RESUMO

Objective: To investigate the cytomorphological and immunocytochemical features of tumor cells in the ascites of ovarian plasmacytoma (SOC). Methods: Specimens of serous cavity effusions were collected from 61 tumor patients admitted to the Affiliated Wuxi People's Hospital of Nanjing Medical University from January 2015 to July 2021, including ascites from 32 SOC, 10 gastrointestinal adenocarcinomas, 5 pancreatic ductal adenocarcinomas, 6 lung adenocarcinomas, 4 benign mesothelial hyperplasia and 1 malignant mesothelioma patients, pleural effusions from 2 malignant mesothelioma patients and pericardial effusion from 1 malignant mesothelioma. Serous cavity effusion samples of all patients were collected, conventional smears were made through centrifugation, and cell paraffin blocks were made through centrifugation of remaining effusion samples. Conventional HE staining and immunocytochemical staining were applied to observe and summarize cytomorphological characteristics and immunocytochemical characteristics. The levels of serum tumor markers carbohydrate antigen 125 (CA125), carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) were detected. Results: Of the 32 SOC patients, 5 had low-grade serous ovarian carcinoma (LGSOC) and 27 had high-grade serous ovarian carcinoma (HGSOC). 29 (90.6%) SOC patients had elevated serum CA125, but the difference was not statistically significant between them and patients with non-ovarian primary lesions included in the study (P>0.05); The serum CEA was positive in 9 patients with gastrointestinal adenocarcinoma and 5 patients with lung adenocarcinoma, and the positive rate was higher than that in SOC patients (P<0.001); The serum CA19-9 was positive in 5 patients with gastrointestinal adenocarcinoma and 5 patients with pancreatic ductal adenocarcinoma, and the positive rate was higher than that in SOC patients (P<0.05). The serum CA125, CEA and CA19-9 were within the normal range in 4 patients with benign mesothelial hyperplasia. LGSOC tumor cells were less heterogeneous and aggregated into small clusters or papillary pattern, and psammoma bodies could be observed in some LGSOC cases. The background cells were fewer and lymphocytes were predominant; the papillary structure was more obvious after making cell wax blocks. HGSOC tumor cells were highly heterogeneous, with significantly enlarged nuclei and varying sizes, which could be more than 3-fold different, and nucleoli and nuclear schizophrenia could be observed in some cases; tumor cells were mostly clustered into nested clusters, papillae and prune shapes; there were more background cells, mainly histiocytes. Immunocytochemical staining showed that AE1/AE3, CK7, PAX-8, CA125, and WT1 were diffusely positively expressed in 32 SOC cases. P53 was focally positive in all 5 LGSOCs, diffusely positive in 23 HGSOCs, and negative in the other 4 HGSOCs. Most of adenocarcinomas of the gastrointestinal tract and lung had a history of surgery, and tumor cells of pancreatic ductal adenocarcinoma tend to form small cell nests. Immunocytochemistry can assist in the differential diagnosis of mesothelial-derived lesions with characteristic "open window" phenomenon. Conclusion: Combining the clinical manifestations of the patient, the morphological characteristics of the cells in the smear and cell block of the ascites can provide important clues for the diagnosis of SOC, and the immunocytochemical tests can further improve the accuracy of the diagnosis.

OBJECTIVE@#To compare different methods for calculating sample size based on confidence interval estimation for a single proportion with different event incidences and precisions.@*METHODS@#We compared 7 methods, namely Wald, AgrestiCoull add z2, Agresti-Coull add 4, Wilson Score, Clopper-Pearson, Mid-p, and Jefferys, for confidence interval estimation for a single proportion. The sample size was calculated using the search method with different parameter settings (proportion of specified events and half width of the confidence interval [ω=0.05, 0.1]). With Monte Carlo simulation, the estimated sample size was used to simulate and compare the width of the confidence interval, the coverage of the confidence interval and the ratio of the noncoverage probability.@*RESULTS@#For a high accuracy requirement (ω =0.05), the Mid-p method and Clopper Pearson method performed better when the incidence of events was low (P < 0.15). In other settings, the performance of the 7 methods did not differ significantly except for a poor symmetry of the Wald method. In the setting of ω=0.1 with a very low p (0.01-0.05), failure of iteration occurred with nearly all the methods except for the Clopper-Pearson method.@*CONCLUSION@#Different sample size determination methods based on confidence interval estimation should be selected for single proportions with different parameter settings.

