Abstract Background Increased malignancy frequency is well documented in adult-systemic lupus erythematosus (SLE), but with limited reports in childhood-onset SLE (cSLE) series. We explored the frequency of malignancy associated with cSLE, describing clinical and demographic characteristics, disease activity and cumulative damage, by the time of malignancy diagnosis. Method A retrospective case-notes review, in a nationwide cohort from 27 Pediatric Rheumatology centres, with descriptive biopsy-proven malignancy, disease activity/damage accrual, and immunosuppressive treatment were compiled in each participating centre, using a standard protocol. Results Of the 1757 cSLE cases in the updated cohort, 12 (0.7%) developed malignancy with median time 10 years after cSLE diagnosis. There were 91% females, median age at cSLE diagnosis 12 years, median age at malignancy diagnosis 23 years. Of all diagnosed malignancies, 11 were single-site, and a single case with concomitant multiple sites; four had haematological (0.22%) and 8 solid malignancy (0.45%). Median (min-max) SLEDAI-2 K scores were 9 (0-38), median (min-max) SLICC/ACR-DI (SDI) score were 1 (1-5) Histopathology defined 1 Hodgkin's lymphoma, 2 non-Hodgkin's lymphoma, 1 acute lymphoblastic leukaemia; 4 gastrointestinal carcinoma, 1 squamous cell carcinoma of the tongue and 1 anal carcinoma; 1 had sigmoid adenocarcinoma and 1 stomach carcinoid; 3 had genital malignancy, being 1 vulvae, 1 cervix and 1 vulvae and cervix carcinomas; 1 had central nervous system oligodendroglioma; and 1 testicle germ cell teratoma. Conclusion Estimated malignancy frequency of 0.7% was reported during cSLE follow up in a multicentric series. Median disease activity and cumulative damage scores, by the time of malignancy diagnoses, were high; considering that reported in adult series.

ABSTRACT Leishmania infantum is a protozoan that causes visceral leishmaniasis (VL) in the Americas and some regions of Europe. The disease is mainly characterized by hepatosplenomegaly and fever, and can be fatal. Factors related to the host and parasite can contribute to the transmission of Leishmania and the clinical outcome. The intraspecific genetic variability of L. infantum strains may be one of these factors. In this study, we evaluated the genetic variability of L. infantum obtained from bone marrow smear slides from patients in the Sao Paulo State, Brazil. For this, the minicircle of the kDNA hypervariable region was used as target by Sanger sequencing. By analyzing the similarity of the nucleotides and the maximum likelihood tree (Fasttree), we observed a high similarity (98%) among samples. Moreover, we identified four different profiles of L. infantum. In conclusion, L. infantum strains from Sao Paulo State, Brazil, showed low diversity measured by minicircle of the kDNA hypervariable region.

