RESUMO

Disintegration time is a key parameter that affects the palatability and compliance of oral soluble films. At present, there is no standard method to determine the disintegration time of oral soluble films. In this study, we compared the six methods (pharmacopoeial disintegration method, petri dish method, sponge surface method, slide frame and ball method, partially immersed into liquid (without weight attached) and partially immersed into liquid (with weight attached)) to determine the in vitro disintegration time of oral soluble films with different thickness, and evaluated the correlation with the in vivo disintegration time. The results showed that the repeatability and correlation of pharmacopoeial disintegration method and the partially immersed into liquid method (with weight attached) were excellent, with the endpoint of disintegration testing easy to determine. Partially immersed into liquid method (with weight attached), properly simulating the physiological condition in oral cavity, showed strong operability, good repeatability and in vitro-in vivo correlation, and was suitable for in vitro disintegration evaluation of oral soluble film dosage form. The adult sensory evaluation study was a research-based clinical trial conducted with informed consent from all subjects in accordance with the ethical requirements of Good Clinical Practice.

RESUMO

Dexmedetomidine is an α2 adrenoceptor agonist and has cardioprotective effect,the mechanism of which is being studied.Increasing studies have proved the clinical value of dexmedetomidine in reducing postoperative complications and improving the prognosis of patients.Therefore,this review summarizes the cardiac protection mechanism of dexmedetomidine based on the existing studies and expounds the application of dexmedetomidine in the perioperative period of cardiovascular surgery.

RESUMO

Objective@#This study aimed to explore the protective effect of procyanidin B2 (PCB2) on acute liver injury induced by aflatoxin B (AFB ) in rats.@*Methods@#Forty Sprague Dawley rats were randomly divided into control, AFB , AFB + PCB2, and PCB2 groups. The latter two groups were administrated PCB2 intragastrically (30 mg/kg body weight) for 7 d, whereas the control and AFB groups were given the same dose of double distilled water intragastrically. On the sixth day of treatment, the AFB and AFB + PCB2 groups were intraperitoneally injected with AFB (2 mg/kg). The control and PCB2 groups were intraperitoneally administered the same dose of dimethyl sulfoxide (DMSO). On the eighth day, all rats were euthanized: serum and liver tissue were isolated for further examination. Hepatic histological features were assessed by hematoxylin and eosin-stained sections. Weight, organ coefficient (liver, spleen, and kidney), liver function (serum alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, total bilirubin, and direct bilirubin), oxidative index (catalase, glutathione, superoxide dismutase, malondialdehyde, and 8-hydroxy-2'-deoxyguanosine), inflammation factor [hepatic interleukin-6 (IL-6) mRNA expression and serum IL-6], and bcl-2/bax ratio were measured.@*Results@#AFB significantly caused hepatic histopathological damage, abnormal liver function, oxidative stress, inflammation, and bcl-2/bax ratio reduction compared with DMSO-treated controls. Our results indicate that PCB2 treatment can partially reverse the adverse liver conditions induced by AFB .@*Conclusion@#Our findings indicate that PCB2 exhibits a protective effect on acute liver injury induced by AFB .

Objective To investigate the correlation between skin elasticity and setup error in optical surface image-guided radiotherapy.Methods The skin elasticity (R7) data of the head,chest and abdomen were extracted and analyzed its correlation with age by systematic literature review.Fifty-four patients diagnosed with nasopharyngeal carcinoma,breast cancer and cervical cancer were recruited in this study.Firstly,the patients were positioned based on the room laser and markers.Subsequently,the patient position was verified by the Varian On-Board Imager,and then C-Rad Catalyst was adopted to obtain surface images in two states (mask or non-mask) as reference images.In the subsequent fraction treatment,after initial positioning,the local calibration was performed by Catalyst,and setup errors in three directions were recorded.Meanwhile,the patient setup was verified by CBCT twice a week.The Pearson correlation analysis was performed to analyze the correlation between setup error and age.Results The skin elasticity was negatively correlated with aging (P<0.01).The correlation coefficient between random error and age in headand-neck cancer were 0.645,0.624 and 0.866 in the AP,SI and LR directions (all P<0.05) for male patients without mask,respectively.The system error was significantly correlated with age in the LR direction (P<0.05) for male patients,and in the AP direction (P< 0.05) for female patients with head-and-neck cancer without mask.The setup error had a significant correlation with skin elasticity in male patients with head-and-neck cancer,and the sequence of absolute value of correlation coefficient was LR > SI > AP.Conclusion In optical surface-guided radiotherapy of head and neck cancer,skin elasticity may be a significant index for assessing the setup errors in male patients.