Objective:To observe the expression of adenosine monophosphate activated protein kinase (AMPK)/transcription factor EB (TFEB) autophagy signaling pathway protein in type 2 diabetes (T2DM) complicated with nonalcoholic fatty liver disease (NAFLD) rats after intervention with Shihu mixture (SHM). Method:Among 40 male SD rats, 10 rats were randomly selected as normal group according to body weight. The remaining 30 rats were fed with high-fat and high-sugar diet for 6 weeks, and then intraperitoneally injected with streptozotocin (STZ) to establish a T2DM NAFLD model. They were divided into normal control group (10 mL·kg-1·d-1), model control group (10 mL·kg-1·d-1), metformin group (100 mg·kg-1·d-1), SHM group (11.3 g·kg-1·d-1). The rats in each group were gavaged for 4 weeks. After gavage, the rats were euthanized. Abdominal aortic blood and liver tissue were collected to detect fasting blood glucose (FBS), glycated serum protein (GSP), insulin (INS), triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C) content. Htoxylin eosin (HE) staining was performed to observe changes in liver tissue morphology. Western blot was used to detect AMPK/TFEB signaling pathway-related protein expression. Result:Compared with the model control group, FBS and GSP of the SHM group and the DMBG group decreased (P<0.05), while INS increased (P<0.05). Compared with the model group, HDL increased in the SHM group and the DMBG group (P<0.05), whereas TC, TG and LDL contents decreased (P<0.05). Liver HE staining results showed that both SHM and Metformin could improve the liver morphology of T2DM and NAFLD rats to some extent. Western blot results showed that p-AMPK/AMPK of SHM and metformin increased (P<0.05), while the expressions of TFEB and LC3Ⅱ increased (P<0.05). Conclusion:SHM can improve glucose and lipid metabolism by activating AMPK/TFEB autophagy signaling pathway, so as to improve liver pathological morphology.

Objective:To investigate the effects of different local complications of acute pancreatitis (AP) on the microcirculation of multiple organs in the upper abdomen.Methods:A dynamic volume perfusion computed tomography (DVPCT) scan in the upper abdomen was prospectively conducted in 101 patients with AP and 24 patients with neither AP nor other obvious upper abnominal lesions diagnosed in People′s Hospital of Deyang City from April 1 to October 31, 2019, 86 patients with AP (AP group) and 21 controls (control group) were enrolled in the study. AP patients were divided into no local complications group (21 cases), acute peripancreatic fluid collection (APFC) group (19 cases), acute necrotic collection (ANC) group (27 cases), walled-off necrosis (WON) group (11 cases) and walled-off necrosis with infection (WONI) group (8 cases). The blood flow (BF) of pancreas, liver, spleen, two kidneys and adrenal glands was measured by deconvolution. The hepatic arterial perfusion (HAP), hepatic portal perfusion (HPP) and hepatic perfusion index (HPI) of each group were calculated by maximum slope. T test was used for statistical analysis. Results:The BF of pancreas, spleen and left adrenal gland of ANC group was (139.89±34.28), (141.42±47.85) and (107.87±26.41) mL·min -1·(100 g) -1, respectively, the BF of pancreas, spleen and left adrenal gland of WON group was (130.00±44.83), (106.12±38.16) and (98.38±41.39) mL·min -1·(100 g) -1 respectively, and the BF of pancreas, spleen and left adrenal gland of WONI group was (127.91±35.86), (102.09±23.73) and (105.66±27.01) mL·min -1·(100 g) -1, respectively, which were all lower than those of control group ((161.22±31.60), (174.00±62.73) and (134.53±36.36) mL·min -1·(100 g) -1), and the differences were statistically significant ( t=2.440, 2.043, 2.943; 2.296, 3.796, 2.548; 2.448, 4.479, 2.154; all P<0.05). The BF of left kidney cortex of WONI group was lower than that of control group ((247.44±39.32) mL·min -1·(100 g) -1 vs. (294.80±39.13) mL·min -1·(100 g) -1), and the difference were statistically significant ( t=2.910, P<0.05). The HAP of ANC group, WON group and WONI group was (18.63±9.54), (19.10±7.47) and (19.51±6.26) mL·min -1·(100 g) -1, respectively, and the HPI was (25.01±15.51)%, (45.98±31.42)% and (35.92±24.95)%, respectively, which were all higher than those of control group ((12.18±5.14) mL·min -1·(100 g) -1 and (13.44±6.49)%), and the differences were statistically significant ( t=2.997, 3.088, 3.235; 3.503, 3.397, 2.517; all P<0.05) . The HPP of ANC group, WON group and WONI group was (72.37±21.76), (48.83±35.10) and (57.55±29.45) mL·min -1·(100 g) -1, respectively, which were all lower than that of control group ((86.43±17.98) mL·min -1·(100 g) -1), and the differences were statistically significant ( t=2.391, 3.331 and 3.226, all P<0.05). The HAP and HPI of APFC group were both higher than those of control group ((18.67±10.24) mL·min -1·(100 g) -1 vs. 12.18±5.14) mL·min -1·(100 g) -1 and (23.75±20.41)% vs. (13.44±6.49)%), and the differences were statistically significant ( t=2.572 and 2.108, both P<0.05) . Conclusions:AP complicated with ANC, WON and WONI can reduce the BF of pancreas, spleen and left adrenal gland, and WONI can induce the decrease of BF of left kindney cortex. AP complicated with ANC, WON and WONI can increase HAP and HPI, but decrease HPP. Furthermore, AP complicated with APFC can increase HAP.