Abstract Introduction Seasonal influenza A (H3N2) virus is an important cause of morbidity and mortality in the last 50 years in population that is greater than the impact of H1N1. Data assessing immunogenicity and safety of this virus component in juvenile systemic lupus erythematosus (JSLE) is lacking in the literature. Objective To evaluate short-term immunogenicity and safety of influenza A/Singapore (H3N2) vaccine in JSLE. Methods 24 consecutive JSLE patients and 29 healthy controls (HC) were vaccinated with influenza A/Singapore/ INFIMH-16-0019/2016(H3N2)-like virus. Influenza A (H3N2) seroprotection (SP), seroconversion (SC), geometric mean titers (GMT), factor increase in GMT (FI-GMT) titers were assessed before and 4 weeks post-vaccination. Disease activity, therapies and adverse events (AE) were also evaluated. Results JSLE patients and controls were comparable in current age [14.5 (10.1-18.3) vs. 14 (9-18.4) years, p = 0.448] and female sex [21 (87.5%) vs. 19 (65.5%), p = 0.108]. Before vaccination, JSLE and HC had comparable SP rates [22 (91.7%) vs. 25 (86.2%), p = 0.678] and GMT titers [102.3 (95% CI 75.0-139.4) vs. 109.6 (95% CI 68.2-176.2), p = 0.231]. At D30, JSLE and HC had similar immune response, since no differences were observed in SP [24 (100%) vs. 28 (96.6%), p = 1.000)], SC [4 (16.7%) vs. 9 (31.0%), p = 0.338), GMT [162.3 (132.9-198.3) vs. 208.1 (150.5-287.8), p = 0.143] and factor increase in GMT [1.6 (1.2-2.1) vs. 1.9 (1.4-2.5), p = 0.574]. SLEDAI-2K scores [2 (0-17) vs. 2 (0-17), p = 0.765] and therapies remained stable throughout the study. Further analysis of possible factors influencing vaccine immune response among JSLE patients demonstrated similar GMT between patients with SLEDAI < 4 compared to SLEDAI ≥ 4 ( p = 0.713), as well as between patients with and without current use of prednisone ( p = 0.420), azathioprine ( p = 1.0), mycophenolate mofetil ( p = 0.185), and methotrexate ( p = 0.095). No serious AE were reported in both groups and most of them were asymptomatic (58.3% vs. 44.8%, p = 0.958). Local and systemic AE were alike in both groups ( p > 0.05). Conclusion This is the first study that identified adequate immune protection against H3N2-influenza strain with additional vaccine-induced increment of immune response and an adequate safety profile in JSLE. Abstract Objective: To assess factors associated with emotional changes and Hyperactivity/Inattention (HI) motivated by COVID-19 quarantine in adolescents with immunocompromising diseases. Methods: A cross-sectional study included 343 adolescents with immunocompromising diseases and 108 healthy adolescents. Online questionnaires were answered including socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and validated surveys: Strengths and Difficulties Questionnaire (SDQ), Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0). Results: The frequencies of abnormal emotional SDQ scores from adolescents with chronic diseases were similar to those of healthy subjects (110/343 [32%] vs. 38/108 [35%], p = 0.548), as well as abnormal hyperactivity/inattention SDQ scores (79/343 [23%] vs. 29/108 [27%], p = 0.417). Logistic regression analysis of independent variables associated with abnormal emotional scores from adolescents with chronic diseases showed: female sex (Odds Ratio [OR = 3.76]; 95% Confidence Interval (95% CI) 2.00-7.05; p < 0.001), poor sleep quality (OR = 2.05; 95% CI 1.08-3.88; p = 0.028) and intrafamilial violence during pandemic (OR = 2.17; 95% CI 1.12-4.19; p = 0.021) as independently associated with abnormal emotional scores, whereas total PedsQL score was inversely associated with abnormal emotional scores (OR = 0.95; 95% CI 0.93-0.96; p < 0.0001). Logistic regression analysis associated with abnormal HI scores from patients evidenced that total PedsQL score (OR = 0.97; 95% CI 0.95-0.99; p = 0.010], changes in medical appointments during the pandemic (OR = 0.39; 95% CI 0.19-0.79; p = 0.021), and reliable COVID-19 information (OR = 0.35; 95% CI 0.16-0.77; p = 0.026) remained inversely associated with abnormal HI scores. Conclusion: The present study showed emotional and HI disturbances in adolescents with chronic immunosuppressive diseases during the COVID-19 pandemic. It reinforces the need to promptly implement a longitudinal program to protect the mental health of adolescents with and without chronic illnesses during future pandemics.

No final de dezembro de 2019, unidades de saúde regionais da cidade de Wuhan identificaram casos de uma pneumonia de etiologia desconhecida. Os casos eram epidemiologicamente ligados ao mercado atacadista Huanan, onde eram vendidos animais vivos, frutos do mar e carnes variadas. Cerca de 66% dos trabalhadores do local tinham sido acometidos pela enfermidade. A disseminação da doença seguiu a direção do rápido trânsito de pessoas do mundo globalizado. Em março de 2020, a COVID-19 já tinha chegado a 150 países, motivando a Organização Mundial da Saúde a classificar a ocorrência como uma pandemia. A crise sanitária causada pela COVID-19 destacou a importância do conceito de saúde única: a saúde humana está intimamente ligada à saúde dos animais, à saúde do ambiente e à inocuidade dos alimentos. As zoonoses correspondem a 61% dos agentes infecciosos que afetam humanos e 75% das doenças emergentes (Taylor et al., 2001). Estima-se que haja cerca de 1,7 milhão de vírus desconhecidos circulando entre aves e mamíferos, dos quais 631 a 827 mil poderão ter capacidade de infectar humanos (IPBES, 2020). A presente publicação reúne análises sobre a origem e sobre as ações de prevenção e de controle da COVID-19, com o objetivo de identificar estratégias para o enfrentamento da emergência e da reemergência de zoonoses, baseadas em princípios da saúde única.

