Study on the clinical effect of minimally invasive mastectomy with Mammotome in the treatment of breast benign tumor / 中国基层医药

Objective:To study the clinical effect of Mammotome minimally invasive breast circumcision in the treatment of breast cancer.Methods:A prospective study was carried out in 800 breast benign tumor patients admitted to the People's Hospital of Anji County from January 2017 to December 2018.The patients were divided into control group(400 cases) and observation group(400 cases) according to the random digital table method.The control group received open breast tumor resection, and the observation group received minimally invasive mastectomy.The postoperative complications, recovery time, aesthetic effect of breast appearance, serum inflammatory factors and cellular immune function were compared between the two groups.Results:The operation time and bleeding volume in the observation group were (17.59±8.23)min, (23.57±7.64)mL, respectively, which were lower than those in the control group [(29.83±11.91)min, (54.28±17.59)mL]( t=16.910, 30.027, all P<0.05). The incidence of postoperative complications in the observation group was 1.50%(6/400), which was lower than 5.75%(23/400) in the control group(χ 2=10.340, P<0.05). The time of incision healing and hospitalization in the observation group were (4.07±1.39)d, (5.58±2.31)d, respectively, which were shorter than those in the control group [(5.56±1.75)d, (7.92±2.78)d]( t=13.334, 12.948, all P<0.05). The excellent and good rate of breast appearance aesthetics in the observation group was 97.25%(389/400), which was higher than 89.25%(357/400) in the control group(χ 2=20.336, P<0.05). After operation, the serum levels of inflammatory factors in the observation group were lower than those in the control group( t=17.360, 14.717, 14.109, all P<0.05). CD 3+ and CD 4+ /CD 8+ in the two groups were lower than before operation(control group: t=20.788, 16.121; observation group: t=11.616, 8.348, all P<0.05), and CD 3+ and CD 4+ /CD 8+ in the observation group were higher than those in the control group( t=10.655, 7.328, all P<0.05). Conclusion:Mammotome minimally invasive mastectomy can effectively reduce the surgical trauma of breast cancer patients, reduce the postoperative complications, reduce the impact on their cellular immune function, and help to speed up the recovery and improve the appearance of breast.

Due to the improvement of infant survival rates,Candidas have been proved to be the third most common pathogen of late-onset sepsis in NICU,and invasive fungal infection of high-risk infants is in-creasing attention. As the diagnosis is difficult,treatment is often delayed,high mortality and severe disability are also caused,it's becoming a research hot spot to assess whether antifungal prophylaxis is beneficial. Now a number of studies have been performed to discuss the prophylactic role of fluconazole and nystatin,but has not yet reached a consensus. This review described the influence of chemoprophylaxis on fungal infection, colonization and drug resistance.

Application of narcotic analgesic in 29 hospitals of Nanjing from 2010 to 2012 / 药学实践杂志

Objective To evaluate the current situation and trend of narcotic analgesics used in 29 hospitals of Nanjing from 2010 to 2012 and provide references for reasonable clinical drug utility .Methods Consumption of narcotic analgesics in 29 hospitals of Nanjing was analyzed retrospectively in terms of sales value , DDDs, DDC, etc.The DDDs, sales value and variety of narcotic anal-gesics were analyzed statistically according to the level , type and district of hospitals .Results There were a total of 13 kinds of nar-cotic analgesics in the 29 hospitals and the sales value increased yearly .During the 3 years,the DDDs of fentanyl citrate injection ,fent-anyl transdermal system and remifentanil hydrochloride were ranked in the forefront and the DDDs rank of controlled -release morphine and controlled-release oxycodone hydrochloride was stable .The sales value and DDDs in Drum Tower District were greater than other districts and increased yearly .Conclusion The application of narcotics analgesic in 29 hospitals of Nanjing was rational and complied with the relevant national regulations .

Effect of n-butanol Extract from Potentilla anserina on Hypoxiainduced Calcium Overload and SERCA2 Expression of Rat Cardiomyocytes / 中草药·英文版

ObjectiveTo investigate the effect of n-butanol extract from Potentilla anserina (NP) intervention on hypoxia-induced Ca2+ overload and SERCA2 expression of rat cardiomyocytes.MethodsPrimary cultured myocardial cell from SD neonatal rat (1-3 d) was used in the establishment of hypoxia model.After hypoxia for 3 h,the Ca2+ concentration of myocardial cells was measured with fura-2/AM fluorescent probe,and the biochemical indicator intracellular Ca2+-ATPase was examined and the mRNA and its protective protein levels of the sarcoplasmic reticulum (SR) Ca2+-ATPases (SERCA2) were assayed with RT-PCR,Western-blotting,and immune-cytochemical staining in each group.ResultsThe results showed that NP decreased Ca2+ concentration,increased the activity of Ca2+-ATPase,and improved the mRNA and protein expression of SERCA2 in hypoxia-injured myocardial cells as compared with the model group.ConclusionThese results indicate that NP could attenuate the Ca2+ overload.The mechanism might be explained as that NP could elevate the SERCA2 level,increase the activity of myocardium in rats,and further enhance the capacity of SR Ca2+ re-uptake.