Objective@#To investigate the clinical application and efficacy of treatment in inverted nipple with a kind of lactation function preserved method of minimally invasive surgery.@*Methods@#96 patients with type Ⅱ and type Ⅲ inverted nipple admitted by our hospital from January 2013 to December 2017 were selected for treatment by modified self-made nipple orthotic device. Following up 7 months to 5 years to observe the effect of nipple correction and lactation function.@*Results@#87 cases of the patients received satisfactory therapeutic effect, and the lactation function was not affected; 2 cases of type Ⅱ and 3 cases of type Ⅲ inverted nipple were evaluated as medium, with mild nipple retraction; 4 cases were worn for less than 1 month, and the outcome was poor.@*Conclusions@#The modified nipple orthotic device is suitable for type Ⅱ and Ⅲ patients with inverted nipple, which is simple and effective method in nipple inversion.

The system error was significantly correlated with age in the LR direction (P<0.05) for male patients, and in the AP direction (P<0.05) for female patients with head-and-neck cancer without mask. The setup error had a significant correlation with skin elasticity in male patients with head-and-neck cancer, and the sequence of absolute value of correlation coefficient was LR>SI>AP.@*Conclusion@#In optical surface-guided radiotherapy of head and neck cancer, skin elasticity may be a significant index for assessing the setup errors in male patients.

BACKGROUND: Anterior cervical transpedicular screws placement technique provides nice mechanical stability and is of very promising application prospects. However, the technology is difficult to operate and has a high risk, and has not been widely used. OBJECTIVE: To design a new three-dimensional (3D) printing template for bilateral anterior cervical transpedicular screws placements and assess its feasibility and safety in anterior cervical pedicle screw placement. METHODS: Six cadaveric cervical specimens, including three males and three females, were used in this experiment. Data of thin layer CT scanning of the specimens were saved in DICOM format and then imported into Mimics 17.0 software. Following 3D reconstructions of the cervical spine, guiding holes for C3-C7 bilateral anterior cervical transpedicular screws trajectories were designed; pedestals for the guiding holes were then designed via reversely thickening the bony structure of the anterior and 1/2 superior-anterior surface of vertebral body, and 1/2 anterior joint surface of bilateral processus uncinatus. Practical objects of the templates were obtained via 3D printing and were then used for guiding bilateral anterior cervical transpedicular screws replacements from C3 to C7. CT scanning was conducted again and the accuracy of anterior cervical transpedicular screws replacements was evaluated from sectional CT images. The difference of deviational angles on axial plane (α1, α2) and sagittal plane (β1, β2) between real and simulated trajectories were compared in Mimics 17.0 software. RESULTS AND CONCLUSION: (1) A total of 60 anterior cervical transpedicular screws were successfully inserted; 57 screws were completely located in pedicles and were judged as grade 0, representing an accuracy of 95.0%. The other three anterior cervical transpedicular screws perforated from pedicles, including grade 1 perforation in two screws (3.3%) and grade 2 perforation in one screw (1.7%). (2) By comparing real and simulated trajectories, the medical and lateral deviational angles were (0.867±0.787)° and (0.783±0.792)°, respectively (P > 0.05); the cephalad and caudal deviational angles were (1.362±1.380)° and (1.314±1.300)°, respectively (P > 0.05). (3) With the help of the 3D printing template designed in this study, bilateral anterior cervical transpedicular screws replacements could be smoothly carried out at high inserting safety.

