RESUMO

Objective To evaluate the efficacy and safety of locating the target bronchus and treating through the chest wall and airway intervention in 29 cases with lung abscess secondary to pulmonary bulla. Methods In 29 cases the lung abscess secondary to pulmonary bulla had a mean diameter of 14 cm, of which 25 with COPD. Implement-ing 4 different ways to locate the target bronchus of lung abscess: Preoperative pulmonary CT, conventional fiber bronchoscopy, and locating the target bronchus by infecting colored saline separately forward and backward. Under guiding of CT, the central venous catheter was inserted via chest wall into lung abscess, which had pus suction, lavage, medicine injection and continuous negative pressure suction and then under direct vision of bronchoscopy, all of the target bronchi of pulmonary absess were occluded with fibrin glue. Results All of the 29 cases, the target bronchi of lung abscess were localized by lung CT in 17, by conventional bronchoscopy in 23, and by infecting col-ored saline forward in 26 and backward in 24 respectively. Cured in 25 and improved in 3, the total effective rate was 96.6%. The adverse effects:chest pain in 3, pneumothorax in 2, pyothorax, pyopneumothorax, subcutaneous em-physema, the drainage catheter falling off and dislocating separately in 1. Conclusion It was simple, safe and effec-tive to treat lung abscess secondary to lung bulla through the chest wall and airway intervention.

Objective To evaluate efficacy and safety of two methods for gas-filled and liquid-filled balloon of Swan-Ganz catheter to locate the bronchus of pulmonary air leakage. Methods All of 38 cases with pneumothorax were randomly divided into two groups. Group A : the bronchus of pulmonary air leakage were detected by injecting gas into balloon of Swan-Ganz catheter. Group B: the bronchus of pulmonary air leakage were detected by injecting physiological saline into balloon of Swan-Ganz catheter. Results The locating success rate in Group A was 89. 5% (17/19) and Group B was 94. 7% (18/19) ( P>0. 05). The mean time of locating in Group A was (13. 3 ±4. 1) min and Group B was (9. 6 ±3. 2) min( P <0. 05).In Group A, the dislocation rate was 47.4% and adjacent bronchial compression rate was 68.4% , and was respectively higher than Group B 21. 1% and 26. 3% ( P < 0. 05). Patients in both groups were well tolerated without any obvious adverse effects. Conclusion It is a more simple, quicker, the same safe and effective to locate the broken bronchus of pneumothorax by injecting physiological saline into balloon of Swan-Ganz catheter.

0.05).The concentration of IL-6 in sputum of multi-virus infection group(122.51?39.86)ng/L was higher than in single virus infection group(65.30?34.92)ng/L.The concentration of IL-6 in sputum of bacteria-virus mixed infection group(120.31?46.62)ng/L was higher than in bacteria or virus single infection group(83.61?47.83)ng/L.Conclusion Streptococcus pneumonia and influenza virus A infection are important factors in AECOPD at early stage.Virus infection would prolong recovery time,increase inflammation of the airway and even induce bacteria infection.Therefore,we should pay more attention to the virus infection in COPD patients,especially A-type influenza virus.

RESUMO

AIM: To observe the changes of endogenous hydrogen sulfide/cystathionine-?-lyase(H2S/CSE) system while acute lung injury induced by LPS in rats.METHODS: Eighty rats were randomly divided into six groups(n=8): Ⅰ,control group;Ⅱ,LPS 1 h group;Ⅲ,LPS 3 h group;Ⅳ,LPS 6 h group;Ⅴ,LPS 9 h group;Ⅵ,LPS 12 h group.The ALI model of rats was prepared with LPS.The rats were respectively killed at 1,3,6,9 or 12 h after administration of LPS.The morphological changes of lung tissues were observed by light and electron microscope.The lung coefficient and the wet-to-dry weight ratio were measured.The contents of IL-1? and IL-10 in serum,the H2S level in plasma and the CSE activity in lung tissue were respectively detected.RESULTS: ⑴ In LPS 1 h group,the morphology,the lung coefficient,the wet-to-dry weight ratio,the H2S level and the CSE activity showed no changes compared with the control group.The contents of IL-1? and IL-10 were increased compared with the control group(IL-1?,P