Objective:To evaluate the application of one-stop dynamic volume perfusion CT (dVPCT) in upper abdomen, and its feasibility of replacing conventional enhanced CT, perfusion, and angiography.Methods:A total of 94 patients with upper abdominal perfusion examinations were retrospectively enrolled in Deyang People's Hospital of Sichuan Province from April 2017 to June 2019. The data of another 64 patients underwent routine upper abdominal enhancement with 64-slice CT (28 patients) and dual-source CT (26 patients) were analyzed in the same period. The radiation dose and image quality were compared. According to different contrast agent concentration and dosage, 4 perfusion groups and 2 conventional enhanccement groups were divided, including 60 ml iohexol group (350 mg/ml) and 60 ml ultravist group (370 mg/ml), 60 ml and 80 ml iomeprol group (400 mg/ml), 64-slice routine group and dual-source CT routine group. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the liver, pancreas and abdominal aorta images in the arterial and portal vein phases were measured by two radiologists in a blinded way. Subjective image quality was evaluated by two radiologists using a 5-point Likert Scale in a blinded method. Statistical analysis was performed using analysis of variance and kappa test. Imaging findings and typical cases of perfusion were retrospectively analyzed.Results:Radiation dose: the effective dose of each perfusion group was lower than that of the 64-slice spiral CT scan, but higher than that of the dual-source CT routine scan. The SNR and CNR of the 80 ml iomeprol dVPCT group were better than that of 64-slice spiral CT and dual-source CT routine scan ( P<0.05), and were better than that of 60 ml iohexol, ultravist and iomeprol dVPCT groups ( P<0.05). However, the subjective index was lower than that of the dual-source CT routine scan group ( P<0.05). The comprehensive information of multi-phase images, vascular images and perfusion quantitative parameters of volume perfusion data reconstruction in this group is superior to conventional enhanced CT in the detection of lesions, visualization of normal tissues and blood vessels. Conclusion:One-stop dVPCT imaging of the upper abdomen has lower radiation dose with good image quality and more diagnostic information. dVPCT with 80 ml Iomeprol (400 mg/ml) can obtain much better images.

OBJECTIVE: To investigate the status of anxiety and depression in field training military officers and soldiers and analyze the impact of coping styles and personality traits.METHODS: Self-rating Anxiety Scale, Self-Rating Depression scale, Simple Coping Style Questionnaire and Eysenck Personality Questionnaire were used to evaluate 494 field training military officers and soldiers by judgment sampling method. RESULTS: The rates of anxiety and depression symptoms of the officers and soldiers in field training were 13.0% and 23.1%, respectively. The results of multivariate logistic regression analysis showed that the higher the negative coping style, mental quality and neurotic scores of the officers and soldiers in the field, the lower the scores of internal and external, the worse the interpersonal relationship and family relationship, the higher the risk of anxiety(P<0.05). The higher the mental quality and neurotic scores of the officers and soldiers in the field, the lower of the position, the lower the scores of positive coping style and internal and external, the worse the interpersonal relationship and family relationship, the higher the risk of depression(P<0.05). CONCLUSION: Anxiety and depression are the main negative emotions in the field training officers and soldiers. Negative coping style, pathological personality tendency and interpersonal relationship are the main influencing factors of negative emotion.

