RESUMO

Background: Thyroid nodules are commonly present in adult population but are rare in pediatric age group. Inspite of being rare, thyroid nodule have a higher chance of malignancy in children. Thus, pediatric patients presenting with thyroid nodule found clinically or incidentally should be worked up to rule out any possibility of malignancy. The besthesda system for reporting thyroid cytopathology (TBSRTC) is widely used for reporting in adults. The present study aims to use TBSRTC for pediatric thyroid lesions reporting.Methods: All pediatric patients with age ? 18 years presenting with thyroid nodule during January 2018 to April 2019 were included in the study, fine needle aspiration (FNA) was done and findings were compared with histology. Statistical analysis was done using SPSS version 18.Results: 42 pediatric patient were included in the study, out of which 2.38% were malignant and suspicious for nmalignant each and 83% were benign.Conclusion: TBSRTC is quite sensitive and specific reporting guideline in pediatric population as in adult population with 100% accuracy in diagnosing benign and malignant cases. This is useful for avoiding unnecessary surgeries.

Gout is an acute inflammatory disorder caused by deposition of monosodium urate crystals. Gouty tophus can be the initial presenting feature of chronic disease. Diagnosis of periarticular nodules can be a challenge for clinicians. Fine needle aspiration can provide quick, cost-effective and definitive diagnosis for the early management of these patients. Here, we present the case of gouty tophi of the right-hand index finger in a 45-years-old male leading to restricted finger movements

Background: The study was done in Obstetric & Gynaecology   Department of Dr. VRK Women's Medical college to prevent the preterm labor which one is better either Nifedipine or Isoxpurine hydrochloride. Methods: In one month period total 1884 patients  attended OPD of Obstetric & Gynaecology. Out of 416 patients came with preterm labor.200 patients chosen for tocolysis.100 patients were given Nifedipine and 100 patients Isoxpurine hydrochloride. Results: Preterm labor incidence was 22% while preterm delivery incidence was 20.9%. Nifedipine was more effective than Isoxpurine hydrochloride for preterm labor whether it is diagnosed either in early stage or late stage. In early-diagnosed preterm labor, Nifedipine had higher efficacy than Isoxpurine (P value 6.45 × 10−6) and also higher efficacy than its own in late diagnosed preterm labor (P value 2.08 × 10−5). Conclusions: In  India preterm labor incidence is very high. So Nifedipine is better option than Isoxpurine hydrochloride especially when patient is having early signs of preterm labor.

Background: In India, RUBELLA & CMV Antibodies screening is routinely carried out, but due to absence of clear intervention, its value is questionable. This retrospective study evaluated the usefulness of rubella & cmv antibody screening during pregnancy. Methods: Blood is collected from pregnant women & children, tested for rubella and CMV specific IgM Antibody by capture ELISA. The data were analysed to determine the incidence of rubella &CMV infection during pregnancy & in congenital infections. Results: In asymptomatic pregnancy females (n=252) rubella positivity was 1.58% and in woman with BOH (n=110) it was 3.66%, while CMV passivity was 2.9% in both asymptomatic and BOH women. In children (n=100) the overall positivity for rubella and CMV specific IgM antibodies was 7.5% and 12.5% respectively. The incidence of rubella and CMV infections in pregnant women & women with BOH has declined. In children, also the congenital rubella syndrome has declined but for the past 5 years the incidence of CMV infection remained same without any change. Conclusions: The incidence of rubella over the past 5 years can be reduced after giving the rubella vaccine to school going girls & women with reproductive age apart from routine vaccine. But CMV infection is a big problem due to unavailability of vaccine & safe treatment against CMV infection, so routine screening for rubella and CMV should be reserved for women with obstetric complications only.

Aims: To compare the apical extrusion of Enterococcus faecalis after instrumentation with three different Ni-Ti rotary instruments- An in vitro study. Settings and Design: In vitro study Methods and Material: Forty freshly extracted mandibular premolars were mounted in bacteria collection apparatus and root canals were contaminated with a suspension of Enterococcus faecalis. The contaminated teeth were divided into 4 groups of 10 teeth each according to rotary system used for instrumentation: Group1: Hyflex files, Group 2: GTX files, Group 3: Protaper files and Group 4: control group (no instrumentation). Bacteria extruded after preparations were collected into vials and microbiological samples were incubated in BHI broth for 24 hrs. The colony forming units were determined for each sample. Statistical Analysis Used: Statistical analysis was done using one way ANOVA followed by post hoc independent “t” test. Results: GTX files extruded least amount of bacteria followed by Hyflex files. Maximum extrusion of E. faecalis was seen in rotary Protaper group. Conclusion: Least amount of extrusion was seen with GTX files followed by Hyflex files and then rotary Protaper system.
