RESUMO

OBJECTIVE@#To explore the clinical characteristics and prognosis of multiple myeloma(MM) patients with secondary primary malignancies.@*METHODS@#The clinical data of newly diagnosed MM patients admitted to the First Affiliated Hospital of Zhengzhou University from January 2011 to December 2019 were retrospectively analyzed. The patients with secondary primary malignancies were retrieved, and their clinical features and prognosis were evaluated.@*RESULTS@#A total of 1 935 patients with newly diagnosed MM were admitted in this period, with a median age of 62 (18-94) years old, of which 1 049 cases were hospitalized twice or more. There were eleven cases with secondary primary malignancies (the incidence rate was 1.05%), including three cases of hematological malignancies (2 cases of acute myelomonocytic leukemia and 1 case of acute promyelocytic leukemia) and eight cases of solid tumors (2 cases of lung adenocarcinoma, and 1 case each of endometrial cancer, esophageal squamous cell carcinoma, primary liver cancer, bladder cancer, cervical squamous cell carcinoma, and meningioma). The median age of onset was 57 years old. The median time between diagnosis of secondary primary malignancies and diagnosis of MM was 39.4 months. There were seven cases with primary or secondary plasma cell leukemia, the incidence rate was 0.67%, and the median age of onset was 52 years old. Compared with the randomized control group, the β2-microglobulin level in the secondary primary malignancies group was lower (P=0.028), and more patients were in stage I/II of ISS (P=0.029). Among the 11 patients with secondary primary malignancies, one survived, ten died, and the median survival time was 40 months. The median survival time of MM patients after the secondary primary malignancies was only seven months. All seven patients with primary or secondary plasma cell leukemia died, with a median survival time of 14 months. The median overall survival time of MM patients with secondary primary malignancies was longer than that of the patients with plasma cell leukemia (P=0.027).@*CONCLUSION@#The incidence rate of MM with secondary primary malignancies is 1.05%. MM patients with secondary primary malignancies have poor prognosis and short median survival time, but the median survival time is longer than that of patients with plasma cell leukemia.

OBJECTIVE: To analyze the diagnosis of occupational disease and the detection status of suspected occupational disease and occupational contraindication in recent years in a petrochemical enterprise. METHODS: The data of new cases of occupational disease reported by a petrochemical enterprise from 2008 to 2019, the cases of suspected occupational disease and occupational contraindication from 2015 to 2019 were collected. The related data was descriptively analyzed. RESULTS: A total of 30 new cases of occupational disease were reported in this petrochemical enterprise from 2008 to 2019. Among them, there were 16 cases of occupational noise deafness, 10 cases of chronic occupational benzene poisoning, 2 cases of occupational leukemia caused by benzene, 1 case of occupational solvent gasoline poisoning and 1 case of occupational pneumoconiosis. Among the 30 cases of occupational diseases, 28 cases(93.3%) were related to benzene and noise exposure, and 19 cases(63.3%) came from the chemical and oil refining divisions. From 2015 to 2019 in this petrochemical enterprise, 24 cases of suspected occupational disease were reported, of which 17(70.8%) suspected cases were diagnosed as occupational disease, and 63 cases of occupational contraindication were reported, including 47(74.6%) cases of occupational contraindication caused by noise. CONCLUSION: Benzene and noise should be taken as the key factors of occupational hazard for prevention and control in the petrochemical enterprises, and control measures should be adopted for special operation links and job posts to control the hidden dangers of benzene and noise that exceeds the standard limits.

