RESUMO

OBJECTIVE@#To evaluate the impact of sperm source and sperm parameters on the outcome of intracytoplasmic sperm injection (ICSI).@*METHODS@#This retrospective study included 433 ICSI cycles from June 2005 to December 2008 in Reproductive Medical Center of Xiangya Hospital. The patients were divided into 2 major groups according to the source of spermatozoa used for ICSI: ejaculated (group A, n=336) and epididymal (group B, n=97). Group A was divided into 3 subgroups according to the sperm parameters: normal (Group A1, n=95), single parameter defect (Group A2, n=119), and multiple parameter defect (Group A3, n=122).@*RESULTS@#The basic characteristics among the 4 groups had no statistic difference (P>0.05), and the difference in the fertilization rate, normal fertilization rate, cleaving embryo rate, good quality embryo rate, implanted rate, clinical pregnancy rate, and early abortion rate among the 4 groups were not significant (P>0.05).@*CONCLUSION@#The outcome is similar no matter whether the spermatozoa is from ejaculated sperm or epididymis. ICSI can treat male infertility of various factors, and the outcome is the same with one or multiple sperm parameter abnormality. ICSI with epididymal spermatozoa through percutaneous epididymal sperm aspiration is effective for infertility due to obstructive azoospermia.

RESUMO

Objective To investigate the clinical value for treatment of diabetic foot with PTA and PTA combined cinepazide maleate. Methods In 24 cases of diabetic associated vascular disease of lower limb,12 cases were treated with PTA and other 12 cases were treated with PTA combined einepazide maleate,We analysed and compared clinical effects before and after the procedure, together with 3 months follow up.Results In patients treated with PTA,the clinical symptom scores of posttreatment and follow-up decreased;ABI and TcPO_2 increased significantly. The clinical symptom score and ABI of follow-up remained, stable, but TcPO_2 decreased significantly.Control angiography showed improvement in degree of vascular stenosis and peripheral staining of 11 patients after treatment. The vascular patency remained in 12 patients and the peripheral staining decreased in 7 patients on follow-up.In patients treated with VIA combined cinepazide maleate, the clinical symptom score,ABI and TcPO_2 after treatment and on follow-up showed no signifcant changes compared with those in patients treated by PTA.F,Control angiography showed that the degree of vascular stenosis and peripheral staining were improved in 12 patients after treatment. The vascular pateney was maintained and peripheral staining was improved on follow-up.Before and after treatment, there were no significant differences in clinical symptom score.ABI and TcPO_2 between patients treated with PTA and PTA combined cinepazide maleate, however, there were significant differences in clinical symptom score and TcPO_2 on follow-up.Conclusion PTA can significantly improve clinical symptom of diabetic foot and the application of cinepazide maleate is a benefitial and necessary supplement.PTA combined cinepazide maleate can be taken as one of the conventional treatment plans for diabetic foot.(J Intervent Radiol,2007,16:811-815)

RESUMO

Objective To investigative CT findings of gastrointestinal neurilemmoma, and to improve the recognition and diagnosis ability of the disesase. Methods Twelve cases of gastrointestinal neurilemmoma proven surgically and pathologically were included. The unenhanced CT and double phase enhanced CT were performed in all patients. The clinical feature and CT findings with regard to the location, size, contour, margin, growth type, contrast enhancement pattern, contrast enhanced degree, presence or absence of ulcer were retrospectively reviewed by two experienced radiologists. Results On unenhaneed CT, all the patients present masses with homogeneous attenuation in the wall of gastrointestinal tract, compared with muscle.Three of the gastric neurilemmoma showed endoluminal growth pattern, and 6 had a predominantly exophytic growth pattern, the tumor had both endoluminal and exophytic growth pattern in 3 cases. The tumors were round or oval with distinct margin in 10 of 12 gastrointestinal neurilemmoma, and lobulated with indistinct margin in 2 cases.Three of the gastric neurilemmoma had ulceration. On contrast- enhanced CT,all the tumors showed homogeneously solid mass.No obvious enhancement on the arterial phase showed moderate enhancement on portal vein phase in 10 cases,2 of gastrointestinal neurilemmoma which manifested mild enhancement on the arterial phase showed obvious enhancement on portal vein phase. Conclusion CT findings of most gastrointestinal neurilemmoma have characterstic, which appeared as round or oval, distinct margin, and homogeneously solid tumors, slow but fairly uniform and moderate enhancement throughout the mass.