Objective: To review and summarize the multimodal analgesia regimen in total hip arthroplasty (THA), and to provide basis for individual analgesia in clinic. Methods: The literature of multimodal analgesia regimen in THA in recent years was systematically searched and summarized. The perioperative pain management regimens of THA in authors' center were introduced at the same time. Results: Postoperative pain of THA is an important factor affecting patient satisfaction, so pain management is one of the key points in perioperative management of THA. At present, multimodal analgesia regimen is used in THA, which works by blocking pain conduction pathway at different levels, including wound ice compress, non-steroidal anti-inflammatory drugs, local infiltration anesthesia, peripheral nerve block, intraspinal anesthesia, central analgesics, and so on. Conclusion: Multimodal analgesia regimen is the perioperative pain management of THA, which includes many analgesic methods. The safe and effective analgesic methods should be chosen according to the specific conditions of patients in order to achieve individualized analgesia.

Aim To establish non-alcoholic fatty liver disease mouse model and study different kinds of lymphocytes in C57BL/6J mouse model. Methods SPF male C57BL/6J mice were randomly divided into control group ( normal diet ) and model group( normal diet with high fat diet by gavage) . Models of non-alcoholic fatty liver disease were established. At 12th and 16th weeks, body weight, liver index, serum TC, TG, HDL, LDL, ALT, AST were measured. Pathological examination of fat deposition in liver was performed. Flow cytometry was used to assay the percentage of natural killer cells, T helper cells, natu-ral killer T cells and IL4 +NKT cells in mouse liver. Results Liver index, serum TC, LDL, ALT, AST were significantly higher in model group(P<0.05) after 16 weeks. Pathological sections showed that liver fat deposition in model group was quite severe and large lipid droplets spread through the mouse liver. The percentage of natural killer T cells increased significantly( P<0.05 ) and the percentage of IL4 +NKT cells increased even more obviously(P<0.01). Conclusions C57BL/6J mice fed with normal diet and high fat diet by gavage can form a good non-alcoholic fatty liver disease mouse model. In this model, the number and activity of natural killer T cells are significantly changed, and natural killer T cells may be the new target of the mechanism and drug treatment of nonalcoholic fatty liver dis-ease.

Objective To evaluate the advantage of 3D printing technique in treatment of complicated ace-tabula fracture,we compared the early clinical outcomes of surgical treatment for complex acetabula fracture with traditional surgery(group A)and that assisted by 3D printing technique(group B).Methods A prospective anal-ysis of 35 complicated acetabula fractures was performed at our department,13 patients cured assisted with 3D printing technique. The other 22 patients performed traditional surgery. Surgical trauma,systematic inflammation, immediate reduction effect were evaluated by Matta image assessment standard,and rehabilitation functional status evaluated by Harris score system and Merled' Aubigne and Postel Grading standard. Results There were signifi-cant differences between the two groups regarding intraoperative blood loss,postoperative drainage,operation time (P<0.05),and group B was superior to group A.C reactive protein,leucocyte and creatine kinase MM in periph-eral blood in group A were much higher than those in group B at 24 hours postoperatively(P<0.01).The result of reduction of fracture is poor in group A,but excellent rate of reduction is about 84% in group B. The result of Merled' Aubigne and Postel scores grading was superior in group B(P = 0.013). The excellent and good rate of Harris function score in B group was significantly higher than that in group A(P=0.027).Conclusion The treat-ment of complicated acetabula fracture assisted by 3D printing is excellent in short-term follow up,achieving good reduction with limited trauma,long-term prognosis needs to be further followed up.

