RESUMO

Sleep disorder is a common problem that damages the quality of life in the elderly. At the same time, it is also related to the occurrence and development of insulin resistance, metabolic syndrome, Alzheimer's disease and other diseases. With the progress of aging in China, the sleep problems of the elderly have attracted more and more attention.Based on the sleep characteristics of elderly and the clinical manifestations of sleep disorders, this paper expounds the current situation using sleep disorders assessment scale, and puts forward the urgent need for independent development of sleep disorder assessment scale suitable for the elderly in China.

RESUMO

Medical microbiology is one of the compulsory courses of basic medical sciences, which lays an important theoretical foundation for the follow-up study of infectious diseases, contagions, tumors, and so on. The course of medical microbiology in our college adhered to the concept of student-centered, diversified teaching, scientific evaluation, and continuous improvement. Teaching design was a cross-link of general theories, specific chapters, clinical cases, theory and practice, and the ideological and political education throughout the curriculum. Lectures adopted the mode of offline teaching (such as flipped classroom, case analysis, and comprehensive experimental design), online assistance (such as classroom test, stage test, extracurricular homework, and questionnaire survey), and combined process evaluation. This teaching mode also reflected the deep integration of information technology and classroom teaching. With the development and construction in these years, medical microbiology has completed the renewal of curriculum resources, the construction of online question bank, the construction and design of ideological and political case bank, and process evaluation (10 points of usual score + 10 points of case study + 10 points of experimental performance + 100 points of final examination multiplied by 70%). There was no significant difference in the results of qustionnaire survey in terms of the improvement of independent learning ability, curriculum evaluation system and satisfaction feedback. Students in Batch 2019 were most satisfied with the teaching of keeping pace with the times and the guidance of positive outlook on life and values.

RESUMO

Objective To compare the adverse effects and prognosis of locally advanced nasopharyngeal carcinoma (LANC) patients between the gemcitabine combined platinum (GP) regimen and other platinum-containing chemotherapy regimens by meta-analysis. Methods We searched relevant databases and included the studies about comparing GP and other chemotherapy regimens in the treatment of LANC. Methodological quality was assessed for each included study. Statistical analysis was carried out using RevMan5.3 and Stata15 software. Results The patients on GP regimen had a lower incidence of severe leukopenia and severe gastrointestinal reaction but a higher incidence of severe thrombocytopenia and hepatoxicity than those on other regimens (P < 0.05). The patients on GP regimen had a higher distant metastasis-free survival (P=0.004). Conclusion GP regimen can be used as a safe, alternative and economical induction chemotherapy regimen for locally advanced nasopharyngeal carcinoma.

RESUMO

Objective:To investigate the expression of serum matrix metalloproteinase 3 (MMP-3) in patients with rheumatoid arthritis (RA) and analyze its correlation with bone erosion and disease activity.Methods:100 RA patients diagnosed in the First People's Hospital of Changde from July 2018 to December 2019 were selected as the RA group, and 35 healthy volunteers who came to the hospital for physical examination at the same time were selected as the control group. The clinical data of the patients were collected, and the serum MMP-3 levels of the patients and healthy volunteers were detected by enzyme-linked immunosorbent assay (ELISA). The serum MMP3 levels of RA patients with different disease activity, different imaging stages and different bone mineral density were compared, and the correlation with clinical indicators were analyzed.Results:⑴ RA patients were grouped according to the Disease Activity Score (DAS) 28. Serum MMP-3 levels in the highly active group (300.87±15.93)ng/ml and in the moderately active group (213.78±12.79)ng/ml were higher than those in the clinical remission group (82.87±8.19)ng/ml increased significantly. ⑵ The serum MMP3 level in the RA group (190.98±13.43)ng/ml was significantly higher than that in the healthy control group (69.97±10.63)ng/ml. ⑶ There were significant differences in serum MMP-3 levels among RA patients with different imaging stages ( P<0.05). The level of MMP-3 in stage Ⅲ (206.18±13.58)ng/ml and stage Ⅳ (301.72±13.43)ng/ml were significantly higher than those in stage Ⅰ (89.16±10.13)ng/ml. ⑷ RA patients were divided into normal group, osteopenia group, and osteoporosis group according to bone mineral density. The serum MMP-3 level in the osteopenia group (180.87±12.69)ng/ml and osteoporosis group (289.54±13.28)ng/ml were significantly higher than that in the normal group (121.05±8.45)ng/ml. ⑸ Serum MMP-3 levels were positively correlated with C-reaction (CRP), erythrocyte sedimentation rate (ESR), DSA 28, rheumatoid factor (RF), joint swelling index, joint tenderness index, and platelet count in the RA group ( P<0.05). Conclusions:The serum MMP-3 plays an important role in the progression of RA, and is closely related to RA disease activity and bone erosion. It is expected to become a serological indicator for predicting RA bone erosion and radiological progress.

RESUMO

Objective:Taking medical microbiology in the pathogen biology as an example, to explore the effects of process evaluation in basic medicine teaching. Methods:Multiple methods, including classroom performance, stage test, case analysis, final examination, questionnaire survey and so on, were used in the study to systematically evaluate the teaching process. According to the students' suggestions, feedback problems and solutions were publically given to students in the form of PPT, forming a process evaluation with "evaluation-feedback-correction" loop as the core. Collected data were analyzed statistically with SPSS 16.0.Results:The students were impressed by the contents with various teaching methods and satisfied with the evaluation system. Most of them thought that their abilities had been improved in many ways during the learning process. 96.8% of the students supported the reform, and 88.6% of the students hoped to apply this system to other disciplines. The scores of case analysis, final examination and overall evaluation in the experimental group were significantly higher than those of the control group ( P<0.01). Conclusion:Both teachers and students got profit from the process evaluation based on feedback loop. The teaching quality of medical microbiology would be further improved by a constantly enhanced evaluation system.

RESUMO

Bronchopulmonary dysplasia (BPD) is a severe complication of preterm birth, especially in those with very low birth weight or extremely low birth weight. The incidence of BPD increased with more survival of very preterm infants.Neonates with BPD are more oxygen-dependent and require prolonged mechanical ventilation.Respiratory and neurological outcomes in the childhood and even early adulthood, including pulmonary symptoms,re-admittance rate, pulmonary hypertension and dysfunction, will be adversely influenced.Meanwhile,BPD patients will develop more problems in cognition, education and behavior.
