OBJECTIVE To analyze the influential factors for potentially inappropriate medication (PIM) in elderly cancer patients. METHODS The data of elderly cancer patients hospitalized in a hospital from January to December 2021 were collected. According to the Beers standard of the American Geriatrics Society in 2019 (hereinafter referred to as the “2019 version of Beers standard”) and Criteria for Potentially Inappropriate Drug Use in Chinese Elderly (2017 version) (hereinafter referred to as the “Chinese PIM standard”), the PIM status of elderly cancer patients was retrospectively analyzed. Multivariate Logistic regression analysis was used to identify influential factors for PIM. RESULTS A total of 293 patients were included in the study. According to the 2019 version of Beers standard, 211 patients (72.01%) had PIM, of which 204 (69.62%) had PIM related to drugs, 6 (2.05%) had PIM related to diseases or symptoms, 46 (15.70%) had PIM that should be used with caution, 32 (10.92%) had PIM with drug-drug interaction that should be avoided, and 11 (3.75%) had PIM based on renal function; the top 5 drugs in the list of incidence were proton pump inhibitors, metoclopramide, the first-generation antihistamines as promethazine, analgesics as ibuprofen and megestrol. According to the Chinese PIM standard, there were 132 patients (45.05%) with PIM, of which 119 (40.61%) had PIM related to drugs, involving 25 drugs (included 7 high-risk drugs and 18 low-risk drugs), and 24 (8.19%) with PIM in disease status; top 4 drugs in the list of incidence were promethazine, megestrol, ibuprofen and cimetidine. Multivariate Logistic regression analysis showed that compared with patients with hospital stay≤10 days, patients with hospital 20054) stay between 11 and 30 days had a higher risk of PIM [odds ratio (OR)=8.836 8, 95% confidence interval (CI) (3.217 8, 31.940 9), P=0.000 1]; compared with the patients with the 65895198。E-mail:fjman@cmpt.ac.cn number of clinical disease diagnosed≤5, patients with the number of clinical disease diagnosed≥11 had a higher risk of PIM [OR=10.930 1, 95%CI (3.000 9, 70.922 9), P=0.001 8]; compared with surgical treatment, patients receiving antineoplastic drugs had a higher risk of PIM [OR=2.209 5, 95%CI (1.180 2, 4.176 9), P=0.013 6]. CONCLUSIONS Elderly cancer patients have multiple diseases, complicated medication, and a high incidence of PIM. The length of hospital stay (11-30 d), the number of clinical disease diagnosed (≥11) and anti-tumor drugs are main influential factors for PIM in patients.

Although the development of COVID-19 vaccines has been a remarkable success, the heterogeneous individual antibody generation and decline over time are unknown and still hard to predict. In this study, blood samples were collected from 163 participants who next received two doses of an inactivated COVID-19 vaccine (CoronaVac®) at a 28-day interval. Using TMT-based proteomics, we identified 1,715 serum and 7,342 peripheral blood mononuclear cells (PBMCs) proteins. We proposed two sets of potential biomarkers (seven from serum, five from PBMCs) at baseline using machine learning, and predicted the individual seropositivity 57 days after vaccination (AUC = 0.87). Based on the four PBMC's potential biomarkers, we predicted the antibody persistence until 180 days after vaccination (AUC = 0.79). Our data highlighted characteristic hematological host responses, including altered lymphocyte migration regulation, neutrophil degranulation, and humoral immune response. This study proposed potential blood-derived protein biomarkers before vaccination for predicting heterogeneous antibody generation and decline after COVID-19 vaccination, shedding light on immunization mechanisms and individual booster shot planning.