Abstract Objective: To evaluate physical and mental health indicators in adolescents with preexisting chronic immunocompromised conditions during coronavirus disease 2019 (COVID-19) quarantine. Methods: A cross-sectional study included 355 adolescents with chronic conditions and 111 healthy adolescents. An online self-rated survey was used to investigate socio-demographic features, healthcare routine, and the quarantine impact on physical and mental health. The validated self-reported version of the Strengths and Difficulties Questionnaire (SDQ) was also applied. Results: The median of age [14 (10-18) vs. 15 (10-18) years, p = 0.733] and frequencies of female (61% vs. 60%, p = 0.970) were similar between adolescents with preexisting chronic conditions and healthy adolescents during quarantine of COVID-19 pandemic. The frequencies of abnormal total difficulties score of SDQ were similar in patients and controls (30% vs. 31%, p = 0.775). Logistic regression analysis showed that being female (OR = 1.965; 95% CI = 1.091-3.541, p = 0.024), fear of underlying disease activity/complication (OR = 1.009; 95%CI = 1.001-1.018, p = 0.030) were associated with severe psychosocial dysfunction in adolescents with chronic conditions, whereas school homework (OR = 0.449; 95% CI = 0.206-0.981, p = 0.045) and physical activity (OR = 0.990; 95% CI = 0.981-0.999, p = 0.030) were protective factors. Further analysis of patients with chronic immunocompromised conditions and previous diagnosis of mental disorders (9%) compared with patients without diagnosis showed higher median of total difficulties score (p = 0.001), emotional (p = 0.005), conduct (p = 0.007), peer problems (p = 0.001) and hyperactivity (p = 0.034) in the former group. Conclusion: Adolescents with preexisting chronic immunocompromised conditions during COVID-19 quarantine were not at higher risk of adverse health indicators. Being female, fear of underlying disease activity/complication, and household members working outside of the home were relevant issues for adolescents with preexisting chronic conditions. This study reinforces the need to establish mental health strategies for teens with chronic conditions, particularly during the pandemic.

Abstract Objective To evaluate the reproductive and histological characteristics of fresh cultured ovarian tissue from transgender male patients. Methods An in vitro pilot study in which samples were collected during sex reassignment surgery for transgender male patients. The ovarian cortex was cut into fragments of 2 mm, 3mm, and 4 mm, and placed in a 96-well plate suitable for cultivation at days 0, 2, 4, 6, and 8, when the histology was analyzed. Results Stromal hyperplasia was observed in all samples, and it was not associated with the obtainment of primordial or primary follicles. Peripheral reduction in cell count was also a recurrent finding. Primordial and primary follicles were identified with a heterogeneous pattern in fragments from the same patient and from different patients, and follicles in more advanced stages of development (secondary and antral) were not found. There was an association between the diameter of the ovarian fragments and the identification of primary follicles (p=0.036). The number of days in culture was associated with histological signs of tissue damaging in the fragments (p=0.002). The total number of follicles identified in the samples with 2mm in diameter was significantly lower than in those that measured 4mm in diameter (p=0.031). Conclusion A diameter of 4mm is suitable for ovarian tissue culture with the benefit of ease of handling. Even after prolonged exposure to testosterone, the ovarian fragments presented primordial and primary follicles, maintaining viability throughout the days they were exposed to the culture. Freezing the ovarian cortex of transgender patients who will undergo surgery for gender reassignment would be an interesting option, in the future, for the preservation of fertility.


Resumo Objetivo Avaliar as características reprodutivas e histológicas de tecido ovariano cultivado a fresco de pacientes transexuais masculinos. Métodos Estudo experimental in vitro e piloto, no qual amostras foram coletadas durante a cirurgia de redesignação de sexo para pacientes transexuais masculinos. O córtex ovariano foi cortado em fragmentos de 2mm, 3mm, e 4mm, e colocado em placa de 96 poços própria para cultivo nos dias 0, 2, 4, 6 e 8, quando a histologia foi analisada. Resultados Hiperplasia estromal foi observada em todas as amostras, e não esteve associada à obtenção de folículos primordiais ou primários. A redução periférica no número de células também foi um achado recorrente. Folículos primordiais e primários foramidentificados com padrão heterogêneo emfragmentos domesmo paciente e em fragmentos de pacientes diferentes, não sendo encontrados folículos em estágios mais avançados de desenvolvimento (secundários e antrais). Houve associação entre o diâmetro dos fragmentos ovarianos e a identificação dos folículos primários (p=0,036). O número de dias de cultura esteve associado a sinais histológicos de lesão tecidual nos fragmentos (p=0,002). O número total de folículos identificados nas amostras de 2mm de diâmetro foi significativamente menor do que nas de 4mm de diâmetro (p=0,031). Conclusão O diâmetro de 4mm parece ser mais adequado para a cultura de tecido ovariano com a vantagem de fácil manejo. Mesmo após exposição prolongada à testosterona, os fragmentos ovarianos apresentavam folículos primordiais e primários, e manteve a viabilidade ao longo dos dias de exposição à cultura. No futuro, o congelamento da cortical do ovário de pacientes transgêneros que se submeterão à cirurgia de redesignação sexual poderia ser uma opção interessante para a preservação da fertilidade.

