RESUMO

Objective To investigate the recovery of affected side kidney function after upper urinary tract obstruction was relieved Methods 78 patients who had been diagnosed with upper urinary tract obstruction were enrolled from January to December of 2015 in our hospital.All patients received the surgery to relieve the obstruction.GFR of the affected side kidney was done after one months of the surgery.The average age was(51.3 ± 12.8)years old.The reason of obstruction was upper urinary tract calculi (72 cases) and upper urinary tract stenosis (6 cases) respectively.All the patients received CT and ECT before and after operation.All the patients was divided into 3 groups by the decreased degree of affected side kidney function,including moderately decreased group [15 rnl/min ≤ GFR < 30 ml/min,n =43,mean value =(23.1 ±5.0) mL/min],severely decreased group [7.5 rnL/min≤ GFR < 15 ml/min,n =23,mean value =(11.2 ± 2.3) ml/min],and extreme severely decreased group [GFR < 7.5 ml/min,n =12,mean value =(4.3 ± 2.9)ml/min].Linear correlation analysis was used to analysis the relationship analysis between the GFR value (pre-operation,post-operation) and the renal cortical thickness.The follow up time of extreme severely decreased group extended to 8-12 months.Results The GFR of moderate decreased group recovered to(30.6 ± 8.5) ml/min,regained averagely (7.56 ± 7.62) ml/min;the severely decreased group recovered to (13.1 ± 4.5) ml/min,regain (1.94 ± 3.38) ml/min.Extreme severely decreased group recovered to (11.1 ± 3.4) ml/min,regained averagely (6.75 ± 4.76) rnl/min,the GFR mean value after operation 8-12 months recovered to (12.7 ± 3.6) ml/min.All groups got significant recovery of kidney function of the affected side.The correlation coefficient between GFR value and the renal cortical thickness was 0.59 (before the surgery) and 0.70 (after the surgery) respectively (P < 0.05).Conclusions Most of affected side kidneys got different degree of recovery.Obstruction influenced the accuracy of ECT at the time of evaluating the actual renal function before operation.The kidneys which had been supposed should be resected in presurgical evaluation could recover to the level of kidney reserve after the surgery.

RESUMO

Objective To summarise and analyze the clinical effecacy of endovascular treatment(internal holmium laser incision,bal-loon dilation,ureter dilator,rigid ureter dilation)for ureteral stricture.Methods The clinical data of 628 patients from January 2010 to Jan-uary 2015 in our hospital were analyzed.The relevant operation indicators,postoperative complications and recovery condition were recorded and analyzed.Results The operation time was 5.5 to 29 minutes,with average time of 16.5 minutes,no ureteral avulsion,ureteral fragmen-tation and massive haemorrhage happened.All patients were followed up for 6 to 36 months,591 cases(94.1%)were cured,29 cases (4.6%)of postoperative stricture recurrence received endovascular treatment again,8 patients(1.3%)conversion to open ureterolithotomy. Conclusion Endovascular treatment of ureteral stricture is diversified within holmium laser incision,it has the advantages of shorter opera-tion time,fewer complications,less trauma,repeatability and so on,which is an effective and safe treatment method.

Objective To evaluate the diagnostic accuracy of CMS50F for screening in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Methods Sixty-four volunteers with suspected OSAHS underwent simultaneous noc?turnal polysomnography (PSG), micromovement sensitive mattress sleep monitoring system(MSMSMS)and CMS50F. The ap?nea-hypopnea index (AHI) detected by PSG and MSMSMS was used as the diagnostic standard for OSAHS. The reliability of CMS50F for monitoring sleep was assessed. Results There was no statistic difference in CMS50F-ODI3 and PSG-AHI be?tween normal, mild and moderate OSAHS groups(P>0.05). The CMS50F-ODI3 was smaller than the PSG-AHI in severe OSAHS patients(P 0.05). The value of CMS50F-ODI3 was smaller than MSMSMS-AHI in severe OSAHS patients (P < 0.05). There was also a significant correlation between CMS50F- ODI3 and MSMSMS-AHI (r=0.867,P <0.05). MSMSMS-AHI≥5 events per hour was used as the threshold value to diagnose OSAHS, the sensitivity and specificity of CMS50F were 94.5%and 88.9%. Conclusion CMS50F can be used as a portable and reliable device for screening of pa?tients suspected OSAHS.

