RESUMO

Objective To explore the reasons and preventive measures of early dislocation after total hip arthroplasty in elder patients. Method A retrospective study was done to analyze dislocation time, reason and time of 168 elderly patients with early anti-dislocation after total hip arthroplasty in joint surgery. Results Only 7 patients (4.1%) had type I joint dislocation, including 2 male and 5 female patients aged 65~89 years. The dislocation happened in 4~5 weeks postoperatively, mainly resulting from hip joint over flexion when urinating in bed, sleep-turning, loaded-moving, walking and stoop and diachoresis. Conclusions For the elderly patients after total hip replacement, it is type I dislocation which happened 4 ~ 5 weeks after operation, more femal than male, reasons including over-exercrse. Effective prevention measures includes regular rehabilitation training, early precautions enhanced mental support and safety nursing.

RESUMO

Objective We aimed to assess effectiveness of Risk Score Scale (RSS) for aortic Dissection (AD) in early triage stage.Methods A total of 251 patients with acute chest or back pain complaints were allocated by triage nurses according to the pre-established RSS from 2011 to 2013.87 cases were assessed to have 5 points or more,and 80 cases were assessed to have 6 points or more from 95 patients diagnosed as aortic dissection (the diagnosed group) by CT angiography and magnetic resonance imaging while excluded 156 cases of control after discharge (control group).The general conditions,distribution of aortic dissection risk factors,aortic dissection risk score,sensitivity and specificity of RSS between two groups were compared.The misdiagnosis rate,missed diagnosis rate,RSS triage sensitivity and specificity and OR value were compared between RSS ≥5 points and RSS ≥6 points.Results There was significant differences in abrupt onset of severe pain intensity,ripping or tearing pain,pulse deficit or systolic blood pressure differential between extremities [upper extremity≥20 mmHg (1 mmHg=0.133 kPa) or lower extremity≥ 10 mmHg],x2 value was 62.6,94.2 and 20.2.RSS≥5 points had high sensitivity and specificity,lower missed diagnosis rate and high OR value for triage application.Conclusion RSS≥5 points could serve as a reliable evidence for early triage of aortic dissection.

RESUMO

Objective To investigate the effect of comparison-type computer-assisted teaching system in the teaching of emergency nursing.Methods 130 students participated in emergency nursing training in emergency department in 2010 were chosen as the control group,another 130 students in 2011 were chosen as the experimental group.The control group used traditional teaching mode,while the experimental group was given comparison-type computer-assisted teaching system mode.The test results and the satisfaction degree of teachers were compared.Results The test results of the control group was(86.70 ±7.63),lower than (90.09 ± 7.78)of the experimental group.The satisfaction degree of teachers was higher in the experimental group than that in the control group.The evaluation to comparison-type computer-assisted teaching system was good.Conclusions The application of comparison-type computer-assisted teaching system can improve the teaching mode,reduce the workload of teachers,increase the interest of autonomous learning for students and enable them to master first aid skills accurately,it has a certain value in application.
