RESUMO

ObjectiveTo analyze the community structure and dynamics of parasitic fleas on the body surface of host animals and nested fleas in different seasons in the natural foci of wild rat plague in Yulong County,Yunnan Province, to explore the relationship between seasonal fluctuation of fleas and the prevalence of plague among animals, so as to provide evidence for plague prevention and control in the natural foci. MethodsNanxi Village, Huangshan Town, the core area of plague epidemic in Yulong County, was selected as the monitoring sample area in December 2019 (winter), August 2020(summer), October 2020(autumn) and March 2021(spring). Host animals were captured by rattrap at night and rat nests were excavated for collecting parasitic fleas on host animals and rat nest fleas in different seasons. Excel 2010 and SPSS 26.0 software were used to analyze the data, and Chi square test was used to compare the rate. Community ecological indicators were used to analyze the community structure and species diversity of the host animals and their parasitic fleas. ResultsA total of 355 vector fleas were captured, belonging to 7 species of 5 genera in 2 families. 441 small animals were captured and 138 rat body fleas were detected with the flea infection rate of 14.51% and the flea index 0.31. 96 effective rat holes were excavated and 217 fleas were detected with the flea infection rate of 35.42% and the flea index 2.26. Among the four seasons, the flea infection rates of rat body and rat nests were higher in summer and winter, showing a significant difference in general (χ2=15.851, P<0.01; χ2=16.398, P<0.01). The dominant species of flea community were Ctenophthalmus quadratus, Stenischia humilis, Neopsylla specialis and Frontopsylla spadix, with a dominance index of 0.434, 0.254, 0.180 and 0.110, respectively. The diversity and evenness of rat body fleas showed a distribution characteristic of decreasing, increasing and then decreasing again with season changes, and both were the highest in spring, while the ecological dominance showed an opposite trend. The diversity and evenness of rat nest fleas showed a distribution characteristic of increasing first and then decreasing in summer, autumn and winter, with the highest in autumn, while the ecological dominance was diametrically opposite. ConclusionThe fleas community structure is relatively stable in Yulong County, but the number of species in the community is unevenly distributed by seasons, and the status of dominant species is prominent. Local authorities should carry out timely preventive deratization and depulization measures according to the results of daily monitoring, so as to effectively avoid the prevalence and spread of plague among animals in plague foci.

Objective:To understand the current research status of master of professional degree students in clinical medicine in provincial teaching hospitals with moderate scientific research level under the " dual-track" training system.Methods:Sojump online survey was conducted to investigate the cognition, current situation and self-evaluation of all clinical master degree students in three grades in a provincial teaching hospital, and the research status of professional degree and scientific degree students was compared and analyzed respectively.Results:The proportion of scientific degree students participating in scientific research projects was significantly higher than that of professional degree students. The proportion of professional degree students participating in scientific research projects was still not high even in the third year of graduate students. However, there was no difference between scientific degree and professional degree students in the publication of scientific research papers. The scientific degree of scientific research knowledge is significantly higher than that of professional degree students. Although scientific degree students receive more scientific research guidance from their supervisors, professional degree students communicate more with their supervisors, and the results show that professional degree students are significantly more satisfied with their supervisors, graduate policies and scientific research policies than scientific degree students. In addition, the results of graduate students found that the degree of research stress of both scientific and professional degrees exceeded 50%.Conclusions:Scientific degree is better than professional degree in research status because of professional characteristics and more research guidance from their supervisors. However, through the reform of professional degree training mode based on scientific research project management in recent years, professional degree students have been able to communicate more with their supervisors, and their satisfaction with their supervisors and colleges has significantly increased. In addition, sufficient attention should be paid to high scientific research pressure of medical graduate students.

Objective@#To investigate the correlation between sleep quality and the use of smart phone before bed in middle school students, and to provide a reference for relevant prevention and control.@*Methods@#A total of 3 749 students from 4 middle school which were randomly selected by stratified cluster sampling method were assessed with selfdesigned questionnaire for the students’ general information,smart phone usage before bed,as well as the Epworth Sleepiness Scale(ESS),and Insomnia Severity Index(ISI).@*Results@#A total of 63.17% of students in the seventh grade used mobile phones before going to bed. 67.04% in the eighth grade, 81.18% in the ninth grade, 83.54% in the first grade of high school and 80.11% in the second grade of high school. Mobile phones usage before bed gradually increased with grade. Students with ≥2 h smart phone usage before bed(group 2) had higher scores than those with <2 h smart phone usage before bed(group 1) in each item of ESS, except dozing during traffic jam(P<0.01). The group 2 had higher scores than group 1 in ESS and ISI scores(P<0.01). @*Conclusion@#Smart phone usage before bed in middle school students is very common. The risk of sleep disorders increased with the duration of smart phone usage before bed. Parental supervision should be involved in the intervention of smart phone usage before bed among middle school students.