<p><b>OBJECTIVE</b>The aim of this study was to evaluate the effects of obstructive sleep apnea-hypopnea syndrome (OSAHS) on semicircular canal function.</p><p><b>METHODS</b>By means of a series prospective study at Department of Otolaryngology Head and Neck Surgery of our hospital, the study was performed on 77 patients suffering from OSAHS in a period from 2012 to 2014, who underwent polysomnography (PSG) and caloric test. The maximal slow-phase velocity (SPV) and unilateral weakness (UW) were used to measure the vestibular function. Severity of OSAHS was evaluated by the lowest oxygen saturation (LSaO₂) and apnea hypopnea index (AHI). The SPV after cool was signed, warm test was performed for each ear, and the sum of left ear SPV were calculated, and then, the SPV of right ear was counted as the same way. Finally, the relationships between LSaO₂, AHI, age, BMI, and SPV of caloric test were analyzed.</p><p><b>RESULTS</b>Caloric vestibular tests in the 77 OSAHS patients demonstrated abnormal findings in 52 patients (67.5%) and normal vestibular functions in the remaining 25 patients (32.5%). Of the 52 patients with an abnormal test result, 16 (20.8%) patients had unilateral vestibular hyporeflexia and 36 (46.7%) patients revealed a bilateral vestibular hyporeflexia. There was no linear relationship between AHI, age, BMI with SPV of caloric test (P > 0.05). The SPV had significant difference between Lower LSaO₂group (LSaO₂< 50%) and higher LSaO₂group (LSaO₂≥ 80%) (P < 0.05). LSaO₂was lower in patients undertaken bilateral vestibular hyporeflexia.</p><p><b>CONCLUSIONS</b>OSAHS patients with long-term intermittent hypoxia can disturb the vestibular organs and reduce semicircular canal function. The heavier hypoxemia will lead to the lower reflex of semicircular canal, with the heavier degree of hypoxemia, and the bilateral horizontal semicircular canal involvement may also be higher at the same time. Due to the effect of vestibule centre compensatory, OSAHS patients lack of dizziness and symptoms from balance disturbances such as typically acute vestibular damage.</p>

<p><b>OBJECTIVE</b>To explore the personality and character of obstructive sleep apnea hypopnea syndrome (OSAHS) patients.</p><p><b>METHODS</b>Subjects, recruited from May 2013 to February 2014, were assigned to the severe OSAHS group (56 cases), mild-moderate OSAHS group (59 cases), and control group (42 cases) on the basis of apnea hyponea index (AHI). Subjects were assigned to the severe hypoxemia group (24 cases), mild-moderate hypoxemia group (91 cases) on the basis of PaO2. The psychological aspects of subjects were assessed by using the Minnesota multiphasic personality inventory (MMPI).</p><p><b>RESULTS</b>Compared between OSAHS group and the control group, differences of 6 clinical scales depression (D), hysteria (Hy), masculinity (Mf), paranoia (Pa), anxiety (A), ego strength (Es) were significant (t value was 2.609, 2.133, -2.294, 2.520, 2.041, 2.675 respectively, all P < 0.05). The scores of OSAHS group were higher than the control group on five clinical scales, depression (D), hysteria (Hy), paranoia (Pa), anxiety (A), ego strength (ES). The scores of OSAHS group were lower than the control group on clinical scale masculinity (Mf). Compared between severe OSAHS group and mild-moderate OSAHS group, differences of 6 clinical scales depression (D), paranoia (Pa), psychasthenia (Pt) anxiety (A), manifest anxiety scale (MAS), dependency (Dy) were significant (t value was 2.460, 2.086, 2.181, 2.121, 2.954, 1.982, respectively). The scores of severe OSAHS group were all higher than the mild-moderate OSAHS group on these six clinical scales. Compared between severe hypoxemia group and the contrast group, differences of 4 clinical scales depression (D), masculinity (Mf), paranoia (Pa), ego strength (Es) were significant (t value was respectively 2.992, -2.221, 2.164, 2.165, all P < 0.05). The scores of severe hypoxemia group were higher than the control group on 3 clinical scales, depression (D), paranoia (Pa), ego strength (ES), and lower than the control group on clinical scale masculinity (Mf). Compared between severe hypoxemia group and mild-moderate hypoxemia group, psychasthenia (Pt) were significant (t value was 1.984). The scores of severe hypoxemia group were higher.</p><p><b>CONCLUSIONS</b>Compared with health people, OSAHS patients have special personality and character. The degree of OSAHS can infect the personality and character of OSAHS patients.</p>