Abstract Objective: To analyze the effects of flower essence bouquets on the signs and symptoms of stress in nursing students. Method: Randomized clinical trial, triple blind, with two groups, flower essence group and placebo group, carried out with 101 nursing students. The groups used the formula for 60 days at a dosage of 4 drops 4 times a day. The outcome was evaluated using the Baccaro Test and the Perceived Stress Scale applied at the beginning and at the end of the intervention. The outcome analysis was performed using the mixed effects model, with 〈 = 5% and the effect size verified by the Cohen's d test. Results: There was no significant difference between the groups in stress reduction (p > 0.05). Both groups showed a reduction in scale scores (p < 0.001) with large effect size. There was an influence of the COVID-19 pandemic in the reduction of Baccaro Test scores. Conclusion: The intervention with flower essence therapy was not more effective than placebo in reducing stress signs and symptoms. Brazilian Registry of Clinical Trials: UTN U1111-1257-5715.


RESUMEN Objetivo: Analizar los efectos del buqué de esencias florales en los señales y síntomas del estrés en estudiantes de enfermería. Método: Ensayo clínico randomizado, triple ciego, con dos grupos: floral y placebo realizado con 101 estudiantes de enfermería. Los grupos utilizaron la fórmula por 60 días con la dosis de 4 gotas 4 veces al día. El resultado se evaluó por las escalas: Prueba de Detección del Estrés (Baccaro) y Escala de Estrés Percibido aplicadas al principio y al final de la intervención. El análisis del resultado se realizó por el modelo de efectos misto, con 〈 = 5% y el tamaño de efecto averiguado por la d de Cohen. Resultados: No hubo diferencia significativa entre los grupos en la reducción del estrés (p > 0,05), ambos los grupos tuvieron reducción en los scores de las escalas (p < 0,001) con tamaño de efecto grande. Hubo influencia de la pandemia COVID-19 en la reducción de los scores de la Prueba de Detección del Estrés (Baccaro). Conclusión: La intervención con terapia floral no fue más efectiva que el placebo en la reducción de señales y síntomas de estrés. Registro Brasileño de Ensayos Clínicos: UTN U1111-1257-5715.


RESUMO Objetivo: Analisar os efeitos do buquê de essências florais nos sinais e sintomas do estresse em estudantes de enfermagem. Método: Ensaio clínico randomizado, triplo cego, com dois grupos, floral e placebo, realizado com 101 estudantes de enfermagem. Os grupos utilizaram a fórmula por 60 dias na dosagem de 4 gotas 4 vezes ao dia. O desfecho foi avaliado pelas escalas Teste de Baccaro e Escala de Estresse Percebido, aplicadas no início e no final da intervenção. A análise do desfecho foi feita pelo modelo de efeitos misto, com 〈 = 5% e o tamanho de efeito verificado pelo teste d-Cohen. Resultados: Não houve diferença significativa entre os grupos na redução do estresse (p > 0,05). Ambos os grupos tiveram redução nos escores das escalas (p < 0,001) com tamanho de efeito grande. Houve influência da pandemia COVID-19 na redução dos escores do Teste de Baccaro. Conclusão: A intervenção com terapia floral não foi mais efetiva que o placebo na redução de sinais e sintomas de estresse. Registro Brasileiro de Ensaios Clínicos: UTNU1111-1257-5715.

Resumo Fundamento A pandemia da COVID-19 representa uma enorme carga para o sistema de saúde do mundo. Apesar de pacientes pediátricos terem sido relativamente poupados em comparação a adultos, estudos recentes mostraram um número crescente de pacientes críticos com Síndrome Inflamatória Multisistêmica Pediátrica (SIM-P) com disfunção cardiovascular importante. No entanto, pouco se conhece a respeito da relação entre anormalidades cardíacas e biomarcadores inflamatórios e de coagulação. Objetivos Investigar anormalidades ecocardiográficas em pacientes pediátricos com COVID-19 admitidos em um hospital terciário. Métodos Este foi um estudo longitudinal retrospectivo, baseado na revisão de prontuários médicos e ecocardiogramas de pacientes (0-19 anos) admitidos em um hospital terciário entre 30 de março e 30 de junho de 2020. Para a análise estatística, o nível de significância foi estabelecido em 5% (p<0,05). Resultados Foram incluídos 48 pacientes, 73% com doenças pré-existentes, 20 (41,7%) com SIM-P. A idade mediana foi 7,5 (0-18,6) anos; 27 (56,2%) eram do sexo masculino. A duração mediana de internação foi 15,4 (2-92) dias e sete (14,6%) pacientes morreram. Um total de 70 ecocardiografias foram realizadas, 66,7% submeteram-se ao exame somente uma vez, e 33,3% várias vezes. Vinte e três (48%) pacientes apresentaram anormalidades no ecocardiograma: oito (16.6%) disfunção sistólica do ventrículo esquerdo, seis (12.5%) disfunção sistólica do ventrículo direito, e 12 (25%) dilatação da artéria coronária (Z-score>+2,5). Anormalidades ecocardiográficas foram significativamente associadas com SIM-P, admissão na unidade de terapia intensiva pediátrica, suporte ventilatório/vasoativo, e morte ( p <0,05). Níveis significativamente mais altos de d-dímero (ng/mL) foram detectados em pacientes com disfunção ventricular esquerda [16733(4157-115668) vs. 2406.5(190-95040)], disfunção ventricular direita [25769(3422-115668) vs. 2803.5(190-95040)] e dilatação da artéria coronária [9652.5(921-115668) vs. 2724(190- 95040)] (p<0,05). Conclusão Anormalidades ecocardiográficas eram frequentes nos pacientes pediátricos com COVID-19 e associadas com piores desfechos clínicos. Exacerbação das vias de inflamação e coagulação pode exercer um importante papel na lesão cardiovascular nesses pacientes.