Objective To investigate the expression of lncRNA HOTAIR, HOTAIR, CRNDE and AFAP1-AS1 in lung cancer patients with bone metastasis (LCWBM), and to elucidate the diagnostic value of lncRNAs for LCWBM. Methods Serum was collected from 38 LCWBM patients and 38 lung cancer without bone metastasis (LCWOBM) patients. Questionnaires were used to collect basic information of patients. Fasting peripheral venous blood of patients was collected to separate serum. qRT-PCR was used to measure the expression levels of four serum lncRNAs, and their diagnostic value for LCWBM was analyzed. Results The expression of serum HOTAIR was decreased in LCWBM patients, compared with LCWOBM patients (P < 0.05); the AUC of serum HOTAIR diagnosing LCWBM was 0.722 (sensitivity was 70.0%, specificity was 81.3%). And the level of serum HOTTIP was significantly increased in LCWBM patients, compared with LCWOBM patients (P < 0.05); AUC of serum HOTTIP diagnosing LCWBM was 0.784 (sensitivity was 100.0%, specificity was 45.5%). The AUC of serum HOTAIR combined with HOTTIP diagnosing LCWBM was 0.818 (sensitivity, specificity, positive predictive value and negative predictive value were 87.5%, 72.7%, 70.0% and 88.9%, respectively). Conclusion Serum lncRNA HOTAIR and HOTTIP might be potential diagnostic biomarkers for bone metastases in lung cancer patients.

Objective:To explore the effect of combining hyperbaric oxygen inhalation with wet healing in the treatment of chronic refractory ulcers of the skin.Methods:Sixty male Sprague-Dawley rats were randomly formed into a general treatment group (group A), a wet healing group (group B), and a hyperbaric oxygen + wet healing group (group C), each of 20. Chronic and refractory skin ulcers were induced on all of the rats after one week of routine feeding and 6 hours of fasting. Six courses (once daily for 10 days/course)of the three treatments were then administered. After 2, 4 and 6 courses of treatment the wound healing rate and recovery were analyzed using histopathological methods.Results:After 2, 4 and 6 courses of treatment, the average wound healing rates of groups B and C were significantly higher than group A′s average, and that of group C was significantly better than that of group B. After 6 courses of treatment, group C′s average histopathology score was significantly higher than that of group B, and those of groups B and C were both significantly higher than group A′s average.Conclusion:Single wet healing is better than common treatment for chronic skin ulcers, but hyperbaric oxygen inhalation can further improve its effectiveness.

OBJECTIVE@#To explore the clinical and genetic characteristics of a patient with 17-hydroxylase/17,20-lyase deficiency.@*METHODS@#The patient was infertile without contraception. Laboratory examination showed her chromosomal karyotype to be 46, XX. DNA sequencing was performed to detect variants of CYP17A1 gene in the patient and her family members.@*RESULTS@#Sanger sequencing revealed that the patient has carried homozygous variant c.1486C>T in the exon 8 of the CYP17A1 gene, which resulted in substitution of arginine by cysteine (p.Arg496Cys). Her family members were all heterozygotes for the same variant.@*CONCLUSION@#Homozygous variant of the CYP17A1 gene c.1486C>T probably underlay the 17-hydroxylase deficiency in this patient. Above finding has enabled accurate genetic counseling and prenatal diagnosis for her family.

Objective To investigate the effect of Pin1 novel depressor all-transretinoic acid (ATRA) in collagen-induced arthritis (CIA) mice and elucidate the underlying mechanisms.Methods CIA model mice were established,then the qualified ones were treated with ATRA and other medicines respectively.Paw thickness and volume were measured once a week in each group.The production of tumor necrosis factor-α(TNF-α) and interleukin-6 (IL-6) was detected by enzyme-linked immunosorbent assay (ELISA) in serum.Pathological changes of joints tissues were observed by hematoxylin and eosin (HE) staining.Western blot was used to detect the expression of Pin1 and nuclear factor-κB (NF-κB) between different groups.The measurement date were compared with single factor analysis of variance and independent sample t test.Results Compared with healthy control group,the paw thickness,volume and arthritis scores were significantly increased in CIA mice,compared with disease control group,joints swelling and arthritic score index were reduced in both treatment groups;TNF-α and IL-6 expression were significantly increased in CIA mice [(50.1±1.3) pg/ml,(67.6±8.3)) pg/ml] when compare with health control group [(47.1±0.6) pg/ml,(43.0± 5.2) pg/ml] (t=-9.0,P<0.01;t=-6.9,P<0.01),and both were decreased in treatment groups compare with disease control group (TNF-α,F=35.5,P<0.05;IL-6,F=17.15,P<0.01);HE staining study demonstrated that ATRA and Dexamethasone could significantly suppressthe pathological changes of joints tissues;Western blot assay demonstrated that the higher arthritis scores was,the more synovial pin1 expressed in mice,moreover ATRA and Dexamethasone decreased Pin1 and NF-κB expression in CIA mice synovial.Conclusion ATRA can successfully decrease inflammation scores and joints pathology damage in CIA mice via weaken the synovial Pin1 expression.