Objective To investigate the metabolic profile of uric acid and the significance of the altered renal expression of urate trans-porter 1(URAT1) in patients with uric acid nephrolithiasis. Methods The data of 24 patients in our hospital from January 2012 to October 2013 were analyzed retrospectively. Participants in the research were divided into three groups:patients with uric acid nephrolithiasis,other patients with nephrolithiasis and normal participants. The basic clinical data and the related data of uric acid metabolition of participants were collected,URAT1 gene expression in renal tissures of three groups was detected by Real-time PCR technique. All data were statistically ana-lyzed and compared between these groups. Results Uric acid levels in plasma,body mass index and age were significantly higher in patients with uric acid nephrolithiasis than other two groups (P0. 05). The result of Real-time PCR suggested that the URAT1 renal expression was significantly higher in patients with uric acid nephrolithiasis than other two groups (P<0. 05). Conculusion Patients with uric acid nephrolithiasis are closely related with hyperuricemia,but unrelated with renal over-ex-cretion of uric acid. The upregulated URAT1 expression in the kidney may be an important molecular mechanism of the clinical features.

RESUMO

Objective To evaluate the curative effect of metallic stent placed for malignant and chronic benign ureteral obstruction. Methods From October 2013 to April 2014, 10 patients were given placement of the metallic stents for treatment of malignant ureteral ob-struction and chronic benign ureteral obstruction in our institutionl. ECT was performed to test split kidney function after metallic stents placement. Results After the mean follow-up time of 4 months ( ranged from 1 to 7 months) , unilateral renal function improved in 11 cases. And there was no decrease of kidney function among all the patients who were given placement of the metallic stents. Conclusion Metallic stent is a valuable treatment for releasing the malignant and chronic benign ureteral obstruction.

RESUMO

Objective To evaluate the curative effect of polyscope modular flexible ureteroscope combined with Holmium laser lithotrip-sy in the treatment of renal calculi. Methods 53 patients with renal calculi were performed CTU to detect the position of renal calculi before operation. The patients were treated with polyscope modular flexible ureteroscope combined with Holmium laser, and the double J tube were routinely indwelled 4 to 12 weeks after surgery. Results All the 52 patients has successfully completed the operation in the frist stage. The mean operative time was about 38 minutes. 6 patients were treated with flexible ureteroscope for second stage operation. One week after oper-ation, the review showed that there were residual calculi existed in 24 cases and there were 56 cases of post-operative infection, including 17 cases of sepsis or sepsis shock. Conclusion Polyscope modular flexible ureteroscope can be used for all kinds of kidney stones surgery and is characterized by safety and mild tissue injury. But we should pay more attention to the post-operative infection especially urosepsis.

Objective To analyze the clinical efficacy of physical vibration lithecbole in treatment of urinary calculi. Methods Ana-lysed the efficacy of 80 patients who underwent physical vibration lithecbole only or combination therapy with surgery in urinary calculi in our hospital from February 2014 to July 2014. Result There were 1 to 4 times calculi discharge among the 80 patients. One month after the sur-gery, the calculi discharge rate was 33. 3% and the calculi clean rate was 22. 2% in the upper ureteral; the calculi discharge rate was 16. 7%and the calculi clean rate was 50. 0% in the distal ureteral; the calculi discharge rate was 40. 0% and the calculi clean rate was 23. 3% in the upper renal calyx;the calculi discharge rate was 27. 7% and the calculi clean rate was 38. 8% in the middle renal calyx;the calculi discharge rate was 60. 0% and the calculi clean rate was 20. 0% in the lower renal calyx. One month after the surgery of physical vi-bration lithecbole combined with Holium laser lithotripsy, the calculi discharge rate was 52. 1% and the calculi clean rate was 39. 1%. Con-clusion Physical vibration lithecbole is a noninvasive treatment for urinary calculi. It has good efficacy in calculi discharge and it can relieve the pain caused by calculi.

RESUMO

7% (OR=4.640,95%CI=1.064 to 20.239,P=0.041) were the high risk factors for myocardial ischemia in patients with type 2 diabetes.Conclusion Poor control of the 4 factors is the high risk factor for myocardial ischemia in patients with type 2 diabetes.Overall and effective control of such risk factors can decrease the incidence of myocardial ischemia and improve its treatment.

