RESUMO

Objective:To investigate clinicopathological characteristics and efficacy of conversion therapy in patients with metastatic gastric cancer.Methods:The clinicopathological and follow-up data of metastatic gastric cancer patients at the Department of Gastrointestinal Surgery of Peking University People's Hospital from Jan 2018 to Jun 2021 were retrospectively studied. Multivariate Logistic regression analysis was used to identify independent characteristics for pathological complete response (PCR). The influence of stage of metastatic gastric cancer and pathological response on prognosis were analyzed by Kaplan-Meier curve.Results:A total of 31 patients were enrolled, and 13 tumors located at the cardia or fundus, 8 at body, other 10 at pylorus or antrum . Baseline CT evaluation showed retroperitoneal lymph node metastasis in 10 cases, intraperitoneal metastasis in 10 cases, liver metastasis in 2 cases, adrenal and splenic metastasis in 1 case respectively, and multiple metastasis in 5 cases. After conversion therapy, 8 (26%) cases had pathological T0, 16 cases (52%) had pathological N0 and 7 cases (22%) had pathological complete response. Multivariate Logistic regression analysis showed retroperitoneal lymph node metastasis ( OR: 20.082, 95% CI: 2.141-188.315, P=0.009) was the only independent risk factor of PCR. Meanwhile, Kaplan-Meier curve showed pT0 improved disease-free survival significantly ( P=0.021). Conclusions:Metastatic gastric cancer patients with retroperitoneal lymph node metastasis alone had a tolerable conversion therapy effect. pT0 is a significant factor in improving prognosis.

Objective:To explore the effect of tumor deposit (TD) on the prognosis of patients with stage Ⅲ colon cancer after radical resection.Methods:The clinicopathological data of patients with stage Ⅲ colon cancer after radical surgery at the Department of Gastrointestinal Surgery, Peking University People's Hospital from Jan to Dec 2015 were analyzed collected. Clinicopathological characteristics such as tumor location, degree of differentiation, mismatch repair status, lymphatic and venous invasion, and preoperative CEA and CA19-9 levels were used to study the effect of TD on the postoperative survival of patients.Results:Among the 155 patients with stage Ⅲ colon cancer, 37 (23.9%) had tumor deposits. The incidence of tumor deposits was higher in patients with intravascular tumor thrombus and preoperative serum CA19-9 elevation ( χ2=9.567, P=0.002; χ2=11.561, P=0.003); Patients with tumor deposits had worse overall survival and disease-free survival than those without cancer nodules (OS: P=0.029, DFS: P=0.025). Multivariate COX analysis found that tumor deposit was an independent risk factor for postoperative overall survival and disease-free survival ( HR=1.990, 95% CI: 1.032-3.835, P=0.040; HR=2.416, 95% CI : 1.205-3.820, P=0.009). Conclusions:Tumor deposit is an independent risk factor affecting postoperative overall survival and disease-free survival in patients with stage Ⅲ colon cancer. For patients with lymph node metastasis, incorporating TD into TNM staging can more accurately predict the postoperative prognosis.