Objective To investigate the change and effect of SSeCKS(src suppressed c kinase substrates)in the activation of hepatic stellate cells(HSCs).Methods HSCs were isolated from normal rats,the change of SSeCKS mRNA expression on HSCs culture in vitro was determined using real.time PCR.protein level was determined by Western blot and immunofluorescence methods.A rat model of liver fibrosis was established.The expression and location of SSeCKS and α-SMA(α-smooth muscle actin)in liver tissues were detected by immunofluorescence methods.Results SSeCKS mRNA expression WaS loW in freshly isolated HSCs cell and the expression increased in activated HSCs in vitro.In liver fibrosis tissue,the number of SSeCKS-positive cells was increased and these cells were distributed along the sinusoids which also contained α-SMA positive cells.Conclusion The expression of SSeCKS was increased in activated HSCs in vitro.Therefore.SSeCKS may be involved in the liver inflammation and fibrosis.

Objective To study the relation between Child Turcotte classification and pathology, diagonsis, prognosis and fibrosis index as well as its significance. Methods The levels of hyaluronic acid(HA), type Ⅲ procollagen(PCⅢ), Laminin(LN) and type Ⅳ collagen(Ⅳ C) were detected by enzyme linked immunoadsordent assay(ELISA) or radioimmunoassay(RIA). Pathomorphology was observed in 68 patients with cirrhosis. Results Level of HA in serum was positively correlated with cirrhotic severity, but other fibrosis indexes could not reflect cirrhotic severity. Child Turcotte classification was concordant with the pathological changes. The concordance rate of Child Turcotte classification B or C patients with pathologic diagnosis was up to 97.8%. The recovery rate of Child Turcotte classification A patients was up to 95.5%. Fatality rate of Child Turcotte classification C patients was up to 96.9%. Conclusion Child Turcotte classification is closely correlated with the severity of hepatic fibrosis, severity of pathological changes in liver and prognosis of patients. It is of clinical value in the reflection of severity of hepatic cirrhosis.

Determination of schisandrin,schisandrol B,deoxyschizandrin,and schisandrin B in serum containing drug of Compound Wurenchun Capsula / 中草药

Objective To determine schisandrin,schisandrol B,deoxyschizandrin,and schisandrin B in serum containing drug of Compound Wurenchun Capsula.Methods An HPLC method was set up.Li-chrosphere C18 column(250 mm ?4.6 mm,5 ?m) and Phenomenex Description C18(4.0 mm?3.0 mm)protective column were used.Acetonitrile-water was used as gradient mobile phase.The flow rate was 1.0 mL/min.The column temperature was 30 ℃ and the detection wavelength was 210 nm.Results The linear ranges of schisandrin,schisandrol B,deoxyschizandrin,and schisandrin B were within 0.051 2-0.768 0 ?g(r=0.999 5),0.054 0-0.810 0 ?g(r=0.999 6),0.012 3-0.184 5 ?g(r=0.999 8),and 0.039 8-0.597 0 ?g(r=0.999 6),respectively.The average concentration of these four lignans in serum containing drug were 8.021 1,6.231 0,0.530 8,and 5.851 0 ?g/mL,respectively.Conclusion This method is easy,sensitive,specific,and accurate for the assaying of the four lignans in serum containing drug of Compound Wurenchun Capsula.

Comparison of schisandrin and schisandrin B in rat serum and plasma after ig Compound Wurenchun Capsules / 中草药

Objective To compare the content of main constituents of Compound Wurenchun Capsules dissolved in serum and plasma of rats. Methods Serum and plasma containing drug were prepared after ig the preparation to rats. Lichrosphere C_ 18 (250 mm?4.6 mm, 5 ?m) column and Phenomenex Description C_ 18 (4.0 mm?3.0 mm) protective column were used. The mobile phase consisted of methanol-water, eluted in gradient mode. The flow rate was 1.0 mL/min. The column temperature was 30 ℃ and detection wavelength was 254 nm. Results The linear ranges of schisandrin and schisandrin B were 0.051 2 .614 4 and 0.039 8—0.477 6 ?g. The average relative recoveries of schinsandrin and schisandrin B were 96.72%, 101.06%, 102.05%, and 99.03%, 100.18%, 100.28% in low, middle, and high concentrations, respectively. The average contents of schisandrin in serum and plasma were 11.063 2 and 12.883 7 ?g/mL, schisandrin B were 7.490 9 and 12.590 8 ?g/mL, respectively. Conclusion The main constitu-ents of Compound Wurenchun Capsules contained in plasma account for higher than the ones in serum.