Abstract Background COVID-19 pandemic represents a huge burden to the health system in the world. Although pediatric COVID-19 patients have been relatively spared compared with adults, recent reports showed an increasing number of critically ill patients with multisystemic inflammatory syndrome in children (MIS-c), with marked cardiovascular impairment. Nevertheless, little is known about the relationship between cardiac abnormalities and inflammatory and coagulation biomarkers. Objectives to investigate echocardiographic abnormalities in pediatric patients with COVID-19 admitted to tertiary hospital. Methods this was a retrospective longitudinal study, based on the review of medical records and echocardiograms of patients (0-19 years) admitted to a tertiary hospital between March 30 and June 30, 2020. For statistical analysis, the significance level was set at 5% (p < 0.05). Results Forty-eight patients were enrolled, 73% with preexisting diseases, 20 (41.7%) with MIS-c. Median age was 7.5 (0-18.6) years; 27 (56.2%) were male. Median duration of hospitalization was 15.4 (2-92) days and seven (14.6%) patients died. A total of 70 echocardiograms were performed; 66.7% patients were scanned only once and 33.3% multiple times. Twenty-three (48%) patients showed echocardiographic abnormalities: eight (16.6%) left ventricle (LV) systolic dysfunction, six (12.5%) right ventricle (RV) systolic dysfunction and 12 (25%) coronary dilatation (Z-score>+2.5). Echocardiographic abnormalities were significantly associated with MIS-c, admission to the pediatric intensive care unit, multiple organ dysfunction, ventilatory/vasoactive support, and death (p<0.05). Significantly higher d-dimer (ng/mL) levels were detected in patients with LV dysfunction [16733(4157-115668) vs. 2406.5(190-95040)], RV dysfunction [25769(3422-115668) vs. 2803.5(190-95040)] and coronary artery dilation [9652.5(921-115668) vs. 2724(190- 95040)] (p<0.05). Conclusion Echocardiographic abnormalities in COVID-19 pediatric patients were frequent and associated with worse clinical outcomes. Exacerbation of the inflammation and coagulation pathways may play an important role in cardiovascular injury in those patients.

ABSTRACT The assessment of the nutritional status of hospitalized patients is fundamental to the establishment of the diagnosis. For bedridden patients, however, it is not possible to determine simple measures, such as weight and height, which are the most widely used variables for nutritional assessments. Objective: Compare real and estimated anthropometric measures in hospitalized patients. Methods: A cross-sectional study was conducted with adult (>18 years of age) and senior patients (>60 years of age) admitted for clinical or surgical treatment in the general surgery infirmary of Governador Paulo Guerra Restauração Hospital. Data (sex, age, clinical diagnosis, real weight, real height, body mass index, knee height and arm circumference) were collected using nutritional follow-up charts and tabulated using Excel 2016. Statistical analyses were performed in SPSS® version 21.0. Results: One hundred and twenty patients participated in the study (median age: 55 years). Most were adults (73.3%) and women (53.3%). The mean differences in weight between the estimated and real measures were statistically significant (p=0.000), with an overestimation of this variable. Regarding height, the estimated values differed significantly from the real values in both men and women (p<0.000) and the difference was larger among the seniors (mean: -0.072). No significant difference was found between the real and estimated body mass index (p= 0.44). Conclusion: In the comparison of methods for estimating weight and height to real measures, a tendency was found to overestimate these body measures.


