RESUMO

To summarize the status quo of clinical evidence on oral Chinese patent medicine in the treatment of hyperlipidemia through scoping review and thereby provide a reference for clinical application and decision-making in health care. Proprietary Chinese medicines for the treatment of hyperlipidemia (dyslipidemia) were retrieved from relevant catalogs and then screened based on their instructions. Articles on the selected Chinese patent medicines were searched from Chinese and english electronic databases and screened according to the inclusion criteria, followed by data extraction and analysis. The results were described with text and graphs. ①A total of 32 Chinese patent medicines and 1 010 related articles were screened out. ②All the included Chinese patent medicines were made from Chinese medicinal materials, of which 5 were Chinese medicinal extracts, and the remaining 27 were pure Chinese medicinal preparations (the compositions of 1 prescription is confidential). ③As indicated in the instructions, all the 32 Chinese patent medicines can be used to treat hyperlipidemia with the main syndrome of combined phlegm and blood stasis, and the main effect of them is activating blood and resolving stasis. ④Among the 32 medicines, Xuezhikang tablets (Capsules) and Gypenosides Tablets have the lowest price. ⑤For the 328 key clinical studies, 248 were randomized controlled trials and 80 non-randomized controlled trials. ⑥As for the sample size, randomized controlled trials generally included 28-579 cases, and non-randomized controlled trials 24-152 cases. Clinical studies with more than 200 cases accounted for 9.3% (12/129). ⑦The most common method was the comparison of the intervention effect of Chinese patent medicine and western medicine (36.5%) and Atorvastatin Calcium Tablets was the preferred western medicine control. The observation duration was generally 56 days (8 weeks). ⑧In clinical practice, the absolute value of blood lipid decrease and (total) effective rate were often used to evaluate the efficacy, and the effective rate was generally determined based on Clinical Guidelines for New Chinese Medicines. The overall quality of clinical studies on oral Chinese patent medicines is uneven and there is a lack of high-quality clinical evidence. Moreover, there are the risks of unreasonable use and uneasy use of Chinese patent medicine. It is recommended that researchers on proprietary Chinese medicine should further broaden the research ideas, focus on the top-level design of the research plan, and standardize the research process, thereby provide high-quality research evidence for the clinical use of proprietary Chinese medicine. Moreover, more efforts should be made to establish a sound mechanism for information collection and feedback of proprietary Chinese medicine, so as to reduce the risk of uneasy use.We hope that in-depth investigation and discussion should be launched by price, economy, quality evaluation, and other departments headed by the supervision department to make the price of Chinese patent medicine reasonable and fair, thereby promoting the rational use of Chinese patent medicine.

RESUMO

The efficacy and safety of recombinant tissue plasminogen activator (rtPA) need to be improved due to its low bioavailability and requirement of large dose administration. The purpose of this study was to develop a fibrin-targeted nanoparticle (NP) drug delivery system for thrombosis combination therapy. We conjugated rtPA to poly(ethylene glycol)-poly(ε-caprolactone) (PEG-PCL) nanoparticles (rtPA-NP) and investigated its physicochemical characteristics such as particle size, zeta potential, enzyme activity of conjugated rtPA and its storage stability at 4℃.The thrombolytic activity of rtPA-NP was evaluated in vitro and in vivo as well as the half-life of rtPA-NP,the properties to fibrin targeting and its influences on systemic hemostasis in vivo. The results showed that rtPA-NP equivalent to 10% of a typical dose of rtPA could dissolve fibrin clots and were demonstrated to have a neuroprotective effect after focal cerebral ischemia as evidenced by decreased infarct volume and improved neurological deficit (P<0.001).RtPA-NP did not influence the in vivo hemostasis or coagulation system. The half-life of conjugated rtPA was shown to be approximately 18 times longer than that of free rtPA.These experiments suggested that rtPA-conjugated PEG-PCL nanoparticles might be a promising fibrin-targeted delivery system for a combination treatment of thrombosis.

RESUMO

To date, the efficacy of radical surgery (RS) versus conservative surgery (CS) for liver hydatid cysts (LHC) remains controversial. This meta-analysis was conducted to compare the two interventions. PubMed, Embase, and Web of Science were searched from their inceptions until June 2016. Meta-analysis was performed using STATA 12.0 software. We identified 19 eligible studies from 10 countries by retrieval. In total, 1853 LHC patients who received RS were compared with 2274 patients treated by CS. The risk of postoperative overall complication, biliary fistula, and recurrence was significantly lower, and operation time was significantly longer in the RS group. However, no statistically significant differences were found in terms of mortality risk and the duration of hospital stay between RS and CS. No significant publication biases were observed in all the above analyses. In conclusion, RS reduces the rates of postoperative complications and recurrence, whereas no trend toward such a reduction in mortality was observed in LHC patients.

