Fifteen out of thirty-two cases diagnosed as acute posterolateral rotary instability [PLRI] of the Knee were subjected to repair of the acute injury, the mean injury-operation interval was 6 days, all were adult males; their mean age was 27 years old. All showed a preoperative positive external- rotation recurvatum [ERR] test and 9 cases showed a positive posterolateral drawer [PW] test. Six cases showed associated rotary instabilities of the same knee. Peroneal nerve palsy was present in 2 cases; partial in one case and complete in one case. One or more of the components of the posterolateral complex of the knee were injured and repaired at the operation. The mean follow-up period was 3.5 years. The results were rated objectively as good in 66.66%, fair in 26.66% and poor in 6.66% of the cases. Diagnosis and treatment of acute PLRI of knee is a necessity and its surgical repair can result in a more acceptable knee function
