RESUMO

Vertigo is the most common complaints of patients who refer to physician and menier's disease is one of the most common causes of it. Diagnose of menier's disease depend on clinical finding. Vestibular tests are affect the treatment method and prognosis of disease, to differentiate it from other causes of vertigo as well. In this study we performed evaluation and analysis of videonystagmoghrapy [VNG] finding in menier's disease. Total 61 patients with menier's disease referred to ENT outpatient department of Imam Khomeini and Apadana Hospitals were evaluated with VNG and Sacade, Smooth pursuit, Optokinetic, Positional and caloric tests. SPSS 16 software for data analysis was used. Directional preponderance was found in 11.44%, unilateral weakness in 39.33% and Latency of sacad in 42.6% with abnormal results. There was no significant correlation between severity of vertigo, severity of hearing loss, duration of Disease and videonystagmography finding. Unilateral weakness was most important finding in this study

RESUMO

Tinnitus is defined as the perception of sound in the absence of any appropriate external stimulation. About 1-2% of the population are severely disturbed by tinnitus and it may disrupt daily activities and sleep. Several treatments are applied for this disorder and one of these treatments is acupuncture. The aim of this study was to finding scientific evidence for efficacy of acupuncture on this disorder that is resistance to conventional treatment modalities. This study is a clinical trial. In this study, 50 patients with resistance tinnitus to conventional treatment who referred to ENT ward of Emam Khomeini university hospital, Ahvaz, were chosen and divided to two groups [each 25 cases]. The first group was used as treatment group [with acupuncture] and the other control group. Tinnitus Handicap Inventory questioners were completed for both groups before and after the allocated treatment period. All of the 25 treatment group cases underwent daily treatment for 15 secessions of 30 minutes each. The reasults analysis with SPSS software. In case and control groups, the males were 56%, 52% and the females were 44%, 48%. The mean period of disorder in case and control groups were 5.8 and 5.6 years respectively. The results of THI were according to assessment quantity and quality. In case group, there had significant difference before and after treatment [P=0.03]. Also, the case group after treatment and group control had significant difference [P=0.02]. According to this finding, it is thought, the acupuncture is useful for treatment of tinnitus, especially in this condition that there is no proven treatment for it. More studies are necessary to find the better management methods

RESUMO

Rubella virus is a potent teratogen. Risk of rubella virus induced multisystem fetal malformation is highly dependent on the gestational age at the time of infection. Because the rubella vaccine is an attenuated live virus, its use is contraindicated during pregnancy. Our objective was to examine whether exposure to rubella vaccine during 1-4 weeks periconceptional period can cause any adverse pregnancy outcome or Congenital Rubella Syndrome [CRS]. This prospective study was performed in 60 pregnant women who received rubella vaccine inadvertently 1-4 wks pre or post conception. Time of conception was determined by last menstrual period [LMP] and first trimester sonography. In addition to gathering mother's obstetric and demographic information, all neonates were evaluated for CRS signs by systemic physical examination and anti rubella IgG and IgM antibody levels in cord blood samples. Mean maternal age was 21.8 +/- 2.9 years and 58.3% of pregnancies were unintended. In 90% of mothers there were no post vaccination side effects. None of the mothers had a history of drug abuse, smoking or teratogenic exposures. Mean neonatal weight was 3108 +/- 581 gr and 6.7% of them were premature. There was no increase in the rate of adverse pregnancy outcome, [abortion, preterm labour, post term pregnancy, fetal demise, obstetric hemorrhage, pregnancy related major malformation]. No signs of CRS were found in the neonates based on systemic physical exam. Mean value of cord blood anti-rubella IgG level was 143.9 +/- 69.5 IU/mL. Cord blood anti-rubella IgM was negative in all of the neonates. It seems that inadvertent rubella vaccination 1-4 weeks before and after conception does not cause CRS in neonate and therapeutic abortion is not indicated; however since in rare instances clinical manifestations of rubella have appeared years after the congenital infection, women planning to get pregnant should not be vaccinated against rubella one month before and throughout pregnancy

RESUMO

Because of the density of deep critical structures in neck and special anatomy, the choice of treatment and evaluation for penetrating neck trauma still remains controversial. This is a retrospective and descriptive study carried out in Imam Khomeini hospital over 5 years period on 65 patients with penetrating neck trauma. Demographic data of patients were investigated by using descriptive method. According to this study, penetrating neck trauma is more seen among young males injured with stab wounds in zone II of the neck. All negative surgical cases are due to injuries from stab wounds. On the basis of information gathered, damage to major neck vessels is the most usual injury and the only cause of death. In all patients who had evidence of damage in diagnostic evaluation, the problem also found during operation. Patients who didn't have any evidence of injury in diagnostic evaluations and didn't undergo surgery, discharged without any complication. While evaluating and treating patients with penetrating neck trauma in situations without critical conditions and available diagnostic instruments, using them because of their accuracy specially for areas with complicated anatomy for surgery [zone I, III] and also using continuous clinical examination in vulnerable neck areas [zone II] is safe and practical

RESUMO

Hemangioma of the nasal cavity has two types:


Mucosal and intraosseous. Intraosseous hemangioma is less common than mucosal type. Since this lesion is similar to other non hemorrhagic lesions, it may be mistaken. Therefore, primary consultations are important in preoperavtive preparation and operative planning we present a 33-year oldfemale patient with bulging of the left side of her nose and nasal obstracion of the same side. Based on clinnical and paraclinical evalutions, intraosseous cavernous hemangioma of the middle turbinate is diagnosed. According to our refereneses we were not able to find previous reported case of intraosseus hemangioma of the middle turbinate
