RESUMO

ObjectiveTo evaluate the effect of virtual reality (VR) on cognitive function and quality of life in patients with Parkinson's disease. MethodsA systematic search of CBM, CNKI, Wanfang data, VIP, Cochrane Library, Web of Science, Embase, and PubMed was carried out to identify randomized control trials (RCT) about the effect of VR technology on patients with Parkinson's disease from inception to February 29th, 2024. The control group received routine cognitive training, balance training or physical therapy, and the experimental group received VR technology. The quality of articles was evaluated using the Cochrane Collaboration's 5.1.0 RCT risk assessment tool for bias. The meta-analysis was performed using Revman5.4. GRADE was used to evaluate the evidence quality of outcome indicators. ResultsA total of 13 literatures involving 426 patients were included. Allocation concealment and blind methods were not described in most literatures, and selective reporting of research results or other biases was unclear. VR technology could improve the Motreal Cognitive Assessment (MoCA) score (MD = 1.11, 95%CI 0.31 to 1.90, P = 0.006), Trail Making Test (TMT)-A score (MD = -6.25, 95%CI -11.71 to -0.78, P = 0.030) and depression scale score (SMD = -0.56, 95%CI -0.95 to 0.18, P = 0.004) of patients with Parkinson's disease; however, it did not improve TMT-B score (MD = -6.01, 95%CI -28.16 to 16.14, P = 0.590), Unified Parkinson's Disease Rating Scale (UPDRS)-Part II score (MD = -2.11, 95%CI -4.97 to 0.75, P = 0.150) and Parkinson's Disease Questionnaire (PDQ-39) score (MD = -0.92, 95%CI -4.03 to 2.19, P = 0.560). For quality of evidence, MoCA score, UPDRS-Part II score and PDQ-39 score were low, and depression score and TMT score were moderate. ConclusionVR technology can improve the cognitive function and depression of patients with Parkinson's disease; however, no significant improvement is found in activities of daily living and quality of life.

@#The CRISPR-Cas (clustered regularly interspaced short palindromic repeats and CRISPR-associated) system is an "adaptive immune system" found in the genomes of bacteria and archaea which is mediated by RNA and resists foreign nucleic acid invasion.Take advantage of specific recognition of target nucleic acid, CRISPR-Cas system can efficiently edit their target site or accurately regulate gene expression, and now have been developed into a powerful tool for gene editing.According to the different compositions of the effector complex, the system has been divided into two categories: class 1 (type I, type IV, and type III) and class 2 (type II, type V, and type VI).Class 2 system, like the CRISPR-Cas9, is widely used in basic research due to the earliest discovery and best research.However, class 1 has not been maturely developed and utilized though it makes up 90% of the entire CRISPR-Cas system.In this essay, the classification of subtype, the assembly of Cascade complex, the cleavage and degradation mechanism of Cas3, and the application in gene editing of class 1 type I CRISPR-Cas system will be discussed and summarized to provide new ideas and methods for further mechanism studying and application of this category.

Objective To investigate the expression levels of miRNA in kidney biopsies of child patients with nephrotic syndrome and the possibility of miRNA as potential markers in differentiating the pathologic subtypes of nephrosis.Methods Kidney biopsy specimens from 41 child patients with nephrotic syndrome,including 22 with nesangial proliferative glomeplonephritis (MsPGN),8 with minimal change disease (MCD) and 11 with endocapillary proliferative glomerulonephritis (ECPGN),were collected,and adult nephridial tissues from 8 patients without renal inadequacy were selected as controls.The expression levels of miR-191,miR-151-3p,miR-150,miR-30a-5p and miR-19b in nephridial tissues were detected by RT-qPCR,and their correlations with renal function related parameters were analyzed.Results Compared with the controls,the miR-191 levels in kidney tissues of child patients with nephrotic syndrome increased significantly (P < 0.01),while the miR-151-3p levels decreased obviously (P < 0.01).The expression levels of miR-150 in MCD patients were significantly lower than those in MsPGN and ECPGN patients and the controls (P < 0.05).The expression levels of these miRNAs were positively correlated with serum IgG,TP and Cr levels,but negatively with serum TC levels (P <0.05).Conclusion The expression levels of miRNA in kidney tissues of child patients with nephrotic syndrome are related to pathological typing of nephrosis,and miR-150 may be a potential marker which may differentiate MCD from other subtypes of nephrosis.