OBJECTIVE: To prepare Magnolol nano-crystal suspension (MAG-NS), and to conduct quality evaluation. METHODS: The preparation technology of MAG-NS was optimized by central composite design-response surface methodology with OD value of particle size and polydispersity coefficient as evaluation indexes, using volume ratio of organic phase to water phase, ratio of excipient to drug, concentration of magnolol as factors and conduct validation tests. The quality of MAG-NS prepared optimal technology was evaluated. RESULTS: Optimized technology included that the volume ratio of organic phase to water phase was 1 ∶ 5, mass ratio of excipient to drug was 4 ∶ 1, concentration of magnolol was 2 mg/mL. In 3 times of validation tests, average OD value was 0.940 0 (RSD=0.08%), relative error of which to predicted value 0.977 7 was 3.86%. magnolol nano-crystals of MAG-NS prepared by the optimal technology were spherical, uniform in size, smooth in surface, with particle size of (34.88±0.33) nm, polydispersity coefficient of 0.032±0.001 and drug loading amount of (17.83±0.92)%. CONCLUSIONS: Established preparation method is simple and feasible. Prepared MAG-NS is in line with quality requirements. It can provide reference for further development and utilization of MAG-NS.

Objective To evaluate the value of multimodal MRI in differential diagnosis of lung squamous cell carcinoma and adenocarcinoma.Methods Routine sequence,diffusion weighted imaging(DWI)and dynamic enhancement images about 1 6 squamous cell carcinoma and 21 adenocarcinoma patients were analyzed retrospectively.Taken a record about the size,edge,internal signal, enhancement types and apparent diffusion coefficient(ADC)values when b=600 s/mm2,and the difference in the degree of pathological differentiation was studied.Results The average diameter of squamous cell carcinoma was (4.17±2.0)cm,while adenocarcinoma was (3.81±1.8)cm,lobulated and spiculation were the most common signs in both of them.Squamous cell carcinoma showed low T1signal in 12 cases(75%),low T2signal in 7 cases(43.7%),adenocarcinoma showed high T1signal in 10 cases(47.6%),high T2 signal in 14 cases(66.7%).Squamous cell carcinoma had lower ADC value than adenocarcinoma(1.27×10-3mm2/s vs 1.38×10-3mm2/s), and well differentiated tumors had higher ADC values than poor ones,it was statistically significant.The most common time-signal intensity curves were type A in squamous cell carcinoma(62.5%)and type B in adenocarcinoma(50%).Conclusion MRI findings of squamous cell carcinoma and adenocarcinoma are associated with the biological characteristics,squamous cell carcinoma has shorter T2signal and adenocarcinoma has shorter T1signal.Squamous cell carcinoma has lower ADC value than adenocarcinoma and is dominated by outflow curve (type A),these features are helpful in subtype and differential diagnosis.

Objective To investigate the clinical effect of five-step sequential method for hemorrhagic radiation cystitis.Methods From April 2015 to April 2018,512 cases of hemorrhagic radiation cystitis patients in 6 provincial tumor hospital were retrospectively analyzed.12 cases were male patients,and 497 cases were female.Patients'average age was (60.1 ± 8.2) years (53-71 years).Hematuria appeared in 2.9 years on average after radiotherapy(ranged 0.5 to 13 years).According to the severity of hematuria,480 patients were classified as mild and 32 patients as severe.The step 1 was drug conservative symptomatic treatment.Thrombin solution or sodium hyaluronate was used for bladder perfusion in step 2.The step 3 was transurethral electrocoagulation.The step 4 was interventional embolization.The step 5 was hyperbaric oxygen therapy.Mild patients were treated from the first step,and the severe patients were treated rom step 3,and combined with step 1.Overactive bladder symptom score (OABSS) was used to assess the improvement of patients'symptoms.Results No obvious complications occurred in step 1,2,3,5.Mild hip pain occurred at step 4.Recurrent hemorrhage rate were 6.7% (2/30) in mild patients group and 10.5% (2/19) in severe respectively in step 4.The total effective rate of step 1 in mild patients group was 65.2% (313/480),step 1 + 2 was 84.2% (404/480),step 1 + 2 + 3 was 95.6% (459/480),step 1 + 2 + 3 + 4 was 98.3% (472/480),and step 1 +2 +3 +4 +5 was 100% (480/480).The effective rate of step 3 + 1 treatment was 59.4% (19/32) in severe patients group,step 3 + 1 + 4 was 68.8% (22/32),and step 3 + 1 +4 +5 was 100% (32/32).The OABSS scores of mild and severe patients decreased after treatment (P<0.01).Condusion Five-step sequential therapy could be an effective method for the treatment of hemorrhagic radiation cystitis.