ObjectiveTo assess the curative effects of Fangji Huangqi detumescence prescription (FHDP) on synovitis and polarization of synovial macrophages of knee osteoarthritis (KOA) model in rats induced by Hulth method. MethodThirty-six rats were randomly divided into sham operation group, model group, high-dose, medium-dose, and low-dose (29.16, 14.58, and 7.29 g·kg-1) FHDP groups, and loxoprofen sodium (16.2 mg·kg-1) group. KOA model in rats was induced by modified Hulth method. Six weeks after the operation, rats were given high, medium, and low concentrations of FHDP, normal saline (NS), and loxoprofen sodium according to the group to intervene, and sacrificed after 2-week administration. Synovium and cartilage histopathological changes were observed after hematoxylin-eosin (HE) staining. Flow cytometry (FCM) and immunofluorescence (IF) test were used to evaluate the polarization of M1/M2 macrophages. Immunohistochemistry (IMC) and enzyme-linked immunosorbent assay (ELISA) were used to detect the related protein expression levels of macrophage polarization, such as interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), and matrix metalloproteinase-13 (MMP-13) in joint tissues and serum. ResultCompared with the sham operation group, Krenn and Mankin scores in the model group were significantly increased (P<0.01). Compared with the model group, Krenn score was decreased in all administration groups (P<0.05, P<0.01), but there was no significant difference in Mankin score in any administration groups. Compared with the sham operation group, M1/mø (CD38+) ratio in the model group was significantly increased (P<0.01), and M2/mø (CD206+) ratio in the model group was decreased (P<0.05). Compared with the model group, M1/mø ratio in the high, medium, and low-dose FHDP groups was decreased (P<0.05, P<0.01), but M2/mø ratio was increased in all administration groups (the difference had no statistical significance). Compared with the sham operation group, M1/M2 ratio in the model group was significantly increased (P<0.01). Compared with the model group, M1/M2 ratio in all FHDP groups was significantly decreased (P<0.01), and M1/M2 ratio in the high and medium-dose FHDP groups was lower than that in the loxoprofen sodium group (P<0.05). Compared with the sham operation group, the levels of TNF-α, IL-1β, and MMP-13 in synovium and cartilage of the model group were significantly increased (P<0.01), the level of IL-10 was significantly decreased (P<0.01). Compared with the model group, the levels of TNF-α and IL-1β in synovium were decreased in all administration groups (P<0.05), but the difference of the levels of MMP-13 and IL-10 in synovium had no statistical significance. The level of inflammatory mediators in cartilage was not affected in all administration groups. Compared with the sham operation group, the levels of TNF-α and IL-β in serum of the model group were significantly increased (P<0.01), the level of IL-10 was decreased (P<0.05). Compared with the model group, the level of TNF-α in the high-dose FHDP group was decreased (P<0.05), and the level of IL-10 was increased in all administration groups (P<0.05, P<0.01). The difference of the level of IL-β in all administration groups had no statistical significance. ConclusionFHDP attenuated the synovitis of KOA rats. FHDP exert the effect on the releasing of proinflammatory cytokines and MMP by inhibiting the polarization of M1 macrophages in synovium, and had no significant effect on the polarization of M2 macrophages. Modulating the imbalanced polarization of synovial macrophages was a possible mechanism of FHDP on attenuating synovitis and treating KOA.

Objective:To summarize the core research topics of the literature on differentiated thyroid cancer (DTC) , and to analyze the citation status to form the knowledge domain of citation science.Methods:Web of Science was used to search the literature on DTC, which was limited to April 04, 2021. The published records on DTC were identified. VOSviewer 1.6.11 and CiteSpace 5.5.R2 software were used to cluster and visualize the knowledge domain of citation.Results:A total of 8, 629 records on DTC were obtained, including 87, 973 citations, which showed that the publication volume increased year by year. Moreover, the trend of the annual records became more significant after the year of 2005. Meanwhile, it was reported that clinical staging and surgical management, as well as clinical researches on targeted therapy drugs, were treated as the currently hot research topics. The data showed that 115 records were cited more than 100 times, and 14 cited more than 300 times. And two records had been cited more than 1,000 times. Furthermore, the publication year of top15 were from 1988 to 2016, which also illustrated that the development of DTC researches were relatively slow before 2006. And the annual publication volume of DTC increased significantly with the publication of four highly cited records in 2006, even three with which have 1,000 citations. And the results revealed that the Consensus and Clinical Guidelines on DTC issued by ATA, as well as targeted therapy and radioactive 131I therapy on DTC had been highly cited. However, the newer highly cited documents on surgical treatment of DTC were still lacking. Conclusions:The current hot topics on DTC are focused on clinical staging, surgical management and targeted therapy. And a high-quality clinical guidelines and consensus and RCTs of targeted drugs on DTC have a significant impact on its development. Moreover, the further researches need to pay more attention to persistent/recurrent and metastatic DTC.

Objective:To evaluate the efficacy and safety of primary tumor resection for asymptomatic metastatic colorectal cancer with unresectable metastases.Methods:A literature search was conducted in PubMed, the Cochrane Library, Chinese National Knowledge Infrastructure and WanFang databases to identify studies comparing primary tumor resection and systemic treatment for asymptomatic metastatic colorectal cancer with unresectable metastases until March 31, 2021 (inchuding RCTs and non-RCTs) . The search strategy was: (((colorectal cancer) OR (colorectal neoplasms) OR (colon cancer) OR (colon neoplasms) OR (rectal cancer) OR (rectal neoplasms)) AND ((metastatic) OR (stage Ⅳ)) AND (asymptomatic) AND (primary tumor resection)). Review manager (RevMan) (Version 5.3.0, Cochrane Collaboration, Oxford, UK) was used to perform the statistical analysis.Results:A total of 14 original studies (RCT: 1; cohort study: 13) were included in this analysis with a total of 2123 patients (primary tumor resection: 1162 cases, systemic treatment: 961 cases). The results of this Meta-analysis showed that PTR group had a significantly improved overall survival in 1-year overall survival ( HR=0.80, 95% CI: 0.67~0.95, P=0.01), 2-year overall survival ( HR=0.81, 95% CI: 0.71~0.93, P=0.002), 3-year overall survival ( HR=0.82, 95% CI: 0.72~0.92, P=0.001), 4-year overall survival ( HR=0.86, 95% CI: 0.75~0.98, P=0.02) and 5-year overall survival ( HR=0.85, 95% CI: 0.74~0.97, P=0.02). The median survival time of PTR group was 4.35 months longer than that of systemic treatment group ( MD=4.35, 95% CI: 0.99~7.72, P=0.015). Conclusions:The current evidence suggests that primary tumor resection may be a potentially safe and feasible treatment strategy for asymptomatic metastatic colorectal cancer with unresectable metastases. Large sample size prospective randomized controlled trials are needed to validate our findings in the future.