RESUMEN La evaluación del estado nutricional de los pacientes hospitalizados es esencial para establecer su diagnóstico. Sin embargo, para los pacientes postrados en cama, no se pueden realizar medidas sencillas como el peso y la altura, que son las más utilizadas para el diagnóstico nutricional. Objetivo: Comparar mediciones antropométricas reales y estimadas en pacientes hospitalizados. Metodología: Estudio transversal, que incluye pacientes adultos (>18 años) y personas mayores de 60 años, ingresados para tratamiento clínico o quirúrgico en la sala de cirugía general del Hospital da Restauração Governador Paulo Guerra. Los datos (sexo, edad, diagnóstico clínico, peso real, altura real, índice de masa corporal, altura de la rodilla y circunferencia del brazo) se recopilaron mediante los formularios de monitoreo nutricional y se tabularon en el software Excel 2016. Los análisis estadísticos se realizaron con SPSS®, versión 21.0. Resultados: 120 pacientes con mediana de 55 años, en su mayoría adultos (73,3%) y mujeres (53,3%). Las diferencias promedio entre las mediciones estimadas y reales fueron estadísticamente significativas, con sobreestimación del peso (p<0,0001). Con respecto a la altura, fue posible identificar que las medidas estimadas diferían significativamente (p<0,0001) de las reales para hombres y mujeres, y que esta variación era aún mayor entre los ancianos (media: -0,072). No hubo diferencias estadísticamente significativas entre el índice de masa corporal real y el estimado (p= 0,44). Conclusión: Al comparar las metodologías para estimar el peso y la altura con las mediciones reales, fue posible observar una tendencia de los métodos a sobreestimar estas mediciones corporales.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

OBJECTIVE: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI). RESULTS: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p<0.001). CONCLUSION: Self-reported increases in screen time and intrafamilial violence report impacted sleep quality in both healthy adolescents and those with chronic conditions. Decreased health-related quality of life was observed in adolescents with poor sleep quality.

Objetivo: Comparar o mapeamento oficial com um mapeamento probabilístico da infestação por Aedes spp. nos municípios do Rio Grande do Sul, Brasil. Métodos: Estudo ecológico com dados de amostras de criadouros em 2016-2017; obteve-se a classificação oficial em boletins epidemiológicos e estimou-se a probabilidade de infestação por município e semana, ajustando-se um modelo dinâmico de ocupação de sítios aos dados da vigilância epidemiológica municipal. Resultados: 187.245 amostras coletadas em 473 municípios originaram 10.648 detecções de Aedes aegypti e 8.414 de Aedes albopictus; o mapeamento oficial concorda com o probabilístico em municípios da região noroeste e oeste do RS; os mapeamentos discordam nas regiões leste, centro, nordeste e sul, revelando municípios oficialmente não infestados com alta probabilidade de infestação e notificação de arboviroses. Conclusão: A classificação oficial identificou infestação nos municípios infestados do noroeste e oeste, e não identificou infestação em municípios com possíveis falsos zeros e onde ela varia temporalmente.

Results: 187,245 samples collected in 473 municipalities returned 10,648 detections of Aedes aegypti, and 8,414 detections of Aedes albopictus; official mapping agrees with probabilistic mapping in municipalities in the northwestern and western regions of the state. The mappings are not in agreement in the eastern, central, northeastern and southern regions, revealing municipalities officially not infested but with high probability of infestation and notification of arbovirus infection. Conclusion: While official classification identified critically infested municipalities in the state's northwestern and western regions, it did not identify infestation in municipalities with possible false zero errors and where infestation varies over time.

RESUMO

Abstract A 31-year-old male patient developed an ulcer on the glans penis that evolved for three months without healing. We diagnosed it as leishmaniasis using polymerase chain reaction. No immunosuppression or associated diseases were observed. The patient was treated with meglumine antimoniate that cured the lesion in a month post-treatment. Here, we report this case of cutaneous leishmaniasis lesion at the unusual location of glans penis in an immunocompetent individual. The lesion likely developed due to the bite of a vector, highlighting the need for considering cutaneous leishmaniasis among differential diagnosis of sexually transmitted diseases in areas endemic for leishmaniasis.

RESUMO: Objetivo: Avaliar a mortalidade por áreas de Belo Horizonte (BH) durante a pandemia de COVID-19 conforme a vulnerabilidade social, visando a uma estratégia de vacinação. Métodos: Estudo ecológico com análise de mortalidade, segundo setores censitários classificados pelo índice de vulnerabilidade da saúde, composto de indicadores de saneamento e socioeconômicos. Óbitos por causas naturais e COVID-19 foram obtidos do Sistema de Informação sobre Mortalidade, entre a 10ª e a 43ª semanas epidemiológicas (SE) de 2020. Calculou-se o excesso de mortalidade por modelo de série temporal, considerando-se as mortes observadas por SE entre 2015 e 2019, por setor censitário. Taxas de mortalidade (TM) foram calculadas e padronizadas por idade com base em estimativas populacionais do Instituto Brasileiro de Geografia e Estatística (IBGE). Resultados: Houve 16,1% (n = 1.524) de excesso de mortalidade em BH: 11, 18,8 e 17,3% nas áreas de baixa, média e elevada vulnerabilidade, respectivamente. As diferenças entre TM observadas e esperadas por causas naturais, padronizadas por idade, foi igual a 59/100 mil habitantes em BH, aumentando de 31 para 77 e 95/100 mil, nas áreas de baixa, média e elevada vulnerabilidade, respectivamente. Houve gradiente de aumento com a idade nas TM por COVID-19, variando de 4 a 611/100 mil habitantes entre as idades de 20-39 anos e 75+ anos. A TM por COVID-19 por 100 mil idosos (60+ anos) foi igual a 292, aumentando de 179 para 354 e 476 nos setores de baixa, média e elevada vulnerabilidade, respectivamente. Conclusão: Desigualdades na mortalidade, mesmo entre idosos, aliadas à baixa oferta de doses, demonstram a importância de priorizar áreas socialmente vulneráveis durante a vacinação contra COVID-19.