RESUMO

Eighty percent of bacterial infections are related to the formation of bacterial biofilm. Compared with planktonic bacteria, bacterial biofilm is 10-1 000 times more resistant to antibiotics, which is the main cause of current bacterial drug resistance. A comprehensive understanding of the characteristics and resistance mechanisms of bacteria biofilm will help us treat the stubborn infections caused by the bacterial biofilm better and solve the problem of bacterial drug resistance. In this review, the composition and quorum sensing of bacterial biofilm, two major patterns of biofilm formation and drug resistance mechanisms were presented. Furthermore, representative compounds with anti-biofilm activity and compounds synergistic with antibiotics in anti-biofilm actions were introduced. Nano drug delivery strategies used for anti-biofilm in recent years as well as a novel drug delivery system-molecularly imprinted polymer was also introduced.

Objective@#To investigate the clinical effects of integrated traditional Chinese and Western medicine in the treatment of castration-resistant prostate cancer (CRPC).@*METHODS@#A total of 54 CRPC patients were randomly divided into a control and a trial group, all treated by endocrine therapy (oral Bicalutamide at 50 mg per d plus subcutaneous injection of Goserelin at 3.6 mg once every 4 wk) and chemotherapy (intravenous injection of Docetaxel at 75 mg/m2 once every 3 wk plus oral Prednisone at 5 mg bid), while the latter group by Fuyang Huayu Prescription (a Traditional Chinese Medicine [TCM] prescription for tonifying yang and dispersing blood stasis) in addition, for a course of 24 weeks. Comparisons were made between the two groups of patients in the level of serum prostate-specific antigen (PSA), Karnofsky physical condition scores, function assessment of cancer therapy-prostate (FACT-P) scores, and TCM symptoms scores before and after 12 or 24 weeks of treatment.@*RESULTS@#Compared with the baseline, the serum PSA level was significantly decreased after 12 weeks of treatment both in the control ([25.9 ± 39.3] vs [20.0 ± 21.1] μg/L, P 0.05). At 24 weeks, however, the PSA levels in the control and trial groups were slightly increased to (23.1 ± 28.4) and (19.6 ± 23.5) μg/L, respectively, with no statistically significant differences in between (P >0.05). Karnofsky, FACT-P and TCM symptoms scores were all markedly improved in the trial group after 12 weeks of treatment (P 0.05).@*CONCLUSIONS@#TCM Fuyang Huayu Prescription combined with endocrine therapy and chemotherapy is effective for CRPC.