Objective:To observe the clinical characteristics of esophageal reflux after total gastrectomy (ERATG), and to explore the mechanism of occurrence.Methods:Fourteen gastric cancer patients who underwent total gastrectomy were prospectively enrolled in this study. The postoperative symptoms were observed and recorded and 24 h MII-pH with pH monitoring was performed to investigate the characteristics of postoperative reflux.Results:After total gastrectomy patients were with different degrees of ERATG as heartburn, appetite loss, chest tightness and belching. The overall nature of ERATG is mainly weak acid, with a pH between 4 and 7. ERATG involved esophageal-jejunal anastomosis and a length of esophagus 7 cm above the anastomosis. Patients with typical reflux symptoms had a lower pH minimum in the upright position than those without typical symptoms[(4.76±0.71) vs.(5.68±0.37), t=2.866, P<0.05]. Patients with typical reflux symptoms had a higher frequency of reflux of mixed liquid and liquid-air reflux than those without typical symptoms[liquid(31.25±29.76) vs.(4.50±9.14), t=0.011, P<0.05; liquid-air(19.50±12.99) vs.(2.00±2.61), t=0.004, P<0.05]. Conclusion:ERATG is mainly a upward reflux of weakly acidic gas, with typical symptoms of heartburn, appetite loss, chest tightness and belching. Patients with typical symptoms usually have lower pH in the upright position.

Objective:To investigate the role of indocyanine green(ICG) fluorescence imaging in laparoscopic anterior resection for rectal cancer.Methods:A retrospective analysis was performed on 7 patients who had undergone laparoscopic anterior resection with the use of ICG fluorescence imaging at Peking University People′s Hospital between Oct 2018 and Mar 2019. The clinicopathological variables, surgical factors, short-term outcome and complications were analyzed.Results:The median operation time was 185 min. The median estimated blood loss was 50 ml. The median time from ICG injection to anastomotic perfusion was 45 s. One patient received extended proximal resection of bowel due to poor perfusion as suggested by ICG imaging. The median time to soft diet was 4 days, and the median hospital stay was 8 days. The median number of lymph nodes harvested was 16. There were no major complications in all these patients. No adverse events related to ICG were recorded.Conclusions:ICG fluorescence imaging was safe and effective in detecting insufficient blood supply around newly established bowel anastomsis, hence potentially reducing the anastomotic leakage rate.

Objective:To determine the diagnostic value of tumor markers in peritoneal lavage fluid from colorectal cancer patients for tumor peritoneal metastasis.Methods:A total of 227 colorectal cancer patients who undergoing surgical treatment were included. 300 ml of peritoneal lavage fluid was irrigated immediately upon laparotomy for traditional cytology (PLC) testing, 134 patients were tested for tumor marker of peritoneal lavage fluid (pTM). Univariate analysis was performed to determine the risk factors for peritoneal metastasis; pTM ROC curve was used to determine the best cutoff value; paired chi-square test was used to compare the difference between PLC and pTM detection.Results:The positive rate of PLC was 12.3% (28/227). Age>65, stage T3 + , lymph node metastasis, mucinous adenocarcinoma and increased serum CA125, CA19-9 are related to peritoneal metastasis; The best cutoff value of pTM for peritoneal metastasis : pCEA 17.095 ng/dl, sensitivity 58.3%, specificity 93.9%; pCA19-9 4.515 U/ml, sensitivity 83.3%, specificity 80.0%; pCA125 303.2 U/ml, sensitivity 58.3%, specificity 95.7%; pCA-724 3.01 U/ml, sensitivity 66.7%, specificity 95.7%; The best cutoff value of pTM for peritoneal micrometastasis: pCA19-9 3.43 U/ml, sensitivity 100%, specificity 72.2%. The positive rate of pCA19-9 was 29.85%, which was higher than that of PLC (χ 2=2.00, P<0.05). Conclusion:Peritoneal metastasis of colorectal cancer is related to tumor T stage, lymph node metastasis, tumor pathological type, and increased serum CA125 and CA19-9; pTM has diagnostic value for peritoneal metastasis of colorectal cancer.