OBJECTIVE@#To investigate the exprassion of WT1 gene in patients with adult acute myeloid leukemia (AML) and its clinical significance.@*METHODS@#Sixty-three newly diagnosed patients with acute myeloid leukemia were selected. Quantitative RT-PCR was used to detect the expression of WT1 gene in the 63 AML patients and 20 non-AML controls.@*RESULTS@#WT1 gene was highly expressed in AML patents and its expression in the low-risk group was significantly lower than that in middle-risk group and high-risk group (P<0.05), and no significant difference of WT1 gene expression between middle-risk and high-risk group was observed. In the patients of middle-risk and high-risk patients, the expression of WT1 gene in the remission group was significantly lower than that in the patients of non-remission after treatment (P<0.05). The non-remission patients after first treatment in middle-risk and high-risk group were treated with second induction therapy. After second induction therapy, the WT1 expression in remission patients was significantly decreased (P<0.05) in comparison with that in patients still in non-remission. There was a negative correlation between WT1 expression and the 2-year overall survival rate in the newly diagnosed middle and high-risk AML patients.@*CONCLUSION@#The detection of WT1 gene expression can help to divide AML patients into low-/middle-/high-risk groups and to evaluate therapeutic response and clinical prognosis in middle and high-risk AML patients.

Objective To investigate the application effect of swallowing training and ice stimulation combined with electromyography biofeedback therapy in stroke patients with swallowing and coughing .Methods Seventy-nine stroke swallowing and coughing patients treated were collected and were randomly divided into two groups (39 cases in control group,40 cases in observation group ) according to the digital table .The control group was treated with normal swallowing training,while the observation group was treated with ice stimulation combined with electromyography biofeedback therapy.The clinical intervention effect and the VFSS results before and after the intervention were observed and compared .The complications of the two groups were statistically analyzed .Results The total effective rate of the observation group [90.00%(36/40)] was higher than that of the control group [71.79%(28/39),χ2 =4.255,P=0.039].After eight-week intervention,the VFSS score of the observation group was higher than that of the control group(t=12.173,P=0.000);and the incidence rate of complications of the observation group [7.50%(3/40)] was lower than that of the control group [25.64%(10/39),χ2 =4.727,P=0.030].Conclusion Swallowing training and ice stimulation combined with electromyography biofeedback therapy can significantly improve the swallowing function of stroke patients with swallowing and coughing ,and it is safe .

Objective To study the clinical effect of triethanolamine cream in treatment of traumatic wounds and its effect on level of interleukin-2 (IL-2), interleukin-8 (IL-8) and tumor necrosis factor-α(TNF-α).Methods 88 patients with traumatic wounds were selected in our hospital from February 2015 to January 2016, those patients were divided into observation group and control group according to coin method.The control group was treated with alginate dressing, observation group were given triethanolamine cream for treatment.The levels of IL-2, IL-8, TNF-αand visual analogue scale(VAS) between the two groups were compared.The clinical curative effect of the two groups was evaluated.Results After treatment, the total effective rate of the observation group(90.91%) was significantly higher than the control group (68.18%).There were no significant differences in VAS scores between the two groups before treatment, 5, 10,20 days after treatment, the VAS scores of the two groups were significantly lower than before treatment (P<0.05), at 5,10,20 days after treatment, the VAS scores in the observation group were significantly lower than the control group (P<0.05).The level of IL-2 in the observation group were significantly higher than the control group, the levels of IL-8,TNF-αwere significantly lower than the control group (P<0.05).Conclusion Triethanolamine cream can increase the level of IL-2 and decrease the levels of IL-8 and TNF-αin patients with traumatic wounds, it can relieve the pain of patients and has good clinical curative effect.

Objective To investigate the effect of tetramethylpyrazine injection on serum ion and β2-GPI/ox-LDL in patients with IgA nephropathy.Methods 58 IgA nephropathy patients were randomly divided into two gorups with 29 cases in each group.The control group were treated by benazepril and conventional treatment of dipyridamole, the experiment group were treated on the basis of the control group with tetramethylpyrazine injection.10 days of 1 courses, 2 courses of treatment.The serum ion,β2-GPI/ox-LDL and renal function related indexes were tested.Results Compared with before treatment, the serum inorganic phosphorus,β2-GPI/ox-LDL, Scr, BUN, CysC levels and 24 h urinary protein quantitation were lower( P<0.05), the blood calcium level were higher(P<0.05).Compared with the control group, the serum inorganic phosphorus,β2-GPI/ox-LDL, Scr, BUN, CysC levels and 24 h urinary protein quantitation were lower(P<0.05), the blood calcium level were higher(P<0.05), the clinical effect were higher (P<0.05).Conclusion Tetramethylpyrazine injection can effectively decrease the serum phosphorus, β2-GPI/ox-LDL, SCR, BUN, CysC levels and urinary protein amount of patients with IgA nephropathy, elevate blood calcium levels, significantly improve renal function and with less adverse reactions.

