RESUMO

Prior to 1990's, all rhinoplasty surgeries were performed via the intranasal approach which is often called closed rhinoplasty or basic technique. In the closed technique there is lacking of the external scaring, reduced post operative edema, shorter operative time, decreased morbidity, and gives rapid return to a normal appearance. In this study the closed technique was performed in forty cases with aesthetic, functional and traumatic nasal deformities. The patients were collected from the .ENT departments in Al Hussein university hospital in Cairo, Royal commission medical center in Yanbu, KSA, and Al Tamenal general hospital in Ryadh, KSA, during the period from June 2009 to June 2010. A simultaneous septoplasty was done in eleven patients partial turbinectomy in two patients were performed with the basic rhinoplasty procedure to improve nasal breathing. The incisions and other steps of the closed rhinoplasty technique used in this study, allowed exposure of all bony and cartilaginous compartments of the nose and offered the performance of all goals needed for nasal reconstruction. After analysis of the results of this study, the conclusion was that, the closed technique gives satisfactory results for all wanted purposes of rhinoplasty performed

RESUMO

Allergic fungal rhinosinusitis [AFRS] is the most common type of fungal sinusitis. It occurs more commonly in tropical countries like India, Sudan, and K.S.A. This comprehensive study was carried out to evaluate the disease in Yanbu area in KSA which has a hot and humid climate. Ninty seven cases were collected in the Royal commission Medical Centre in Yanbu in the period from Jun 2006 to Jun 2010, their age ranged from 12 to 59 years old, 61 was males and 36 was females. Full ENT and allergic history was taken. Endoscopic examination, radiological and immunological investigations were done then medical treatment was given for all patients, then FESS was done for all patients. Polypi and debris which were collected were sent for bacteriological and histopathological examination. The conclusion of this study showed that one should suspect AFRS in all long standing history of sinusitis inspite of all medical treatment and previous sinus operations and the treatment of choice of this disease is surgical treatment and post-operative close follow up with medical and immunological therapy

RESUMO

Velopharyngeal sphincter reconstruction and oronasal fistula during cleft palat repair is still a matter of challenge to pediatric and ENT surgeons. To improve results of surgical treatment of cleft palate and secondary velopharyngeal incompetence a simple modified procedure for palatoplasty and velopharyngeal sphincter reconstruction is made.: Thirty three patients with cleft palate[26 primary and 7 recurrent were corrected by this procedure from April 2005 to April 2010.In this procedure the well mobilized palatal muscles allow good repair for the nasal layer, and its medial rotation give push back and increases the length of well functioning soft palate, with narrowing of the velopharyngeal isthmus. The modifications in the nasal layer repair prevent the oronasal fistula and elongate the nasal posterior which reduces the velopharyngeal isthmus. All corrected patients showed good results, with no recurrence, no post-operative fistulae, with accepted phonation pattern for all Arabic alphabetic and no nasal regurgitation need for further pharyngoplasty
