RESUMO

A 90-year-old man was diagnosed with primary gastric diffuse large B-cell lymphoma (PGDLBL) by PET/CT examination, gastroscopy, biopsy and histopathological analysis at a regular physical check in April, 2016. The patient received R-CO chemotherapy (rituximab, cyclophosphamide, and vincristine) and radiotherapy subsequently, with enteral nutritional treatment through 3-cavity nasogastric tube due to development of pyloric obstruction. To satisfy patient's strong desire of eating by himself, we performed surgery of exploratory laparotomy and Roux-en-Y gastric bypass (RGB) to relieve pylorus obstruction. Postoperatively, the patient resumed oral feeding, supplemented by nasogastric tube feeding at 1350 - 1550 Kcal daily. He is now 94 years old with fairly well nutrition and normal communication. The outcome of 4 year follow-up suggests that nutritional treatment and palliative medicine are important for improving prognosis and life-quality of very elderly patients with end-stage tumors apart from the effective chemotherapy, radiotherapy, and surgery.

Objective@#To analyze the prevalence of overweight and obesity among adolescents in Wuhan and its association with bullying victimization,and to provide information for relevant interventions.@*Methods@#A total of 3 251 adolescents in senior grade of primary school, middle school and high school were selected by using random clustering sampling method and were surveyed by questionnaire, height and weight were measured.@*Results@#Among the 3 251 students, 523(16.09%) were overweight, and 445 (13.69%) were obesity. In the past 30 days, 732 (22.52%) of the adolescents had ever been bullied and 101(3.11%) had often been bullied. The relationship between overweight/obesity and bullying victimization among boys was not statistically significant by controlling areas and age(P>0.05). While girls who were overweight or obesity were more likely to be bullied (OR=1.78,1.58, P<0.05).@*Conclusion@#Overweight and obesity have a certain influence on the physical and mental development of adolescents, which may increase the possibility of being bullied among girls. Strategies should be taken to prevent overweight and obesity among children and adolescents, and a healthy body shape appreciation should be established, to reduce the incidence of bullying.

Objective: To rapidly analyze chemical compositions in raw products and stir-fried products of Gardeniae Fructus,and determine the pharmacodynamic material basis of Gardeniae Fructus Praeparatus. Method: Liquid chromatography-ion trap-time-of-flight-mass spectrometry(LCMS-IT-TOF) was performed on ACQUITY UPLC HSS T3 column(2.1 mm×100 mm,1.8 μm),mobile phase was 0.005%formic acid aqueous solution(A)-acetonitrile(B) for gradient elution(0-5 min,10%-15%B;5-8 min,15%B;8-18 min,15%-95%B;18-20 min,95%B;20-22 min,95%-10%B;22-25 min,10%B),the column temperature was 30℃ and the flow rate was 0.4 mL·min-1,volume of sample injection was 2 μL;electrospray ionization(ESI) was applied for mass spectrometric analysis under positive and negative ion mode,the scanning ranges were m/z 100-1 000.The ion peaks were identified by comparison of control substances,mass spectrometry data analysis and literature references.The peak areas of some ion peaks before and after processing were compared,the changes of chemical compositions in Gardeniae Fructus after processing were investigated. Result: Based on the mass spectral data information and references,a total of 38 compounds were identified,no new compounds were produced after processing.The ion peak areas of main compounds were investigated during the stir-frying processing,the contents of geniposide,genipin-1-β-D-gentiobioside,shanzhiside,chlorogenic acid and crocin in Gardeniae Fructus Praeparatus were decreased;the contents of geniposidic acid and crocetin were increased. Conclusion: The method can rapidly and accurately identify the chemical constituents in Gardeniae Fructus Praeparatus.The changes of iridoids and crocins in Gardeniae Fructus during the stir-frying process may be related to hemostatic activity and protection of cardiovascular and cerebrovascular.

