RESUMO A Política Nacional de Atenção Básica (PNAB) 2017 propõe alterações na composição e na atuação das equipes, com implicações no trabalho do Agente Comunitário de Saúde (ACS). O objetivo deste estudo foi investigar os sentidos que os ACS, atuantes em territórios do Norte de Minas Gerais, imprimem às mudanças propostas pela PNAB e os possíveis desdobramentos para o seu trabalho e para o cuidado à população. Foram realizados oito grupos focais, com participação aproximada de 15 ACS por grupo, no período de julho a dezembro de 2018. Os resultados apontaram distintas implicações da PNAB 2017, que, por um lado, demostram a possibilidade de ampliação do escopo de atuação do ACS com a população residente em áreas rurais e, por outro, revelam uma descaracterização da natureza do seu trabalho educativo, com ameaça à existência da categoria e fragilização dos princípios da universalidade e da integralidade na atenção à saúde da população rural. Há que se discutir as condições e a sobrecarga de trabalho do ACS que atua em área rural e a necessidade de reorganizar o processo de trabalho das equipes da Atenção Primária à Saúde para fazer com que um cuidado mais ampliado e equânime chegue aos domicílios rurais.
ABSTRACT The National Policy of Primary Healthcare (PNAB) 2017 proposes changes in the composition and performance of the teams, with implications in the work of the Community Health Agent (ACS). The aim of this study was to investigate the meanings that ACS, acting in territories of the North of Minas, print to the changes proposed by the PNAB and the possible unfoldings for their work and the care of the population. Eight focus groups were carried out, with an approximate participation of 15 ACS per group, from July to December 2018. The results pointed out the different implications of the PNAB 2017 which, on the one hand, demonstrate the possibility of expanding the scope of the ACS together with the population residing in rural areas and, on the other, reveal a decharacterization of the nature of their educational work with a threat the existence of the category and the fragility of the principles of universality and integrality. It is necessary to discuss the conditions and workload of the ACS working in rural areas and the need to reorganize the work process of the primary healthcare teams in order to achieve a more extended and equitable care for rural households.
RESUMO
O objetivo deste texto é expor as interações entre pesquisadora, profissionais de saúde e usuários do SUS no contexto rural, no material empírico de uma pesquisa de orientação etnográfica. O rural é pensado como o lugar do encontro entre o campo e a cidade, como um espaço singular e como um modo vida. Apoiadas nas contribuições dos estudos etnográficos, utilizaram-se, neste trabalho, recursos como observação participante, descrição densa e entrevistas com os profissionais da equipe de Saúde da Família que trabalham em povoados de um município de Minas Gerais. Acentuam-se, nos resultados e discussão, elementos do material empírico que permitem acompanhar cenas de estranhamento nos encontros proporcionados pela pesquisa.
The objective of this text is to expose the interactions between the researcher, health professionals and SUS users, in the empirical material of an ethnographically oriented research in health. The rural is thought of as an encounter between the countryside and the city, as a singular space , and as a way of life. Supported by the contributions of ethnographic studies, we used field strategies such as participant observation, dense description and interviews with professionals of the Family Health team, who work at villages of the state of Minas Gerais. In the results and discussion section we highlight elements of the empirical material that lead us to follow scenes of estrangement within the encounters produced by the research.