RESUMO

Objective To study the effect of mesenteric lymph duct ligation(MLDL)on sepsis-induced lung and intestinal injuries in rats. Methods Sepsis was induced by cecal ligation and puncture(CLP)in male rats. The rats were randomly divided into sham control group in which rats received sham operation, sepsis group(CLP group)in which rats received saline after CLP operation, and CLP+MLDL group in which rats received mesenteric lymph duct ligation(MLDL)after CLP operation. The rats were sacrificed at 24 h after CLP modeling. Bronchoalveolar lavage fluid, arterial blood, and intestine and lung tissues were collected to determine organ injuries. Results The lung tissue histopathology and blood gas analysis revealed that MLDL markedly alleviated the lung injury(ALI score:6.14 ± 0.51 vs.8.12 ± 0.63,P=0.029 9),improved oxygen partial pressure[(78.67 ± 4.51)mmHg vs.(64.83 ± 2.90)mmHg,P=0.0273],decreased lung W/D ratio(6.12 ± 0.25 vs.7.63 ± 0.49,P=0.021 2),decreased BALF cytokines levels[TNF-α:(828.17 ± 81.89)pg/mL vs.(1 118.17 ± 79.22)pg/mL,P=0.029 1;IL-6:(39.33 ± 5.50)ng/mL vs.(68.67 ± 5.24)ng/mL,P=0.003 4],alveolar permeability[protein levels:2.117 ± 0.289 2 vs.3.033 ± 0.164 7,P=0.020 3,and cell levels:(30.00 ± 3.587)×106/L vs.(43.83 ± 2.358)×106/L,P=0.009 1].However,according to the results of HE and biochemical detection,MLDL had no protective effect on sepsis-induced intestinal injury. Moreover,MLDL could significantly reduce the bacterial loads of the blood and lung, but do not change the bacterial level in the intestines. Conclusion MLDL has a significant protective effect on sepsis-induced lung injury mainly by decreasing bacterial translocation, but had no effect on intestinal injury. This difference may be related to that MLDL inhibits the bacteria spreading from abdominal cavity to other organs in sepsis but it causes their accumulation in the intestines.

RESUMO

The chemical compositions of Anemone raddeana Rhizome, a kind of traditional Chinese medicine, were reviewed, along with its bioactivity and pharmacological properties and method improvements of extracting and detecting triterpenoid saponins. A. raddeana Rhizome is used to treat neuralgia and rheumatism, and is rich in triterpenoid saponins, most of which are pentacyclic, with oleanane as the nucleus. So far, 37 triterpenoid saponins have been determined from the herb. Its reported bioactivity and pharmacological properties have been described as anticancerous, antimicrobial, anti-inflammatory, analgesic, antipyretic, anticonvulsive, antihistaminic, and sedative. It has also been used for the induction of the humoral immune response and treatment of liver fibrosis in chronic hepatitis. However, the herb also has hemolytic effects and can be toxic, which limits its clinical application. Further studies are needed on the pharmaceutical functions, mechanisms, and immunological responses to contribute to the herb's clinical applications.

RESUMO

Objective: To evaluate the activating blood circulation and dissipating blood stasis effects of the compatibility between two effective parts (DT, DF, DST, and DF + DST) of Salvia miltiorrhiza. with different doses on rats with acute blood stasis. Methods: Acute blood stasis rat model was induced by sc injection of adrenaline and ice water bath. The blood stasis mice were ig administered with different effective parts of S. miltiorrhiza. The whole blood viscosity (WBV), plasma viscosity (PV), erythrocyte sedimentation rate (ESR), and haematocrit (HCT) were determined to observe the effects of two effective parts of Danshen on hemorheology of blood stasis rats. And the thrombin time (TT), prothrombin time (PT), activated partial thromboplastin time (APTT), and plasma fibrinogen (FIB) were tested to observe the effects of different effective parts of S. miltiorrhiza on blood coagulation function of blood stasis rats. In addition, the maximum aggregation induced by adenosine diphosphate (ADP) was tested to observe the effect of different active parts of S. miltiorrhiza on platelet aggregation index of blood stasis rats. Then principal component analysis (PCA) and multi-attribute comprehensive index methods were both used to comprehensively evaluate the total activating blood circulation and dissipating blood stasis effects of different effective parts of S. miltiorrhiza. Results: The hemorheological indexes and coagulation parameters of model group had the significant differences with normal group. Compared with model group, the effective parts of S. miltiorrhiza with different doses could improve the blood hemorheological indexes and coagulation parameters in acute blood stasis rats with dose-effect relation. Based on the PCA and multi-attribute comprehensive index methods, the group of high-dose DF + DST had the best effect of activating blood circulation and dissipating blood stasis effect in all medication administration teams. Under the condition of same dose of the effective parts of S. miltiorrhiza, the DF + DST could obviously improve the hemorheology and blood coagulation function in acute blood stasis rats. Conclusion: The combination of DF and DST has the better effects of activating blood circulation and dissipating blood stasis in comparison with those of single part. It could provide the scientific basis for more effective application of the compatibility between salvianolic acids and tanshinones in modern clinic medicine.