Objective: To analyze and research detection data of radioactivity performance of digital radiography (DR) equipment so as to provide reference for purchasing DR equipment in hospital. Methods: Through collected the analysis results of detection data of quality control of 153 DR equipment of third level 1st class hospital, third level 2nd class hospital and county level hospital of Yunnan province. The comparison analysis about these DR equipments depended on the hospital type, nationality of equipment and duration of service. Results: There were 50 DR equipment and 103 DR equipments in the third level 1st class hospital and the third level 2nd class hospital, respectively. And in the third level 1st class hospital, there was 79.17% of DR equipments was imported equipment, while it was 51.45% in the third level 2nd class hospital. Besides, the duration of service of the most of DR equipments was 0-4yeras in the third level 2nd class hospital, while it was 5-7 years in the third level 1st class hospital. Conclusion: Most of the DR equipments in the third level 1st hospitals are imported equipment, and their duration of service were longer. On the other hand, in the comparison between imported equipment and domestic equipment, there are a certainly gap in quality stability, reliability and other aspects.

Cooking fumes (CFs) are mixtures of many toxic components, such as aldehydes, heterocyclic amines, polycyclic aromatic hydrocarbons, fat aerosols and particulate matters. CFs exposure has been proven to be associated with many diseases. Lung cancer takes the leading place among the diseases being reported caused by CFs exposure. Molecular and biochemical studies have found that CFs exposure may lead to lung cancer by gene damage, formation of reactive oxygen species, blockage of related proteins' function, and even cell death. However, reviews about the mechanisms of how CFs exposure leads to lung cancer are still lacking. Elucidation of the mechanisms of lung cancer caused by CFs exposure may provide a new insight into the prevention of lung cancer caused by CFs exposure, as well as laying the foundation for the toxicity study of CFs. In this minor review, the mechanisms of how CFs exposure leads to lung cancer were summarized and discussed.

Triptolide (TP) from Tripterygium wilfordii has been demonstrated to possess anti-inflammatory, immunosuppressive, and anticancer activities. TP is specially used for the treatment of awkward rheumatoid arthritis, but its clinical application is confined by intense side effects. It is reported that licorice can obviously reduce the toxicity of TP, but the detailed mechanisms involved have not been comprehensively investigated. The current study aimed to explore metabolomics characteristics of the toxic reaction induced by TP and the intervention effect of licorice water extraction (LWE) against such toxicity. Obtained urine samples from control, TP and TP + LWE treated rats were analyzed by UPLC/ESI-QTOF-MS. The metabolic profiles of the control and the TP group were well differentiated by the principal component analysis and orthogonal partial least squares-discriminant analysis. The toxicity of TP was demonstrated to be evolving along with the exposure time of TP. Eight potential biomarkers related to TP toxicity were successfully identified in urine samples. Furthermore, LWE treatment could attenuate the change in six of the eight identified biomarkers. Functional pathway analysis revealed that the alterations in these metabolites were associated with tryptophan, pantothenic acid, and porphyrin metabolism. Therefore, it was concluded that LWE demonstrated interventional effects on TP toxicity through regulation of tryptophan, pantothenic acid, and porphyrin metabolism pathways, which provided novel insights into the possible mechanisms of TP toxicity as well as the potential therapeutic effects of LWE against such toxicity.

This study was designed to explore the "multi-components, multi-targets and multi-pathways" intervention mechanism of Huanglian Jiedu decoction (HLJDD) in the treatment of Alzheimer's disease (AD) by pharmacological network technology, which may establish a foundation for drug development and innovative research. Seventeen active constituents of HLJDD with anti-AD activities were submitted to PharmMapper and Molecule Annotation System (MAS 3.0) bioinformatics softwares to predict the target proteins and carry out related KEGG pathways annotation respectively. The network of "active compound-target-pathway" was constructed and analyzed using the Cytoscape 3.4.0 software. The results suggest that 47 pathways are affected by the 17 active components through 59 target proteins, in which 4 target proteins are related to AD and 2 pathways related to neuroinflammation, respectively. The effect of HLJDD on AD may be dependent on clearing/reducing β-amyloid protein, inhibiting Tau hyperphosphorylation, anti-inflammation and immunoregulation.