METHODS:All literatures were retrieved during 2005-2016 using Web of ScienceTM(called"WoS"for short)core collection as database,"Rheumatoid arthritis""Rheumatoid-arthritis""Drug therapy""Medication"as subjects,set-ting literature type as"Article""Review". Bibliometric analysis method was used to list and analyze literatures about drug therapy of rheumatoid arthritis by annual number of articles published,source journals,subjects,agencies,authors and hot issues. RE-SULTS:A total of 2 528 literatures were retrieved from WoS core collection. The number of literatures showed an overall trend of growth.Those literatures came from more than 800 journals,among which top 10 journals published 689 literatures about drug ther-apy of rheumatoid arthritis(27.25%).A total of 18 subjects were involved,and the number of literatures in"Clinical medicine"was the largest,with 1 884 literatures(74.53%).A total of 3 431 research institutions were involved,and the largest number of literatures were issued by the Harvard University,with 68 literatures in total(2.69%). A total of 14 325 researchers were in-volved,and top 10 researchers published 240 literatures about drug therapy of rheumatoid arthritis(9.49%). The frequency of 25 keywords was greater than or equal to 100 times,that of 12 was greater than or equal to 150 times."Rheumatoid-arthritis"had the highest frequency(1 740 times). In basic scientific index(ESI)database,there were 35 literatures with high citation fre-quency. CONCLUSIONS:With the literatures of rheumatoid arthritis therapy increasing year by year,the focus of research in this field is focused on the diagnosis of rheumatoid arthritis and clinical efficacy. In China,no institution or author enters the world front rank in this research field.Attention should be paid to the clinical study on drug therapy of rheumatoid arthritis.

Objective: To explore influence of nursing health education on morning blood pressure surge (MBPS) fluctuations in aged patients with hypertension.Methods: A total of 100 outpatients and inpatients with non-dipper hypertension, who haven't been treated, were selected from Apr 2011 to Sep 2015.According to random number table method, patients were randomly and equally divided into routine treatment group and health education group (received nursing health education based on routine treatment group), both groups were treated for four weeks.MBPS, trough/peak(T/P) ratio and smoothness index (SI) were measured and compared between two groups after treatment.Results: Compared with routine treatment group after four-week treatment, there were significant reductions in systolic blood pressure (SBP) morning surge[(46.95±5.17) mmHg vs.(42.21±5.19) mmHg]and diastolic blood pressure (DBP) morning surge[(16.31±3.23) mmHg vs.(14.21±3.58) mmHg], and significant rise in SBP T/P[(59.22±6.65)% vs.(70.11±6.41)%], DBP T/P[(50.91±5.04)% vs.(56.97±5.12)%], SBP SI[(1.19±0.13) vs.(1.42±0.12)]and DBP SI[(1.11±0.09) vs.(1.29±0.08)]in health education group, P<0.05 all.Conclusion: Health education can effectively improve blood pressure fluctuations and may likely improve prognosis in patients with non-dipper hypertension, which is worth extending.

Objective To investigate the effect of Montelukast Combined with Symbicort in severe asthma treatment and intervention. Methods The treatment according to different interventions will be January 2015 to January 2017 in our hospital 80 cases of severe asthma groups:control group with routine nursing intervention for a letter form treatment, the observation group with high quality nursing the intervention of montelukast Symbicort treatment; detailed records of patients in the two groups of experimental data, and the data for comparative analysis, to explore the combination of montelukast and Symbicort in severe asthma treatment and intervention effect. Results The high quality nursing intervention meng montelukast Symbicort (observation group) for the treatment of severe clinical the effect of nursing intervention in patients with asthma is better than conventional Symbicort (control group) clinical efficacy, clinical symptoms improved better than the control group, nursing satisfaction was higher than the control group, the difference was statistically significant (P< 0.05). Conclusion The high quality nursing intervention of patients with severe asthma montelukast Symbicort treatment effect significantly, can effectively improve the clinical symptoms of patients, nursing satisfaction is high, it is widely used in clinical treatment of patients with severe asthma intervention.