Objective To investigate SIRT2 expression and its significance in colorectal cancer liver metastasis.Methods The expression level of SIRT2 was detected in 25 cases of colorectal cancer tissues and corresponding liver metastasis tissues and another 60 cases of colorectal cancer tissues and corresponding adjacent normal tissues by immunohistochemical method.The clinicopathologic correlations of SIRT2 expression were performed using chi-square test.Kaplan-Meier method was used to evaluate the overall survival.Transwell method was used to evaluate invasiveness of SW480 cell line.qRT-PCR was applied to assess expression level of E-cadherin and MMP2.Results SIRT2 expression significantly decreased in colorectal cancer liver metastases (3.6 ±0.5 vs.6.5 ±0.8,t =15.37,P <0.01).The low expression level of SIRT2 was significantly associated with tumor differentiation (x2 =7.475,P =0.024).Patients with low SIRT2 expression had a shorter overall survival than those with high SIRT2 expression [(42.5 ±6.0)mon vs.(60.5 ±5.4)mon,x2 =5.493,P =0.019].Multivariate analysis showed that SIRT2 expression and distant metastasis were the two factors that were independently associated with prognosis.Overexpression of SIRT2 significantly weakened the invasive ability of SW480 cell line.Conclusion Downregulation of SIRT2 predicts poor prognosis of patients with colorectal cancer for enhancing tumor cell invasive ability.

Objective To investigate the influence of mismatch repair status on clinicopathological characteristics and prognosis in patients with colon cancer.Methods Patients who underwent radical excision for colon cancer between Nov 2012 and Mar 2016 at Peking University People's Hospital were enrolled.Clinicopathological data and prognosis were collected.Multivariate analysis were used to identify independent characteristics of MMR-deficient colon cancer.The influence of MMR-deficient on prognosis of colon cancer were analyzed through Kaplan-Meier curve.Results The overall rate of MMR-deficient in colon cancer was 17.1% (51/299).Multivariate logistic regression analysis showed that low differentiation (OR =3.555,95% CI:1.685-7.640,P < 0.001),right-sided colon cancer (OR =5.645,95% CI:2.483-14.715,P < 0.001) and UICC Stage Ⅰ-Ⅱ (OR =4.099,95% CI:1.863-9.840,P <0.001) were associated with MMR-deficient colon cancer.Conclusion Low differentiation,right-sided colon cancer and UICC Stage Ⅰ-Ⅱ were more common in MMR-deficient colon cancer.

We report a case of follicular variant papillary thyroid carcinoma with bone metastases as the initial symptom,and review the progression,management and short-term outcome of the case.The classification and characterization of follicular variant papillary thyroid carcinoma combined with a review of literature were discussed.We hope that the lessons learn will help better management of the disease.

Objective@#To compare the efficacy and safety of two procedures in the treatment of hemorrhoid: the procedure for prolapse and hemorrhoids (PPH) and stapled transanal rectal resection (STARR).@*Methods@#A retrospective cohort research was conducted. Clinical data of 263 patients undergoing the first elective surgery for grade IV hemorrhoids by the same team of surgeons at our department from January 2015 to December 2016 were analyzed retrospectively, while those had other anorectal diseases, emergency surgery, inflammatory bowel disease, tumor and incomplete clinical data were excluded. PPH was performed in 129 patients and STARR was performed in 134 patients. PPH procedure: a circular purse 2-0 string suture was made at 4 cm above the dentate line; in accordance with the standard protocol, the PPH circular stapling devicewas introduced; the suture was closed, and a pull-through followed; the traction was continued; the stapler was fired; the prolapsed mucosa and submucosa were removed. STARR procedure: 3-5 needles were sutured in the anterior rectal mucosa, protecting the posterior wall mucosa; with the help of a finger the PPH stapler was inserted into the vaginal lumen; the sutures were hooked from both sides of the stapler to maintain traction; according to the disease condition, the suturewas tightened appropriately; stapler was screwed and activated; the anterior wall mucosa was removed; the joint of the both ends of anastomosis was cut; the posterior wall mucosa was removed as well. The short-term efficacy, surgical safety and prognosis of the two groups were compared.@*Results@#There were 67 males (51.9%) in the PPH group and 57 (42.5%) males in the STARR group. The median age of the two groups was 51.0 (22.0, 80.0) years and 49.0 (24.0, 74.0) years, respectively. There were no significant differences in the baseline data between the two groups (all P>0.05). No significant differences in the intraoperative bleeding, length of hospital stay, postoperative analgesic drug use, postoperative bleeding, postoperative infection, etc. were found between two groups (all P>0.05). As compared to PPH group, STAAR group had longer operation time and higher hospitalization cost with significant differences [(44.0±19.3) minutes vs. (26.3±8.5) minutes, t=9.701, P=0.001; (11 047±473) yuan vs. (7674±309) yuan, t=32.826, P=0.001]. One case in STAAR group developed rectovaginal fistula. The median follow-up period of the whole group was 40 (33, 52) months. A total of 108 cases in STARR group and 114 cases in PPH group completed the follow-up. The 3-year disease-relapse rate was 0 in STARR group and 4.2% in PPH group (P=0.042).@*Conclusion@#STARR procedure can improve the prognosis in the treatment of grade IV hemorrhoid, but attention should be paid to the development of complications.