Objective To evaluate the effect of evidence based nursing on the prevention of the complications of PICC .Meth‐ods The randomised controlled trials (RCT) on the evidence based nursing in reducing or preventing the complications of patients with PICC were collected using the databases of PubMed ,CNKI ,WanFang ,CBM and VIP;the data were analyzed by RevMan5 .0 software .Results 17 studies involving 3 092 patients were included in total .The results of Meta analyses showed that compared with the control group ,the complication (Phlebitis ,puncture point infection ,catheter prolapse or ectopic and catheter obstruction) rates of evidence based nursing group with PICC were lower (P<0 .05) than that of the control group .Conclusion Evidence based nursing can effectively prevent the complication of PICC ,improve the quality of nursing and worth of clinical application .

Objective To determine the levels of miR-193a-3p in serum from patients with renal clear cell carcinoma,and eval-uate the potential of miR-193a-3p asa molecular biomarker of renal clear cell carcinoma.Methods Serum samples were taken from 107 untreated hospitalized patients with renal clear cell carcinoma (TNM Ⅰ grade 76 cases,Ⅱ grade 16 cases,Ⅲ grade 2 cases,Ⅳ grade 8 cases and unknown 5 cases)in Nanjing General Hospital from 2010 to 2014 year and 107 age-and gender-matched healthy individuals to determine the level of miR-193a-3p by quantitative reverse-transcription PCR (qRT-PCR). The early diagnostic value of miR-193a-3p for renal clear cell carcinoma was assessed by ROC curve.Results qRT-PCR confirmed that serum miR-193a-3p expression levels was significantly elevated in cancer than in normal controls (3.294± 1.526 vs 1.944±0.600,P =0.000).TNM Ⅰ grade (3.411±1.676 vs 1.944±0.600,P =0.000),Ⅱ grade (2.926±0.927 vs 1.944±0.600,P =0.001),Ⅳ grade(2.926±0.894 vs 1.944±0.600,P =0.000)is significantly higher than that in con-trol group.The area under the ROC curve (AUC)of miR-193a-3p was 0.820 (95% CI,0.756 ~ 0.883),demonstratinga high sensitivity (80.3%)and specificity (93.5%)for the early diagnosis of renal clear cell carcinoma.Conclusion MiR-193a-3p content was significantly elevated in serum from renal clear cell carcinoma and has a high accuracy in diagnosing ear-ly cancer,which holds potential as molecular biomarker for renal clear cell carcinoma.

Objective To use the method of Meta-analysis to evaluate the effect of the evidencebased nursing in preventing the deep vein thrombosis after orthopaedic surgery,to provide the basis for clinical prevention of DVT.Methods The randomized controlled trials (RCT) on the evidence-based nursing in preventing the DVT were collected using the databases of China National Knowledge Infrastructure (CNKI),Wanfang Periodical Databases,Chinese Science Technology Periodical Databases (VIP) and PubMed and analyzed by RevMan 5.2 software.Results Nine RCTs were included in the study.The results of Meta-analysis revealed that the incidence of the DVT could be effectively prevented by evi-dencebased nursing.Conclusions The evidence-based nursing can reduce the incidence of the DVT after orthopaedic surgery.

