RESUMO

Purpose: To determine the efficacy and safety of pediatric accelerated cross linking (CXL). Methods: A prospective study on progressive keratoconus (KC) cases under ?18 years of age. Sixty four eyes of thirty nine cases underwent epithelium?off accelerated CXL protocol. Visual acuity (VA), slit?lamp examination, refraction, pentacam reading of keratometry (K), corneal thickness, and thinnest location pachymetry were noted. Cases were followed up on days 1, 5, and at 1st, 3rd, 6th, and 12th?month post procedure. Results: Statistically, significant improvement of the mean aided VA, K, and mean corneal astigmatism (p < 0.0001) was noted. Mean Kmax reading reduced from 55.5 ± 5.64 (47.4–70.4) diopter (D) preoperatively to 54.41 ± 5.51 (46–68.3) D at 12 months postaccelerated CXL. Two cases had progression. Complications encountered were sterile infiltrate and persistent haze. Conclusion: Accelerated CXL is effective and efficacious in pediatric KC.

The supplementation of Selenium-enriched probiotics is effective in reducing oxidative stress and maintaining meat quality stability in broiler chicken especially under heat stress. An experimental study was conducted to perform Comparative analysis of Selenium yeast with inorganic Se in broilers under heat stress. A total of 120 broilers chicks of one day were assigned to 4 groups each consisting 30 chicks fed on same basal diet but different selenium sources. The basal diet of group D1 was not supplemented with Se source (Negative control), group D2 basal diet was supplemented with inorganic selenium (Sodium selenite 0.22mg/Kg starter phase and 0.15mg/Kg finisher phase), group D3 basal diet was supplemented with commercially available organic selenium (Seleno-methionine 0.22mg/Kg starter phase and 0.15mg/Kg finisher phase) and group D4 basal diet was supplemented with self-developed organic selenium (Se-enriched yeast 0.22mg/Kg starter phase and 0.15mg/Kg finisher phase). The performance parameters i.e. feed intake (FI), live body weight (BW) and FCR were not significantly (p>0.05) effected by selenium supplementation in the starter phase but were significantly (p<0.05) effected in the finisher phase. Selenium supplementation significantly (p<0.05) effected serum Se level in different supplemented groups. Higher serum Se value (58.20±0.06) was recorded in D4 group. Similarly significantly lower selenium value was recorded for D4 and higher was recorded for D1 (11.36±0.08). However lower serum Paraoxonase (PON) value was recorded for D4 (13.24±0.01) and higher for D1 (13.33±0.03). Comparatively self-developed Se enriched yeast increased the Se accumulation and improved antioxidant system. Glutathione peroxidase (GPx) was found higher in D4 (12.333±0.03) followed by D3, D2 and D1 respectively. A suplementação de probióticos enriquecidos com selênio é eficaz na redução do estresse oxidativo e na manutenção da estabilidade da qualidade da carne em frangos de corte, especialmente sob estresse por calor. Um estudo experimental foi conduzido para realizar uma análise comparativa da levedura selênio com o Se inorgânico em frangos de corte sob estresse térmico. Um total de 120 pintos de um dia foi dividido em 4 grupos, cada um consistindo de 30 pintos alimentados com a mesma dieta basal, mas com diferentes fontes de selênio. Introduction: Hypertension and Diabetes are two of the most common chronic non-communicable diseases and multifactorial disorders affecting both developed and developing countries. Method: A cross sectional study was conducted for a period of 3 months among patients’ attendees above the age of 30 years in a tertiary care hospital of Mangalore, between July - September 2018. Data was collected from 150 patient attendees by personal interviews followed by anthropometry, blood pressure measurement and random blood sugar and entered into Microsoft excel. Analysis was done using Statistical package for social sciences (SPSS) software version 21. Results: It was seen that 72(48%) were Normotensive, 44 (29.3%) were Pre- hypertensive and 34 (22.7%) were Hypertensive while 65(43.3%) had a random blood sugar of > 140 mg/dl. There was an association between hypertension and age, gender, type of family, occupation, religion, alcohol intake, smoking habits and physical activity which was statistically significant (p< 0.05). While age more than 45 years, illiteracy, belonging to Christian faith, having a family history of diabetes mellitus and being obese/ overweight were associated with a random blood sugar of >140 mg/dl which was statistically significant (p< 0.05). It was also seen that 23(67.6%) of hypertensive attendees and 18(40.9%) of pre- hypertensive attendees also had diabetes as a comorbidity. Conclusion: Opportunistic screening of individuals for hypertension and diabetes will help in early detection of these diseases. This will help in early prevention and timely treatment