Objective To observe the effect of homemade ankle-foot orthopedic band on motor function of lower extremities in children with foot drop post spastic hemiplegic cerebral palsy (CP). Methods 24 spastic hemiplegic CP children were randomly divided into observa-tion group (n=12) and control group (n=12). Both groups accepted roution rehabilitation training. The control group accepted walking train-ing, and the observation group accepted walking training wearing the homemade ankle-foot orthopedic band, for 12 weeks. They were as-sessed with modified Ashworth Scale (MAS) of gastrocnemius, range of motion (ROM) of ankle active dorsiflexion, and D and E domains of Gross Motor Function Measure (GMFM-88) before and after treatment. Results The score of MAS significantly decreased in both groups after treatment (P<0.01), and decreased more in the observation group than in the control group (P<0.05). ROM and scores of D and E do-mains of GMFM-88 significantly increased in both groups after treatment (P<0.01), and increased more in the observation group than in the control group (P<0.05). Conclusion Walking training with the homemade ankle-foot orthopedic band may further decrease the muscle tone of gastrocnemius, increase the active range of motion of ankle and improve the motor function of lower extremities in children with foot drop post spastic hemiplegic CP.

Objective To investigate the correlation of intima-media thickness (IMT) of the lower peripheral arteries and maximal shear rate (SR) in diabetic (DM) patients and to analyze the related risk factors.Methods A total of 84 diabetic patients and 46 controls were recruited in the study.Color Doppler ultrasonography technique was applied to observe the IMT,diameter(D) and hemodynamics of the lower peripheral arteries and the comparison with biochemical parameters was carried out in order to reveal their correlation to the pathological changes of vascular in DM patients.ResultsIn DM group,the SR was lower than that in control group (P<0.05).The risk factors related to the lower peripheral arteries of DM patients also were significant difference to that of control group (P<0.05).Conclusion The decrease of SR in diabetes patients can cause the thicker IMT,which is one of the important factors leading to atherosclerosis and is correlated to various risk factors for metabolic syndrome.

Surgical complication after biliary tract injury are serious complications of Hepatobiliary surgery. The incidence of iatrogenic bile duct injuries has increased significantly since the number of cholecystectomy operations have increased, laparoscopic cholecystectomy became the "gold standard", mini-cholecystectomy established for the treatment of cholelithiasis. Intraoperative hemorrhage can be life-threatening or may lead the death. The common uses of laparoscopic cholecystectomy and mini-cholecystectomy have made the young surgeons less familiar with open cholecystectomy procedure and the approaches to manage the biliary tract injuries. Uncommonly the patient had to undergo hepatic transplantation secondary to biliary tract surgery with several vessel injuries or biliary cirrhosis. Postoperative bile leakage can be managed by effective drainage as soon as possible. These complications uncommon with the expert surgeons, but common with comparatively inexperienced surgeons. There is no substitute of experience and caution in biliary surgery for optimization technique

RESUMO

To study the outcome of biliary reconstruction in cases of Iatrogenic Bile duct Injury and Structures. Retrospective study conducted from Oct. 1989 to Dec. 2002. Dept. of Hepatobiliary Surgery, First Affiliated Hospital, School of Medicine, Xi'an Jiaotong University, Xi'an, People Republic of China. Ninety four patients who underwent biliary reconstruction. Amongst the 94 patients, 74 injuries resulted from Laparoscopic procedure and 28 from Open Cholecystectomy, while one was the result of Mini Cholecystecomy The patients underwent repair based on Bismuth classification. The outcome was graded as excellent, good and poor. There was no pre-operative death in this series. During the follow-up of 13-15 years, 75 [79.8%] patients had excellent results, 13 [13.8%] good results and 6 [6.4%] poor results. Four patients amongst the poor group died. To reduce the incidence of bile duct injuries it is important to recognize the cystic duct, the common hepatic and the common bile duct junction, and to be familiar with the anatomical variations and the congenital anomalies. The classification of iatrogenic bile duct injuries and strictures, and the timing and technique of biliary reconstruction influence the outcome in these cases

RESUMO

Orthotopic liver transplantation was performed for 43 years. One-year survival rates climbed to as high as 88% and operative mortality rates dropped to as low as zero. Liver resection should still be considered the first-line treatment for patients with hepatocellular carcinoma in non-cirrhotic and resectable livers; however liver transplantation must be the preferred approach for cirrhotic livers. Orthotopic liver transplantation for hepatocellular carcinoma at a more advanced stage was not indicated. Combination therapy with hepatitis B immune globulin and lamivudine for hepatitis B virus patients before and after orthotopic liver transplantation increases survival rates compared to non- hepatitis B virus recipients