This article dealed with the effects of processing method and duration on the major bioactive components (sinigrin and sinapine thiocyanate) in Brassica juncea. The contents of sinigrin and sinapine thiocyanate in decoctions of raw and processed B. juncea were determined and compared by high performance liquid chromatography on a Alltima C18 column (4.6 mm x 250 mm, 5 microm) at 35 degrees C with the acetonitrile-0.1% phosphoric acid as the mobile phrase in gradient elution. The detection wavelength of sinigrin and sinapine thiocyanate was set at 227 nm and 326 nm, and the flow rate was 1.0 mL x min(-1). It was found that with the extended processing duration, the contents of sinigrin and sinapine thiocyanate first increased and then decreased: i.e., 0-2 minutes they increased gradually (for sinigrin, by 9.65% in processed products and 356. 10% in powder; for sinapine thiocyanate, by 12.82% in processed products and 3.41% in powder), and achieved their highest content at 2 min; then, decreased during the next 5 minutes (for sinigrin, by 80.35% in processed products and 82.09% in powder; for sinapine thiocyanate, by 14.29% in processed products and 17.54% in powder), suggesting that processing duration could significantly affect the contents of bioactive components in B. juncea, enzymatic hydrolysis of sinigrin when the seed is crushed in the present of moisture may be responsible for the content change. It is recommended that the slow fire should be the best processing method and the raw seed could be used directly in the water extracts related industrial production.

<p><b>OBJECTIVE</b>To study the effects of Zhenqing Recipe (ZQR) and earthworm on the expressions of transforming growth factor-beta1 (TGF-beta1) and plasminogen activator inhibitor-1 (PAI-1) in renal tissues of type 2 diabetes mellitus (T2DM) rats.</p><p><b>METHODS</b>The T2DM model rats induced by high sucrose/fat feeding and low dose streptozotocin injecting were randomly divided into the model group, the ZQR group, and the earthworm group. A normal control group was also set up. ZQR extract was given at the daily dose of 24 g/kg body weight by gastrogavage to rats in the ZQR group. The earthworm extract was given at the daily dose of 2.6 g/kg body weight by gastrogavage to rats in the earthworm group. Equal volume of distilled water was given to rats in the normal control group and the model group by gastrogavage. The medication was once daily for 8 successive weeks. The fasting blood glucose (FBG), serum PAI-1, 24-h urinary albumin excretion (UAE), and endogenous creatinine clearance rate (CCr) were detected. The structural changes of renal tissues stained with Periodic Acid-Schiff were observed by light microscope. The expressions of TGF-beta1 and PAI-1 were analyzed by immunohistochemistry. And the mRNA and protein expressions of TGF-beta1 and PAI-1 in the renal cortex were detected by Real-time PCR and Western blot respectively.</p><p><b>RESULTS</b>Compared with the normal control group, the FBG, serum PAI-1, UAE, the mRNA and protein expressions of TGF-beta1, and PAI-1 significantly increased in the model group, and CCr obviously decreased (P<0.01). Compared with the model group, FBG, serum PAI-1, UAE obviously decreased (P<0.01) and CCr obviously increased (P<0.05) in the ZQR group. FBG was not obviously lowered in the earthworm group. The serum PAI-1 and UAE both decreased obviously (P<0.05, P<0.01) and CCr increased (P<0.05) in the earthworm group. The effects of ZQR were superior to the earthworm. The renal pathological changes got improved in the ZQR group and the earthworm group. The mRNA and protein expressions of TGF-beta1 and PAI-1 significantly decreased in the ZQR group and the earthworm group, especially in the ZQR group.</p><p><b>CONCLUSIONS</b>Early application of ZQR and earthworm in treatment of T2DM rats with nephropathy was effective in protecting the kidney. Its mechanism may possibly be correlated with inhibition of TGF-beta1 expression, resulting in down-regulation of PAI-1 expression. ZQR showed better effects than earthworm.</p>