Genetically engineered plants have varied applications in agriculture for enhancing the values of food and feed.Genetic engineering aims to introduce selected genetic regions with desirable traits into target plants for bothspatial and temporal expressions. Promoters are the key elements responsible for regulating gene expressionsby modulating the transcription factors (TFs) through recognition of RNA polymerases. Based on theirrecognition and expression, RNA polymerases were categorized into RNA pol II and pol III promoters.Promoter activity and specificity are the two prime parameters in regulating the transgene expression. Since theuse of constitutive promoters like Cauliflower mosaic virus (CaMV) 35S may lead to adverse effects on nontarget organisms or ecosystem, inducible/tissue specific promoters and/or the RNA pol III promoters providemyriad opportunities for gene expressions with controlled regulation and with minimum adverse effects.Besides their role in transgene expression, their influence in synthetic biology and genome editing are alsodiscussed. This review provides an update on the importance, current prospects, and insight into the advantagesand disadvantages of promoters reported thus far would help to utilize them in the endeavour to developnutritionally and agronomically improved transgenic crops for commercialization.

In the present work, the quaternary salts of 4-aminopyridine, i.e., 4-amino-1-[2-(4-bromophenyl)-2-oxoethyl]pyridin-1-iumbromides were obtained by stirring 4-aminopyridine with phenacyl bromides in acetone at room temperature separately.These quaternary salts of 4-aminopyridine were treated with acetylenes (electron deficient), in the presence of anhydrouspossium carbonate in N,N-dimethylformamide solvent to get indolizine derivatives. The structures of newly synthesisedcompounds have been confirmed by spectroscopic techniques, such as liquid chromatography mass spectrometry,1H-NMR, and elemental analysis. Synthesized all compounds were screened for antibacterial and antioxidant activity.The compounds 2e, 2g, and 2j shows inhibition zone against bacteria and compounds 2a and 2f moderately active againstbacteria. All compounds 2a to 2j show 1,1-diphenyl-2-picrylhydrazide radical free radical scavenging activity, NitricOxide free radical scavenging activity, Reducing power scavenging activity, and Lipid peroxidation inhibition activity.

Background: Thrombocyte is important and very essential component of blood and have significant role in maintenance of hemostasis. Thrombocyte count is an important investigation done in various acquired and congenital coagulable states which include conditions like pregnancy. Thrombocyte count is routinely done by automated hematology analyzer method. The automated hematology analyzer counters are not usually available at all centres especially in peripheral and rural side though thrombocytes can also be assessed from the peripheral blood smears, which can be easily and precisely done at any set up. Aim and objective of this study was to compare the thrombocyte estimation by peripheral blood smear method and automated hematology analyzer in pregnant women.Methods: Thrombocyte estimation was done from samples taken from 120 normal pregnant women between December 2018 to March 2019, where samples were Ethylene Diamine Tetra Acetic acid (EDTA) anticoagulated. Thrombocyte was counted manually using PBS (Leishman stain) and hematology analyzer (Sysmex XN1000 series). Thrombocyte counts were expressed in Mean and standard Deviation. Statistical analysis was done by student’s t test using MS excel and SPSS version 17.Results: Thrombocyte count by PBS have mean value of 2.04 lacs/mm3 with standard deviation of 0.56 lacs/mm3 and by automated method have mean value of 1.89 lacs/mm3 and standard deviation of 0.71 lacs/mm3 with p value 0.010. Thus, there was no statistically significant difference found between two methods.Conclusions: Estimation of thrombocyte count on the basis of manual thrombocyte count is a reliable technique and can be used to validate automated thrombocyte counts. It can also be used in under resourced laboratories, where there are no automated counters of good precision available. In fact, all the tests showing abnormal thrombocyte counts must be reported only after cross examining on PBS.

