RESUMO

Objective:To explore the influence of multidimensional integration and 360° support on the function of family and marriage, and quality of life in women patients with systemic lupus erythematosus.Methods:Totally 196 patients with systemic lupus erythematosus from Department of Rheumatology and Immunology, The Second Affiliated Hospital of the Air Force Medical University from August 2016 to November 2017 were included. According to random number table method, these patients were divided into observation group and control group as 98 cases each. Conventional care and hospital discharge were used for control group. On the basis of this, multidimensional integration and 360° support were used for patients of observation group. The function of family and marriage, quality of life in patients were assessed before and after 3 months of the intervention. The treatment adherence was evaluated in 3 months and 6 months after intervention.Results:Before intervention, the marriage family function score, marital satisfaction, conflict resolution methods and the relationship with friends and family, husband and wife exchange scores of the observation group were (2.3 ± 0.5), (24.6 ± 6.1), (25.7 ± 7.1), (28.2 ± 6.9), (28.8 ± 6.9) points, respectively. Three months after intervention, these scores were (2.5 ± 0.7), (31.6 ± 5.0), (31.7 ± 5.3), (28.1 ± 6.8), (29.0 ± 7.1) points, respectively. There was statistically significant difference between before and after the intervention ( t values were -2.371 - 8.631, P < 0.01). These scores of control group before the intervention were (2.3 ± 0.6), (24.5 ± 6.2), (25.2 ± 7.2), (32.5 ± 6.0), (33.9 ± 6.3) points, respectively. Three months after intervention, these scores were (2.3 ± 0.4), (24.5 ± 6.2), (26.1 ± 6.9), (29.1± 4.8), (28.5 ± 7.2) points. Significant differeces were found between before and after the intervention in control group ( t values were -3.878-6.323, P < 0.05 or 0.01). There was statistically significant difference between the two groups after the intervention ( t values were 2.675-8.631, P<0.01). As for observation group, planning (62.8 ± 27.2 vs. 75.5 ± 25.4) and intimate relationship (62.8 ± 25.2 vs. 78.2± 24.9) in quality of life were obviously difference before and after 3 months of intervention ( t values were 3.050, 3.639, P < 0.01). As for control group, planning (62.5 ± 27.6 vs. 65.7 ± 24.9) and intimate relationship (65.8 ±25.2 vs. 63.5 ± 23.8) in quality of life were obviously difference before and after 3 months of intervention ( t values were 2.375, 3.132, P < 0.01). There was statistically significant difference between the two groups after the intervention ( t values were 3.050, 3.639, P < 0.01). The treatment adherence of observation group was significant better than control group. After 6 months intervention, the treatment adherece of observation group was 83.67% (82/98), while the treatment adherece of control group was 44.89% (44/98), significant differences were found btween the two groups ( χ2 value was 0.511, P < 0.01). Conclusion:Multidimensional integration and 360° support obviously improved function of family and marriage, improved the understanding of disease, and self-management ability of patients. Therefore, it can increase the treatment adherence and improve quality of life in SLE patients.

RESUMO

Objective To study the application effect of interdisciplinary collaborative transformational learning mode in undergraduate nursing students′ disaster nursing teaching. Methods A total of 115 undergraduate nursing students in 2014 and 2015 were enrolled in this study. The subjects were divided into two groups: the control group (62 cases) and the experimental group (53 cases). The control group adopted the case teaching mode of full-time teachers, and the experimental group adopted the interdisciplinary collaborative transformation learning mode. The theoretical knowledge, skills, practical skills, critical thinking, humanistic accomplishment and nurse-patient communication skills of two groups of nursing students before and after intervention were compared. Results The scores of theoretical knowledge, operational skills, practical ability, critical thinking, humanistic accomplishment and nurse-patient communication before the intervention were (42.72±10.18), (35.72±7.11), (41.19±5.32), (298.09±3.98), (298.38±6.03), (52.87±6.02) points, (87.21±5.05), (87.02±4.81), (86.13±4.48), (378.47± 9.16), (327.15 ± 12.01) and (69.11 ± 3.38) points after intervention respectively. There was significant difference before and after intervention t=- 57.687-- 14.328, P<0.01). The scores of theoretical knowledge, operational skills, practical ability, critical thinking, humanistic accomplishment and nurse-patient communication before the intervention were (43.90±9.56), (38.21±8.87), (41.39±6.77), (296.76± 10.02), (296.89±6.37), (52.37±5.45) points, (82.27±6.46), (84.06±5.63), (78.21±6.99), (344.18±10.88), (315.16±12.82) and (64.45±3.37) points after intervention, respectively. There was significant difference before and after intervention (t=-38.221--9.823, P<0.01). There was no significant difference between the two groups before the intervention (P>0.05), and the score of the experimental group was significantly higher than that of the control group after intervention (t=- 18.107-- 2.997, P<0.01). Conclusions Introducing interdisciplinary cooperative transformation learning in disaster nursing teaching can promote the enhancement of nursing students'knowledge of disaster succeeding.

Objective To study the impact of earthquake disasters on the mental health of military medical personnel during emergency rescue. Methods Three hundred military medical personnel who participated in the emergency rescue of earthquake disasters in five military hospitals in Shaanxi Province were selected as the study subjects, and their mental health levels were analyzed using the Mental Symptom Checklist 90 (SCL-90). Results In this survey, a total of 300 SCL-90 questionnaires were issued, 300 were recovered, and the effective recovery rate was 100%. Only 30.67%(92/300) of people participated in emergency disaster rescue during investigation, 31.33% (94/300) had psychological problemsafter arriving in the earthquake-stricken area after one week, such as somatization, obsessive-compulsive disorder, anxiety, and horror. SCL-90 scores were 0.45±0.03, 1.13±0.13, 0.79 ±0.08, 1.07±0.09, 1.01±0.14. The psychological stress response rates of the emergency response of military medical staff of different characteristics were different. Among them, the stress response rates of military personnel with different degrees (specialty, bachelor or above) and whether they have participated in rescue work were 74.07% (40/54), 21.95% (54/246), 15.22% (14/92), 38.46% (80/208).The difference was statistically significant (χ2=55.913,16.018, P<0.01). Conclusions The medical staff of the army participating in the earthquake disaster rescue has psychological pressure due to many injuries, environmental risks, lack of supplies, etc. The medical unit should provide adequate training in theory and practice to its personnel and focus on the mental health of rescue workers, give timely psychological counseling to ensure the successful completion of the rescue.