Background Deficiency of neurotrophic factor is associated with the damage of optic nerve in glaucoma.Reaserches showed that ectopically applied neurotrophic factor has a transient neuroprotective effect in glaucoma model,and the viral expression of adeno-associated neurotrophie factor may provide long-term supplementation of neurotrophic factor and neuroprotection in tissues.Objective The present study Was to investigate the neuroproteetive effect of adeno-associated viral(AAV)-mediated brain-derived neurotrophic factor (BDNF)on retinal ganglion cells(RGCs)in DBA/2J mice with experimental glaucoma. Methods 10 clean DBA/2J mice were administered intravitreal injection with 1 microliter of AAV-BDNF-GFP in the left eyes at the age of 6 months,and the right eyes were injected with the same volume of saline solution as control.Intraocular pressure (IOP)was measured with Tonolab in the mice every month.Retinas were obtained after 3 months for the investigation of GFP expression in RGCs using fluorescence microscopy.Immunohistochemistry Was performed by applying TUJ1 and Cy3 antibodies to identify surviving RGCs. Results The IOP of DBA/2J mice were 11.90 mmHg and 11.40 mmHg in the right eyes and left eyes,respectively,at 4 months.The IOP of DBA/2J mice began to rise at 5 months and reached its peak in 8 month-old mice.There was no statistically significant difference in IOPs between the right eyes and the left eyes from 4 month-through 9 month-old mice(t=-1.78-0.61,P=0.11-0.90).Three months after intrlavitreal injection of AAV-BDNF-GFP,GFP was positively expressed in RGCs of retinas with the expression rate of 46.33%±8.08%.The over-expression of BDNF led to more RGCs survival than the control eyes (3168.13±1319.33/mm2 vs 2024.81±796.38/mm2,t=2.75,P=0.02). Conclusion These data suggest that BDNF can exert a protective effect in DBA/2J glaucoma mice.

<p><b>OBJECTIVE</b>To explore the value of lower-limb short latency somatosensory evoked potentials (SLSEP) in predicting early death in patients with massive cerebral infarction.</p><p><b>METHODS</b>Forty-eight patients of massive cerebral infarction were admitted in the Neurological Intensive Care Unit (NICU) between March 2008 and March 2009, and Glasgow-Pittsburgh coma scale (GPCS) and SLSEP were recorded and graded within 24 h after admission. The patients were divided into survival and death groups (including brain death) according to their short-term prognosis. The correlations of SLSEP and GPCS to the mortality were assessed.</p><p><b>RESULTS</b>A significant correlation was found between SLSEP and the mortality in patients with massive cerebral infarction (r=0.484, P<0.001). The positive predictive value of the SLSEP grade 3 to death was 100%, and the patients with malignant middle cerebral artery infarction (mMCAI) appeared to have a 100% mortality.</p><p><b>CONCLUSION</b>SLSEP grade 3 can be a highly specificity in predicting early death in patients with massive cerebral infarction, and it is also of value in determining the timing of surgical intervention of mMCAI.</p>

RESUMO

<p><b>OBJECTIVE</b>The present study investigated the inhibitory effects of Chinese herb component sinapine on activity of acetylcholinesterase (AChE) in cerebral homogenate and blood serum of rats.</p><p><b>METHOD</b>AChE was prepared from cerebral homogenate and blood serum of rats, respectively. Acetylcholinesterase activity assay kit and Chromatometry were used to detect the AChE activity.</p><p><b>RESULT</b>Sinapine significantly inhibited AChE activity in vitro, with more effective on cerebral homogenate (IC50 3.66 micromol x L(-1)) than blood serum (IC50 22.1 micromol x L(-1)).</p><p><b>CONCLUSION</b>Sinapine could significantly inhibit the cerebral AChE activity and may be a promising drug used for prevention and cure of Alzheimer's disease as a cholinesterase inhibitor.</p>