Objective Energy based surgical devices such as high frequency electrotome,Harmonic scalpel and LigaSure are widely used in thyroid operations.This study is to demonstrate the difference of tissue thermal damage among different surgical instruments.Methods 12 beagle dogs were randomly divided into 3 groups,electrotome 15 W groups (group A),three-speed Harmonic scalpel (group B) and LigaSure middle gear group (group C).Patients of each group received energy instruments operating on their thyroid tissue which mimics traditional thyroidectomy.The temperatures of gland tissue during procedures were monitored by infrared thermal imager,and the operated thyroid tissues were histologically analyzed.Results The highest temperature was (83.9±8.2)℃ in the electrotome group,(70.7±7.5)℃ in three-speed Harmonic scalpel group,and (56.6±5.7)℃ in LigaSure group.The highest temperature among the three groups was statistically significant.The electrotome (15 W) caused more serious thermal damage to thyroid tissues than that caused by either the Harmonic scalpel or LigaSure (thermal damaged depth:(0.96±0.07) mm vs (0.74±0.07) mm,P<0.01;(0.96±0.07) mm vs (0.72± 0.11) mm,P<0.01).Nevertheless,the thermal damage had no significant differences between the Harmonic scalpel and LigaSure group (P=0.845).The thermal damage caused by the 15 W electrotome was significantly larger than that in the other two groups,and the difference had statistical significance (P<0.01).Conclusion Compared to the high frequency electrotome,Harmonic scalpel and LigaSure lead less tissue thermal damage during thyroid surgeries,owing to less heat production.In that way,Harmonic scalpel and LigaSure are superior to electrotome in terms of safety.

Objective To investigate the mechanism of lnc-AC079612.1-1∶1 in the metastasis of colorectal cancer by regulating miR-1915 expression and provide a theoretical basis for new therapeutic targets of colon cancer.Methods Continuously harvested 3 pairs of primary tumor and hepatic metastases of patients with synchronous colon cancer with liver metastasis.LncRNA + mRNA Human Gene Expression Microarray V4.0 and 4 × 180K Expression Microarray were used to detect the Expression profile of LncRNA in primary colon cancer and matched liver metastases.The detection of expression profile chip was used to find some lncRNAs that may be closely associated with colon cancer liver metastasis,and through the literature retrieval,the author preliminarily analyzed the co-expression encoding genes of lncRNAs,in order to find lncRNAs that may play key roles in the process of colon cancer liver metastasis.The effect of lnc-AC079612.1.1-1∶1 on the proliferation capacity of colon cancer cell lines was detected in SW480 cell lines and SW620 cell lines of colon cancer through CCK-8 experiment.Effects of lnc-AC079612.1.1-1∶1 on the migration and invasion of colon cancer cells were examined by Transwell cell invasion assay.Through TargetScan,Miranda,LNCipedia and other web tools,miRNAs that may bind to lncRNA and o-6-methylguanine-DNA methyl transferase (MGMT) were predicted.Double luciferase reporter gene assay was used to detect the interaction between miR-1915 andlnc-AC079612.1.1-1 ∶ 1.SPSS 22.0 statistical software was used to analyze the data.The measurement data were expressed as ((x) ± s).T test was compared between the two groups,and multiple groups were compared with F test.Results The expression of lnc-AC079612.1-1 ∶ 1 was 3.94 times higher in the liver metastases than in the primary tumor,and expression of MGMT was 3.74 times higher in the liver metastases than in the primary tumor.The results showed that there was a significant co-expression relationship between the lncRNA and o-6-methylguanine DNA transferase (MGMT) gene,and the co-expression correlation coefficient was 0.994 (P < 0.001).The over expression of lnc-ac079612.1.1-1∶1 could significantly enhance the proliferation capacity of SW480 and SW620 cell lines of colon cancer through the CCK-8 experiment.The over expression of lnc-ac079612.1.1-1:1 could promote the invasion and metastasis of SW480 and SW620 cell lines of colon cancer through Transwell cell invasion experiment.MirR-1915-3p was the only miRNA that predicted to be combined with lnc-ac079612.1-1∶1 and MGMT through TargetScan,Miranda,LNCipedia and other web tools.Lnc-ac079612.1.1-1∶1 was found to bind to miR-1915 by double luciferase assay.Conclusion Lnc-ac079612.1.1-1∶ 1 may participate in the liver metastasis process of colon cancer by regulating MGMT,and may play a role in the liver metastasis of colon cancer through the action of mir-1915 on MGMT.