Objective To explore the relationship between body mass index (BMI) and frailty among community aged people. Methods A total of 1571 aged people in two communities in Zhengzhou, Henan, China were recruited by convenience sampling method. They were divided into normal group, overweight group and obesity group according to BMI, and were assessed with Fried Frailty Criteria. Results The rates of overweight and obesity were 38.6%and 10.9%, respectively. There was significant difference in the level of frailty among dif-ferent groups (χ2=86.659, P<0.001). Multivariate Logistic regression analysis showed that overweight and obesity were independent risk fac-tors of pre-frailty (P<0.05), and obesity was the independent risk factor of frailty (P<0.05). Conclusion The prevalence of overweight and obesity was higher in community aged people in Zhengzhou, and normal BMI was important to postpone the process of frailty.

Objective To explore the distribution of parvovirus B19 (HPVB19) infection in patients with leukopenia.Methods Patients who visited the Affiliated Hospital of Hangzhou Normal University from January 2015 to June 2016 were analyzed.Patients with peripheral leucocytes count less than 3.5×109/L were included in experiment group and healthy people were included in control group.HPVB19 IgG and IgM were detected by enzyme-linked immunosorbent assay, and HPVB19 DNA was detected by quantitative polymerase chain reaction.Differences in continuous data between two groups were compared with two-sample t test and those in categorical data were compared with Chi-square test.Results A total of 79 patients were included in experiment group, including 32 males and 47 females.Ages ranged from 24 to 62 years old.And 126 healthy individuals were included in control group, including 55 males and 71 females.Ages ranged from 28 to 67 years old.The positive rates of HPVB19 IgG, IgM and DNA in experiment group were 34.2%, 5.1% and 3.8%, respectively, while those in control group were 36.5%, 0 and 0, respectively.The detection rates of HPVB19 IgM and DNA between two groups were significantly different (χ2=6.507, P=0.011 and χ2=4.856, P=0.028, respectively).Sequence analysis for 3 of the HPVB19 DNA positive samples showed that there were two single nucleotide polymorphisms in VP1/VP2 sequence from one patient, which contributed to the 153rd (L/H) and 219th (N/Y) amino acids mutations, respectively.Phylogenetic analysis found that two strains belong to genotype 1a and one strain belongs to genotype 1b.Conclusions Detection rate of parvovirus HPVB19 infection (positive rates of HPVB19 IgM and DNA) in leukopenic patients is significantly higher than healthy controls.HPVB19 should be detected before considering transfusion in leukopenic patients in clinical practice.

