RESUMO

Ankle arthrodesis was done for 17 patients divided into two groups. The first group consisted of ten patients treated by an open technique, and the second group included seven patients managed by arthroscopic assessed ankle fusion. The details of the patients, surgical techniques and the results were described in this study. In the second group, postoperative pain was much less and healing time was shorter than the first group. The arthroscopic technique is less invasive but there is a learning curve associated with the maneuver, where severe joint deformity is considered a contraindication for arthroscopic fusion

RESUMO

The operative technique for ACL reconstruction has evolved from the traditional arthrotomy to the arthroscopic-assisted reconstruction and to the latest technique, which is the totally arthroscopic reconstruction. The aim of this work was to analyze the results of a totally arthroscopic ACL technique by patellar tendon autograft with an accelerated postoperative rehabilitation program. Between March 1997 and October 2000, a prospective study of 54 patients suffering from knee instability due to anterior cruciate injury was done in Assiut University Hospital. The patients were 53 males und one female with the age range of 18 years [average 30 years]. Four patients had a previous arthroscopic partial meniscectomy and three patients had a previous failed reconstruction of the ACL. The follow up continued until October 2000 with an average of 24 months period [range 9-42 months]. Out of the 54 patients included in the study, the results of 9 patients were deducted because they were lost to follow up. The overall results were satisfactory in 39 knees and unsatisfactory in 6 knees with 34 patients returned to their pre-injury level of activity. The Lysholm score improved from a preoperative mean of 59 to a postoperative mean of 91. Radiological assessment of tunnels placement revealed the tibial tunnel was 43.11 +/- 4.98%. The tibial tunnel position was found to be posterior in 27 patients, central in 12 patients and anterior in 6 patients. Tibia1 tunnel widening [0.5 - 2 mm] occurred in 7 patients, 3-6 months postoperatively. Arthroscopic ACL reconstruction with bone-patellar tendon-bone autograft with an accelerated postoperative rehabilitation program in young adults leads to reasonable stability and good overall function of the knee joint

RESUMO

Pyogenic arthritis of the knee was treated in 28 patients by arthroscopic decompression, debridement and irrigation with motorized instruments plus suction drainage coupled with parenteral and oral antibiotics. Patients were treated at the Orthopedic Departments of Assiut and Menia University from 1992 to 1996. This series consisted of 21 males and 7 females patients whose ages ranged from 8 to 62 years. The average follow up period was 9.1 months. They were twelve cases of hematogenous arthritis, eleven arthritis secondary to foreign bodies punctures and infiltration and five postoperative arthritis. They were nineteen positive cultures. At the end of follow up period, there were twenty-two cures, five failures due to persistent articular sepsis and one recurrence of infection after an initial remission. The average hospital stay was 3.5 days. Different parameters were used in evaluating the quality of the results, the etiology of the arthritis, the causal germ and the delay prior to arthroscopy. Arthroscopic debridement proved to be effective for the treatment of pyogenic arthritis of the knee with minimal morbidity and short hospital stay

RESUMO

31 high tibial osteotomies [in 30 patients] for varus gonarthrosis performed between 1985 - 1990 were evaluated to determine correction of deformity, functional results and subjective impression. 25 patients had satisfactory follow up evaluations. Factors contributing to success included careful patient selection, over-correction of the limb deformity and precise surgical technique
