RESUMO

OBJECTIVE: To evaluate their roles in the pathophysiology of preterm premature rupture of membranes (PPROM), we checked interleukin (IL)-6, lipid peroxide, oxygen-radical absorbance capacity (ORAC) and antioxidant vitamin in the venous plasma and amniotic fluid of women with PPROM. METHODS: Venous plasma and amniotic fluid was taken from 20 normal pregnant women and 20 PPROM pregnancy women. IL-6 levels was determined by enzyme-linked immunosorbent assay. Lipid peroxide levels were measured by thiobarbituric acid reaction. The ORAC levels and the antioxidant levels were measured by Cao's method and by high performance liquid chromatography. RESULTS: The IL-6 levels in the venous plasma and amniotic fluid of PPROM pregnancy women were significantly higher than normal pregancy (P<0.01, P<0.01). The lipid peroxide levels in the venous plasma and amniotic fluid of PPROM pregnancy women were significantly higher than normal pregancy (P<0.01, P<0.01). The ORAC values in the venous plasma and amniotic fluid of PPROM pregnancy women were significantly higher than normal pregancy (P<0.01, P<0.01). The ORAC values/lipid peroxide levels in the venous plasma and amniotic fluid of PPROM pregnancy women were significantly higher than normal pregancy (P<0.01, P<0.01). The ascorbic acid levels in the venous plasma and amniotic fluid of PPROM pregnancy women were significantly higher than normal pregancy (P<0.05, P<0.01). CONCLUSION: This result suggest that the increased inflammatory bioactivity, increased lipid peroxidation and decreased antioxidant activity may be involved in the pathophysiology of PPROM. Low levels of ascorbic acid appears to be an important determinant of PPROM.

OBJECTIVE: Our purpose was to investigate lipid peroxide levels, total peroxyl radical-trapping antioxidative parameter (TRAP) values, and antioxidant vitamin levels in the maternal venous blood plasma of preterm premature rupture of membranes (PPROM) and to evaluate their roles of pathophysiology in PPROM. METHODS: Samples of maternal blood were obtained from women with normal pregnancy (n=23) and PPROM (n=20). Lipid peroxide levels of maternal venous blood plasma were measured by thiobarbituric acid reaction. The TRAP value of maternal venous blood plasma was measured by Wayner's method, although some reaction conditions were modified. Ascorbic acid and uric acid were measured by high performance liquid chromatography (HPLC) CoulArray detector of water-soluble antioxidants. Retinol, alpha-tocopherol, and gamma-tocopherol were measured by HPLC-CoulArray detector of fat-soluble vitamins RESULTS: Lipid peroxide levels in the maternal venous blood plasma of PPROM were significantly higher than that of normal pregnancy (3.87 +/- 0.24 vs. 3.04 +/- 0.24 nmol/mg protein, p<0.05). TRAP values in the maternal venous blood plasma of PPROM were significantly lower than that of normal pregnancy (0.33 +/- 0.02 vs. 0.40 +/- 0.02 mM, p<0.05). Ascorbic acid levels in the maternal venous blood plasma of PPROM were significantly lower than that of normal pregnancy (383.8 +/- 47.5 vs. 563.3 +/- 73.8 nmol/mg, p<0.05). There were no significant differences in maternal venous blood plasma uric acid, retinol, alpha-tocopherol, and gamma-tocopherol levels between two groups. CONCLUSION: This findings suggest that the imbalance of increased lipid peroxidation and decreased antioxidant activity in maternal blood may be involved in the pathophysiology of PPROM. Low levels of ascorbic acid in maternal blood appears to be an important determinant of PPROM.

RESUMO

Parencymal pulmonary endometriosis is a rare gynecologic problem, characterized by cyclic hemoptysis. Lung is the origin of hemoptysis. The diagnosis is based upon the clinical history and exclusion of other causes of recurrent hemoptysis, changes in the radiologic findings of chest lesion associated with menstrual cycle. We describe three women with recurrent hemoptysis during menstruation and localized lesion on computerized tomography (CT) scanning. The patients were treated with gonadotropin releasing hormone (GnRH) agonist or surgery. We represent three cases with a brief review of literature.