ABSTRACT: Objective: To assess mortality during the COVID-19 pandemic according to social vulnerability by areas of Belo Horizonte (BH), aiming at strategies for vaccination. Methods: Ecological study with mortality analysis according to census tracts classified by the Health Vulnerability Index, a composite indicator that includes socioeconomic and sanitation variables. Deaths by natural causes and by COVID-19 were obtained from the "Mortality Information System", between the 10th and 43rd epidemiological weeks (EW) of 2020. Excess mortality was calculated in a time series model, considering observed and expected deaths per EW, between 2015 and 2019, per census tracts. Mortality rates (MR) were calculated and age-standardized using population estimates from the 2010 census, by the Brazilian Institute of Geography and Statistics (IBGE). Results: Excess mortality in BH was 16.1% (n = 1,524): 11, 18.8 and 17.3% in low, intermediate and high vulnerability areas, respectively. The differences between observed and expected age-standardized MR by natural causes were equal to 59/100,000 inhabitants in BH, increasing from 31 to 77 and 95/100,000 inhabitants in the areas of low, intermediate and high vulnerability, respectively. There was an aging gradient in MR by COVID-19, ranging from 4 to 611/100,000 inhabitants among individuals aged 20-39 years and 75+ years. The COVID-19 MR per 100,000 older adults (60+ years) was 292 in BH, increasing from 179 to 354 and 476, in low, intermediate and high vulnerability areas, respectively. Conclusion: Inequalities in mortality, particularly among older adults, combined with the limited supply of doses, demonstrate the importance of prioritizing socially vulnerable areas during vaccination against COVID-19.

Introdução: Dentre os estudantes acadêmicos da área da saúde, os da enfermagem são os que apresentam maiores manifestações psicológicas e fisiológicas de estresse. Em função da formação, durante o período acadêmico, algumas situações podem ser consideradas estressoras como aulas práticas, avaliações de aprendizado, diferenças entre teoria e a realidade da prática assistencial, situações de limites humanos (doenças/morte), horas intensivas de aula, entre outros. A terapia floral, por sua atuação sutil, pode auxiliar a trazer o equilíbrio emocional ajudando na redução dos sinais e sintomas psicoemocionais do estresse. É uma prática integrativa, não farmacológica e de fácil acesso. Objetivo: Analisar o efeito do buquê de essências florais composto pelas essências: Impatiens, Cerato, Elm, White Chestnut, Olive, Cherry Plum, Larch na redução de sinais e sintomas de estresse dos estudantes de enfermagem medidos pelo Teste de Baccaro, Escala de Estresse Percebido e Instrumento de Avaliação de Estresse de Estudantes de Enfermagem. Método: Ensaio clínico randomizado, triplo cego, com dois grupos: Floral (GF) que recebeu um buquê de essências florais composto por: Impatiens, Cerato, Elm, White Chestnut, Olive, Cherry Plum, Larch e grupo Placebo (GP) que recebeu uma fórmula composta somente do veículo sem essência floral. Os grupos utilizaram a fórmula por 60 dias na dosagem de 4 gotas 4 vezes ao dia. A amostra foi composta por 101 estudantes de enfermagem de duas instituições públicas e duas privadas (49 GP e 52 GF) com pontuação acima de 20 pontos pelo Teste de Baccaro. O desfecho primário foi avaliado pelas escalas: Teste de Baccaro, Escala de Estresse Percebido e Avaliação de Estresse em Estudantes de Enfermagem. O desfecho secundário foi avaliado pela percepção do uso da fórmula pelos estudantes ao final do estudo. As escalas de estresse foram aplicadas no início e no final da intervenção. A escala de COPE Breve e o Questionário de Senso de Coerência de Antonovsky foram utilizadas como variáveis moderadoras. A análise utilizou a estatísitca descritiva, comparação dos grupos pelo t-student, teste de Fisher, qui-quadrado e teste Wilcoxon-Mann-Whitney de acordo com o tipo de variável e tamanho de efeito pelo teste d-Cohen. Adotou-se nível de significância =5%. Resultados: Não houve diferença significativa entre os GF e GP na redução do estresse medido pelo Teste de Baccaro e Escla de Estresse Percebido (p>0,05), ambos os grupos tiveram redução nas pontuações das escalas (p<0.001) com tamanho de efeito grande nos dois grupos. Na Avaliação de Estresse do Estudante de Enfermagem não houve redução significativa entre os grupos e para os dois grupos ao longo do tempo. A analise de interação mostrou que houve interferência ao longo do estudo do ano do curso na Escala de Estresse Percebido e o fato de ser aluno USP na Avaliação de Estresse em Estudantes de Enfermagem, e nesta escala também houve uma influência do ano de curso e pandemia. A análise da percepção sobre a utilização do efeito floral mostrou nos relatos do GF efeito terapêutico na mudança de padrões de comportamento, não observados no GP. Conclusão: A intervenção com terapia floral não foi mais efetiva que o placebo na redução de sinais e sintomas de estresse O efeito da terapia floral se evidenciou nos relatos dos estudantes. A fórmula floral contribuiu para a melhora da qualiade do sono, autoconfiança e padrão de comportamento frente a determinadas situações.


