RESUMO

OBJECTIVE To mine the signals of adverse dr ug events (ADE)for tolvaptan based on FAERS database ,and to provide reference for safe use of drugs in clinic. METHODS The data of tolvaptan-induced ADE were collected from FAERS database during the first quarter of 2004 to the third quarter of 2020;the reporting odds ratio (ROR)method and the proportional reporting ratio (PRR)method of disproportional method were used for data mining. RESULTS A total of 4 744 ADE reports of the target drug tolvaptan were extracted ,involving 1 279 ADEs. The reporting countries were mainly the United States and Japan ,etc. A total of 199 ADE signals were obtained ,involving 21 system organ classes (SOCs),which mainly focused on various examinations(n=56),hepatobiliary disorders (n=17),renal and urinary disorders (n=14),etc. Among them ,80 signals were not mentioned in existing instructions for tolvaptan in China ,such as decreased glomerular filtration rate ,positional vertigo , rupture of renal cyst ,renal cyst infection ,pulmonary malignant tumor. CONCLUSIONS Before using tolvaptan ,drug evaluation should be performed well ,especially the patients with basic diseases such as heart failure ,liver insufficiency and renal insufficiency. During treatment ,the indexes of liver function and renal function should be closely monitored ;timely intervention measures should be taken to avoid related injury and disease deterioration caused by ADE when ADE or disease progression occurs.

RESUMO

The diagnosis of food allergy in children is one hotspot attracting people′s attention in recent years.The incidence of it shows an increasing trend which exposes problems in the understanding of children′s food allergy in China, especially in the misdiagnosis and missed diagnosis.To further standardize the diagnosis and treatment of food allergy in children, based on the current domestic, foreign guidelines and relevant research evidence, the guideline recommends 16 clinical hot-button issues in the 4 aspects of diagnosis, treatment, prognosis, and prevention.Finally, a diagnosis flowchart has been formulated.The guideline aims to improve the standard diagnosis and treatment of food allergies in children in China.

OBJECTIVE:To provid e reference for pharmaceutical workers to better understand Novel Coronavirus Infection : Expert Consensus on Guidance and Prevention Strategies for Hospital Pharmacists and the Pharmacy Workforce (hereinafter referred to as “expert consensus ”),and to apply and practice in specific work ,so as to give full play to the role of pharmacists to help fight the epidemic.METHODS :The background of the formulation and revision of the expert consensus were introduced ,and its main contents and viewpoints were interpreted. RESULTS & CONCLUSIONS :The text of expert consensus is divided into 8 parts,mainly including disease diagnosis and treatment [SARS-CoV- 2 infection related background ,clinical manifestations and diagnosis, treatment],hospital pharmacy (prevention and control strategy ,work guidance ),drug and facility support management(key drug/facility/equipment support ,management and use of the drug in special circumstances ),information sources and related resources ,etc.,which comprehensively and detailedly provide information ,guidance and strategies for coronavirus SARS-CoV-2 infection prevention and control to play the role of pharmacists in hospital pharmacy well ,do well in the protection of staff in different pharmaceutical posts ,drug security work in response to epidemic situation ,and develop pharmaceutical care. So far,the understanding of SARS-CoV- 2 in the pharmaceutical industry is relatively limited. Based on the accumulated experience and progress in epidemic prevention and control ,the expert consensus will be updated and improved continuously ,so as to provide guidance and help for hospital pharmaceutical personnel.

OBJECTIVE: To standardize and guide the formulation process of guideline and guarantee the evidence-based formulation of Rapid Advice Guideline for Intravenous Azithromycin in Pediatrics scientifically. METHODS: The protocol of the guideline was introduced, such as guideline-making institution and registration, establishment of project group, scope of application, declaration of interest and funding support, identification of the clinical issues and outcome indexes, evidence retrieval and processing, invastigation of patients preferences and values, economics analysis, development of recommendation, external review and approval, guideline release, dissemination and update, etc. RESULTS: The construction of guidance committee, expert group and secretarial group, the registration (registration number: IPGRP-2016CN013) of the guideline had been achieved according to this protocol; clinical issues (such as indications, usage and dosage, ADR and treatment, drug use in special population) and effectiveness, safety outcome indexes had been formed. The recommendations have been issued by the end of 2017; the full text and interpretation of the guideline will be published in 2018; the guideline will be updated in 2022. CONCLUSIONS: This protocol provides the methodological and programmatic guide and provides reference for evidence-based formulation of the guideline scientifically so as to promote clinical rational drug use.

