RESUMO

Se describen distintos aspectos del rinofima: historia, etiopatogenia, histopatología, epidemiologia, clínica, diagnósticos diferenciales, tratamientos y pronóstico. Se hace hincapié en que es una enfermedad localizada en la nariz asociada a desórdenes generales, y que repercute en las áreas psicológica y social. Se presenta un caso clínico operado con cirugía plástica. Los autores plantean una hipótesis sobre la etiopatogenia y la fisiopatología de la enfermedad; y consideran a los tratamientos de rinofima como paliativos y no curativos


Different aspects of rhinophyma were described: history, etiopathogeny, histopathology, epidemiology, clinical, differential diagnoses, treatments and prognosis. It is emphasized that it is a localized disease in the nose associated to general disorders, and that it brings psychological and social problems. We present a case operated with plastic surgery. The authors present a hypothesis about the etiopathogeny and pathophysiology of the disease and consider rhinophyma treatments as palliative and non-curative.

ABSTRACT Purpose: This study aims to evaluate the link between preoperative parameters and oxidative stress (OS) markers in the bladder wall of men undergoing open prostatectomy. Materials and Methods: From July 2014 to August 2016, men aged ≥ 50 years and presenting with LUTS were prospectively enrolled. Preoperative assessment included validated questionnaires (IPSS and OAB - V8), lower urinary tract ultrasound and urodynamics. Bladder biopsies were taken during open prostatectomy for determination of OS markers. Increased OS was defined by increased concentration of malondialdehyde (MDA) and / or decreased concentration of antioxidant enzymes (superoxide dismutase and / or catalase). P<0.05 was regarded as statistically significant. Results: Thirty - eight consecutive patients were included. Mean age was 66.36 ± 6.44 years, mean prostate volume was 77.7 ± 20.63 cm3, and mean IPSS was 11.05 ± 8.72 points. MDA concentration was increased in men with severe bladder outlet obstruction (BOO grade V - VI according to the Schaefer's nomogram) in comparison with BOO grade III - IV (p = 0.022). Patients with severe LUTS also had higher MDA concentration when compared to those with mild LUTS (p = 0.031). There was a statistically significant association between increased post - void residual urine (cut off ≥ 50 mL) and not only higher levels of MDA, but also reduced activity of SOD and catalase (p < 0.05). Conclusions: This pilot study showed that severity of LUTS and BOO were associated with increased MDA concentration in the bladder wall of men undergoing open prostatectomy. Further studies are still needed to assess the role of non - invasive biomarkers of OS in predicting bladder dysfunction in men with LUTS.

Introducción: La esplenectomía laparoscópica se ha convertido en la técnica Gold Standard para la patología benigna del bazo. Debido a la posición anatómica, fragilidad y gran riego sanguíneo del bazo este procedimiento requiere un equipo con formación en cirugía laparoscópica de avanzada. Objetivos: Presentación de la experiencia inicial en esplenectomía laparoscópica realizada en un sistema médico asistencial cerrado por un equipo entrenado en cirugía laparoscópica de avanzada. Lugar de aplicación: Diseño: Retrospectivo, observacional, de corte transversal. Población: 6 esplenectomías laparoscópicas programadas realizadas entre agosto de 2010 y junio de 2012. Material y métodos: Se realizaron 6 esplenectomías laparoscópicas entre agosto de 2010 y junio de 2012. La vía de abordaje fue laparoscópica con 3 trócares. Resultados: En todos los casos se usaron 3 puertos de trabajo, no requiriendo conversión a cirugía abierta. En 2 casos se utilizó morcelación del bazo y en 4 se amplió la incisión umbilical para extraer la pieza. Conclusiones: La esplenectomía laparoscópica es una técnica segura y efectiva, en nuestra casuística ningún paciente requirió convertir la técnica a cirugía abierta y no tuvimos morbilidad. Debe ser realizada por un equipo con experiencia en cirugía laparoscópica de avanzada.


Background: Laparoscopic splenectomy has becom e the gold standard for benign splenic disease. Design:Retrospective, observational, cros-sectional study.Population: 6 laparoscopic splenectomy scheduled betweenAugust 2010 and June 2012. Methods:We’ve performed 6 laparoscopic splenectomy between August 2010 and June 2012. The surgical approach waslaparoscopic with 3 trocars. Results: In all cases we used three working ports, not requiring conversion to traditional open surgery. In 2 cases we used morcellation of the spleen and in 4 umbilical incision wasextended to remove the piece.Conclusions:Laparoscopic splenectomy is a safe and effective,none of our patients required convertion to traditional open surgery and had no morbidity. Must be performed by a trained laparoscopic team.

Introducción: El cáncer de vesícula ocupa el 5° lugar de aparición de todos cánceres digestivos. En nuestro país es dos veces más frecuente que los tumores ubicados en la vía biliar principal intra o extrahepática. Objetivos: Reporte de caso y revisión bibliográfica. Reporte de Caso: Mujer de 52 años, sin antecedentes clínicos de relevancia, que consulta por dolor abdominal, náuseas y vómitos de 14 días de evolución. Se realiza una ecografía que evidencia engrosamiento parietal y litiasis vesicular. En el intraoperatorio se evidencia Vesícula en Porcelana con infiltración hepática a nivel del fondo.Discusión: Destacar la importancia de una correcta interpretación de las imágenes, a los fines de tomar la conducta que mayor beneficio representa para el paciente.


Introduction: Gallbladder cancer occupies the 5th place of occurrence among all digestive system cancers. In our country, it is twice more frequent than tumors of the intra or extrahepatic principal biliary tract. Objectives: Case report and literature review. Case Report: 52-year-old woman, with no relevant clinical history, with 14 days of abdominal pain, nausea and vomit. The ultrasound scan reveals parietal thickening and gallstones. During intrasurgical exploration, porcelain gallbladder is found, with hepatic infiltration at fundus. Discussion: To highlight the importance of a correct image interpretation, in order to choose the most beneficial treatment for the patient.