Introduction: Among healthcare college students, the nursing ones are those demonstrate the biggest psychological and physiological stress manifestations. On account of the daily academic life, some situations can be considered stressors such as practical classes, learning assessments, differences between theory and reality of care practice, human boundary situations (disease/death), intensive class hours, amongst others. The floral therapy, for its subtle action, can help bring the emotional balance by helping to reduce the psycho-emotional signs and symptoms of stress. It s an integrative practice, no pharmacological and with easy access. Objective: To analyse the effect of a flower essence bouquet composed by the essences: Impatiens, Cerato, Elm, White Chestnut, Olive, Cherry Plum, Larch in the reduction of nursing students signs and symptoms of stress measured by Baccaro s Test, Perceived Stress Scale and Scale of Assessment of Stress Among Nursing Students. Method: Randomized clinical trial, triple blind, with two groups Floral (FG) that received a flower essence bouquet composed of: Impatiens, Cerato, Elm, White Chestnut, Olive, Cherry Plum, Larch and Placebo group (PG) that received a formula composed only of the vehicle without flower essence. The groups used the formula for 60 days in the dosage of 4 drops 4 times a day. The sample was composed of 101 nursing students from two public and two private institutions (49 PG e 52 FG) with score above 20 points by the Baccaro s Test. The primary outcome was rated by the scales: Baccaro s Test, Perceived Stress Scale and Scale of Assessment of Stress Among Nursing Students. The secondary outcome was rated by the students perception of the formula use by the end of the study. The stress scales were applied in the beginning and at the end of the intervention. The Brief COPE scale and the Antonovsky s Sense of Coherence Scale were used as moderators variables. Descriptive analysis of the data was performed, comparing the groups by t-student test, Fisher test, chi-square and Wilcoxon-Mann-Whitney test according to variable type and size effect by d-Cohen test. The significance level = 5% was adopted. Results: There was no significant difference between GF and GP in the stress reduction measured by Baccaro s Test and Perceived Stress Scale (p>0,05), both groups had scale scores reductions (p<0.001) with large effect size in the two groups. In the Scale of Assessment of Stress Among Nursing Students there was no significant reduction between the groups and for the two groups in the long term. The interaction analysis showed that there was interference throughout the academic year study in the Perceived Stress Scale and the fact of being USP student in the Scale of Assessment of Stress Among Nursing Students, and this scale had also an influence in the academic year and pandemic. The perception analysis about the use of the floral effect showed in the FG reports therapeutic effects in the changing behavior patterns, not observed in the PG. Conclusion: The floral therapy intervention was not more effective than placebo in reducing signs and symptoms of stress. The effect of flower essence therapy was evident in the students' reports. The floral formula contributed to an improvement in sleep quality, self-confidence, and behavioral patterns when faced with certain situations.

ABSTRACT Purpose To evaluate the biomechanical properties of a novel total hip replacement femoral stem. Methods Eight pairs of femurs from dog cadavers were used. The femurs were separated into different groups. A novel femoral stem with a convex proximal portion (Stem B) was biomechanically evaluated and compared to awell-known veterinary collared stem (Stem A). Femoral stems were inserted into the contralateral femurs from the same dog, forming 16 constructs. A flexo-compression load was applied on the axial axis of each sample. Maximum strength, deflection, stiffness, and energy absorption were analysed. Results Group B constructs showed significantly higher values (p ? 0.05) for the variables, except stiffness. The mean maximum strength was 1,347 ± 357 N for Group A and 1,805 ± 123 N for Group B (p ? 0.0069). The mean deflection was5.54 ± 2.63 mm for Group A and 10.03 ± 3.99 mm for Group B (p ? 0.0056). For the energy variable, the force was 6,203 ± 3,488 N/mm for Group A and 12,885 ± 5,056 N/mm for Group B (p ? 0.0054). Stem B had greater maximum strength, deflection, and energy. Conclusions The new stem was effective in neutralizing the impact of axial flexion-compression stresses during biomechanical tests in cadaveric models.