OBJECTIVE@#To understand the epidemiological characteristics of workers who are burn patients, and to provide basis for prevention and treatment of burn at work.@*METHODS@#We investigated 4 078 burn workers in 9 cities in Hunan provincial enterprises, and different trades managed by municipal government from January 1st, 2005 to December 31st, 2010.@*RESULTS@#The incidence rate of employment injury was 94.84 per 10 thousand workers each year in Hunan. That of the enterprises directly managed by Hunan Medical Insurance Bureau was 93.71 per 10 thousand workers each year, and that of the enterprises managed by Medical Insurance Bureaus of cities or districts was 95.02 per 10 thousand workers each year. The burn incidence accounted for 2.00% of the total work injury in Hunan Province (7.35% in the enterprises directly managed by Hunan Medical Insurance Bureau and 1.39% in districts and industries). There were 4 078 burn patients from the sorted units [1 823 out-patients and 2 255 in-patients; 3 498 males (85.78%) and 580 females (14.22%) with age of (38.64±11.56) years]. The ratio between males and females was 6.03:1. Among the 4 078 patients, 2 979 were from the rural area (71.78%), with 1 100 out-patients and 1 379 in-patients. The burn was mostly mild and moderate (64.66%), and mainly occurred on the face, head, arms, and legs (61.55%). The main causes of burn were heating power (74.1%), chemical stuff (14.37%), and electric power (10.78%). The death rate was 1.42%. The burn accidents often happened between July and September. The average time from being burned to be in the hospital for treatment was 3.91 hours.@*CONCLUSION@#Burn accidents, especially mild and moderate burn, occur mainly in young, married and poorly educated males from rural areas. Heating power, chemical stuff, and electric power are the key causes for burn. Work injury prevention measures should be improved in high risk enterprises.

Objective To compare the cost-effectiveness of intervention protocols with or without rehabilitation approaches for burn patients.To explore the pros and cons of the two treatment programs,especially in terms of efficacy.Methods Data on 164 burn patients hospitalized in four hospitals in Hunan Province were collected and compared in terms of the therapeutic interventions employed,the direct medical costs and the effects obtained at the end of the first two months of hospitalization.The patients were divided into an early rehabilitation therapy group and a control group,based on the interventions they received.Results The average total direct medical cost of the early rehabilitation group was significantly lower than that of the control group [((¥)59 380.80 ±196.87) versus ((¥)67 471.62 ±217.23)].The average Vancouver scar scale (VSS) results in the early rehabilitation group were significantly better than those of the controls,but their Zung's Self-rating Anxiety Scale and Barthel index scores were not.The average cost-effectiveness ratio (C/E) of the early rehabilitation group as judged by these scores was lower than that of the control group.Conclusion Early rehabilitation can improve the therapy of burns patients and reduce subsequent expenditures.It is a cost-effective approach to treating burns patients.

OBJECTIVE@#To compare the cost-effect of 2 therapeutic regimens and to find out a better therapeutic regimen with reasonable price and better effect, hoping to improve the quality of burn patients and reduce the waste of medical resources.@*METHODS@#We collected 1017 burn patients from Hunan and Sichuan, and divided them into a research group (the moist exposed burn therapy group) and a control group (the escharectomy and grafting group). We analyzed the cost and effect of the 2 groups from general condition, direct medical cost during treatment and curative effect.@*RESULTS@#The average total cost of the research group [93633.71 (82260.79, 107576.34) yuan] was significantly lower than the control group [175077.93 (131433.23, 228918.83) yuan] (P<0.05). The total effective rate of the research group was higher (97.28%) than that in the control group (92.89%) (P<0.05). The cost-effect of the research group (962.52) was better than that of the control group (1884.79).@*CONCLUSION@#Cost-effect analysis is objective and direct in the evaluation of different treatments. The moist exposed burn therapy is a better and cheaper therapeutic regimen.

Objective To study the expression characteristics of cytokeratin 19(CK19) in different live tissues from victims with third-degree burns to explore the possible mechanisms of tissue reparation and regeneration in orthophoria.Methods 10 young victims with third-degree burns were included in this study.Alive tissues at different regeneration stages were studied by immunohistochemistry with streptavidin-biotin-peroxidase method using specific anti-CK19 monoclonal antibody. Only cytoplasm expression was considered as specific.Normal skin tissue and chronic skin ulcer were used as controls.The study was focused on the distribution and morphological features of CK19-positive cells.Results ⑴In normal skin ,CK19-positive cells were seen in basialis layer of epiderm and cutaneous appendages.⑵CK19-positive cells were not found in subcutaneous tissue at 3~6 days after the injury,but they were seen in granulation tissues.⑶A lot of CK19-positive cells were detected in early regeneration tissues.⑷In late regeneration tissues,CK19-positive cells were detected in basialis layer of epiderm and cutaneous appendages.⑸Advanced regenerationed epidermal tissue was similar to that of normal.⑹No CK19-positive cells were observed in all of 6 cases with chronic skin ulcer.Conclusion It is possible that victims with third-degree burns could be healed by tissue regeneration orthophoria.