Objective:To systematically evaluate the effect of brain-computer interface (BCI) on upper-limb motor function after stroke, and compare the effects under different interfaces. Methods:Randomized controlled trials (RCTs) about BCI for upper-limb motor function after stroke were retrieved from databases of PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang Data and CBM, from inception to October, 2020. The quality of the trials was assessed and the data were extracted according to the Cochrane Handbook of Systematic Review. A meta-analysis was carried out with RevMan 5.3 and ADDIS 1.16.8. Results:Ultimately, 14 RCTs involving 504 patients were included. Meta-analysis showed that BCI could obviously improve the Fugl-Meyer Assessment-Upper Extremities (FMA-UE) score (MD = 6.81, 95%CI 1.51 to 12.11, P < 0.05), Action Research Arm Test score (MD = 7.68, 95%CI 0.49 to 14.88, P < 0.05) and modified Barthel Index score (MD = 8.91, 95%CI 5.57 to 12.25, P < 0.001) after stroke. Subgroup analysis showed that FMA-UE score could be improved by BCI for both more than four weeks (MD = 9.44, 95%CI 1.83 to 17.04, P < 0.05) and less than four weeks (MD = 5.18, 95%CI 2.84 to 7.51, P < 0.001). For the types of interface, the probabilities of the best effects from network meta-analysis ranked as electrical stimulator (P = 0.53), visual feedback (P = 0.41) and machine assistance (P = 0.06). Conclusion:BCI, especially with electrical stimulator interface, could obviously improve upper-limb motor function and activities of daily living for stroke patients.

OBJECTIVE@#To investigate the effect of the exosomes from bone marrow mesenchymal stem cells (BMSCs) transfected with silence plasmid of Piezol small interference RNA (siRNA)on osteoarthritis (OA) animal model.@*METHODS@#Twenty male SD rats with specific pathogen free (SPF) were selected, ranging in age from 5.46 to 6.96 months, with a mean of (6.21± 0.75) months;and ranging in weight from 385.76 g to 428.66 g, with a mean of (407.21±21.45) g. BMSCs were extracted. The siRNA silencing plasmid of piezo1 was constructed by siRNA technology. After lentivirus was transfected into BMSCs, the exosomes were extracted. At the cellular level, BMSCs were divided into blank plasmid group and siRNA silencing plasmid group according to whether siRNA-Piezo1 was transfected or not. The osteogenic induction ability of siRNA-Piezo1 on BMSCs was detected by RT-PCR and Western blot. At the animal model level, the OA model was established by surgical resection of cruciate ligament of knee joint.According to different treatment schemes, SD rats were divided into 4 groups:blank control group, model group, BMSCs group and exosome group. SD rats in the blank control group were not treated. In the model group, the cruciate ligaments of rats were excised and OA animal model was established. In BMSCs group, BMSCs were injected into knee joint under CT guidance after OA model establishment, and the cell volume was 5×10@*RESULTS@#The lentivirus transfection efficiency was(92.11±4.22)%. RT-PCR showed that the relative expression of Piezo1 mRNA in blank plasmid group was 1.07±0.06, which was significantly different from that of 0.31±0.01 in siRNA silencing plasmid group (@*CONCLUSION@#Piezo1 siRNA silencing vector can promote the differentiation of BMSCs into chondrocytes and effectively inhibit the progression of OA, so as to delay the disease of OA.

Objective: To explore the effect of modified Danggui Shaoyao San on matrix metalloproteinase-2(MMP-2), intercellular adhesion molecule-1 (ICAM-1), emorheology and inflammation in patients with chronic pelvic inflammatory disease. Method: Patients with chronic pelvic inflammatory disease during May 2017 to June 2018 were randomly divided into treatment group and control group, with 37 cases in each group. The control group was orally treated with levofloxacin tablets and metronidazole tablets. In addition to the therapy of the control group, the treatment group was also given modified Danggui Shaoyao San. Traditional Chinese medicine(TCM) symptom scores, MMP-2 and ICAM-1, interleukin-1β(IL-1β), IL-6, IL-4, IL-10, and transforming growth factor-β (TGF-β), grain-megakaryocyte colony stimulating factor (GM-CSF), tumor necrosis factor-alpha (TNF-α), whole blood viscosity (ηb), plasma viscosity (ηp), erythrocyte aggregation index (AI), fibrinogen (Fib) before and after treatment were compared between two groups. The efficacy, adverse reactions and recurrence were observed in two groups. Result: The clinical efficacy of treatment group was better than that of the control group (Z=2.791, PPα, IL-1β, IL-6, GM-CSF,ηb,ηp, AI, Fib levels in the treatment group after treatment were significantly lower than those in control group (Pβ1 levels in treatment group after treatment were significantly higher than those in control group (Pχ2=6.198, PConclusion: Modified Danggui Shaoyao San has a significant clinical efficacy in the treatment of CPID, and can effectively relieve clinical symptoms and greatly reduce the recurrence rate, which may be related to the improvement of the regulation of MMP-2 and ICAM-1 levels, the inhibition of inflammatory reactions and the improvement of hemorheology.