Objective To study the risk factors and to provide preventive measures of pulmonary infection for patients with severe brain injury.Methods A total of 320 patients with severe brain injury were analyzed with hospital-based case-control study method.A total of 101 patients complicated with lung infection were set as the experimental group.Another 101 patients with no pulmonary infection according to the proportion of 1:1 choice over the same period were set as the control group.The related factors were analyzed with the univariate analysis and the multiple factors Logistic regression analysis.Results Univariate analysis showed that the occurrence of pulmonary infection of patients with severe brain injury was closely related to the old age,low GCS score,long time stay in the bed,long time hormone application,long time antibiotic application,the combined underlying disease,use of mechanic ventilation,long-term smoking history and indwelling gastric tube.Multivariable Logistic regression analysis showed that the application of mechanic ventilator,the combined underlying disease and long time stay in bed were the independent risk factors for pulmonary infection of patients with severe brain injury,but GCS score was a protective factor.Conclusions The occurrence of lung infection were related to the ventilator application,the combined underlying disease,long time stay in bed and low GCS score.Considering these risk factors,the medical staffs should take preventive measures early to control lung infection.

Objective To evaluate the impact of programmed nursing intervention based on MEWS for potential accidents in critical patients.Methods The randomized controlled trials (RCT) on programmed nursing intervention based on MEWS for potential accidents in critical patients were collected using the databases of PubMed,Medline,FMJS,CNKI,Wanfang database,VIP.Data were analyzed with RevMan5.0 software.Results Four RCTs were included in the study.All were Chinese articles.The incidence of accidents in the intervention group was significandy lower than that of the control group,the difference was statistically significant.Conclusions Programmed nursing intervention based on MEWS can effectively reduce the potential accidents happened in critical patients.

Objective To evaluate the effect of the bundle of care interventions in preventing ventilator-associated pneumonia (VAP) in the mechanic ventilated patients.Methods The randomized controlled trials (RCT) on the bundle of care interventions in preventing ventilator-associated pneumonia (VAP) were collected using the databases of PubMed,Medline,Embase,Cochrane library,China National Knowledge Infrastructure (CNKI),Wanfang Periodical Databases and Chinese Science and Technology Periodical Databases (VIP),data were analyzed by RevMan5.0 software.Results Ten RCTs were included in the study.The results of Meta-analysis revealed that the incidence of ventilator associated pneumonia,mechanical ventilation time,ICU length of hospital stay and the mortality rate in the intervention group were different from the control group,the difference had statistical significance.Conclusions The bundle of care interventions can reduce the incidence of ventilator-associated pneumonia,mechanical ventilation time,ICU length of hospital stay and the mortality rate.

Objective To discuss the application effect of evidence-based nursing in preventing the phlebitis using the method of Meta analysis,and provide basis for clinical prevention of phlebitis.Methods The randomized controlled trials (RCT) on the evidence-based nursing in preventing the phlebitis were collected using the databases of China National Knowledge Infrastructure(CNKI),Wanfang Periodical Databases,Chinese Science Technology Periodical Databases (VIP) and PubMed which were analyzed by RevMan 5.0 software.Results Seven RCTs were included in the study.The results of meta-analysis revealed that the incidence of the phlebitis was effectively prevented by evidence-based nursing.Conclusions The evidence-based nursing can reduce the incidence of the phlebitis.

RESUMO

Objective To investigate epidemiological characteristics in hospitalized adolescents and children who suffered from traffic injuries,such as source and location of an accident,the victim's gender arid age,and the site of injury.Methods The chosen subjects were 163 hospitalized children aged 1 to 18 years old,who were admitted to general hospitals in Shanghai in the years of 2004 to 2005 as a result of traffic injuries.Descriptive statistics and analysis were used to interpret the data.Results Children of immigrant workers and suburban residents were the main victims ;the main cause of injury was the collision between the motor vehicle and the pedestrian;this type of traffic injury was the most prevalent among children between 5 to 9 years old;and the locations of the injury was most commonly found in the head,the face, and the extremities.Conclusions From this statistical evidence,it could be concluded that it would be worthwhile for the government to pay more attention to children of immigrants and suburban residents to effectively educate them about traffic safety and knowledge,tt is also advisable to improve the safety protection and post - traffic injury care of children passengers.