RESUMO

OBJECTIVE: This study evaluated the possible role of 2 additional tumor markers to CA125 in discriminating between benign and malignant ovarian tumors. METHODS: Serum samples from 1,346 patients were obtained on seven days before operation. All patients underwent surgery for ovarian tumors. Serum levels of 3 tumor markers were compared to histology. Concentrations of tumor markers (CA125, CA72-4, CA19-9) were detected by enzyme immuno- or immunoradiometric assays. Normal range of these markers was defined as CA125

RESUMO

Malignant fibrous histiocytoma (MFH) is a common sarcoma of the soft tissue, usually occurring in the extremities and less commonly in the retroperitoneal space, the abdominal cavity, or other site such as the skin or the head and neck resion. However, MFH of the liver is quite a rare neoplasm. This tumor has five distinct histological subtypes: storiform pleomorphic, myxoid, giant cells, inflammatory and angiomatoid. Recently, we experienced a case of primary MFH in a surgically removed liver. This case is reported with a review of the literature.

RESUMO

Primary hepatic leiomyosarcoma is a rare tumor, most frequently occurring in liver parenchyma. We recentlyencountered an exophytic hepatic leiomyosarcoma ; CT scans indicated an indistinct border, with the parenchyme ofthe liver and parenchymal beaking suggesting a primary hepatic mass. We present an unusual case of primaryleiomyosarcoma which showed exophytic growth.

RESUMO

PURPOSE: There are several kinds of classificatian dealing with the staging of the gastric adenocarcinoma. However, such different staging systems pose difficulty in the inter- institutional or intemational comparison of the disease status and the treatment results. The purpose of this study is to evaluate each staging system and to assess the usefulness of the new UICC-TNM staging system (1997) for gastric adenocarcinoma. MATERIAL AND METHODS: We retrospectively analysed 473 cases of gastric adenocarcinoma who were operated at the Department of General Surgery, Hanyang University Hospital during the period from 1992 to 1996. Using these cases, we analyzed their cumulative 5-year survival rate according to three kinds of staging systems; old UICC-TNM staging system (1987), new UICC-TNM staging system (1997), and the Japanese staging system for gastric carcinoma (1993). RESULTS: The follow up rate was 94.3% and the median follow up period was 30.3 months. All of these three systems showed a statistically significant survival difference according to their different classifications. When the distribution of stage between old and new UICC-TNM staging system was compared, 95 cases (20.1%) were subjected to stage shifting, which involves 12.1% of up-staging and 8.4% of down-staging. Stage shifting was most prominent in stage IIIb (68.8%). The cumulative 5-year survival rate according to the new UICC-TNM staging system was 99.1% in stage Ia, 81.4% in stage Ib, 75.2% in stage II, 45.9% in stage IIIa, 21.0% in stage IIIb, and 19.4% in stage IV. CONCLUSION: We conclude that the new UICC-TNM staging system is simple, practically convenient, and highly reproducible, and it showed a statistically significant survival difference according to their staging classification.

RESUMO

We report a rare case of carcinosarcoma arising in the stomach. The tumor is presented in the posterior wall of the gastric lower body and antrum of a 56-year-old man. Grossly the tumor has polypoid appearance with diffuse surface ulceration and measures 5.5 cm in diameter. The tumor is accompanied with three separate well differentiated early gastric adenocarcinomas (two EGC type IIc & IIa). Microscopically, the tumor consists of moderately well differentiated adenocarcinoma in the periphery, and lobulated sarcomatous areas in the center, which shows chondroid differentiation. Transitional areas between adenocarcinoma and chondrosarcoma are evident. Immunohistochemical studies show positivity for cytokeratin and carcinoembryonic antigen in the epithelial component, and for vimentin and S-100 in the sarcomatous component. The transitional areas are positive in carcinoembryonic antigen, vimentin, S-100, and cytokeratin. The tumor extended to the subserosa and showed metastasis of only adenocarcinomatous component in six out of 47 dissected perigastric lymph nodes.

RESUMO

Neurilemmoma is the most frequent solitary tumor of the peripheral nerves. It may occur anywhere in the body, but their occurrence in the penis is rare and unusual. Treatment of this lesion is simple surgical excision. We report a case of neurilemmoma of the penis in a 27 year-old male and review the literature.