Despite recent advances in molecular biology and very advanced techniques leading to early detection of malignant tumours and despite newer drugs and use of nanotechnology an adjunct measures for targeted therapy, cancer remains the most deadly enemy and killer of mankind. Thus cancer remains the single most factor for increase mortality and morbidity of human being as well as for entire nation, impacting its manpower and economy. It is this vicious cycle that needs to be controlled, broken and irradiated which at present appears to be a herculean tasks although not an impossible one.

Papillary thyroid carcinoma (PTC) and Hashimoto’s thyroiditis (HT) are common diseases in clinical practice. Hashimoto’s thyroiditis is the most common autoimmune thyroid disease and the most common cause of hypothyroidism. It is characterized by diffuse lymphocyte infiltration, fibrosis, and parenchymal atrophy. Papillary thyroid carcinoma is the most common malignant neoplasm of the thyroid. There is a high prevalence of association between HT and PTC FNAB is useful for diagnosing PTC in patients with HT associated thyroid lesions. The presence of HT in patients with PTC was associated with disease presentation at an earlier stage, less aggressive clinical course and better outcome. Here we have presented a case of hashimoto's thyroiditis with papillary thyroid carcinoma in a 37 years old female who came with complaints of swelling in front of neck. Cytology and radiology showed features of HT but, on histopathology it was diagnosed as HT with PTC.

Lactating adenomas are rare benign tumors of breast, frequently associated with pregnency and lactation. They are common in the third trimester of pregnancy during second and third decade. They present similarly as other benign lesions of breast and differentiation from them preoperatively is difficult both clinically and radiologically. it is characterised by typical changes of secretory epithelium leading to formation of a well-diffrentiated benign tumor. Cytology and histopathology remains the definitive tests for confirming the diagnosis. Surgery is the treatment of choice. They tend to regress spontaneously and prognosis is generally good. Here we have presennted such a rare case of lactating adenoma arising from 28 years old lactating women.

Fibroadenomas are the most common benign tumor of the breast. it can occur in women of any age but the peak incidence seen in young adolescent women during the second and third decades of life, Fibroadenoma is a biphasic tumor, composed of an epithelial and a stromal component. Even though they are solitary lesions, multiple fibroadenomas can also occur especially in the older age group. Breast fibroadenomas seemed to display a wide spectrum of proliferative and non-proliferative histologic changes. Of all these changes, carcinoma arising from fibroadenoma is a rare occurrence. Here, we have presented such a rare case of carcinoma in situ arising from the long standing multiple fibroadenoma in a postmenopausal women. Clinical and radiological features are not helpful in identifying carcinoma arising from fibroadenoma. Histopathological examination of the excised specimen should be done for the confirmation of the diagnosis.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal (GI) tract. Most of the cases are located in the stomach followed by the small intestine. They arise from the interstitial cells of cajal, which are located in the wall of the intestine. Malignant GISTs are rare type of tumors in GIT. Small intestine GIST is more likely to be malignant compared to stomach. Here we reported such a rare case of malignant GIST in a 54 years old female patient who came to the hospital with complaints of pain and mass in abdomen. Ultrasound revealed a mass originating from the duodenum. Provisional clinical diagnosis of duodenal carcinoma was considered preoperatively. Patient underwent complete surgical excision of the tumor. Histopathological examination confirmed the diagnosis of malignant GIST. As the recurrence rate for malignant GIST is high, patient was kept on follow up.

Aims: This in vitro study was designed to investigate the exogenous erosive potential of most commonly used pediatric syrup medicaments on primary teeth. Place and Duration of Study: Department of Pediatric Dentistry, Vishnu Dental College, Bhimavaram, India, 2012 to 2013. Methodology: The erosive potential of 17 syrups was assessed by measuring their inherent pH, titratable acidity, viscosity and ability to erode enamel. The inherent pH and titratable acidity of syrups were measured using digital pH meter and viscosity was determined using Brookfield viscometer. Enamel surface changes were evaluated using optical 3D profilometer. Results: Api –up (appetizer) showed the least pH and Azee (Azithromycin) exhibited the highest pH. The titratable acidity was greater for Api-up (5.40ml) and least for Tonoferron (0.20ml). Asthalin exhibited highest viscosity and Ondem showed least viscosity. All the test syrups caused some degree of enamel loss, however, Api – up caused the greatest and Azee caused the least degree of enamel loss at three time intervals. Conclusion: The medicated syrups with low inherent pH and high viscosity showed to cause a greater dissolution of enamel. However, certain syrups with low viscosity had shown higher degree of enamel loss due to their inherent acidic pH and those with high viscosity Showed lesser degree of enamel loss due to their basic pH. Hence, these results proved that inherent pH is the most critical factor for erosion.

