RESUMO

Objective To explore the efficacy of tigecycline combined with cefoperazone/sulbactam in the treatment of infections due to multiple drug resistant strains and pandrug-resistant acinetobacter baumannii, so as to guide the reasonable clinical medication.Methods A total of 16 cases of ventilator associated pneumonia caused by multiple drug resistant strains and pandrug-resistant acinetobacter baumannii treated in the First Hospital of Shijiazhuang from November 2012 to November 2014 were retrospectively analyzed, and the severity of the infection, clinical efficacy and mortality were observed.Results The multiple drug resistant strains and pandrugresistant acinetobacter baumannii were frequently detected in the 16 patients.Fifteen cases had been used other antibiotics before tigecycline, such as imipenem, cefoperazone/Shubatan, minocycline etc.The severity of underlying disease was assessed with the acute physiology and chronic health score(APACHE Ⅱ sore) within 24 h of admission, on the first day of tigecycline (TGC) therapy and after 7 days of TGC therapy.It showed that the average APACHE Ⅱ score were (25±6.0), (24.2±6.4) and (17.8±6.6) within 24 hours of admission(P<0.01), on the first day of TGC therapy and after 7 days of TGC therapy.Thirty days after application of the TGC, the bacterial eradication rate was 56.25% (9/16).The effective rate was 87.5% (14/16).The failure rate was 12.5% (2/16).Conclusion The effect of the tigecycline combined with cefoperazone/sulbactam on the clearance of the multiple drug resistant strains and pandrug-resistant acinetobacter baumanniiis is satisfied.

RESUMO

Objective To investigate whether miR-200a suppresses cell proliferation by targeting AP-2γexpression, and reveal molecular mechanism that miR-200a functions as a tumor-suppressor in neuroblastoma cells. Methods Dual-luciferase reporter gene assay was employed to examine the effect of miR-200a on AP-2γpromotor luciferase activity. Neu-roblastoma cells were transfected with miR-200a mimics, and the expressions of AP-2γmRNA and protein were detected by RT-PCR and Western blot assay. The effects of AP-2γdown-regulation on cell proliferation were observed after AP-2γshRNA was transfected into neuroblastoma cells. Neuroblastoma cell proliferation was detected by MTS assay after being co-transfected with miR-200a mimics and AP-2γplasmid. Results Results showed that miR-200a could inhibit proliferation of neuroblastoma cells at cell viability (66.33 ± 5.13) compared with that of control group (100 ± 0), and also miR-200a can bind to the 3'untranslated region of AP-2γpromotor and inhibit its luciferase activity with an inhibit ratio at (0.624±0.051). AP-2γmRNA and protein expressions were significantly down-regulated when miR-200a was over-expressed in neuroblas-toma cells. Furthermore, results showed that shRNA-mediated down-regulation of AP-2γthat suppressed the cell prolifera-tion of neuroblastoma at (62.5±2.4) by comparing with the control group (100±0). Moreover, restoring AP-2γexpression re-versed the effect of miR-200a with a cell viability suppression at (92.4±1.4). Conclusion miR-200a suppresses cell prolif-eration by targeting AP-2γexpression in neuroblastoma cells.

BACKGROUND:Adipose-derived mesenchymal stem cels have rich sources that are easily obtained, which can be used to treat acute myocardial infarction. OBJECTIVE: To investigate the therapeutic effect of adipose-derived mesenchymal stem cel transplantation on acute myocardial infarction in rats. METHODS:Rat models of acute myocardial infarction were made and subjected to adipose-derived mesenchymal stem cel transplantation in comparison with model and control (sham operation) groups. RESULTS AND CONCLUSION:Echocardiography findings showed significant improvement in the left ventricular end-systolic diameter, left ventricular end-diastolic diameter and ejection fraction in the cel transplantation group compared with the model group (P < 0.05). Hematoxylin-eosin staining showed that myocardial infarction was evident in the model group, in which, there were rarely viable myocardial tissues and few vessels in the infarcted region, but in the cel transplantation group, there were evident survived myocardial tissues and transplanted cels. The percentage of infarct size was significantly lower in the cel transplantation group than the model group (P < 0.05). Immunohistochemical staining showed that adipose-derived mesenchymal stem cels were able to survive in the infarcted myocardial tissues, and the expression of cardiac troponin T in the cel transplantation group was significantly higher than that in the model group (P < 0.05). Experimental data show that adipose-derived mesenchymal stem cel transplantation can protect the myocardial tissues after myocardial infarction, and effectively improve the myocardial function.

