RESUMO

The research investigated factors affecting migration, distant registration and their expected effects on leprosy service in Bangladesh. This qualitative study was conducted from April 2018 to September 2019. 130 leprosy-affected people (LAP) were selected for in-depth interview (IDI) and 23 professionals for Key informant's interview. A Semi-structured interview guide was used for data collection. Different factors were identified by key informants (KIs) and IDI participants (IDIPs) both for migration and for distant registration. Main factors for migration of LAP, mentioned by both groups, were fear of social exclusion. Lack of treatment facilities under the treatment category, job transfer under financial category and marriage under the personal category are the other main influencing factors for migration. Reasons for leprosy cases registering in distant places are similar to reasons for migration: lack of expert professionals under treatment category, relatives' influence under the personal category and job transfer under the financial category. It is concluded that migration and distant registration should be considered in studying the geographical distribution of leprosy. Interpretations proposed by key informants may not reflect the actual field situation. The opinions of those directly affected by leprosy should be sought and heeded in designing and evaluating leprosy programmes.

Background:Parkinson’s disease (PD) is a condition in which part of the brain becomes progressively damaged over many years. This study represents the pattern of Parkinson’s disease and help to identify various drugs which are being used at different health care levels in Bangladesh.Methods:Cross-sectional technique was applied as study design in this research work. We accessed the patients with formulated questionnaire of the Department of Neuroscience of National Institute of Neurosciences and Hospital, Dhaka Medical College and Hospital and Bangabandhu Sheikh Mujib Medical University (BSMMU) for data collection from January 2017 to August 2019.100 patients were selected in the ages between 25-80 years, among them 66were male and 34females.Results:A total number of 100 Parkinson’s disease patients (male 66%, female 34%) were recruited for this study. Genetic factor (56%) is the main cause of PD found in this study. Among various symptoms, the prominent symptoms were voice disorders (96%), slowness of movement (90%), mask-like face expression (86%), tremor (80%), sensory and sleep difficulties (78%), excessive sweating (60%) and insomnia (56%). It was observed that along with physiotherapy, drugs used to manage PD were levodopa (14%), carbamazepine (12%), quetiapine (12%), haloperidol (11%), pramipexole (10%), trihexyphenidyl HCl (10%), carbidopa (8%), amlodipine (8%)andclonazepam (8%).Conclusions:Disgrace exists in the personal life and social context of the PD patients which also unfavourably affects their psychosocial aspects of life. Our population-based data provide evidence for a protective effect of Parkinson’s disease in our country.

RESUMO

Oral Myiasis is a condition in which the soft tissues of different parts of oral cavity are invaded by the parasitic larvae of flies known as Maggots. This study presents a case of oral myiasis in 12 years old mentally retarded girl. Clinically, the patient had multiple perforations of the palatal mucosa at mid-hard palate and tunneling of the palatal gingivae from lateral incisor to the first molar on either side communicated to the palatal perforations. The patient was treated by mechanical removal of the maggots with simple curettage and irrigation with paraffin, povidone iodine and normal saline. After complete larvae removal, the wounds healed normally.

OBJECTIVE:To assess the incidence of prostate adenocarcinoma in patients undergoing radical cystoprostatectomy due to bladder cancer in Iranian men. MATERIALS AND METHODS: Fifty cystoprostatectomy specimens removed due to bladder malignancy (2004-2005) at two referral centers (Shaheed Modarress and Shaheed Labbafinejad Hospitals, Tehran, Iran) were examined for the coincidental finding of prostate cancer (PCa). At the time of surgery the patient's serum PSA was less than 4 ng/mL and there were no suspicious lesions by digital rectal examination. Pathologic grade, stage, morphometric volume, number of tumor foci and association with areas of high grade prostatic intraepithelial neoplasia (HGPIN) were assessed by light microscopy. All specimens were totally embedded and whole-mounted. Clinically significant cancers were defined as tumors with 0.5 mL volume, Gleason pattern 4 or 5, pT3, positive surgical margin, and multifocality > 3. RESULTS: Incidentally detected cancer was found in 7 (14 percent) of cystoprostatectomy specimens. HGPIN was present in 1 (14.3 percent) of the cystoprostatectomies with incidentally detected prostate cancer. None of cystoprostatectomies without prostate cancer had HGPIN. Four (57 percent) of the detected cancers were significant. CONCLUSION: We conclude that incidentally detected prostate cancer in Iran is lower than the rates reported in other countries. Further studies are warranted for better declaration of variability of prostate cancer between different ethnic groups.
