Musculoskeletal complications have been observed following severe sustained coughing. Abdominal muscle hematoma can occur as a result of acute or chronic collection of blood following disruption of blood vessels or muscle tear. RESUMO

Leptospirosis can present with a wide range of clinical manifestations. Its severe form, called Weil’s disease, can result in multiorgan dysfunction and death. Hence, an early diagnosis is very essential. The patient being reported had a fever with maculopapular rash, which was initially treated as viral fever. He was later diagnosed to have Weil’s disease; which improved with penicillin therapy. Cutaneous manifestation in leptospirosis is uncommon, and awareness about this can help to prevent life-threatening complications.

Isolated facial nerve palsy is commonly seen with Bell’s palsy. However, it can be associated with a central lesion at the level of the ipsilateral facial nucleus or the facial nerve at the pons. Multiple sclerosis is a chronic autoimmune inflammatory disease characterized by axonal degeneration and demyelination of the central nervous system. Isolated cranial neuropathies are rarely seen with multiple sclerosis. The patient being reported is a 20-year-old female who developed isolated facial nerve palsy, which was initially treated as Bell’s palsy. However, she was found to have multiple sclerosis on MRI brain. Isolated facial nerve palsy due to multiple sclerosis is a rare scenario and can often get misdiagnosed and treated as Bell’s palsy.

Hyperammonemia, a known cause of encephalopathy, is commonly seen in patients with liver disease. Non-cirrhotic hyperammonemia is an uncommon condition. Patients with uncontrolled diabetes can have elevated levels of ammonia. Diabetic ketoacidosis is a life threatening complication in diabetes. This is a case of a middle-aged male who presented in encephalopathy. He was found to have diabetic ketoacidosis with hyperammonemia. Encephalopathy as a result of hyperammonemia in diabetes mellitus is a rare scenario.

Hyponatremia is a common disorder in the geriatric population. Patients can present in altered sensorium, seizures, and even coma. Syndrome of inappropriate antidiuretic hormone secretion (SIADH) and diuretic therapy are the common causes in the elderly. However, acute urinary retention due to benign prostatic hyperplasia (BPH) resulting in hyponatremia is a rare scenario. Here, we report the case of a 60-year-old male patient who developed hyponatremia following urinary retention due to benign prostatic hyperplasia (BPH); which is a rare scenario.