Objective To determine the dose?response relationship of bupivacaine coadministered with sufentanil for subarachnoid block in severely preeclamptic patients undergoing cesarean delivery. Methods Two hundred patients with severe preeclampsia, of American Society of Anesthesiologists physi?cal statusⅠ?Ⅲ, scheduled for elective cesarean delivery, were divided into 4 groups ( n=50 each) using a random number table: bupivacaine 4 mg group ( group B4 ) , bupivacaine 6 mg group ( group B6 ) , bupivacaine 8 mg group ( group B8 ) , and bupivacaine 10 mg group ( group B10 ) . In B4 , B6 , B8 and B10 groups, bupivacaine 4, 6, 8 and 10 mg plus 2.5μg sufentanil in 2.5 ml of normal saline were injected into the subarachnoid space, respectively. Effective anesthesia was defined as bilateral sensory block of T6 achieved at 10 min after intrathecal administration when measured by pin?prick test, and with no need for epidural supplementation ( lidocaine ) . A probit analysis was used to estimate the 50% effective dose ( ED50 ) and 95% effective dose ( ED95 ) with 95% confidence intervals for bupivacaine, coadministered with sufentanil, when used for subarachnoid block in severely preeclamptic patients underwent cesarean de?livery. Results When coadministered with sufentanil, the ED50 and ED95 ( 95% confidence interval) of bupivacaine were 5.67 ( 5. 20-6. 10) mg and 8. 82 ( 8. 14-9.87) mg, respectively, for subarachnoid block in severely preeclamptic patients underwent cesarean delivery. Conclusion The ED50 and ED95 of bupivacaine for subarachnoid block, when coadministered with sufentanil 2.5 μg, are 5.67 and 8.82 mg, respectively, in severely preeclamptic patients undergoing cesarean delivery.

Objective To explore the expression and clinical signiifcance of microRNA-200a in childhood B-cell acute lymphoblastic leukemia (ALL). Methods Bone marrow samples were collected from 45 children with B-cell ALL and 18 chil-dren without hematology disease as control. Total RNA was acquired from bone marrow. qRT-PCR was performed to detect the expression level of miR-200a. Results The relative expression level of miR-200a in B-cell ALL group was signiifcantly lower than that in control group (P<0.05);the expression of miR-200a in children over 10 years old was signiifcantly lower than those in children under 10 years old (P<0.05);the expression of miR-200a in newly diagnosed samples was lower than those in those samples taken on Day 33 and at Week 12, respectively (P<0.05, P<0.01). In addition, the expression of miR-200a in low-risk group was higher than those in mid-risk and high-risk group, respectively (P<0.05). Conclusions Low level of miR-200a had a close correlation with the development and prognosis of childhood B cell ALL, which could be used as a potential target of thera-py and a biomarker of childhood B cell ALL in the future.

Background and purpose:MicroRNAs are 19 to 25-nucleotide noncoding RNA molecules. The aim of this article was to investigate the expression and clinical signiifcance of microRNA-141 in childhood B-cell acute lymphoblastic leukemia (ALL). Methods:Bone marrow samples were collected from 35 children with B-cell ALL and 15 children with non hematologic disease. Total RNA was acquired from bone marrow. Real-time PCR was performed to detect the expression level of miR-141. Results:The relative expression level of miR-141 in B-cell ALL group was remarkably lower than those in the control (P<0.05). The expression of miR-141 in newly diagnosed samples was lower than those in Day 30 and Week 12 samples respectively (P<0.05). Besides, the expression of miR-141 in Day 30 samples was lower than those in week 12 samples (P<0.05). The expression of miR-141 in children over 10 years old was signiifcantly lower than those in children under 10 years old (P<0.05). The expression of miR-141 in low-risk group was higher than those in mid-risk and high-risk group respectively (P<0.05), and there was signiifcant difference between mid-risk and high-risk groups (P<0.05). Conclusion:MiR-141 is likely to have tumor suppressor effect and to be a potential prognostic biomarker in childhood B cell ALL.

Objective To evaluate the reliability of Pain Vision method for assessment of labor pain.Methods Eighty-nine nulliparous parturients who were at full term and at the latent period of the first stage of labor,aged 19-31 yr,weighing 55-85 kg,were enrolled in this study.The degree of labor pain was assessed using Pain Vision method and VAS.Pain degree is calculated from two parameters,current perception threshold and pain compatible electrical current by using a somatosensory evoked potential stimulator.The former parameter was defined by the lowest electrical current detected; the latter parameter defined by the electrical current judged as being compatible with the intensity of ongoing pain.Linear correlation of VAS scores with pain degree was analyzed.Results There was no correlation between pain degree and VAS scores,and the correlation coefficient was 0.206 (P > 0.05).Conclusion Pain Vision method can not be applied for objective assessment of labor pain.