Objective To evaluate different methods in detecting intraperitontal free cancer cells (IFCCs) in patients with gastric cancer and to clarify the relationship between positive IFCCs and short-term prognosis.Methods A total of 119 gastric cancer patients who underwent surgical treatment were enrolled.Peritoneal lavage was performed with 300-400 ml saline respectively at three points of time:immediately after abdominal cavity entry;when surgical operation was completed;when extensive intraoperative peritoneal lavage was done.The IFCCs were detected with methods of traditional centrifugal cytology,membrane cytology,ICC and RT-PCR.The survival curve of patients with gastric cancer was drawn using Kaplan-Meier method.Results The positive rate of PLC was 16.8%,20.7% and 11.2% respectively at 3 timepoints (P < 0.05).The positive rates of ICC were 28.6%,38.8% and 20.7% respectively at 3 timepoints.The positive rates of RT-PCR were 39.3%,69.5% and 50.8% respectively at 3 time points.The positive rate of IFCCs detected through RT-PCR was higher than that of PLC and ICC (P < 0.05).The short-term prognosis of patients with positive IFCCs was worse than those with negative results detected with any three method at the timg point immediately after opening the abdomen (P < 0.05).At the timg point immediately after removing the tumors,the short-term prognosis of patients with positive IFCCs detected with PLC was worse (P < 0.05).Conclusion The short-term prognosis was poor in patients with positive IFCCs.It is the best time to detect IFCCs before radical resection.Surgical procedures increase the risk of shedding of IFCCs.

Gastric stump cancer(GSC) is defined as newly developed remnant stomach cancer following gastrectomy. This definition initially referred to carcinoma detected in the remnant stomach more than 5 years after the primary surgery for a benign disease. Subsequently, this timeframe was extended to 10 years after the primary surgery for a malignant disease. Recently, the concept of "carcinoma in the remnant stomach(CRS)" proposed by the Japanese Gastric Cancer Association was introduced in China. The new definition encompasses all carcinomas arising in the remnant stomach following gastrectomy, irrespective of the histology of the primary lesion, extent of resection, or reconstruction method. It includes all carcinoma types that have developed in the remnant stomach, such as newly developed cancer, recurrent cancer, remaining cancer, and multiple cancers. Considering the current diagnosis and treatment status of gastric cancer in China, if CRS is to be used as a direct equivalent to GSC in clinical practice, confusion may arise concerning disease identification and diagnosis. Following several discussion rounds, a meta-analysis of the literatures at home and abroad, and a multicenter national retrospective study with a large sample population, the "Chinese surgeons' consensus opinion for the definition of gastric stump cancer (version 2018)" was completed. By reviewing the detailed evidence-based medicine supporting the consensus document, this paper aims to assist clinical diagnosis and enhance future academic exchange.