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis. Despite advances in control strategies, inadequate treatment and failure to comply with drug regimens have resulted in TB to emerge as one of the most common and deadly infectious diseases worldwide. The emergence of drug-resistant TB has evolved as a formidable obstacle for comprehensive TB control. Drug-resistant TB can be classified as multi-drug-resistant TB, extensively drug-resistant TB and totally drug resistant TB (TDR-TB). There is a paucity in the development of new drugs against drug-resistant mycobacteria. The focus has shifted to the exploration of anti-mycobacterial properties of drugs approved for other indications. Thioridazine, a drug approved for use in schizophrenia is one such potential agent, which has shown anti-mycobacterial activity. There is evidence of anti-mycobacterial action of Thioridazine in in-vitro and mouse models. There is a compelling need for new anti-mycobacterial drugs that are more effective and have less toxicity. Further clinical trials are advocated favoring the use of thioridazine as an adjuvant in the treatment of TB, especially TDR-TB.

Snakebites are considered a neglected tropical disease that affects thousands of people worldwide. Although antivenom is the only treatment available, it is associated with several side effects. As an alternative, plants have been extensively studied in order to obtain an alternative treatment. In folk medicine, Azima tetracantha Lam. is usually used to treat snakebites. The present study aims to provide a scientific explanation for the use of this plant against snakebite. The extracts of shade dried leaves of A. tetracantha were tested for in vitro inhibitory activity on toxic venom enzymes like phosphomonoesterase, phosphodiesterase, acetylcholinesterase, hyaluronidase etc. from Bungarus caeruleus and Vipera russelli venoms.

Increasing interest is being envisaged by environmental researchers in controlling the phosphates pollution in natural waters by evoking the surface sorption characteristics of bio-waste materials of flora origin. In the present work, a thorough investigation has been made to find powders of leaves or their ashes of different classes of plants, having the affinity towards phosphate ions. 10 plant leaves and their ashes have been found to have sensitivity towards Phosphate ion. Investigations have been made in optimizing the various physico-chemical parameters such as equilibration time, pH and sorption dosage for the maximum removal of phosphates. The % of removal is found to be increasing with the increase in pH and equilibration time. The ashes of leaves shows more sorption towards phosphates than powders of leaves.79% to 90% removal of phosphate from polluted waters are found at optimum conditions with the sorbents investigated in this work. The interference of foreign ions on the % of extraction of phosphate reveals that some divalent cations are found be synergizing the extraction and thereby % of removal of Phosphate is increased even in the presence of 500 ppm of the said ions.

Snake venoms are rich in enzymes such as phospholipase A2, proteolytic enzymes, hyaluronidases and phosphodiesterases, which are well characterized. However, L-amino acid oxidase (LAO EC.1.4.3.2) from snake venoms has not been extensively studied. A novel L-amino acid oxidase from Bungarus caeruleus venom was purified to homogeneity using a combination of ion-exchange by DEAE-cellulose chromatography and gel filtration on Sephadex® G-100 column. The purified monomer of LAO showed a molecular mass of 55 ±1 kDa estimated by SDS-PAGE. The specific activity of purified LAO was 6,230 ± 178 U/min/mg, versus 230 ± 3.0 U/min/mg for the whole desiccated venom, suggesting a 27-fold purification with a 25% yield. Optimal pH and temperature for maximum purified enzyme activity were 6.5 and 37ºC, respectively. Platelet aggregation studies show that purified LAO inhibited ADP-induced platelet aggregation dose-dependently at 0.01 to 0.1 µM with 50% inhibitory concentration (IC50) of 0.04 µM, whereas at a 0.08 µM concentration it did not induce appreciable aggregation on normal platelet-rich plasma (PRP). The purified protein catalyzed oxidative deamination of L-amino acids while the most specific substrate was L-leucine. The purified LAO oxidizes only L-forms, but not D-forms of amino acids, to produce H2O2. The enzyme is important for the purification and determination of certain amino acids and for the preparation of α-keto acids.(AU)