Objective To compare the efficacy of laryngeal tube suction Ⅱ (LTS Ⅱ ) and ProSeal laryngeal mask airway (PLMA) for airway management during general anesthesia. Methods One hundred and twenty adult ASA Ⅰ or Ⅱ female patients, aged 30-50, with body mass index < 30 kg/m2, undergoing general anesthesia for elective surgery were randomly divided into 2 groups ( n = 60 each) : PLMA group and LTS Ⅱ group. Each group was further divided into 2 subgroups: PLMA controlled ventilation group (group PC), PLMA spontaneous breathing group (group PS), LTS Ⅱ controlled ventilation group (group LC) and LTS Ⅱ spontaneous breathing group (group LS) . The rate of successful insertion was recorded. PetCO2 , peak airway pressure, lung compliance and incidence of gas leakage during controlled ventilation were recorded before operation, 10 min after the start of operation and at the end of operation. The tidal volume and PetCO2 were recorded in patients breathing spontaneously when the breathing was stable. The bucking and body movement were observed during removal of LTS Ⅱ or PLMA. The side effects in 24 h after surgery were recorded.Results Insertion was successful in all the patients. During either spontaneous breathing or controlled ventilation, the peak airway pressure was significantly higher in the patients with LTS Ⅱ than in the patients with PLMA ( P < 0.05), and there wag no significant difference in the other ventilatory parameters between the two devices. There was no significant difference in the incidences of postoperative complications among the groups ( P > 0.05) . Conclusion The efficacy of PLMA for airway manage ment is better than that of LTSⅡ during general anesthesia.

Objective To investigate the effect of perhexiline injection on oxygen free radical in old patients with heart failure(CHF). Methods 32 healthy parsons were taken as controls. 58 old patients with CHF were ran-domly divided into two groups. The treatment group received both basic therapy and perhexiline injection transfusion for 14 days. Malondialdehyde (MDA) ,total antioxygen capacity(TAOC) ,oxidation-low-density-lipoprotein-cholester-ol (OX-LDL-C) and superoxide dismutase(SOD) were measured before the therapy and after the therapy. Results MDA [ (5.59±1.95 )μmol/L and OX-LDL-C [ ( 808.64 ± 156.34) μg/L] were significantly higher in the old patients with CHF] than control group [ (4.04 ± 0.67 μmol/L ) and ( 538.67 ± 125.43 ) μ/L ] ( P < 0.05 ) ; SOD [ (68.69 ± 20.43 )μU/L ] and TAOC [ ( 7.72±2.13 ) kU/L ] were significantly lower in the old patients with heart failure than the matched group [(105.67±13.86 )μU/L and (9.68±1.86 ) kU/L ] ( P <0.05 ), which were posi-tively associated with CHF severity. After the improvement of the cardiac function, MDA, OX-LDL-C;SOD and TAOC recovered significantly. Conclusion The therapeutic action of perhexiline injection in old patients with CHF can re-ly on the promotion of eliminating oxygen free radical inside organism.

Objective To investigate the significance of urinary telomerase activity (TA) in the diagnosis and recurrence screening of bladder carcinoma. Methods TA in urine of 46 cases of superficial bladder carcinoma and 46 normal subjects was estimated by the method of PCR ELISA, and it was monitored continuously in 20 cases after transurethral resection of tumors. Results TA in urine of patients with bladder carcinoma increased significantly as compared with the controls.It dropped to normal range after tumor removal, but rose again with tumor recurrence.The specificity and sensitivity of TA detection were 93.5% and 47.8% respectively. TA was positively related to the grade of tumors whereas there was no association between the preoperative value of TA and early recurrence. Conclusions The examination of TA in urine is helpful for the diagnosis of bladder carcinoma and it is related to the differentiation degree of tumors.It may become an important supplement to cystoscopy in the screening of tumor recurrence.

Objective: To investigate whether Vigconic VI-28 capsule, a formulated traditional Chinese medicine containing radix Ginseng, cornu cervi pantotrichum and semen cuscutae, can assist tumor chemotherapy in a mouse model. Methods: Female C57BL/6 mice were s. c. injected with viable Lewis lung cancer ( LLC) cells (106 cells/mouse) . The mice were then treated with cyclosposphamide (Cyp, 40 mg/kg bodyweight, once every other day). One group was intra-gastrically given 2% VI-28 (0.5 ml/mouse, every other day) during the course of the chemotherapy. By day 28, the mice were sacrificed and their thymic indices and tumor indices were calculated and compared. Splenocytes were collected for analysis of their immunological status. Histological study was carried to examine the solid tumors. Results: Fourteen days after the injection of LLC cells, solid tumors developed in most of the animals, reaching 1 ~ 1.8 cm diameters by 28 th day Compared with mice of the LCC + Cyp group, thymus glands from the LLC + Cyp + VI-28 group were significantly heavier. Splenocytes of the same group responded better to ConA stimulation in vitro. Histochemical examination of the tumor tissues revealed that tumors of the Cyp + VI-28 group were better differentiated (less aggressive) than that of the Cyp group. Conclusion: Vigconic VI-28 capsule can promote recovery of immune system in mice undergoing chemotherapy and help Cyp to control the growth of tumor cells in vivo.

AIM: To examine the electrophysiological characteristics of transient outward potassium current(Ito 1) in repolarization 1 phase from the canine right ventricular M cells. METHODS: By use of whole cell patch-clamp technique, we quantitatively researched the ionic intensity, density of Ito 1 and the notch magnitude of action potential in repolarization 1 phase. RESULTS: (1) The activating process of Ito 1 of canine right ventricular M cells presented evident voltage-dependency.