Objective To compare the MRP8/14 level in Normal people,benign prostatic hyperplasia(BPH) patient and prostate cancer patient,and explore the relationgship between MRP8/14 and prostate cancer.Methods 150 cases of normal people,150 cases of BPH patients and 150 cases of prostate cancer patients were chose from December 2012 to December 2014 in our hospital.ELISA method was used to detect the MRP8/14 level in each group.ROC cure was used to analyse the prediction value of MRP8/14 for prostate cancer.According to the cut off value of MRP8/14,prostate cancer patients were divided into MRP8/14 low value group (MRP8/14 < cut off value) and MRP8/14 high value group (MRP8/14 ≥cut off value),and the difference of patient's clinical characteristics and survival function between high value group and low value group were explored.Results The MRP8/14 level of normal people was (966.7 ± 152.8) ng/ml,while the BPH patient was (1 207.0 ± 190.6) ng/ml,and the prostate cancer patient was (2 833.3 ± 1 101.5) ng/ml,the difference is statistically significant.ROC analysis result showed that the AUC for the prediction of prostate cancer was O.887 (95% CI 0.841-0.934),with a high statistical significance,indicating that MRP8/14 may possess high prediction value for prostate cancer.In addition,the cut off value was 2 845.682 ng/ml,with the specifity of 76.4% and sensitivity of 85.1%.According to the cut off value of MRP8/14,prostate cancer patients were divided into the low MRP8/14 group (< 2 845.682 ng/ml) and the high MRP8/14 group (≥2 845.682 ng/ml).Among the 150 prostate cancer patients,88 cases were in the low MRP8/14 group and 62 cases were in the high MRP8/14 group.Comparations of the baseline characteristics of the two groups showed that amount of patients belong to ECOG PS =1,Gleason score =8-10,organ involvement > 2 and tumour stage > Ⅲ were much more in MRP8/14 high value group.PSA level,LDH level,AKP level,CRP level and Alkaline Phosphatase level were also significantly increased in MRP8/14 high value group.Besides,prostate cancer patient with an average follow-up of 2 years,a total of 32 cases of patients died,12 cases in the MRP8/14 low value group with mortality of 13.6%,20 cases in MRP8/14 high value group with mortality of 32.3%.Kaplan Meier survival function curve shows 2 years survival rate of patients in high value group was significantly reduced.Cox regression analysis showed that MRP8/14 was possible risk factors associated with mortality,and the independent predictors for all-cause mortality during follow-up.Conclusion MRP8/14 was significantly increased in prostate cancer patients,and it was an independent predictor of 2-year mortality in prostate cancer patients.

ObjectiveTo investigate the role of reducing the door-to-needle time for patients with acute ischemic stroke based on the quality improvement program of PDCA cycle.MethodsConsecutive patients with acute ischemic stroke admitted to hospital were registered prospectively from January 1, 2016 to September 30, 2016.Questionnaires and time tracking method were used to investigate the door-to-needle (DNT) and its influencing factors.PDCA cycle method was used to improve the stroke channel workflow and the changing trend of DNT was analyzed.ResultsA total of 71 patients with acute ischemic stroke were enrolled.After 3 PDCA cycles, DNT (median, interquartile range) from 100.0 min (65.5-127.0 min) reduced to58.0 min (45.5-80.0 min) (Z=11.689, P<0.001), the proportion of the patients with DNT ≤60 min increased from 19.05% to 60.00% (χ2=7.893, P=0.019).Conclusions The quality improvement program of PDCA cycle may effectively reduce the time of DNT in patients with acute ischemic stroke.

RESUMO

Objective To observe the operative effect of selective decompressive laminectomy and long segment pedical screw internal fixation for degenerative scohosis beyond 3 segments. Methods During January 2003 to July 2009, 52 cases with degenerative scoliosis beyond 3 segments were treated by posterior selective decompressive laminectomy, intervertebral implanted bone fusion and long segment pedical screw internal fixation, vetebral plate fusion seperately, and all the cases were followed up. Results All the patients were followed up for 12-60 months after operation. The status of back leg pain relief were evaluated through Oswestry score system. The Oswestry score of 1 week,3 months and 12 months after operation were (25.91 ± 9.05), (26.75 ± 8.37), (26.90 ± 8.45) scores respectively and there was statistical significance compared to the preoperative [(46.35± 8.12) scorns] (P <0.05). The average coronal Cobb angle was corrected from 15.23°± 3.35° to 10.02°± 3.26° 1 week after operation (P < 0.05). All the internal implant didn't appear loosing or fracture confirmed by X-ray at 3,6,12 months after operation. The intervertebral implanted bone fusion and vetebral plate fusion all got well in 18-24 months after operation. Conclusion Selective decompressive laminectomy and long segment pedical screw internal fixation for the degenerative scoliosis can get better operative effect.

