RESUMO

PURPOSE: The purpose of this study is to identify the relationship among clinical competence of diabetic diet education on standardized patients, self-efficacy, communication skill, learning satisfaction, and professional values of nursing students. METHODS: The present study was a descriptive research. The participants were 144 junior nursing students in a university. The clinical competence on diabetic diet education was assessed by trained evaluators when nursing students performed diabetic diet education using the standardized patients. The students' self-efficacy, communication skill, professional values, and learning satisfaction were measured by themselves using the questionnaires. RESULTS: The clinical competence on diabetic diet education showed positive correlations with communication skill and learning satisfaction, while the competence was not correlated to self-efficacy and professional values. CONCLUSION: The results of this study suggest that clinical competence on diabetic diet education may be more related to communication skills and learning satisfaction than self-efficacy and professional values. Promoting communication skills and learning satisfaction using a standardized patient may increase nursing students' competence on diabetic diet education.

RESUMO

PURPOSE: In this study differences in levels of performance and importance of nursing jobs were explored in relation to levels of work experience. METHOD: The sample was chosen by convenient sampling (N=363) and a cross-sectional study design was used. Frequencies and percentages were used for descriptive statistics. ANOVA was utilized to examine differences in performance and importance of nursing jobs among three groups of nurses, divided based on levels of work experience (less than 1 year, 1 to 4 years, and 5 or more years). RESULTS: Regarding performance of nursing jobs, novice nurses provided more support for patients, but provided less assessment of patients (i.e., investigation of levels of nutrition and exercise, and observation of verbal, and non-verbal behavior, and levels of cognition). Regarding the importance of nursing jobs, nursing linked to exercise and activities, and post-operative and excretion care were considered to be less important among novice nurses than among experienced nurses. CONCLUSION: Different training programs should be developed and provided to nurses according to levels of work experience. By doing so, it will be feasible to minimize differences in quality of nursing care triggered by different levels of work experience.

PURPOSE: The purpose of this study was to examine the effects of Danjeon Breathing Exercise (DBE) on vital capacity, physical fitness, anxiety and depression among older adults. METHODS: This study employed the nonequivalent control group pretest-posttest design. The experimental group participated in a 12-week DBE program. Vital capacity and physical fitness were measured with a health measurement system (HELMAS). Anxiety and depression were measured with SCL-90-R-K. Data were collected from 37 community-dwelling older adults (experimental group=21, control group=16) in the Seoul metropolitan area. The data were analyzed by chi2-test, Mann-Whitney test and ANCOVA. RESULTS: The experimental group taking DBE reported significant increases in flexibility than the control group, but differences in vital capacity and balance were not significant. Older adults taking DBE showed significant decreases in the anxiety and depression levels. CONCLUSION: A DBE program may be a useful nursing intervention for older adults to improve their flexibility and to reduce anxiety and depression.

PURPOSE: This study was to develop the clinical competency evaluation, and to examine the effects of the developed evaluation by comparing it with existing evaluation on clinical competency, communication skill, and self-efficacy of nursing students. METHODS: Design was a randomized control group post-test design. The subjects were 102 senior nursing students(Experimental group: 48, Control group: 54) at K university in seoul, Korea. The experimental group took the clinical competency evaluation using standardized patients and the control group took the existing evaluation using a doll. The clinical competency and communication skills were measured by evaluators, and self-efficacy was self reported by the nursing students. RESULTS: The experimental group had higher scores in clinical competency, communication skills, and self-efficacy than those in the control group. CONCLUSION: Through these results, practice education of nursing education programs need to activate the clinical competency evaluation using standardized patients.

PURPOSE: The purpose of this study was to develop a multimedia learning program for patients with diabetes mellitus (DM) diet education using standardized patients and to examine the effects of the program on educational skills, communication skills, DM diet knowledge and learning satisfaction. METHODS: The study employed a randomized control posttest non-synchronized design. The participants were 108 third year nursing students (52 experimental group, 56 control group) at K university in Seoul, Korea. The experimental group had regular lectures and the multimedia learning program for DM diet education using standardized patients while the control group had regular lectures only. The DM educational skills were measured by trained research assistants. RESULTS: The students who received the multimedia learning program scored higher for DM diet educational skills, communication skills and DM diet knowledge compared to the control group. Learning satisfaction of the experimental group was higher than the control group, but statistically insignificant. CONCLUSION: Clinical competency was improved for students receiving the multimedia learning program for DM diet education using standardized patients, but there was no statistically significant effect on learning satisfaction. In the nursing education system there is a need to develop and apply more multimedia materials for education and to use standardized patients effectively.