<p><b>OBJECTIVE</b>To observe the effect of S100A4 gene silencing mediated by small interfering RNA (siRNA) on the proliferation of bladder cancer stem cells (CSCs) and their capacity of xenograft tumor formation.</p><p><b>METHODS</b>MB49 bladder cancer stem cells (MCSCs) were isolated and identified. The differentially expressed protein S100A4 was identified in MCSCs using isobaric tags for relative and absolute quantitation technology (iTRAQ). A siRNA targeting S100A4 was constructed and transfected into MCSCs, and its inhibitory effects on S100A4 expression in MCSCs were assessed with Western blotting and qPCR. The effects of siRNA-mediated S100A4 silencing on the proliferation and xenograft tumor formation ability of MCSCs were observed.</p><p><b>RESULTS</b>Among the 65 differentially expressed proteins identified by iTRAQ combined with LC/MS/MS, S100A4 protein showed the most distinct differential expression in MCSCs. Transfection of MCSCs with S100A siRNA significantly inhibited the expressions of S100A4 at both mRNA and protein levels, caused obvious suppression of the cell proliferation, and attenuated the xenograft tumor formation ability of the cells in nude mice.</p><p><b>CONCLUSION</b>S100A4 in MCSCs is associated with the recurrence and metastasis of bladder cancer. S100A4 may serve as a potential therapeutic target for eliminating bladder CSCs.</p>

Objective To discuss the clinical efficacy of hot-tonifying needling in treating stomachache due to deficient cold in spleen-stomach.Method Forty patients with stomachache due to deficient cold in spleen-stomach were divided into a treatment group and a control group by using random number table method according to registration order, 20 cases in each group. The control group was intervened by twisting tonifying method, while the treatment group was intervened by hot tonifying needling method. The Visual Analogue Scale (VAS) was observed before and after the treatment, and the clinical efficacies were compared.Result The VAS scores were significantly changed after the treatment in both groups (P<0.05); there was a significant difference in comparing the VAS score between the two groups after the treatment (P<0.05); the difference in the change of VAS score between the two groups was statistically significant (P<0.05). The total effective rate was 95.0% in the treatment group, versus 60.0% in the control group, and the difference was statistically significant (P<0.05), and there were significant differences in comparing the clinical efficacy and relapse rate between the two groups (P<0.05).Conclusion Hot-tonifying needling is effective in treating stomachache due to deficient cold in spleen-stomach, with less adverse effects and lower relapse rate.

Objective·To explore the molecular classification and prognostic prediction of gastric cancer based on nonnegative matrix factorization (NMF).Methods·Cases of gastric cancer were acquired from Gene Expression Omnibus (GEO). Expression profiling of lncRNA was performed by using a lncRNA-mining approach. NMF model was built with Consensus Cluster Plus package. The relationship among NMF subgroups and clinical relevance was assessed. Results·According to the molecular classification based on NMF, samples were divided into three subgroups. Significant difference was observed in relapse state, lymph node ratio, Lauren classification, TNM stage and age of onset among three subgroups. High-risk group was identified with shortest relapse time by survival analysis both in GSE62254 and GSE15459. Multivariate Cox proportional-hazards regression showed that NMF model based molecular classfication could be regarded as an independent risk factor for gastric cancer. Gene set variance analysis (GSVA) and gene set enrichment analysis (GSEA) showed that the high-risk subgroup was enriched in several tumor development pathways. Conclusion·Based on NMF model, the molecular classification of gastric cancer can be used for treatment decision and prognostic prediction.

