RESUMO

Background: The objective of the present study was to compare the efficacy of terbinafine and griseofulvin in patients with tinea corporis in a tertiary care hospital, Madurai.Methods: About 60 patients are selected from the outpatient department of Dermatology according to inclusion and exclusion criteria. They were divided into 2 groups of 30 patients each. Group 1 received tab. terbinafine 250 mg OD and group 2 received 250 mg BD for 4 weeks. All patients were investigated at baseline, end of 2nd week and at end of 4 weeks. Effectiveness of both the drugs were determined by achieving clinical as well as mycological cure. The results were recorded, tabulated and analysed using student抯 t test.Results: Patients in group 1 showed higher clinical and mycological cure rate when compared with group 2.Conclusions: Oral terbinafine is the effective antifungal agent in the treatment of extensive tinea corporis infection.

Background: Infertility rates were highest in women living in urban areas and increases proportionately with level of education. The social consequences due to infertility affect the women more often. It is necessary to identify and treat the amount of stress and various coping methods by infertile women population. We have evaluated psychological impact of infertility and coping strategies employed in women population attending infertility OPD at department of obstetrics in a tertiary care teaching hospital in Tamil Nadu. Material and methods: This was a hospital based cross sectional study done in 150 women attending infertility OPD at a tertiary care hospital in Tamil Nadu for a period of 12 months from January 2014 to December 2014. A structured and validated proforma was designed for the purpose of data collection. The tool was validated by including the inputs from five experts in the subject area. Categorical variables were presented as frequencies and percentages. Quantitative variables were presented as mean and standard deviation. IBM SPSS version 21 was used for statistical analysis. Results: Majority of the study subjects belonged to 21 to 25 years of age group 35.7% and 45.5% of the women had recent onset infertility. 17% women had difficulty in falling asleep was reported. 58.9% of the women reported with high levels of anxiety. 26% women have decrease in weight was noticed. Whereas 48.2% women reported feeling guilt and 35.7% women reported feeling of pessimism and suicidal tendency.16.28% women participants reported to have immersing in household activities followed by 13.95% with hobbies and 11.63% with crying spells. Page 115 Conclusion: Our study revealed that infertile women experienced high levels of anxiety, guilt, feeling of pessimism and suicidal tendency. The coping strategy adopted by majority of the women was engaging in household activities and hobbies. There should be parallel psychological counselling should be advocated strongly to all women with infertility taking treatment along with family members.

Autologous chondrocyte implantation (ACI) is a widely accepted procedure for the treatment of large, fullthickness chondral defects involving various joints, but its use in developing countries is limited because of high cost and failure rates due to limited resources and support systems. Five patients (age <45 years) with focal cartilage defects received ACI at University of Malaya from 2006 to 2007 and followed up for 36 months. The average presubjective Knee Evaluation Forms (IKDC) improved from 38.44±6.29 to 25.6±8.04 postoperatively, the Oxford Knee Score (OKS) went from 25.6±8.04 to 13.96±1.63 and the American Knee Society Score (AKSS) improved from 80±14.33 to 92.96±5.82 post-operatively. Thus improvements were seen in the IKDC and AKSS score but not in the OKS. Magnetic resonance images showed the presence of cartilage tissue filling in the lateral and medial patellar facet and medial femoral condyle in three patients. Failures were seen in two patients, both with patellar defects and over the age of 36 years. Treatment with autologous chondrocyte implantation for focal cartilage defect in lateral and medial patellar facet and medial femoral condyle showed early improvement which was maintained at 3 yrs follow-up. ACI provided satisfactory outcome in focal cartilage defects involving the femoral condyle.

Background & objectives: Mesencymal stem cells (MSCs) derived from foetal tissues present a multipotent progenitor cell source for application in tissue engineering and regenerative medicine. The present study was carried out to derive foetal mesenchymal stem cells from ovine source and analyze their differentiation to osteogenic linage to serve as an animal model to predict human applications. Methods: Isolation and culture of sheep foetal bone marrow cells were done and uniform clonally derived MSC population was collected. The cells were characterized using cytochemical, immunophenotyping, biochemical and molecular analyses. The cells with defined characteristics were differentiated into osteogenic lineages and analysis for differentiated cell types was done. The cells were analyzed for cell surface marker expression and the gene expression in undifferentiated and differentiated osteoblast was checked by reverse transcriptase PCR (RT PCR) analysis and confirmed by sequencing using genetic analyzer. Results: Ovine foetal samples were processed to obtain mononuclear (MNC) cells which on culture showed spindle morphology, a characteristic oval body with the flattened ends. MSC population CD45-/CD14- was cultured by limiting dilution to arrive at uniform spindle morphology cells and colony forming units. The cells were shown to be positive for surface markers such as CD44, CD54, integrinβ1, and intracellular collagen type I/III and fibronectin. The osteogenically induced MSCs were analyzed for alkaline phosphatase (ALP) activity and mineral deposition. The undifferentiated MSCs expressed RAB3B, candidate marker for stemness in MSCs. The osteogenically induced and uninduced MSCs expressed collagen type I and MMP13 gene in osteogenic induced cells. Interpretation & conclusions: The protocol for isolation of ovine foetal bone marrow derived MSCs was simple to perform, and the cultural method of obtaining pure spindle morphology cells was established. Criteria proposed for defining MSCs by this study includes the cell adherence to culture plates, specific surface protein profiles and differentiation to osteogenic lineage. The MSCs and osteogenic differentiated cells in this ovine animal model may serve as a large source for stem cell applications in regenerative medical therapies.

Plasma IL-6 and serum MDA were significantly high in microalbuminuria and macroalbuminuria as compared to normal controls and normoalbuminuria. Plasma TAC was significantly decreased in all the groups of diabetic patients as compared to normal controls. Significant increase in lipid parameters and AIP were observed in macroalbuminuria and microalbuminuria as compared to normal controls. Plasma IL-6 showed negative correlation with HDL-c (r=-0.255) and significant positive correlation was observed between MDA and HbA1c (r=0.537) in diabetic patients. Conclusion: Increased IL-6, MDA levels and decreased plasma TAC levels in diabetic patients revealed inflammation with increased oxidative stress, which may involve in the development of renal damage. The associated altered lipid profile and high risk AIP indicates the risk of developing cardiovascular complications in diabetic patients with macroalbuminuria.

The present investigation was carried out to study the removal of nickel using a non-conventional adsorbent, rice husk. The effects of contact time, adsorbent dosage, pH, concentration of the metal ion and isotherm models are studied in a batch process. A steady trend was observed for all variables. The adsorption percentage increased with increase in contact time, adsorbent dosage and pH, but there was a decrease in adsorption with increase in concentration of the metal ion. K, the adsorbent capacity and 1/n, the adsorption intensity values showed that rice husk, a non-conventional adsorbent is efficient in removing Ni (II) from an aqueous solution. The adsorption data fitted well into Freundlich adsorption isotherm.