PURPOSE: The purpose of this study was to test the effects of tailored diabetic education on blood glucose control and self-care for patients with type 2 diabetes on insulin therapy. METHODS: The participants were 60 patients (experimental group: 30, control group: 30) with type 2 diabetes on insulin therapy. The patients were being seen at a university hospital in Seoul, Korea. Group diabetic education and tailored diabetic education were given to the experiment group while group diabetic education only was given to the control group. Data were collected before and three months after the education. chi-square test, t-test, and ANCOVA were used to analyze the data. RESULTS: No significant differences in postprandial (PP2hrs) glucose and HbA1c levels were found between the two groups. Participants in the experiment group showed statistically significant differences in the area of self-glucose test, management of insulin injection, and life style change compared to those in the control group. CONCLUSION: The results indicate that tailored education for patients with diabetes on insulin therapy improve self-glucose test, management of insulin injection, and life style. Therefore it is suggested that tailored education can be applied in diabetic education to improve self-care.

PURPOSE: The purpose of this study was to examine the effect of topical lidocaine cream on pain and anxiety during the AV fistula puncture among hemodialysis patients. METHODS: The study employed one group repeated measurement design. The data were collected from 50 hemodialysis patients who received AV fistula puncture. The topical lidocaine cream was applied 30 minutes before the puncture. The data were measured total 3 times (T1=without lidocaine, T2=2% lidocaine, T3=5% lidocaine). Pain was measured by VAS and a behavioral checklist. Anxiety was measured by Korean manual of SCL-90-R. RESULTS: Patients with 5% lidocaine cream reported significantly lower of VAS pain score than those with 2% lidocaine and without lidocaine. Patients with 2% lidocaine cream reported significantly lower of behavioral pain scores than those without lidocaine, but less effective than 5% lidocaine cream. Patients with 2% lidocaine cream reported significantly lower of anxiety scores than those without lidocaine, but less effective than 5% lidocaine cream. CONCLUSION: Topical application of lidocaine cream for 30 minutes before AV fistula puncture significantly decreased pain and anxiety among hemodialysis patients. Specifically 5% lidocaine was more effective than 2% lidocaine for both pain and anxiety.

PURPOSE: The aim of this study was to design and evaluate a scale for measuring reliability and validity in the field of oriental nursing education. METHOD: A questionnaire was administered to 495 university nursing students using a convenience sampling method. The period of time for collecting data was from September 2003 to January 2004. RESULT: The derived outcome tool consisted of 6 factors and 22 inquires on the basis of a conceptual frame of three domains (knowledge, attitude, and skill), As a result of the item analysis, 22 items were selected and the internal consistency alpha coefficient was .767. The value of Cronbach' Alpha of knowledge(factor 1) was .885, attitude(factor2)was .756, and skill (factor 3) was .610. The three factors accounted for 65.110% of the variance in the total scale. Addressing the explanatory variance of each domain Cognitive domain was 22.477%, affective domain was 20.543%, and psychomotor domain was 17.090%. CONCLUSION: Further studies need to be done to verify educational evaluation and apply our outcomes to oriental nursing education.

PURPOSE: The purpose of this study was to develop and to evaluate a health promotion program for elderly. METHOD: Subjects were 63 elderly women (experimental group:33, control group:30). The study was a nonequivalant control group pretest-posttest design. The data was analyzed with an SPSS Window program, then the data was computed for the purpose of each study. RESULT: 1. In designing the program, the experimental group was given health education - 2 times per week, for 8 weeks - and they participated in recreation programs and stretching exercises five times a week. 2. Stretching exercises including health education, significantly affected the total Cholesterol, HDL-cholesterol, triglycerides, health behavior and self-efficacy. 3. The body fat weight was not significantly affected by the stretching exercises. CONCLUSION: This program was tested to promote the health of elderly and verified as an effective nursing intervention program, because the outcome of this program ascertains that this program enhances self efficacy of exercise, reduces Cholesterol and triglyceride levels' increases HDL-cholesterol, and helps promote the understanding of heath behavior.

PURPOSE: This study was to examine the effects of the Dan-Jun Breathing Exercise Program on pulmonary function and psychological health promotion of women in midlife. METHOD: Experimental group(20) was matched to control group(20) according to age, education, religion and marital status. The Dan-Jun Breathing Exercise Program was carried out for 80 minutes a day, 3 times a week for 12 weeks. FVC and FEV1.0/FVC(%) were measured by using the Health Management System developed by the Korean Physical Science Institution. The scores of depression, anxiety and hostility were measured by the Korean Manual of Symptoms-Checklist -90 revision. RESULT: 1) FVC of the experimental group was higher than that of the control group, FEV1.0/FVC(%) was not higher than that of the control group. 2) The scores for depression, anxiety and hostility in the experimental group were lower than those of the control group. 3) FVC of 6 weeks and 12 weeks experiment in the experimental group was higher than that of pre-experimental group. CONCLUSION: The Dan-Jun Breathing Exercise Program promotes the Pulmonary function and psychological health of women in midlife.

