RESUMO

To investigate the influence of leukoaraiosis (LA) on the functional outcomes of subcortical stroke in the subacute phase after onset. We retrospectively analyzed 41 patients with subacute subcortical infarct at a single center from 2011 to 2015. We explored the relationship between LA severity at admission/transfer (initial evaluation) and functional outcome at the time of discharge (follow-up evaluation), as assessed using the modified Rankin Scale (mRS), Functional Ambulation Category (FAC), and modified Barthel Index (mBI). LA severity was graded as mild, moderate, or severe according to the Fazekas scale. Scores of the mRS, FAC, and mBI were compared in patients grouped based on LA severity: no LA (n = 12), mild LA (n = 19), and moderate-to-severe LA (n = 10). Significant inter-group differences were observed in all 3 scores at both the initial and follow-up evaluations. After adjustment for age, scores at follow-up evaluation were significantly different between the 2 groups. LA is related to functional outcomes of subcortical stroke in the subacute phase after onset. After adjustment for age, severe LA was correlated with poor functional outcomes in the subacute phase.

Objective: To classify four new Sparassis strains (CLM1, CKM1, CKM2, and KJM1) using the internal transcribed spacer sequence and to elucidate their β-glucan content and mycelial growth. Methods: Two different microbiological media were used to determine growth rate. The β-glucan contents were analyzed using the Megazyme Mushroom and Yeast Beta-Glucan kit. To determine the genetic relationships, phylogenetic trees were constructed using ClustalX. Multiple sequence alignments were printed and shaded with the BOXSHADE 3.21 program. Results: In this study, four new Sparassis strains were isolated from the southern region of the Korea Peninsula. They were all classified into the Sparassis latifolia clade as a monophyletic group based on the internal transcribed spacer sequence. Mycelial growth rate of the CLM1 strain was highest in potato dextrose agar and potato dextrose agar larch. The β-glucan content of the CLM1 strain was highest at 29.5% (w/w). A high degree of sequence divergence was detected in the RNA polymerase second largest subunit II gene (RPB2) within Sparassis spp. tested. The putative amino acid sequences of the RPB2 had a distinct sequence. The nucleotide sequences of the RPB2's intron were also divergent among Sparassis spp., even though their nucleotide length was well conserved within Sparassis latifolia. Conclusions: These results indicate that the nucleotide sequences and the amino acid sequences of RPB2 can be used to identify individual Sparassis sp. The Sparassis strain CLM1 may be best for developing a remedy to prevent or treat cancer and other chronic diseases.

OBJECTIVE: To describe a hand-stretching device that was developed for the management of hand spasticity in chronic hemiparetic stroke patients, and the effects of this device on hand spasticity. METHODS: Fifteen chronic hemiparetic stroke patients with finger flexor spasticity were recruited and randomly assigned to an intervention group (8 patients) or a control group (7 patients). The stretching device consists of a resting hand splint, a finger and thumb stretcher, and a frame. In use, the stretched state was maintained for 10 minutes per exercise session, and the exercise was performed twice daily for 4 weeks. Spasticity of finger flexor muscles in the two groups was assessed 3 times, 4 weeks apart, using the Modified Ashworth Scale (MAS). Patients in the intervention group were assessed twice (pre-1 and pre-2) before and once (post-1) after starting the stretching program. RESULTS: Mean MAS (mMAS) scores at initial evaluations were not significantly different at pre-1 in the intervention group and at 1st assessment in the control group (p>0.05). In addition, no significant differences were observed between mMAS scores at pre-1 and pre-2 in the intervention group (p>0.05). However, mMAS scores at post-1 were significantly lower than that at pre-2 in the intervention group (p0.05). In addition, mMAS scores at post-1 in the intervention group were significantly decreased compared with those at the 3rd assessment in the control group (p<0.05). CONCLUSION: The devised stretching device was found to relieve hand spasticity effectively in chronic hemiparetic stroke patients.