Aim To observe the effect of pyrimidine dithiocarbamate (PDTC) on the variance of disc morphology and the expressions of TNF-α, MMP-9 in the cervical disc in cervical dynamic equilibrium rat models, and to investigate the roles of PDTC in the process of intervertebral disc degeneration and the mechanism involved.Methods Fifty-four SD rats were divided into three groups randomly, then the dynamic equilibrium rat model was established by cutting the nuchal superficial and deep muscle of the rats.The dynamic equilibrium rats with PDTC solution intraperitoneal injection after operations were defined as PDTC group (group A), the models with saline intraperitoneal injection after operations as saline group (group B), the rats of fake operation with saline intraperitoneal injection as blank group (group C), and the animals were sacrificed in batches 10 weeks, 12 weeks, 16 weeks respectively after operation.The C5, C6 vertebrae and C5/6 discs were harvested, and the disc morphology was observed.TNF-α, MMP-9 mRNA expressions were detected by q-PCR and protein expression was observed by Western blot.Results Compared with the saline group, the morphology of disc in PDTC group was destructed slightly and fiber ring arranged orderly.TNF-α, MMP-9 gene and protein expressions had no obvious changes (P>0.05) in blank group (group C) at each time point.The expressions of IL-6, MMP-9 mRNA increased with time in group B, but the amount increased fast firstly, and slow lately, reaching peak in 12 weeks.The expression of TNF-α, MMP-9 protein became steady in group B from 10 weeks compared with other time points(P>0.05).TNF-α, MMP-9 genes and proteins expression decreased obviously in PDTC group (group A) compared with saline group (group B) (P<0.01) at each time point, but higher than blank group C(P<0.01) at each time point.Conclusions TNF-α and MMP-9 are important inflammatory factors involved in rat cervical disc degeneration, PDTC relieves the degeneration of rat cervical disc by reducing the expression of TNF-α and MMP-9 via disturbing the NF-κB signal pathway probably, and PDTC may become potential medicine for disc degeneration.

<p><b>PURPOSE</b>Traumatic diaphragmatic rupture (TDR) needs early diagnosis and operation. However, the early diagnosis is usually difficult, especially in the patients without diaphragmatic hernia. The objective of this study was to explore the early diagnosis and treatment of TDR.</p><p><b>METHODS</b>Data of 256 patients with TDR treated in our department between 1994 and 2013 were analyzed retrospectively regarding to the diagnostic methods, percentage of preoperative judgment, incidence of diaphragmatic hernia, surgical procedures and outcome, etc. Two groups were set up according to the mechanism of injury (blunt or penetrating).</p><p><b>RESULTS</b>Of 256 patients with a mean age of 32.4 years (9-84), 218 were male. The average ISS was 26.9 (13-66); and shock rate was 62.9%. There were 104 blunt injuries and 152 penetrating injuries. Preoperatively diagnostic rate was 90.4% in blunt injuries and 80.3% in penetrating, respectively, P < 0.05. The incidence of diaphragmatic hernia was 94.2% in blunt and 15.1% in penetrating respectively, P < 0.005. Thoracotomy was performed in 62 cases, laparotomy in 153, thoracotomy plus laparotomy in 29, and combined thoraco-laparotomy in 12. Overall mortality rate was 12.5% with the average ISS of 41.8; and it was 21.2% in blunt injuries and 6.6% in penetrating, respectively, P < 0.005. The main causes of death were hemorrhage and sepsis.</p><p><b>CONCLUSIONS</b>Diagnosis of blunt TDR can be easily obtained by radiograph or helical CT scan signs of diaphragmatic hernia. For penetrating TDR without hernia, "offside sign" is helpful as initial assessment. CT scan with coronal/sagittal reconstruction is an accurate technique for diagnosis. All TDR require operation. Penetrating injury has a relatively better prognosis.</p>

<p><b>OBJECTIVE</b>To investigate the relationship between degree of abnormal deeper lateral femoral notch between anterior cruciate ligament tear.</p><p><b>METHODS</b>The radiograph and MRI image material of 16 patients with anterior cruciate ligament injury from January 2013 to November 2013 were reviewed including 14 males and 2 females with an average age of 28.3 years old ranging from 18 to 52 years. Eleven cases was on right side and 5 on left. Survey tool of PASC imaging system was used to measure the depth of lateral femoral notch in patients with abnormal indicated by lateral X-ray view or sagittal view of MRI in knee joint,while clinical data,physical examination,image material in arthroscopy of these patients were retrospective researched.</p><p><b>RESULTS</b>Four patients had an abnormal lateral femoral notch with the depth of 2 mm on lateral X-ray and sagittal MRI, while positive anterior drawer sign and Lachman test as well as anterior cruciate ligament tears on MRI, and completed tears were comfirmed on the operation of arthroscopy. Two patients without abnomal lateral femoral notch on lateral view of X-ray while with the depth of 1 mm on sagittal view of MRI were also coupled with positive anterior drawer sign and Lachman test as well as anterior cruciate ligament tears on MRI, and one of them were comfirmed completed anterior cruciate ligament tears on the arthroscopy operation and completed tear could not comfirmed on another one because of disagreed with arthroscopy operation.</p><p><b>CONCLUSION</b>There appears to be an association between abnormal lateral femoral notch on lateral view of knee with anterior cruciate ligament tears. An abnormal deeper lateral femoral notch is an indirect evidence for anterior cruciate ligament tears.</p>