PURPOSE: This study focused on affirming of the effects of Dan-jun breathing exercise experienced by women in midlife. METHOD: The data were collected via direct interview from 20 participants, who participated in a Dan-Jun exercise program during three months. Collected data were analyzed by content analysis. RESULT: 1. As for the health problems of women in midlife before the Dan-jun exercise program, 93 items were extracted in the content analysis. These items were categorized into 59 attributes and 15 higher attributes. 1) Of the 59 attributes of health problems, shoulder pain (30%) was the most dominant. 2) The 15 higher attributes consisted for the physical domain of, weakness, pain, joint stiffness, sexual problem, sleep disorder, gastro-intestinal disorder, menstrual irregularity, circulatory disorder, respiratory disorder, constipation, urinary incontinence, and for the psychological domain, anger, emptiness, depression, and anxiety. 2. As for the effects experienced by women in midlife after the Dan-jun exercise program, 169 items were extracted in the content analysis. These items were categorized into 85 attributes and 14 higher attributes. 1) Of 85 attributes, lightening of physical condition (55%) was the most dominant. 2) The higher attributes consisted in physical domain of, recovery of vigor, pain relief, menstrual regularity and improvements in flexibility, sexual problem, sleep disorder, gastro- intestinal disorder, circulatory disorder, constipation, respiratory disorder and urinary incontinence, and for the psychological domain, relaxation, cultured mind and self-confidence. CONCLUSIONS: Dan-jun breathing exercise program for three months showed positive effects on physical and psychological health in women in midlife. In this context, it can be also considered as a significant nursing intervention for maintenance and promotion of the health of these women.

RESUMO

The purpose of this study was to develop a DanJeon Breathing Exercise Program for health promotion and to examine the effects of a DanJeon Breathing Exercise Program on the physical and emotional health promotion of women in midlife. The design utilized for this study was quasi-experimental with a nonequivalent control group pretest-posttest design. This study was done between February 1 and July 4, 2000 and the subjects of the study were 40 women in midlife living in Seoul. There were 20 members in experimental group who participated in the DanJeon Breathing Exercise Program and 20 members in the control group who were chosen as matched to members of the experimental group according to age, education and religion. The DanJeon Breathing Exercise Program was carried out for 80 minutes a day, 3 times a week for 12 weeks. The percent of body fat, back strength, flexibility (trunk flexion), balance and vital capacity were measured using the Health Management System developed by the Korea Physical Science Institution. Blood tests were done at D infirmary for total cholesterol, and the atherogenic index was calculated using an established formula. The scores of anxiety and depression were measured by a questionnaire with 10 questions on anxiety and 13 questions on depression. It was developed from the Korean Manual of Symptoms-Checklist-90 revision. The collected data were processed using the SPSS PC program and analyzed using X2-test and t-test. The result of this study are as follows: 1. The percent of body fat for the experimental group who participated in the DanJeon Breathing Exercise Program was lower than that of the control group. 2. The degree of back strength, flexibility (trunk flexion) and balance of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 3. The vital capacity of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 4. The total cholesterol level and atherogenic index of the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. 5. The scores for anxiety and depression in the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. In conclusion, DanJeon Breathing Exercise Program promotes the physical and emotional health of women in midlife. The DanJeon Breathing Exercise Program can be utilized as a nursing intervention for the promotion of health in women in midlife.

The purpose of this study was to investigate the effect of a hand massage program on anxiety, vital sign and pain in clients with ischemic heart disease. The design utilized for this study was quasi-experimental with a nonequivalent control group non-synchronized design The subjects were fifty-four patients, twenty-eight for the experimental and twenty-six for the control group, who were admitted with ischemic heart diease at a cardiac intensive care unit in K medical center of K university. This study was carried out from May, 1999 to March, 2000. The level of anxiety and pain measured by Visual Analogue Scale, systolic blood pressure, diastolic blood pressure and pulse rate were measured before and after hand massage, the state of Anxiety was measured by the Spielberger (1970) scale at admission and after hand massage for three days. The collected data were processed by using the SPSS PC program and analyzed using X2 -test and t-test. The result of this study are as follows: 1. The scores of VAS anxiety and State of anxiety of the experimental group were lower than those of the control group. 2. The degree of systolic blood pressure, diastolic blood pressure and pulse rate of the experimental group were lower than that of the control group. 3. The score of VAS pain of the experimental group was lower than that of the control group. The results suggested that hand massage can decrease VAS anxiety, State of anxiety, vital signs and VAS pain of patients who were admitted with ischemic heart disease at cardiac intensive care unit Therefore, it is proposed that hand massage is an appropriate nursing intervention to relieve anxiety of the patients who were admitted with ischemic heart disease at a cardiac intensive care unit.