<p><b>OBJECTIVE</b>To explore the prognostic value of the tumor-ratio-metastasis (TRM) staging system for carcinoma in the remnant stomach(CRS).</p><p><b>METHODS</b>Clinicopathological data of 91 CRS patients who underwent surgery at Peking University People's Hospital between March 1992 and December 2017 were retrospectively analyzed. According to the ratio of metastatic lymph node to dissected lymph node, the R staging was obtained, and the pN staging was replaced by the R staging to create the TRM staging. To compare the predictive accuracy of TRM and tumor-node-metastasis (TNM, UICC version 7), the R staging and pN staging were included in the prognostic factor analysis model, and the survival curve, c-index, and 95% confidence interval (CI) of the TRM staging and TNM staging system were compared. A higher c-index value means higher prediction accuracy.</p><p><b>RESULTS</b>Of 91 CRS patients, 77 were male and 14 were female with the mean onset age of (65.2±10.4) years. The mean interval from the first operation to CRS onset was 156(6-600) months. The primary diseases of 49(53.8%) cases were benign and of 42(46.2%) cases were malignant. The median number of retrieved lymph node (RLN) was 8 (0-38), and 64 patients (70.3%) had an RLN ≤15. Lymph node metastasis occurred in 50 patients (54.9%). pN staging result was as follows: 41 cases in N0 stage, 14 in N1 stage, 19 in N2 stage, and 17 in N3 stage. R staging result was as follows: 41 cases in R0 stage, 4 in R1 stage, 19 in R2 stage, and 27 in R3 stage. TNM staging result was as follows: 13 cases in stage I(, 25 in stage II(, 10 in stage III(a, 23 in stage III(b, and 6 in stage III(c. TRM staging result was as follows: 13 cases in stage I(, 24 in stage II(, and 4 in stage III(a, 18 in stage III(b, and 18 in stage III(c. Univariate analysis showed that tumor diameter ≥7 cm (HR=2.696, 95%CI: 1.307-5.563, P=0.007), T3-4 stage (HR=4.350, 95%CI: 1.949-9.707, P=0.000), N2-3 stage (HR=1.883, 95%CI: 1.167-3.038, P=0.009), R2-3 stage (HR=1.642, 95%CI: 1.026-2.628, P=0.039), TNM III(-IIII( stage (HR=2.448, 95%CI:1.490-4.021, P=0.000), and TRM III(-IIII( stage (HR=2.504, 95%CI:1.515-4.137, P=0.000) were related to prognosis. Tumor diameter, pT staging, and pN staging were included in the Cox multivariate analysis, and the result showed that pT staging (HR=5.507, 95%CI:2.254-13.454, P=0.000) and pN staging (HR=1.698, 95%CI: 1.022-2.789, P=0.041) were independent risk factors for overall survival of CRS in this group. While R staging replaced pN staging and was included in the Cox multivariate analysis together with tumor diameter and pT staging, the result showed that R staging was not an independent risk factor for CRS in this group (HR=1.622, 95%CI: 0.866-2.329, P=0.164). Survival curve revealed pN and TNM staging systems provided better stratified curves according to each staging than R and TRM staging systems. The overall survival c-index of TNM and TRM staging systems was 0.813(95%CI: 0.732-0.826) and 0.809(95%CI: 0.741-0.847) respectively, and no significant difference in predictive accuracy was found (P=0.693). In 42 patients with primary malignance, the overall survival c-index of TNM and TRM staging systems was 0.774(95%CI: 0.589-0.901) and 0.761(95%CI: 0.596-0.912) respectively, and there was no significant difference in predictive accuracy as well (P=0.881).</p><p><b>CONCLUSION</b>TRM staging is not superior to TNM staging (7th UICC) in evaluating the resected samples of CRS.</p>