Objective:To screen and identify the protein that interacts with the adhesion protein of Mycoplasma genitalium (MgPa)from T7-phage display cDNA library of human uroepithelial cells(SV-HUC-1).Methods:Recombinant adhesion protein of My-coplasma genitalium(rMgPa)was used as target molecule to biopan the T7 phage display cDNA library of SV-HUC-1 cell,the selected positive clones were analysed using DNA sequencing and BLAST analysis and identified by means of indirect ELISA,Dot immunoblot and Far-western blot.Results:After four rounds of biopanning,positive phages were obviously enriched.According to the results of DNA sequencing and BLAST analysis,the selected randomly 32 positive clones included 7 kinds different sequences,of which the number of RPL35 repeats was the most.The results of indirect ELISA,Dot immunoblot and Far-western blot showed that 7 representative phages could bind specifically with rMgPa.Conclusion:60S ribosomal protein L35(RPL35) may be the interacting protein of MgPa,which lays the experimental foundation for understanding the function of MgPa and the pathogenesis of Mycoplasma genitalium.

To study the antirheumatic substance of Loranthus parasiticus and observe the relationship between its in vivo distribution and meridian tropism in rats by establishing adjuvant arthritis models corresponding to effectiveness. All rats except the negative control group were injected with 0.1 mL Freund's complete adjuvant on the left foot. After 8 days, the rats in negative control group and model group were given with normal saline while the rats in positive control group were given with tripterygium glycosides suspension 10 mg•kg-1, and the rats in L. parasiticus treatment groups were given with high(10 g•kg ⁻¹), medium(5 g•kg ⁻¹) and low(2.5 g•kg ⁻¹) dose decoction for 21 days. The left rear ankle joint diameter of rats were measured every 7 days from the 9th day of modeling. On the 22nd day, eyeball blood of part rats in L. parasiticus high-dose group was taken at different time points, and then they were sacrificed to take heart, liver, spleen, lung, kidney, stomach, large intestine, small intestine and brain tissues. For the remaining rats, eyeball blood was taken 30 min after drug treatment, and their left rear ankle joints were taken to detect interleukin (IL)-1β and tumor necrosis factor (TNF)-α levels in serum by ELISA method; rutin, avicularin and quercitrin levels in the tissues of high-dose group were detected by HPLC; pharmacokinetic parameters were analyzed by using DAS 2.0. Our results showed that L. parasiticus decoction could significantly improve the paw edema situation of adjuvant arthritis model rats, and reduce IL-1β and TNF-α levels in rat serum. The in vivo efficacy substance analysis in rats showed that rutin was only present in the stomach with a small amount. AUC0-t of avicularin was stomach > small intestine > kidney, and the duration time in vivo was kidney=stomach > small intestine > lung > heart. AUC0-t of quercitrin was stomach > kidney > liver > heart > lung > spleen > small intestine > brain > large intestine > serum, and the duration time in vivo was kidney=liver=small intestine=brain=lung=spleen=heart=stomach > large intestine > serum. The research indicated that L. parasiticus decoction was effective in treating rats with adjuvant arthritis. Avicularin and quercitrin are important ingredients of L. parasiticus in antirheumatism therapy. The distribution of avicularin and quercitrin in rats were consistent with traditional understanding that L. parasiticus could attribute to the kidney and liver meridians.