Objective@#To understand the antiretroviral treatment (ART) discontinuation rate and its influencing factors among HIV infected patients who initiated ART between 2012 and 2015 in Wenshan prefecture, Yunnan province.@*Methods@#Demographic characteristics, baseline informations of ART, initial treatment regimens and follow-up status of a total of 4 354 patients who initiated ART from 2012 to 2015 in Wenshan prefecture were collected. A historical cohort study was used to describe the discontinuation incidence rate of ART. Life table was used to estimate cumulative retention rate and Cox proportional hazard model was used to determine the influencing factors of ART discontinuation.@*Results@#The percentage and incidence rate of discontinuation were 25.1% (1 092/4 354) and 14.53 per 100 person-years among patients who initiated ART from 2012 to 2015 in Wenshan prefecture. ART retention rates were 88%, 83%, 78%, 74% at 6th month, 12th month, 18th month, 24th month, respectively. The multivariate Cox proportional hazard regression model showed that male patients were at a higher risk of discontinuation (HR=1.24, 95%CI: 1.09-1.41) than female patients, patients aged ≥50 years were at a higher risk of discontinuation (HR=1.27, 95%CI: 1.06-1.53) than patients aged<30 years, discontinuation hazard among patients who were unmarried or divorced or widowed was 1.30 times (HR:1.30, 95%CI: 1.14-1.48) as patients who were married or cohabitation, discontinuation hazard among patients infected with HIV through injection drug use (IDU) was 1.49 times (HR:1.49, 95%CI: 1.23-1.82) as those infected through heterosexual transmission, patients with a baseline CD4 cell count<350/μl (HR=1.34, 95%CI: 1.13-1.58) or ≥500/μl (HR=1.36, 95%CI: 1.09-1.71) were at a higher risk of discontinuation than those with a baseline CD4 cell count from 350/μl to 500/μl, patients initiating ART in 2014 (HR=1.25, 95%CI: 1.04-1.50) or in 2015 (HR=1.26, 95%CI: 1.02-1.55) were at a higher risk of discontinuation than those initiating ART in 2012.@*Conclusion@#There is an uptrend for ART discontinuation rate in Wenshan prefecture. Male, 50 years or older, unmarried or divorced or widowed, transmission route as IDU, baseline CD4 count <350/μl or ≥500/μl are risk factors of ART discontinuation.

Objective To evaluate the effects of antiviral therapy in prevention of tumor recurrence after curative resection in patients with hepatitis B virus (HBV) related hepatocarcinoma. Methods The data of 78 HBV related hepatocellular carcinoma patients having underwent hepatocarcinoma curative resection were retrospectively analyzed. The patients were divided into 2 groups according to the treatment method and the serum replication level of HBV DNA: 45 patients received antiviral therapy after hepatocarcinoma curative resection (treatment group), including 28 cases of preoperative HBV DNA high replication level (HBV DNA ≥ 107 copies/L), and 17 cases of preoperative HBV DNA low replication level (HBV DNA 0.05). The tumor-free survival rates at 6, 12, 18 and 24 months after operation in patients with HBV high replication level of treatment group were 78.6%, 46.4%, 32.1%and 10.7%, and in patients with HBV low replication level of treatment group were 82.4%, 64.7%, 47.1%and 35.3%;these in patients with HBV high replication level of control group were 50.0%, 15.0%, 5.0%and 0, and in patients with HBV low replication level of control group were 92.3%, 46.2%, 30.8%and 15.4%. The tumor-free survival rates in patients with HBV high replication level of control group were significantly lower than those in patients with HBV low replication level of treatment group and patients with HBV low replication level of control group, and there were statistical differences (P0.05). Multivariate analysis result showed that preoperative serum HBV DNA ≥ 107 copies/L, lack of antiviral therapy after operation and tumor low differentiation were independent risk factors for tumor recurrence after operations (OR=1.987, 2.119 and 2.539;P<0.05 or<0.01). Conclusions The serum HBV high replication levels and lack of antiviral therapy after operation are independent factors in influencing tumor recurrence in HBV related hepatocarcinoma patients after operation. It might be better that patients with HBV high replication level should receive an antiviral therapy as early as possible after operation.

Objective To explore the influence of hyperbaric oxygen therapy (HBOT) on the learning and memory ability using a rat model of vascular dementia (VD).Methods Sixty Sprague-Dawley rats were selected and divided into a normal group,a sham operation group,a VD group and an HBOT group,each of 15 rats using a random number table.The improved four-vessel method was used to establish a VD model in all except the rats in the normal group,but for the sham operation group,the vertebral artery was not subjected to firing and the arteria carotis communis was not clipped.The HBOT group was then given HBOT for 7 days.The Morris water maze test was used to evaluate their learning and memory ability of all 4 groups.Results Compared to the normal group,the mean escape latency of the sham operation group,the VD group and the HBOT group was significantly longer in each case (P<0.05).Their platform quadrant crossing times were also significantly less,and their quadrant swimming distance as a proportion of their total swimming distance was significantly smaller (P<0.05).Compared with the sham operation group,the mean escape latency of the VD and HBOT groups was significantly longer (P<0.05),the times of crossing platform quadrant were less and their quadrant swimming distance was a smaller proportion of their total swimming distance.As compared with the VD group,the mean escape latency of the HBOT group was significantly shorter [(25.73±6.20) s],the average platform quadrant crossing times were significantly more [(9.51±2.25) times] and the platform quadrant swimming distance of the HBOT group was significantly longer [(494.72±208.26)],representing a significantly larger proportion of the total swimming distance [(28.71 ± 5.32) %] (P<0.05).Conclusion HBOT can effectively improve learning and memory in vascular dementia,at least in rats.

