RESUMO

Clear cell papulosis (CCP) is a rare skin condition characterized by asymptomatic multiple hypopigmented maculopapules distributed primarily on the lower aspect of the abdomen or along the milk lines in young children. The characteristic histological feature of CCP is the presence of clear cells scattered among basal keratinocytes. We describe the case of a 3-year-old girl with clear cell papulosis. Immunohistochemical staining results were variable based on the different kinds of CEA antibodies. Immunostaining specification of CEA glycoproteins may provide a clue for the differential diagnosis of clear cell papulosis with other diseases such as primary extramammary Paget's disease.

BACKGROUND: Melasma is a common acquired hyperpigmentation disorder that predominantly affects the face. It frequently occurs in women with darker skin types and severely impacts quality of life. OBJECTIVE: To characterize the clinicoepidemiological features and triggering or aggravating factors of melasma in Korean patients. METHODS: This cross-sectional study was conducted at the dermatology clinics of five university hospitals in Korea. Between January 2011 and August 2012, 411 patients with melasma completed a questionnaire about the clinical and aggravating factors associated with their melasma. RESULTS: The study population consisted of 400 women and 11 men aged 22~73 years (mean age, 42.8±9.92 years). Triggering or aggravating factors were sun exposure (68.4%), pregnancy (27.0%), and emotional stress (24.8%). Interestingly, 61.1% of patients complained of sensitive/inflammatory features such as erythema, itching, and a stinging sensation. Dryness was the most common aggravating factor, followed by erythema/redness and itching/stinging. Concomitant pigmentary disorders included post-inflammatory hyperpigmentation in 15.1% of patients, followed by pigmented contact dermatitis, and acquired bilateral nevus of Ota-like macules. CONCLUSION: It is well known that sun exposure and hormonal changes are the most common triggers of melasma; however, sensitive/inflammatory features may aggravate melasma in East Asian patients. Therefore, these individual and racial differences should be considered in the prevention and treatment of melasma.

Cutaneous metastasis from gastric adenocarcinoma is uncommon, and the eyelid is a rare metastatic site. Three patterns of clinical presentation of eyelid metastasis have been described: nodular, infiltrative, and ulcerated. The infiltrative pattern, also known as an inflammatory diffuse pattern or mask-like metastasis, can be easily misdiagnosed as cellulitis or contact dermatitis. Here, we report a case of gastric adenocarcinoma in a 75-year-old man who presented with a localized erythematous plaque on his eyelid that developed four months earlier. The patient had been treated with an antimicrobial agent owing to suspicion of preseptal cellulitis. Gastric adenocarcinoma metastasis was diagnosed on the basis of histopathological examination and immunophenotyping (i.e., cytoplasmic epithelial membrane antigen, cytokeratin- 7, cytokeratin-20, and carcinoembryonic antigen). For patients with malignant neoplasms, persistent skin lesions similar to cellulitis or contact dermatitis should be suspected of metastasis derived from an internal malignancy, even for very rare sites of metastasis.

PURPOSE: This study was conducted to investigate and compare the characteristics and relations of nursing practice environment, professionalism and job satisfaction among nurses in general hospitals according to hospital size. METHODS: The participants included 314 staff and charge nurses who were working in the general medical/surgical nursing units in one large hospital, three medium sized hospitals, and four small hospitals. Data collected through using self-report questionnaire were analyzed using the SPSS and SAS statistical programs. RESULTS: Nursing practice environment and job satisfaction had significant differences according to hospital size. Both of these scales were highest for medium hospitals and lowest for small hospitals. For all hospital sizes there were positive correlations between each of the variables. Multiple regression analysis showed that both nursing practice environment and job satisfaction were affected by hospital size, but professionalism was not. CONCLUSION: The results of this study indicate that nursing practice environment and job satisfaction vary with the size of the hospital. Therefore, further study is necessary to identify the work environment variables of nurses for performance management and to implement appropriate policies.

OBJECTIVES: To determine whether a novel marine micro-organism with anticancer properties, H31, the metabolic product of Bacillus SW31, has anti-tumor effects on head and neck cancer, and potential for apoptotic-enhancing anti-cancer treatment of affected patients. METHODS: The cell viability and apoptosis assays were performed. Changes in the signal pathway related to apoptosis were investigated. Then, the therapeutic effects of H31 were explored in mouse xenograft model and drug toxicity of H31 was examined in zebrafish model. RESULTS: We identified the anticancer activity of H31, a novel metabolic product of Bacillus SW31. Bacillus SW31, a new marine micro-organism, has 70% homology with Bacillus firmus and contains potent cytotoxic bioactivity in head and neck cancer cells using MTT assay. Combined with c-JUN, p53, cytochrome C, and caspase-3, H31 induced apoptosis of KB cells, a head and neck cancer cell line. In a separate in vivo model, tumor growth in C3H/HeJ syngeneic mice was suppressed by H31. In addition, in a zebrafish model used for toxicity testing, a considerable dose of H31 did not result in embryo or neurotoxicity. CONCLUSION: Growth inhibition and apoptosis were achieved both in vitro and in vivo in head and neck cancer cells after exposure to H31, a metabolite from the marine Bacillus species, without any significant toxicity effects even at considerable H31 dose concentrations.