Objective To study the characteristics of the binding of 125 I-VIP with VIP receptor in H22 hepatocarcinoma cells in mice and in vitro. Methods VIP was radio-labelled with carrier free 125 I with the chloramine T method and purified with sephadex G-50 column chromatography. Experiments of saturation binding and competition binding were carried to study the characteristics of the binding between 125 IVIP and the VIP receptors in the H22 cells in vitro. Experiments of dynamic biodistribution and competion distribution were performed to verify the characteristics of the binding between 125 I-VIP and VIP receptors in mice with H22 hepatocarcarcinoma. Results The labelling rate of 125 I to VIP was 73.8%. The specific radioactivity of 125 I-VIP was 18.2 TBq/mmol and the radiochemical purity was 98%. The binding of 125 I-VIP with VIP receptors in H22 cells was inhibited by non-labelled VIP in a dose-dependent manner. Two classes of specific high affinity binding sites for VIP expression in H22 cells were detected with scatchard analysis. The Kd was 1.74 nmol/L and 28.63 nmol/L and Bmax was 0.27 pmol/10 6 cells and 5.57 pmol/10 6 cells for high and low affinity binding sites respectively. RESUMO

Objective To explore the inchoate changes in patients with Graves disease (GD) treated with 131I and appraise the therapeutic effect. Methods According to the thyroid size, patients’ age, thyroid 131I intake rate and course of the disease, 303 patients with GD were given individual 131I doses. Six months later, the symptom, signs, and thyroid hormone levels of every case were compared with those recorded before. Results The symptom and the level of thyroid hormones were significantly meliorated after 131I treatment. Six cases became goggle-eyed after treatment. Among the 303 patients, 249(82.18%) fully recovered, 21 (6.93%) partially remitted, 24(7.92%) developed hypothyroidism, and 9(2.97%) showed ineffective. Conclusion 131I treatment for GD with an individual dose can exert a satisfied therapeutic efficacy. It may as serve the first choice for GD treatment.

<p><b>OBJECTIVE</b>To explore the diagnostic value of MRI and SPECT in early postburn brain edema in severely burned dogs.</p><p><b>METHODS</b>Twenty-six mongrel dogs were randomized into control (n = 6) and burned groups in which every 5 dogs were allotted to each of following time points: 6, 12, 18 and 24 postburn hours (PBHs). The dogs in burn groups were inflicted with 50% TBSA of III degree skin burn and were infused with 5% glucose solution after 6 PBHs, so that severe early postburn brain edema was produced. MRI and SPECT were employed to observe dynamically the brain of dogs in all groups. The results were collected and compared with one another.</p><p><b>RESULTS</b>The results indicated that with MRI brain morphological change of early brain edema could be shown as early as within 12 PBH and diffuse brain edema became more obvious with elapse of time. The changes might be difficult to be found by MRI when T(1)WISIR decreased below 10%. T(2)WI SIR increased by 8.29% at 24 PBH with blurred demarcation between the brain gray and white matters. There was diffused and progressive nuclide ((99)TCm-ECD) concentration in the brain tissue as shown by SPECT at 6 PBH. The radio-nuclide taking ratio increased significantly after 12 PBH, especially at 24 PBH (P < 0.01) when compared with that before burn.</p><p><b>CONCLUSION</b>Combined application of MRI and SPECT could evidently increase sensitivity and specificity in the diagnosis of early postburn brain edema.</p>

Objective To quantitatively analyze type 1 receptor of insulin-like growth factors (IGF1Rs) on the HepG 2 cells. Methods 1H7 (monoclonal antibody against IGF1R) was labeled with radionuclide 131 I by Iodogen method. The mixture was purified with Sephadex G-50 column, 131 I-1H7, and its unlabeled counterpart 1H7 competitively bound to IGF1Rs on the HepG 2 cells. The average number of IGF1R on the HepG 2 cells was analyzed by Scatchard method. Results The Bmax of HepG 2 cells (2.0?10 5) in every well was 6.746?10 -12 mol/L. The expression of IGF1R on the HepG 2 cells was 2.03?10 4/cell. Conclusion IGF1Rs are expressed on the HepG 2 cells with high density and high affinity, which may be used in other related studies.