To investigate the outcome of locoregionally advanced nasopharyngeal carcinoma [NPC] treated with induction chemotherapy followed by chemoradiotherapy. Between June 2005 and October 2007, 604 patients with locoregionally advanced NPC were analyzed, of whom 399 and 205 were treated with conventional radiotherapy and intensity-modulated radiotherapy [IMRT] respectively. Meanwhile, 153 patients received concurrent chemotherapy, and 520 were given induction chemotherapy. With a median follow-up time of 65 months, the 3-, and 5-year overall survival [OS], locoregional free survival [LRFS], and distant-metastasis free survival [DMFS] rates were 82.5% vs. 72.6%, 90.6% vs. 87.1%, and 82.5% vs. 81.2%, respectively. Induction chemotherapy was not an independent prognostic factor for OS [P=0.193] or LRFS, but there was a positive tendency for DMFS [P=0.088]. GP regimen [gemcitabine + cisplatin] was an independent prognostic factor for OS [P = 0.038] and it had a trend toward improved DMFS [P = 0.109]. TP regimen [taxol + cisplatin] was only a significant prognostic factor for DMFS [P =0.038]. Adding induction chemotherapy had no survival benefit, but GP regimen benefited overall survival and had a trend toward improved DMFS. GP regimen may be superior to TP/FP regimen [fluorouracil + cisplatin] in treating locoregionally advanced NPC

RESUMO

This infant cereal chiefly consisted of rice meal, fullfat milk powder, rice germ and dried yeast, and fortified with ferrous lactate and calcium carbonate. This mixture contained 14.1% protein, 8.6% fat, 64.3% carbohydrate, 750mg% calcium, 10mg% iron, 1.0mg% zinc and proper amount of various kinds of vitamins.1. Growth test; 84 weaning SD rats were randomly divided into 6 groups:(A) Infant cereal group;(B) Control I; (C) Control II; (A') Infant cereal group; (B') Control I; (D) Casein group. The latter three groups contained 10% protein which were designed for calculating PER. All these different preparations were fed to 6 groups for 4 weeks. The results showed that the body weight gain, the FE, CE and PER were not significantly different between group A and B but there were significant difference between group A and C. The Hb was increased from 11.76g% ( ) and 11.46g%() to 14.30g% and 14.46g% respectively in group A. The bones developed well and no decalcification occured.2.Nitrogen balance test:(A). Infant cereal group; (B) Control I. The results indicated that the TD, BV and NPU of group A were 98.08%, 85.73% and 84.09% and of group B were 97.81%, 87.51% and 85.59% respectively. Both groups obtained the results of positive nitrogen balance, no significant statistical difference. The contents of hepatic and body nitrogen of two groups were also not significantly different.These data demonstrate that this infant cereal is able to promote the growth of weanling rats, to retain positive body nitrogen and to prevent the occurrence of anemia and rickets.

Baby cake'' is a kind of baby food which has been traditionally used in certain parts of China. It consists half of rice powder and half of flour. The lysine is the first limiting amino acid of "baby cake'' and it is also deficient in calcium.In order to improve the nutritional value of "baby cake", we enriched it with 0.25% lysine and 0.122% calcium.72 Wistar derived rats were devided into 3 groups: group A was fed with "baby cake" enriched with 0.25% lysine, group B enriched with 0.25% lysine and 0.122% calcium and group C with original "baby cake" as control.The results revealed: body weight, weight and length and density of bones all were better in enriched groups and the best was in the group enriched with lysine and calcium. All were statistically significant.