OBJECTIVE: To describe the basic concepts of pharmaceutical inter-changeability and to discuss issues involved in the process of medicine substitution as well as the present situation of medicine substitution in our and other countries. METHODS: Collecting literatures on pharmaceutical inter-changeability, generic medicines and brand-name medicines, after analyzing and summarizing the information on pharmaceutical inter-changeability and medicine substitution were reviewed. RESULTS: The substitution of generic medicines with brand-name medicines involves in issues about bioequivalence, therapeutic equivalence, benefit, harm and so on. Our and other countries take appropriate measures to carry out the substitution of generic medicines with brand-name medicines. CONCLUSION: With following certain rules, generic medicines can substitute for brand-name medicines. This can be better to ensure the safety of drug use in clinical practice.

To simultaneously determine paeoniflorin, ferulic acid, prim-O-glucosylcimifugin and 4'-O-beta-glucopyranosyl-5-O-methylvisamminol in Zhengtian pills. In the test, Insertil ODS-C18 column (4.6 mm x 250 mm, 5 microm) was adopted, with acetonitrile-0.05% phosphoric acid solution as the mobile phase for gradient elution. The flow rate was 1.0 mL x min(-1), the column temperature was 30 degrees C and the detection wavelength was 230 nm. According to the results of the test, paeoniflorin, ferulic acid, prim-O-glucosylcimifugin and 4'-O-beta-glucopyranosyl-5-O-methylvisamminol showed good linear relations between peak areas and sample sizes in 11.37-170.5, 2.188-32.82, 2.896-43.44, and 3.000-45.00 mg x L(-1) (r = 0.999 9, 0.999 9, 1.000 0, 1.000 0), respectively. The average recoveries (n = 6) were 102.3% (RSD 1.2%), 99.71% (RSD 1.9%), 101.2% (RSD 1.2%), and 99.40% (RSD 2.4%), respectively. The above four components were determined in five batches of samples by using the established method, with satisfactory results. The method was so simple, accurate and highly reproducible that it could be used for quality control of the four components in Zhengtian pills.

This study is to observe the protection effect of amentoflavone (AMT) in Selaginella tamariscina against TNF-alpha-induced vascular inflammation injury of endothelial cells. On the basis of TNF-alpha induced human umbilical vein endothelial cell, observe the influence of AMT on endothelial active factor, the contents of SOD and MDA, the protein expression of vascular endothelial adhesion molecules and inflammatory factor; study the effect of its common related signal pathways such as NF-kappaB; research the effect of AMT against TNF-a induced human umbilical vein endothelial cell injury by means of MTT, ELISA, Western blotting and the cell immunofluorescence. The results showed that AMT could increase the content of NO and decrease the levels of VCAM-1, E-selectin, IL-6, IL-8 and ET-1; enhance the activity of SOD, reduce the content of MDA; downregulate the protein expressions of VCAM-1, E-selectin, NF-kappaBp65 and up-regulate IkappaBalpha, attenuate the NF-kappaBp65 transfer to cell nucleus. AMT has the effect of protect vascular endothelial and maybe via the signal pathway of NF-kappaB to down-regulate the inflammation factor and oxidative damage factor of downstream.