RESUMO

Objective: A novel and generally applicable approach was established for the interaction analysis on compatibility of Danshen-Honghua (Salviae Miltiorrhizae Radix and Carthami Flos) pair with different preparations and proportions when decocting together by using HPLC-PDA and fuzzy chemical identification strategy with poly-proportion design. Methods: A simple project was initially developed for the rapid identification and classification of four types of components in Danshen-Honghua based on the establishment of relevant component data base, recognition of the reference compound peaks, selection of the characteristics of ultraviolet spectrum, and formation of group networks. In this study, the accurate structures of the chemical components did not need to be determined, and only the constituents attributed to different groups were further considered for quantitative analysis. Results: A total of 47, 57, and 43 constituents from different preparations of Danshen-Honghua were classified into four kinds of chemical groups, and they were quantitatively analyzed furtherly according to semi-symmetric proportion design. The results showed that the preparation of methanol-water could significantly promote the dissolution of most salvianolic acids, chalcones, and flavonoids, while the contents of tanshinones in methanol were higher than others. The relative dissolution rate range of total optimization for the four types of components were from 5:1 to 5:2, and the highest proportion of the total relative dissolution were all 5:2. Conclusion: The most reasonable range may appear 5:1-5:2 for water preparation and be consistent with clinical ratio, which provides the reference for clinical application of Danshen-Honghua pair, and for modern research approach of herb pairs.

RESUMO

Xin-Sheng-Hua granule (XSHG) is a popular remedy commonly used in clinic for the treatment of lochiostasis after delivery. To comparatively investigate the roles of herb pairs containing Angelicae Sinensis Radix (Danggui) upon the formula by evaluating the blood coagulation and hemorheology function in acute blood stasis rats, acute blood stasis rat model was established by ice water bath and subcutaneous injection of adrenaline. And the blood stasis mice were administrated intragastrically with different samples of the formula minus herb pairs containing Danggui and the whole formula (XSHG, SHD, DY, DC, DT, DH, DJ and DZ). The whole blood viscosity (WBV), plasma viscosity (PV), erythrocyte sedimentation rate (ESR) and haematocrit (HCT) were applied to evaluate the effects of the formula minus herb pairs containing Danggui on hemorheology of blood stasis rats. The thrombin time (TT), activated partial thromboplastin time (APTT), prothrombin time (PT), and plasma fibrinogen (FIB) were used to observe the effects of the formula minus herb pairs containing Danggui on blood coagulation function of blood stasis rats. Additionally, the maximum aggregation induced by adenosine diphosphate (ADP) was tested to observe the effect of different samples on platelet aggregation index of blood stasis rats.Afterwards, multi-attribute comprehensive index methods and principal component analysis were both applied to comprehensively assess the total effects of the formula minus herb pairs containing Danggui on promoting blood circulation and dissipating blood stasis. Compared with normal group, the hemorheological parameters and coagulation indexes of model group had statistical differences (P<0.01). Compared with model group, different samples (XSHG, SHD, DY, DC, DT, DH, DJ and DZ) could improve the blood hemorheology indexes, coagulation parameters and platelet aggregation in acute blood stasis rats. According to multi-attribute comprehensive index methods and the principal component analysis, the effects of promoting blood circulation by removing blood stasis became poor when excluding herb pairs containing Danggui from the formula, the sample DY and DC had the weakest effect of activating blood circulation and dissipating blood stasis, and the effect of sample DY was slightly poorer than DC. The orders of contribution of herb pairs containing Danggui on the formula were Danggui-Yimucao>Danggui-Chuanxiong>Danggui-Honghua>Danggui-Zhigancao>Danggui-Taoren>Danggui-Jiangtan. In conclusion, various herb pairs containing Danggui played different roles on the effects of improving the abnormality of hemorheology and coagulation function. And the herb pairs Danggui-Yimucao were particularly important for the formula, which was consistent with the characteristics of XSHG and the traditional effect of Yimucao. Moreover, it could lay foundation to further reveal the compatibility mechanism of XSHG.