Background and purpose:It has beenreported that miR-1284 is associated with gastric cancer lymph node metastasis in the research of microRNA microarray in human gastric cancer tissues. But the specific role of miR-1284 in gastric cancer has not been reported. The aim of this study was to investigate the effect of miR-1284 over-expression on the gene expression profiling and invasion/metastasis of human gastric cancer SGC-7901 cells. Methods:Gastric cancer SGC-7901 cells of LV-miR-1284 group were transfected with lentiviral vectors of miR-1284, cells of LV-NC-GFP group were transfected with lentiviral vectors without miR-1284, and cells of control group were not transfected with lentiviral vectors. The expression of miR-1284 was detected by the real-time fluorescent quantitative PCR. Differential expression genes were detected by the microRNA chip. Target genes of miR-1284 were predicted by the bioinformatics. Invasive ability was detected by the Transwell invasion assay. Metastasis ability was detected by subcutane-ously transplanted tumor model of nude mice.Results:Compared with LV-NC-GFP and control groups, the expressions of miR-1284 and 20 genes were up-regulated, and the expression of 17 genes was down-regulated in LV-miR-1284 group. One hundred and thirty-eight target genes of miR-1284 were predicted by the bioinformatics website. Compared with invasive cell number of LV-NC-GFP group (168.67±4.55) and control group (170.33±3.08), the ability of invasion ofcells was weakened in LV-miR-1284 group (70.00±2.37). Compared with the liver metastasis rate of LV-NC-GFP group (85.71%) and control group (85.71%), the ability of metastasis of cells was weakened in LV-miR-1284 group (14.29%). Conclusion:The ability of invasion and metastasis of SGC-7901 cells is suppressed by over-expression of miR-1284. The mechanism may be related to regulating the expression ofSUMO1 andJUNgenes.

AIM:To evaluate the correlation between microRNA-1284 (miR-1284) and gastric cancer, and to investigate the underlying mechanism.METHODS: The expression of miR-1284 was examined by real-time PCR in 63 gastric cancer ( GC) tissue samples and 63 non-malignant adjacent tissue samples.The correlation between miR-1284 and the clinicopathological feature of GC was analyzed.Lentiviral vector containing miR-1284 was constructed and transfected into GC SGC-7901 cells.After transfection, the expression of miR-1284 was examined by real-time PCR.The cell activity was evaluated by CCK-8 assay.The cell cycle and apoptosis were determined by flow cytometry.The ability of cell migra-tion was measured by wound-healing assay.The potential target gene of miR-1284 was predicted by online bioinformatic softwares.The expression of JAG1 mRNA was examined by real-time PCR.The protein levels of JAG1, Notch1 and NF-κB were analyzed by Western blotting.RESULTS:Compared with non-malignant adjacent tissue samples, the results of real-time PCR showed significant downregulation of miR-1284 in 42 GC tissue samples ( P0.05), but significantly associated with histologic grading (P<0.05).Compared with LV-NC-GFP group and control group, after transfection of miR-1284 in LV-miR-1284 group, the expression of miR-1284 was significantly in-creased (P<0.05), the percentages of apoptotic cells and the cells in G0/G1 phase were significantly increased (P<0.05), the cells activity and ability of migration were significantly decreased (P<0.05), and the expression of JAG1, Notch1 and NF-κB was significantly decreased (P<0.05).CONCLUSION:The inhibitory effect of miR-1284 on gastric cancer may be associated with the regulation of its targeting gene JAG1.