<p><b>OBJECTIVE</b>To investigate the nutritional status, and provide evidence for nutritional treatment option.</p><p><b>METHODS</b>A total of 452 patients with gastrointestinal cancer were selected, including 156 gastric cancer,117 colon cancer, and 180 rectal cancer. The nutritional risk screening 2002(NRS2002) was applied to grade the nutritional risk. A multi-frequency bioelectrical impedance analysis was used to measure the patients' body composition. Albumin (Alb), prealbumin(PA), transferring(Tf), retinol binding protein(RBP), red blood cell(RBC), hemoglobin (Hb), haematocrit(Hct) were measured after fasting.</p><p><b>RESULTS</b>The rate of patients with NRS2002 score more than 3 was 70.5%(110/156) for gastric cancer, 53.8%(63/117) for colon cancer, and 46.7%(86/180) for rectal cancer. The score for impaired nutritional status more than 1 for gastric cancer was higher than that for colorectal cancer(P<0.05), while patients with disease score more than 2 was less for gastric cancer(P<0.05). Body mass index(BMI), obesity degree, fat content, fat percentage, and arm circumference were lower in gastric cancer patients as compared to colorectal cancer patients(P<0.05); but protein percentage, muscle percentage, ratio of muscles of arm, and cell mass percentage were higher in gastric cancer patients(P<0.05). The proportions of patients with low Alb, PA, Tf, BC, Hb, Hct were higher for gastric cancer and colon cancer(P<0.05).</p><p><b>CONCLUSION</b>Patients with gastric cancer are prone to fat loss and therefore have a higher nutritional risk and malnutrition than those with colorectal cancer. Combination of body composition analysis and laboratory examination may achieve comprehensive evaluation of the nutritional status of patients, and provide the evidence of nutritional therapy by being combined with NRS2002 score.</p>

RESUMO

The self-renewal and differentiation of adult stem cells are closely related to their niches. Naturally, spermatogonial stem cells (SSCs) are the only adult stem cells in the body, which can transfer genetic information into the offspring. An insight into the modulation of the self-renewal and differentiation of SSCs can help elucidate the mechanisms of spermatogenesis and investigate the proliferation and differentiation of other adult stem cells. Therefore, the SSC system provides an ideal model for researches on the adult stem cell niche. More and more evidence indicates that the self-renewal and differentiation of SSCs are regulated by their niches. Based on our previous work and other related findings recently reported, this article presents an overview on the biological properties of SSC niches and their relationship with the self-renewal and differentiation of SSCs, focusing on the basic properties and components of SSC niches and various regulatory factors they produce.

<p><b>OBJECTIVE</b>To investigate the expression of RhoA and NF-κB in gastric carcinoma and their correlation with clinicopathological fearures. To determine the effective prognostic factors of long-term suivival of gastric carcinoma patients.</p><p><b>METHODS</b>The role of RhoA and NF-κB in gastric carcinoma was assessed by tissue array technology and the levels of RhoA and NF-κB expression in paraffin-embedded tissues was quantified by immunohistochemistry from 189 cases of gastric carcinoma, 54 cases of their adjacent tissues, and 32 cases of normal gastric mucosa. The prognosis of gastric carcinoma was evaluated by Kaplan-Meier survival analysis and Cox multivariate regression analysis.</p><p><b>RESULTS</b>The positive rates of RhoA expression were 84.7%, 68.5% and 65.6% in gastric carcinoma, adjacent tissues and normal mucosa, respectively. The expression of RhoA in gasric carcinoma was significantly higher than that in adjacent tissues and normal mucosa (P < 0.05). The positive rates of NF-κB expression were 75.1%, 42.6% and 15.6%% in gastric carcinoma, adjacent tissues and normal mucosa, respectively. The expression of NF-κB in gasric carcinoma was significantly higher than that in adjacent tissues and normal mucosa (P < 0.05). RhoA was positively linked with NF-κB (r = 0.203, P = 0.005). In gastric carcinoma, the expression of RhoA was related with depth of invasion (P < 0.05), and the expression of NF-κB was related with depth of invasion and lymph node metastasis (P < 0.05). The Kaplan-Meier survival analysis showed that the tumor size, lymph node metastasis, depth of invasion, expression of RhoA and NF-κB can shorten the cumulative survival rate. With these paramaters entering the Cox multivariate regression analysis mode, it was revealed that expression of NF-κB, lymph node metastasis and depth of invasion are independent prognostic factors.</p><p><b>CONCLUSIONS</b>The overexpression of RhoA and NF-κB is involved in the occurrence and development of gastric carcinoma. RhoA is positively linked with NF-κB. They are correlated with the invasion and metastasis of gastric carcinoma. The expression of NF-κB, lymph node metastasis, depth of invasion are independent prognostic factors playing an important role in prediction of the clinical outcome after radical resection of gastric carcinoma.</p>
