RESUMO

PURPOSE: To compare the image quality and ligament traceability in ankle images obtained using Volume Isotropic Turbo Spin Echo Acquisition (VISTA) MRI with and without fat suppression. MATERIALS AND METHODS: The signal-to-noise ratios (SNRs) in images from a phantom and from the ankle of a volunteer were compared. Ten ankles from 10 non-symptomatic volunteers were imaged for comparisons of contrast ratio (CR) and ligament traceability. All examinations were performed using VISTA sequences with and without fat suppression on a 3T MRI scanner. The SNRs were obtained from images with subjects and without subjects (noise-only). Contrast ratios from images of the 10 ankles were acquired between fluid and tendon (F-T), F-cartilage (C), F-ligament (L), fat (f)-T, f-C and f-L. Two musculoskeletal radiologists independently scored the traceability of 7 ligaments, in sagittal, axial and coronal images respectively, based on a 4-point scale (1 as not traceable through 4 as clearly traceable). The Wilcoxon signed-rank test was used to compare the CR. Fisher's exact test and Pearson's chi-squared test were used to compare the ligament traceability. RESULTS: The SNRs did not differ significantly between the two sequences except in bone marrow. VISTA SPAIR showed the higher CR only in F-T (p = 0.04), whereas VISTA showed higher CR in f-T (p = 0.005), f-C (p = 0.005) and f-L (p = 0.005). The calcaneofibular ligament traceability with VISTA was superior to that obtained with VISTA SPAIR (p < 0.05) in all planes. CONCLUSION: VISTA showed significant superiority to VISTA SPAIR in tracing CFL due to the superior CR between fat and ligament.

RESUMO

Some reports provide conclusive evidence of close interactive regulation between the taste receptor and sympathetic nervous system. We report a middle-aged male patient with gustatory change after cervical sympathetic ganglion block (CSGB) who had been suffering from hypersensitivity to sour taste since developing complex regional pain syndrome (CRPS) type 1, diagnosed according to the revised CRPS criteria. Despite receiving two high doses of prednisolone therapy, he experienced the recurrence of CRPS symptoms. We attempted other therapy treatments, including pamidronate intravenous infusion, non-steroidal anti-inflammatory drugs, opioids, tricyclic antidepressants, and CSGB. Following each CSGB administration, the patient reported decreased hypersensitivity to sour-tasting foods, such as kimchi and oranges, with decreased pain and reduction of dysautonomic symptoms. This case demonstrates that overactivation of the sympathetic nervous system may influence sensitivity and regulation of gustatory receptors; therefore, a patient demonstrating CRPS symptoms, including taste alterations, may respond positively to CSGB therapy.

RESUMO

PURPOSE: To compare the relative values of various fast breath-hold imaging sequences for superparamagnetic iron-oxide (SPIO)-enhanced hepatic MRI for the assessment of solid focal lesions with a 3T MRI unit. MATERIALS AND METHODS: 102 consecutive patients with one or more solid malignant hepatic lesions were evaluated by spoiled gradient echo (GRE) sequences with three different echo times (2.4 msec [GRE_2.4], 5.8 msec [GRE_5.8], and 10 msec [GRE_10]) for T2*-weighted imaging in addition to T2-weighted turbo spin echo (TSE) sequence following intravenous SPIO injection. Image qualities of the hepatic contour, vascular landmarks and artifacts were rated by two independent readers using a four-point scale. For quantitative analysis, contrast-to-noise ratio (CNR) was measured in 170 solid focal lesions larger than 1 cm (107 hepatocellular carcinomas, nine cholangiocarcinomas and 54 metastases). RESULTS: GRE_5.8 showed the highest mean points for hepatic contour, vascular anatomy and imaging artifact presence among all of the subjected sequences (p<0.001) and was comparable (p=0.414) with GRE_10 with regard to lesion conspicuity. The mean CNRs were significantly higher (p<0.001) in the following order: GRE_10 (24.4+/-14.5), GRE_5.8 (14.8+/-9.4), TSE (9.7+/-6.3), and GRE_2.4 (7.9+/-6.4). The mean CNRs of CCCs and metastases were higher than those of HCCs for all imaging sequences (p<0.05). CONCLUSION: Regarding overall performances, GRE using a moderate echo time of 5.8 msec can provide the most reliable data among the various fast breath-hold SPIO-enhanced hepatic MRI sequences at 3T unit despite the lower CNR of GRE_5.8 compared to that of GRE_10.

RESUMO

Clear cell hidradenoma is an uncommon benign skin appendageal tumor that typically involves the dermal layer of the head, face, and extremities. The breast is a rare site for this lesion, with only two documented cases, which were determined based on mammogram and sonogram findings. We present a case of clear cell hidradenoma of the axillary tail with radiological findings and a literature review.

OBJECTIVE: To evaluate the quality of life and psychologic status in parents of children with cerebral palsy (CP). METHOD: We studied 94 parents of children with cerebral palsy (case) and 60 parents of normal children (control). The functional level of CP was determined based on the gross motor functional classification system (GMFCS), and type of CP was classified by clinical features. We collected the data through questionnaires obtained from the parents, which consisted of Short Form Health Survey-36 (SF-36), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) and Family APGAR score. The data were statistically analyzed. RESULTS: The mean scores of SF-36 in parents of CP children was significantly lower than controls (60.09<67.38, p< 0.001). BDI (p<0.001) and BAI (p=0.002) scores of parents of CP children were significantly higher than control group, and familial APGAR score was lowered in the CP group compared to the control group. There were no differences in the scores of SF-36 in parents according to the severity of CP (p=0.844). CONCLUSION: The quality of life of parents of children with CP was significantly lower than that of control parents. Also psychologic status and familial function were poorer in parents of CP. It is necessary to evaluate and support for parent's psychologic status and quality of life in the comprehensive rehabilitation of CP.

Vascular tumors of the breast are a rare disease. A hemangioma in the male breast is an extremely rare benign disease. We report a case of a 53-year-old-man with a capillary hemangioma of the breast.
