RESUMO

Objective To analyze the endemic situation of schistosomiasis in Wuhu City from 2009 to 2018, so as to provide evidence for adjusting and developing the control strategy. Methods The data regarding the endemic situation were retrospectively collected and analyzed in Wuhu City from 2009 to 2018. Results The prevalence of human Schistosoma japonicum infections reduced from 0.450% in 2009 to 0.035% in 2018 in Wuhu City, and a reduction rate of 92.22% (χ2 = 1 6128.307, P < 0.01), and the prevalence of S. japonicum infections decreased from 0.483% in 2009 to 0 in 2018 (χ2 = 27.570, P < 0.01) in livestock. The area of snail habitats increased from 1 501.07 hm2 in 2009 to 4 408.62 hm2 in 2018, with an increase of 193.70%. No infected snails were found in Wuhu City since 2012, and no egg positives were detected in humans and livestock since 2016. Conclusions Currently, the endemic situation of schistosomiasis is at a low level in Wuhu City; however, there are still factors affecting schistosomiasis transmission. In the future, the integrated strategy with emphasis on the control of infectious source and the surveillance-response system should be intensified to consolidate the achievements of schistosomiasis control.

RESUMO

Objective To investigate the characteristics of the colon polyp in a northwest district of China. Methods 112 cases of patients from 1~18 years old, diagnosed with polyps by means of colonoscopies reference centers were followed-up from January 2015 to December 2016. The clinical variables evaluated in this study included: gender, age, symptoms, age at onset of symptoms, age at identification of the polyp, interval of time between the onset of symptoms and the endoscopic diagnosis of colonic polyps, and family history of colon polyp and/or colorectal cancer. The characteristics of the polyps also included: number, morphological type, diameter, histology, and distribution. The case records of these patients were reviewed. The demographics, clinical course, laboratory results, endoscopic and histopathological features, treatments, and outcomes were analyzed. Results In the 112 patients, the median age was 6.9 years, of which 64.3% (72/112) were male. All the patient had the symptom of hematochezia and just a little of the patients have the symptoms of abdominal pain, prolapse of anal polyp and diarrhea. 9.8% (11/112) was mild anemia and the anemia patient all had the symptom of hematochezia of more than 6 months and have no connection with the polyp location. Juvenile polyps were identified in 91.9% (103/112) patients, inflammatory polyp were identified in 7.1% (8/112) and 0.9% (1/112) were identified with adenoma. 1.8% (2/112) of the patient had the family history of colorectal polyp. Conclusions The morbidity of colorectal polyp in children and adolescents in our region is high. The male was significantly more common seen than female. A little proportion of the patient had the family history of colorectal polyp. The hematochezia could be seen in all patients A little proportion of the patient was mild anemia and the anemia patient all had the symptom of hematochezia of more than 6 months. Mainly the polyp was located in the rectum and sigmoid. Juvenile polyps were the major pathologic type and then the inflammatory polyp and adenoma. The adenoma may located in other place than rectum and sigmoid.

Objective To evaluate the effects of different dosages of rocuronium on abdominal wound closure under general anesthesia.Methods Ninety ASA class I or II patients scheduled for elective abdominal surgery under general anesthesia were randomized into three groups with 30 cases each.Each group received rocuronium 1 time of ED95 (0.3 mg/kg),0.5 time of ED95 (0.15 mg/kg),0.2 time of ED95 (0.06 mg/kg) respectively when closing the abdominal wall.TOF recovered to 25%, 75%, 90%, time of eye-opening on command, time of head-lift for more than 5 seconds,extubation time, the rate of upper airways obstruction after extubation and the satisfaction score for the muscle relaxation to general surgeons were recorded.Results TOF values had no significantly different among the three groups before abdominal wound closure (P>0.05).Compared with group A and group B,time of TOF in group C was significangtly increased after administration of rocuronium (P<0.05) . Time of TOF recovery to 25%, 75%, 90%in group C were significangtly decreased (P<0.01).Compared with group A and group B, time of eye-opening on command ,head-lift for more than 5 seconds and extubation time were significantly shorter in group C.There were 8,4,0 patients undwent upper airways obstruction among each group respectively after extubation (P<0.05).The satisfaction score for muscle relaxation among three groups had no significant difference.Conclusion An additional administration of 0.2 time of ED95 (0.06 mg/kg) rocuronium before abdominal wound closure in abdominal surgery can achieve satisfactory muscle relaxation for surgeons.

<p><b>OBJECTIVE</b>To investigate the effects, side-effects and security of bupivacaine, ropivacaine combined with fentanyl in postoperative continuous epidural analgesia.</p><p><b>METHODS</b>A total of 1 600 postoperative continuous epidural analgesia patients receiving different agents in SICU were divided into two groups: 0.1% bupivacaine +5 microg/ml fentanyl group (group B, n = 920) and 0.2% ropivacaine +2 microg/ml fentanyl group (group R, n = 680). The effects (visual analog-scale score and content to analgesia), side effects were analyzed retrospectively in the two groups.</p><p><b>RESULTS</b>Compared with group B, patients in group R had higher analgesia contentment (P < 0.05), but no difference in visual analog-scale score was found in the two groups. The incidences of urinary retention, nausea and vomiting, skin itching in group B were significantly higher than those in group R (P < 0.05). In each group, patients over sixty had higher ratio of hypotension than those under sixty (P < 0.05); The female patients had a higher incidence of nausea and vomiting than male patients (P < 0.05); The incidence of debility and numbness of lower limbs in patients with lumbar segments epidural analgesia was higher than those with thoracic analgesia (P < 0.05).</p><p><b>CONCLUSIONS</b>0.1% bupivacaine +5 microg/ml fentanyl and 0.2% ropivacaine +2 microg/ml fentanyl can provide adequate pain relief in postoperative continuous epidural analgesia, and 0.2% ropivacaine +2 microg/ml fentanyl comes with less side effects. The incidence of complication is related with analgesics, age, gender and the position of epidural puncture.</p>