<p><b>OBJECTIVE</b>The purpose of this study was to determine the feasibility of transcatheter pulmonary valve replacement in sheep up to 6 months post procedure.</p><p><b>METHODS</b>Fresh sheep pericardium treated with a 0.6% glutaraldehyde solution for 36 hours was sutured to a valvular ring and then fixed onto a newly designed nitinol self-expandable stent. Thoracotomy was performed in sheep (23.5 +/- 3.1) kg under general anesthesia and the device was delivered into the native pulmonary valve of the sheep via the anterior wall of right ventricle by catheter and fooled for 6 months.</p><p><b>RESULTS</b>One sheep died 4 months after the procedure due to in-stent thrombosis. Another 4 animals survived the 6-month observing period. Angiographic and hemodynamic measurements confirmed good positioning and function of the stents with a competent valve immediately post procedure and 6 months post the procedure in surviving animals.</p><p><b>CONCLUSION</b>Implantation of the nitinol self-expandable stent in the pulmonary valve position by a transcatheter approach is feasible and good function of transcatheter implanted memory nitinol valved stents was shown after 6 months of implantation in sheep.</p>

RESUMO

[Objective]To evaluate the surgical results and value of posterior lumbar decompression and intervertebral bone fusion with pedicle screw interval fixation in degenerative sympotomatic lumbar scoliosis.[Method]From Jan.1999 to Apr.2006,37 cases with degenerative sympotomatic lumbar scoliosis underwent posterior lumbar decompression and intervertebral bone fusion with pedicle screw interval fixation,including 21 male and 16 female patients,aged 62.6 years old averagely.All the patients were assessed by routine X-ray and Bending X-ray.CT and MRI were performed necessarily.According to patients ' symptom,total or semi-laminectomy,intervertebral bone fusion with pedicle screw interval fixation were performed.[Result]The fellow-up period varied from 1 year to 6 years(averaged,2 years and 7 months),clinical symptoms were effectively relieved after operation in all cases.Neurological functions were evaluated using JOA system,the clinical success rate in all 37 patients was 91.9%.The corrective rate was 54.8%.The average solid arthrodsis time was 11.6 weeks.The height of intervertebral disc and corrective rate had no loss.All patients X-ray of the third month and the first year didn 't show the interplantation 's clinch and breakage.[Conclusion]In the treatment of the elderly patients with degenerative sympotomatic lumbar scoliosis,first of all,the particular principle of surgical operation should be applied,the old age and long course of disease are not absolute contraindication.In addition,The technique of posterior lumbar decompression and intervertebral bone fusion with pedicle screw interval fixation can be safely performed in the elderly patients with degenerative sympotomatic lumbar scoliosis.

<p><b>OBJECTIVE</b>To investigate the inhibitory effects of antisense oligonucleotides to different sequences on VEGF gene expression by human hepatoma cells.</p><p><b>METHODS</b>SMMC7721 cells were cultured under normoxic or hypoxic conditions for 24 h, followed by being transfected with different antisense oligonucleotides (A06513 to cap structure, A06514 to translation initiation, A06515 to Exon-3 and A06516 to translation terminal). The total RNAs from the cells were extracted and the VEGF expression were examined with RT-PCR. The relative concentrations of VEGF transcripts in SMMC772 cells from different groups were determined using GAPDH (glyceraldehyde-3-phosphate dehydrogenase) cDNA as internal standard.</p><p><b>RESULTS</b>In response to the hypoxic challenge, SMMC7721 cells upregulated VEGF mRNA; Comparative to the control (no oligonucleotides), A06513, A06514, A06515, and A06516 had obvious sequence-specific inhibitory effect on VEGF gene expression, with the ratio of VEGF over GAPDH of 0.49+/-0.08, 0.71+/-0.12, 0.72+/-0.11 and 0.86+/-0.12, respectively (F=12.21, P< 0.05). A06513 showed the strongest inhibitory effect (P<0.01).</p><p><b>CONCLUSION</b>The antisense oligonucleotides complementary to VEGF cap structure, may become a potential alternative for antisense gene therapy of HCC.</p>

Objective To compare MRI wi th CT discography (CTD) for diagnostic assessment of lumbar disc disruption. Methods Paired comparative examination in 16 patients with ch ronic lower back pain without radicular pain and no disc herniation was conducte d using CT or MRI. The standard of CTD classification and positive disc was for mulated and the correlation between the induced lower back pain and dosage used in CTD was observed. Results For a total of 21 discs in the 16 patients, CTD showed the disc as type 2 in 12 discs and type 5 in 1 disc with 13 positive discs, while MRI only showed the high-intensity zone of poster ior annulus in 6 discs as the indirect sign of disc disruption and disc degenera tion in 7 discs. Conclusion CTD was the only method for showing the direct sign of disc disruption. The induced lower back pain was rel ated with the type of disc disruption. MRI can show some of the indirect signs of disc disruption and CTD can show the direct sign of disc disruption.