The purpose of this study is to compare the physical health, anxiety and depression of the Dan-Jeon Breathing trained group with those of a non-trained group and to analyze the relationship between the training period of Dan-Jeon Bre athing method and physical health, anxiety, and depression. The number of people in the Dan-Jeon Breathing trained group over 3 months was 149 and in the non-trained group 142. Data were collected from August to October 1999. As tools, 35 physical symptom questions were chosen from The Cornell Medical Index modified by Nam(1965) and from the Symptom Checklist-90-Revision(SCL-90-R) by Kim(1984); 10 questions on anxiety and 13 questions on depression were used in the study. The statistics of the study were gathered by using SPSS Window; the analysis was made by applying x(2)-test, t-test, Pearson correlation, ANOVA and Scheffe tests. The results were as follows. 1. The Dan-Jeon Breathing trained group had lower physical symptom score and showed less anxiety and depression than the non-trained group, which supported the 1st, 2nd, 3rd hypotheses. 2. The longer the training period of Dan-Jeon Breathing, the lower the physical symptom score, which supports the 4th hypothesis, but the 5th and, 6th hypotheses were not supported because anxiety and depression did not get lower as the training period of Dan-Jeon Breathing got longer. 3. The physical symptom score, anxiety and depression that were made by the general characteristics of Dan-Jeon Breathing trained group were analyzed. As a result, the physical symptom score of women was greater than that of men. Physical symptoms score and the rate of anxiety and depression were different by education levels. The degree of anxiety was different by age. Those who have a religion have a higher physical symtoms score than people who have no religion. There was no difference in physical health score, anxiety and depression according to marital status, economic status and occupations. The results suggest that the physical and mental health status of Dan-Jeon Breathing trained group is better than that of the non-trained group. Physical symptoms scores gets lower as the training period of Dan-Jeon Breathing gets longer which results in the improvement of physical health status.

This study was performed to analyse the tendency of the self-care studies, to investigate the variables related to self-care, which could be applied for the data establishing the theory of self-care. to accomplish the objectives of this study, the researchers analyzed 83 studies, available that contained the area of self-care, which was published in local and foreign areas between 1981 and Feb. 1997. The findings of this study are as follows: 1) Studies published in Korea were about twice more than foreign studies. According to the statistics, the number of studies in this study tended to increase year by year. Between 1991 and 1995 were most. Analysis on the subject of this studies showed, that they were most dealt with chronic diseases an adults. 2) The theoretical definition of self-care were most referred to Orem's self-care. For the main concept of studies, self care performance was the most. 3) The correlational study was most often used design and experimental study is tended to increase. The questionnaires were the most often used data collection method. There was lots of different variable measuring technique to evaluate main concepts. 4) In the results of analysis on the propositions related to the self-care performance, a client's education, social support, self-efficacy, and level of knowledge as a causal variable were found. Also, the physiological index improved and quality of life were fond to be significant effective variables.

The result of studying papers about management of lower back pain published in domestic and foreign nursing and medical magazines in these 10 years is as follows; 1. General characteristic of lower back pain; 1) In sex distribution, there were more men than women in 5 papers and more women than men in 4 papers among 9 papers surveryed. 2) In age distribution, thirties to forties of patients had more lower back pain as surveyed by general hospitals and fifties to sixties of patients had more lower back pain as surveyed by oriental medical hospitals. 3) In cause factor, there were 50 to 65% of sprain, 32 to 44% of herniated intervertebral disc and 13 to 29% of degenerative changes. 4) In symtom distribution, there were 26 to 57% of lower back pain, 42 to 65% of lower back pain with radiating pain and 34 to 99% of paravertevral muscle spasm. 5) In period of pain management distribution, 18 to 40% of patients experienced pain for Jess than 6 months and 59 to 82% of them experienced pain for more than 6 months in 3 papers among 4 papers. 6) In surveying the treatment, 66 to 88% of patients had conservative treatment and there were treatments of general hospital, oriental medicine, self remedy and traditional practice in conservative treatment. 7) In job distribution, 12 to 50% of them were housekeepers, 23 to 31% office workers, 4.6 to 36% blue color workers and 11 to 15% students. 2. As psychological character lower back pain paients had anxiety, depression, anger-hostility, phobic anxiety, neurasthenia, hypochondriasis, and interpersonal sensitivity. 3. To distinguish the cause of lower back pain, plain lumbar roentgenogram, straight leg rasing test, eletromyelogic findings, somatosensory evoked potentials CT and MRI were performed. 4. To relieve lower back pain, epidural adhesiolysis, epidulal injection of local anesthetic in mixture with steroid, lumbar spinal root block, low level laser therapy, acupuncture like transcutaneous nerve stimulation(ALTENS), topical capsaicin and lumbar orthotics were used in medical field, and relaxation technique was used in nursing field. 5.