PURPOSE: It is difficult to estimate definite intraocular pressure of neonates. We intended to evaluate neonatal normal intraocular pressure using Tono-pen, and compared mean intraocular pressures between groups divided by sex, birth weight, and gestational age. METHOD: We measured the intraocular pressure with Tono-pen in 504 eyes of 252 newborn, who admitted in our hospital from June 2000 to January 2001 at their the second day of life. One observer measured the intraocular pressure with Tono-pen five to ten times after proparacain HCL 0.5% instillation on regular time. Only 5% of Tono-pen's coefficient of variance was considered valuable. Neonates suspected having congenital glaucoma or retinopathy were excluded. Conclusively, 462 eyes of 231 newborns were included. Mean gestational age was 38 weeks and 6 days. Mean birth weight was 3.23 kg. We achieved average intraocular pressures of neonates having gestational age between 38 weeks and 42 weeks, and birth weight between 3.0 kg and 3.5 kg. We achieved also average intraocular pressure of neonates divided by sex, gestational age, and birth weight, compared them with average intraocular pressure of normal neonates by 3-way ANOVA. RESULT: Average of intraocular pressure of normal neonates was 13.61+/-2.02 mmHg. Intraocular pressure of normal neonates is not statistically significant in the classification by sex, gestational age, and birth weight.(p-value>0.05) CONCLUSION: These value will aid the ophthalmologists in the diagnosis and screening of congenital glaucoma.

PURPOSE: To report appropriate treatment of medial rectus muscle injury after functional endoscopic sinus polypectomy and ethmoidectomy METHODS: The author experienced the right medial rectus muscle injury after functional endoscopic polypectomy and ethmoidectomy in a 42-year-old man who complained of decreased visual acuity, ocular pain, binocular diplopia, and exophthalmos in the right eye at the first postoperative day. His best corrected visual acuity of right eye was 0.3, Intraocular pressure was 26 mmHg, and pupillary light reflex was decreased. On exophthalmometry, the right eye was more exophthalmic by 3 mm than the left eye. After steroid therapy and befunolol eyedrops instillation, the visual acuity was improved to 0.8 and intraocular pressure was decreased to 18 mmHg. There was severe limitation of medial gaze and about 65 prism diopters of right exodeviation in red filter test. In computerized tomography, bony defect in the right orbital medial wall and defect in the right medial rectus muscle were confirmed. On the third postoperative day, silastic sheet was applied in dehiscent medial wall under endoscopic examination in otorhinolaryngologic outpatient department. Because exodeviation and diplopia had not improved, on the 61th postoperative day, right lateral rectus muscle recession 12 mm was carried out, and then, on the 88th postoperative day, Hummelsheim operation on right eye was done. RESULTS: Right exodeviation was decreased by about 40 prism diopters after right lateral rectus muscle recession 12 mm. and then, after Hummelsheim operation, he had right exodeviation 20 prism diopters of and less binocular diplopia. We prescribed 10 prism diopters of prism lens in each eye, but he complained of decreased visual acuity and cosmetic problem. So, we prescribed 5 prism diopters of prism lens in each eye. At the last follow-up he is satisfied with his visual acuity and cosmetic aspect.

RESUMO

PURPOSE: We investigated the serial developement of programmed cell death or apoptosis as a mechanism of cell death by inducing chronic retinal ischemia in experimental permanent common carotid artery ligation in rats. We also hope this model to be applied in study of normal tension glaucoma or ischemic ocular disease. METHODS: After ligation of both common carotid arteries of rats under anesthesia, they were sacrificed on the 1st, 3rd, 5th, 7th, 14th, 28th, and 63rd day and then enucleation was done. After separating retina, we did TdT-dUTP nick-end labeling (TUNEL)stain and Bax immunochemistry stain to study apoptotic change and we also did hematoxylin eosin stain to evaluate retinal morphologic changes. RESULTS: Apoptotic cells were showed in ganglion cell layer cheifly on TUNEL stain and Bax immunochemistry stain on the first day after ligation of common carotid artery, as time as gone, apoptotic cell increased and then developed in all layers on the 28th day. Cell density decreased in ganlion cell layer, inner nuclear layer, and outer nuclear later on hematoxylin eosin stain on the third day; as time as gone, cell density further decreased and thickness of inner plexiform layer and nuclear layer decreased also. CONCLUSIONS: This result shows that apoptosis has the important role in the cell death at the ischemic retina. The results of this study will also provide baseline information for the further study on normal tension glaucoma or ocular ischemic diseases compared to transient retinal ischemia model induced by elevation of intraocular pressure.

The purpose of this study was to verify the validity of the Patient Severity Classification Tool by examining the correlations between the APACHE III and the Patient Severity Classification Tool and to propose admission criteria to the ICU. The instruments used for this study were the APACHE III developed by Knaus and thePatient Severity Classification Tool developed by Korean Clinical Nurses Association. Data was collected from the 156 Medical ICU patients during their first 24 hours of admission at the Seoul National University Hospital by three trained Medical ICU nurses from April 20 to August 31 1999. Data were analyzed using the frequency, X2, Wilcoxon rank sum test, and Spearman rho. There was statistically significant correlations between the scores of the APACHE III and the Patient Severity Classification Tool. Mortality rate was increased as patients classification of severity in both the APACHE III and the Patient Severity Classification Tool scored higher. The Patient Severity Classification Tool was proved to be a valid and reliable tool, and a useful tool as one of the severity predicting factors, ICU admission criteria, information sharing between ICUs, quality evaluations of ICUs, and ICU nurse staffing. 1) This paper was awarded the first prize at the Seoul National Hospital Nursing Department Research Contest.