<p><b>OBJECTIVE</b>To study the clinicopathological features and prognosis of gastric stump cancer (GSC) following subtotal gastrectomy for gastric cancer, to compare the clinicopathologic differences between narrow GSC and generalized GSC, and to compare the prognosis between GSC and primary proximal gastric cancer (PPGC) after radical resection.</p><p><b>METHODS</b>Literatures of GSC-associated clinical study were searched by computer from the Cochrane Library, Medline, PubMed, CNKI, Wanfang and VIP databases, and the retrieval period was from the establishment of database to December 31, 2017.</p><p><b>INCLUSION CRITERIA</b>(1) GSC was defined as a carcinoma arising in the gastric remnant after radical gastrectomy for gastric cancer, and confirmed by the pathological or histological examination, the elapsed time from the initial operation was not considered in the definition. (2) Retrospective or prospective clinical cohort study. (3) Study included at least one of below items: gender, anastomotic type in gastric cancer surgery, the interval between the initial surgery and diagnosis of GSC, the location, treatment, pathological differentiation, pathologic stage, lymph node metastasis rate and prognosis of GSC. (4) When similar studies were reported by the same institution or author, either the better quality study or the newest publication was chosen.</p><p><b>EXCLUSION CRITERIA</b>(1) Abstracts, reviews, case reports, meeting record, editorials and repeated research. (2) Studies including patients with initial non-gastric cancer. In this study, gastric stump cancer(GSC) after gastric cancer was divided into two groups: the incidence without limit interval time (generalized GSC group) and above 10 years (narrow GSC group). Selective trials were Meta-analyzed by the Stata13.0 software and statistical analysis was performed using SPSS 21.0 software.</p><p><b>RESULTS</b>A total of 27 literatures were finally enrolled, which comprised 1463 GSC patients, including 1146 males and 317 females. The generalized group and narrow GSC group had 921 and 542 patients respectively. The generalized GSC group and the narrow GSC group did not significantly differ in terms of previous reconstruction mode, types of differentiation, pathologic T staging, postoperative pathology tumor-node-metastases staging, and distant metastasis rate (χ=2.341, 0.926, 0.350, 0.965, 2.311 respectively, all P>0.05). As compared to generalized GSC group, narrow GSC group had higher ratio of male patients (82.8% vs. 75.7%, χ=9.909, P=0.002), more lesions locating in anastomotic stoma (37.8% vs. 26.1%, χ=18.091, P=0.000), higher ratio of patients undergoing radical resection (84.2% vs. 70.3%, χ=11.738, P=0.001), higher positive rate of postoperative lymph node (45.8% vs. 34.5%, χ=6.319, P=0.012), and larger size of tumor [(5.9±2.2) cm vs. (4.5±1.9) cm, t=9.151, P=0.000]. The overall 5-year survival rate and postoperative pathology stage III(-IIII( survival ratio in narrow GSC group were higher compared to general GSC group (42.7% vs. 30.6% and 27.5% vs. 18.1%, respectively), which were significantly different (χ=10.938, P=0.000; χ=4.128, P=0.042), while the postoperative pathology stage I(-II( survival ratio was not significantly different between two groups (67.3% vs. 67.0% respectively, χ=0.015, P=0.92). There was no significant difference in the 5-year survival rate between GSC with radical resection and PPGC(RR=1.04, 95%CI:0.79-1.36, P=0.805) and the 5-year survival rate of same postoperative pathology stage was not significantly different between two groups (I(-II( stage: RR=1.08, 95%CI:0.93-1.26, P=0.328; III(-IIII( stage: RR=0.59, 95%CI:0.33-1.04, P=0.111).</p><p><b>CONCLUSIONS</b>There are some different clinicopathological features between the generalized and the narrow GSC after gastric cancer surgery. The prognosis of GSC after radical resection is similar to primary proximal gastric cancer.</p>

Gastric stump cancer (GSC) is a carcinoma arising from the remnant stomach following gastric surgery for benign or malignant disease, and is more common in men. The risk of morbidity has an obvious time dependence. GSC incidence is likely to rise with lengthening of the initial operation interval. The GSC time interval after malignant disease is significantly shorter than that of benign disease. GSC etiologies mainly include duodenogastric reflux and denervation of the gastric mucosa resulting in the change of the gastric environment after gastrectomy and the Helicobacter pylori infection. Due to atypical clinical symptoms, GSC is always identified at an advanced stage and the long-term survival rate is low. An optimal endoscopic surveillance system is essential to improve early detection rates. Treatments in GSC and primary gastric cancer are the same and include resection of the lesion and radical lymph node dissection. R0 resection is an important prognostic factor. Here we review previous reports with respect to epidemiological characteristics, etiology, clinical symptoms, treatment, and prognosis of GSC.

Objective To compare the surgical outcomes of RAC versus LAC for the surgery of sigmoid-rectal cancer.Methods In this study,19 patients received robotic assisted colectomy (RAC)and 37 patients received laparoscopic assisted colectomy (LAC).Results The duration of surgery was longer in the RAC group (240 ± 76) min vs.(177 ± 74) min,t =3.006,P =0.004.The blood loss was less in the RAC group (67 ±40) vs.(153 ±151) ml,t =-2.422,P=0.019.The time to tolerate solid food was shorter in the RAC group (6.1 ±2.2)d vs.(7.4 ±4.4)d,t =-1.329,P =0.189 and the postoperative hospital stay was shorter in the RAC group (8.5 ± 2.6) d vs.(10.0 ± 3.6) d,t =-1.577,P =0.121.Mobidity and number of lymph nodes harvested were similar in both groups.7 cases were converted to open surgery in LAC group but none in RAC group.Conclusion Compared to laparoscopic surgery,Robotic-assisted colorectomy was safe and associated with less blood losses,lower conversion rate to open surgery and faster recovery of bowel function.
