RESUMO

The traumatic C1-C2 unifacetal joint dislocation associated with oblique fractures of the C2 body and arch is a rare injury that can be effectively treated with gentle closed reduction under fluoroscopic guidance followed by halo vest immobilization. To our knowledge, however, there are only a few reported cases of this injury. Recognition of associated conditions including vertebral artery compromise, concomitant cervical spine fractures, and life-threatening injuries is paramount to the successful treatment of these patients. A 29-year-old patient, who presented a unifacetal dislocation at C1-C2 with associated oblique fractures of the C2 body and arch, is reported. Closed reduction with Gardner-Wells tong and halo vest application was performed. After 3 months of application of halo vest, the fractures of C2 body and arch had the complete union and the C1-2 dislocation had the good reduction. He had no pain and discomfort in his neck for about 2 years after halo vest removal.

RESUMO

Sturge Weber syndrome (SWS) is a rare, congenital neurocutaneous syndrome. We rarely experienced neurocutaneous disorders in forensic practice but there are unexpected sudden deaths of patients with SWS due to epilepsy, intracranial hemorrhage, thromboses and secondary accident. We introduce a case of drowning death probably due to an epileptic attack of a person diagnosed as a patient of SWS by an autopsy. We review some of the neurocutaneous syndromes and a discussion of autopsy approach is presented.

PURPOSE: We analyzed the results of treatment of dislocation of the extensor tendons over the metacarpophalangeal joints. MATERIALS AND METHODS: Thirteen patients who had received treatment for dislocation of the extensor tendons over the metacarpophalangeal joint were reviewed. Ten patients had traumatic dislocations, two had spontaneous dislocations, and one had congenital dislocation. The long finger was most frequently affected. The other fingers were affected almost equally. Displacement of the extensor tendon always occurred in the ulnar direction in the long and ring fingers. The index and little fingers exhibited different patterns of dislocation. RESULTS: Nonsurgical treatment was undertaken in two cases. Surgery was performed in 11 cases. No recurrent dislocations were reported in any of the patients. Conculsions: Based on our experience, patients seen within 2 weeks of injury initially should be treated with splinting of the involved metacarpophalangeal joint. Chronic dislocations should be treated with a primary repair of the defect in the sagittal band. When the sagittal band is absent or deficient, the tendon must be stabilized using a loop procedure with a tendon slip.

RESUMO

A statistical observation on the morphology of the normal I.V.P. of 500 cases in Korean young men was followed; 1. Age distribution was from 20 to 40 years old. The majority was 325 cases (65%) between 20 and 25 years of age. 2. Upper poles of the both kidneys were located at the T12 level and lower poles located at the L3 Level in supine position. 3. The average angle of every kidney to vertebral column in supine position was 18.3 degrees +/- 5.06 in right and 10.0 degrees +/- 5.01 in left side. 4. The average length of every kidney in supine position was 12.12+/-O.70l cm. in right side and 12.44 +/- 0. 664 cm. in left side. 5. The average width of every kidney in supine position was 6.37 +/- O.539 cm. in right side and 6.75 +/- 0.447 cm. in left side.

RESUMO

Extended pyelolithotomy was performed in five patients who had renal stones which were considered to be difficult to remove by ordinary pyelolithotomy. This operative technique was relatively simple and easy, and uneventful recovery followed in all cases.

RESUMO

A case of 23 years old boy having ureteral injury caused by accidental gunshot wound. He had successfully treated by side to side anastomosis of the ureter following removed the injuries side of the ureter under the splinting ureteral catheter and nephrostomy.

RESUMO

A case of congenital giant hydronephrosis with capacity of 3, 700cc associated with hypertension was reported. Nephrectomy was done by lumbar approach in lateral position. Removed kidney showed marked cystic dilatation and marked ureteropelvic junction stricture. Hypertension was disappeared 3 days after operation.

RESUMO

Authors present here in a calyceal diverticulum complicated by chronic pyelonephritis, which was diagnosed on urinalysis, IVP, RGP and symptoms. Partial nephrectomy was done and confirmation was made by specimen gross findings and biopsy. The microscopic findings are as follows: The lining epithelium of the inner surface of the cystically dilated calyx is made up of transitional epithelial cells. The epithelium in the areas is eroded. The subepithelial interstitial tissue is fibrotic and infilterated with large numbers of chronic inflammatory cells, which also invade the underlying renal parenchyma.