Doxazosin, a high selective alpha1-adrenoceptor antagonist, is considered as the first-line therapy for the patients with benign prostatic hyperplasia (BPH) and also produce several side effects in cardiovascular system. In this study, we observed the isometric vasoconstrictive responses of the rabbit isolated arterial rings to electric field stimulation and noradrenaline ( NA ) to study the effects of R-doxazosin ( R-DOX ) and S-doxazosin ( S-DOX ) on the alpha1-adrenoceptor-regulated vasoconstrictive responses in the rabbit isolated ear artery, mesenteric artery and pulmonary artery, and the effects of higher concentration of S-DOX and R-DOX on presynaptic alpha2-adrenoceptor-regulated purinergic vasoconstriction in the rabbit isolated saphenous artery. We found that R-DOX and S-DOX competitively inhibited the vasoconstriction induced by NA in the rabbit isolated ear artery, mesenteric artery and pulmonary artery. The pA2 values of R-DOX and S-DOX against NA in the rabbit isolated ear artery, mesenteric artery and pulmonary artery were 7. 91 +/- 0. 03 and 7. 53 +/- 0. 05, 7. 80 +/- 0. 05 and 7. 29 +/-0. 07, 8. 32 +/- 0. 06 and 7. 97 +/- 0. 07, respectively. The pA2 values of R-DOX in the three arterial preparations were significantly higher than those of S-DOX (P < 0. 01). R-DOX and S-DOX at the concentrations of 0. 1 - 10 micromol x L (-1) did not affect the vasoconstriction induced by electric stimulation in the rabbit isolated saphenous artery. R-DOX and S-DOX at 100 micromol x L(-1) in the rabbit isolated saphenous artery completely inhibited the vascular responses to exogenous NA, but did not affect the vascular responses to exogenous adenosine triphosphate (1 mmol x L(-1) ). It is reasonable to suggest that R-DOX and S-DOX competitively inhibit the vasoconstriction induced by NA in the rabbit ear artery, mesenteric artery and pulmonary artery, and the pA2 values of S-DOX are significantly lower than those of R-DOX. The higher concentration (10 micromol x L(-1)) of R-DOX and S-DOX does not affect the presynaptic alpha2-adrenoceptors at sympathetic nerve terminals of the rabbit saphenous artery.

<p><b>OBJECTIVE</b>To establish the quantitative analysis method of triterpene delta-amyrone in Sedum sarmentosum by HPLC-ELSD.</p><p><b>METHOD</b>The Zorbax Sil Column was used and the mobile phase was dichloromethane-methanol (99:1). The flow rate was set at 0.5 mL x min(-1), the temperature of drift tube was set at 100 degrees C and the nebulizer nitrogen flow rate was 3.0 L x min(-1).</p><p><b>RESULT</b>The linear range of sigma-amyrone was 2.12-10.6 microg. The average recovery was 100.5%.</p><p><b>CONCLUSION</b>The results showed that the method was reliable, simple and precise. It would be beneficial for the study of the quality control of S. sarmentosum.</p>

<p><b>OBJECTIVE</b>To observe the clinical efficacy of Qianlie Sanyu Capsule on benign prostatic hyperplasia (BPH).</p><p><b>METHODS</b>Seventy-two patients with BPH were randomly divided into a treatment group (40 patients) and a control group (32 patients), the former treated by Qianlie Sanyu Capsule and the latter by Longbishu. Observation were made on the changes of the patients' urination' symptoms, living quality, prostatic volume, Qmax uroflowmetry and excel urine before and after treatment.</p><p><b>RESULTS</b>The total efficacy rates were 92.5% in the treatment group, and 72.5% in the control group. Comparison between the two groups showed significant difference (P < 0.01). The urination symptoms, living quality, Qmax uroflowmetry and the excel urine were better improved in the Qianlie Sanyu group than in the Longbishu. There were significant differences between the two groups (P < 0.05), but none in reducing effect on the prostatic volume.</p><p><b>CONCLUSION</b>Qianlie Sanyu Capsule is an effective drug for BPH.</p>
