RESUMO

Adenoma malignum is a well differentiated form of adenocarcinoma. Despite of benign histologic appearance, this tumor has malignant clinical course. Because of its rarity and subtle histologic changes, it may be missed. Therefore it has poor prognosis. Early diagnosis and early treatment is required by thorough examination. We report one case of adenoma malignum of uterine cervix with a brief review of the literature.

RESUMO

OBJECTIVE: To access the pregnancy and delivery in the early thirties. METHODS: From January 1994 to December 2003, we statistically compared and investigated the 21,744 deliveries in Sung Ae Hospital, considering pregnant women aged 30-34 as study groups, and pregnant women aged 35 or older (elderly gravidas) as control groups. RESULTS: The rate of Cesarean section, preterm delivery, gestational diabetes, pregnancy-induced hypertension, placenta previa, placenta abruptio, low birth weight was low statistically in the study groups. But, the percentage of the pregnant complication in the early thirties were increased than of in pregnant women younger than 30. CONCLUSION: Pregnant women in the early thirties were safer from the maternal complication, and neonatal complication than women aged 35 or older (elderly gravidas), but were exposed to these illnesses more than women aged younger than 30. Therefore, to improve the pregnant outcome of women in the early thirties, it is needed to concern their pregnancy and delivery and to take care of before and after childbirth thorougly.

RESUMO

OBJECTIVE: This study was aimed to investigate of newborn babies sex ratio in Seoul Seong Ae hospital and review of future problem. METHODS: We reviewed retrospectively the 21,332 cases of deliveries in women visiting at Seoul Seong Ae hospital from Jannuary 1994 to December 2003 (10 year's period). We investigated 1st baby and 2nd baby sex ratio and in case of 3rd baby, subdividing then into tree groups (first and second are female, first and second are male, first and second are male and female). We arranged the data at the base of childbirth registers and patients' obligation records. RESULTS: The results were as follows, 1. The total sex ratio in last 10 years was 111.2:100 (male:female). 2. 1st baby sex ratio was 109.2:100. 3. 2nd baby sex ratio was 110.6:100. 4. 3rd baby sex ratio was 133:100. CONCLUSION: These findings suggest that unbalance of sex ratio in 3rd baby is extremely serious trend in this country and is expected a tremendous social problems in early 21th century.

We evaluated the clinical and radiologic results of 48 cases (31 patients) who had rheumatoid arthritis of the knee joint and were managed with cementless total knee arthroplasty between November 1988 and December 1994. The mean follow-up period was 3.7 years (range, 2 to 7 years). The knee score of the Hospital for Special Surgery improved from an average of 44 points (range, 5 to 77 points) preoperatively to 83 points (range, 50 to 97 points) at the time of the most recent followup examination. In knee score, 85.4% of cases had excellent or good grade (over 75 points). The range of motion improved from an average of 77 degrees to an average of 103 degrees postoperatively. In tibiofemoral angle, 95.8% of cases were between valgus I degree and 10 degrees. The implants were inserted in an optimal position. Continuous radiolucency was not visible beneath any implant and loosening was not found. In one case, heterotopic ossification was removed to improve the range of motion. There was one case of patellar fracture during operation. Even in relatively poor bone quality in rheumatoid arthritis of the knee, the cementless total knee arthroplasty revealed good clinical and radiologic results.

Ilizarov technique has been successfully applied to limb lengthening for several decade, one of its main drawback is, however, long application of external fixator over 6 months. To lessen this time, simultaneous intramedullary fixation which convert later to static fixation by insertion of interlocking screws has been proposed. The indication for surgical limb lengthening includes familial short stature below the 3rd percentile. We compared the results of thirteen tibial lengthening with Ilizarov method to that of eleven hybrid methods. The hybrid system is composed of an unreamed AO intramedullary nail (IM) and ring fixator. Once the lengthening is completed, distal interlocking screws were inserted. In the group who were treated by hybrid system, the mean lengthening of tibia was 7.1cm (range, 6.3-9.5), the mean of external fixation period 7 months (range, 5-9), the mean healing index 1.2 month/cm (range, 1.1-1.4). On the other hand, the mean lengthening of tibia was 7.5cm (range, 5-10.3), the mean of external fixation period 9.8 months (range, 6-15), the mean healing index 1.4 month/cm (range, 1.1-1.8) in the group treated by Ilizarov method. Mean follow-up period was 23 months (range 14-47). There was no rotational or angular deformities and no loss of lengthening in the hybrid group. However we experienced three cases of angular deformity at the distraction site during lengthening in Ilizarov group. There was no deep infection in both groups. In the hybrid group we found several advantages such as no deformity, no loss of lengthening, and less scar by early removal of fixator, but some disadvantages such as need of subsequent procedures and limitation in intramedullary nailing.
