RESUMO

Overexpression of ABCG2 transporter in cancer cells has been linked to the development of multidrug resistance (MDR), an obstacle to cancer therapy. Our recent study uncovered that the MET inhibitor, tepotinib, is a potent reversal agent for ABCB1-mediated MDR. In the present study, we reported for the first time that the MET inhibitor tepotinib can also reverse ABCG2-mediated MDR in vitro and in vivo by directly binding to the drug-binding site of ABCG2 and reversibly inhibiting ABCG2 drug efflux activity, therefore enhancing the cytotoxicity of substrate drugs in drug-resistant cancer cells. Furthermore, the ABCB1/ABCG2 double-transfected cell model and ABCG2 gene knockout cell model demonstrated that tepotinib specifically inhibits the two MDR transporters. In mice bearing drug-resistant tumors, tepotinib increased the intratumoral accumulation of ABCG2 substrate drug topotecan and enhanced its antitumor effect. Therefore, our study provides a new potential of repositioning tepotinib as an ABCG2 inhibitor and combining tepotinib with substrate drugs to antagonize ABCG2-mediated MDR.

RESUMO

OBJECTIVES@#A study was conducted to investigate the clinical effects of oral digital design on the aesthetic restoration of anterior teeth of cleft lip/palate patients.@*METHODS@#Nine adult cleft lip/palate patients who need aesthetic restoration of anterior teeth were recruited. Digital information of patients' dental arches, the surrounding soft tissue and face were captured by digital camera and scanner. The aesthetic analysis and design were conducted using keynote and 3shape software and were demonstrated to the patients. The optimized treatment plan was ensured by communicating with the patients. Digital wax-up models were exported and printed into resin diagnostic models, which were then utilized in the treatment process to guide the doctors and the technicians in tooth preparation and in making the final restorations, respectively. The adhesive procedure was completed after satisfactory try-in. Aesthetics assessment was conducted in accordance with the anterior esthetic evaluation form. The scores of patient's satisfaction were recorded on a questionnaire containing six items of aesthetic index and doctor-patient communication. Patients were interviewed and examined after 1, 3, 6, and 12 months, respectively, and the clinical effects of restorations were evaluated.@*RESULTS@#All nine patients had satisfactory clinical results. The aesthetic defects of the patients were effectively addressed. All treatments met the requirements of the preoperative digital designs. The patients' scores were all above 90 on the satisfaction scale. At 12 months after the operation, the clinical effects of restorations of all cases achieved A class in each evaluation indicator.@*CONCLUSIONS@#For cleft lip/palate patients with esthetic defect in the anterior teeth, the digital design plays an important role in optimizing the treatment plan and guides the whole treatment process. This design can help clinicians achieve predictable satisfactory aesthetic results.

OBJECTIVE@#To evaluate the predictive value of pre-treatment serum uric acid (sUA) level for the prognosis of newly diagnosed multiple myeloma (NDMM) patients.@*METHODS@#The NDMM patients admitted to our center from January 2014 to December 2018 were analyzed retrospectively, and 94 patients among them who were initially treated with bortezomib-based chemotherapy for at least 4 cycles were included in this study. Clinical characteristics, laboratory data and follow-up information were collected, and the predictive value of sUA on the overall survival (OS) of NDMM was evaluated by using receiver operating characteristic (ROC) curve based on the patient's pre-treatment sUA level and the survival status at the end of follow-up, and the correlation of the sUA level with patient's clinical, laboratory characteristics and overall survival (OS) was further analyzed. The univariate and multivariate Cox proportional-hazards model were used to identify the potential factors affecting OS.@*RESULTS@#ROC analysis showed that the area under the curve for predicting OS in NDMM patients with sUA level was 0.702 (P<0.001), and the optimal cut-off value was 455.4 μmol/L. Compared to patients with low sUA (<455.4 μmol/L), patients with higher sUA (≥455.4 μmol/L) were more likely to have international staging system (ISS) stage III disease, beta2-microglobulin (β@*CONCLUSION@#Pre-treatment sUA level is a potential biomarker for the prognosis evaluation in NDMM patients, which deserves a further exploration and verification.

Objective To study the first-time killing efficacy and the chain-killing efficacy of four gel baits against Blattella germanica: 1% chlorpyrifos, 0.05% fipronil, 2.15% imidacloprid, and 0.5% dinotefuran and provide a basis for drug selection in controlling Blattella germanica. Methods Laboratory killing efficacy test was conducted according to the national standard GB/T 13917.7-2009 and the chain-killing efficacy test was conducted for three rounds.The first round of chain efficacy test was conducted by feeding the cockroaches killed in the laboratory efficacy test, and each next round by feeding the cockroaches killed in the last round.Median lethal time (LT50), 95% confidence limit, and toxicological regression equation of each test were calculated by software DPS V9.01. Results The LT50 of the efficacy test with 1% chlorpyrifos gel bait was 0.745 5 (0.603 4-0.890 3) d.The LT50 of the first, second and third chain experiments increased by 3.30, 2.18 and 2.76 times, respectively.The LT50 of the efficacy test with 0.05% fipronil gel bait was 0.846 5(0.464 7-1.228 0)d, and increased by 5.42, 2.09 and 1.48 times, respectively, in the first, second and third chain experiments.The LT50 of the efficacy test with 2.15% imidacloprid gel bait was 3.192 1(2.865 0-3.506 0)d, and increased by 1.13, 1.65 and 1.15 times, respectively in the first, second and third chain experiments.The LT50 of the efficacy test with 0.5% dinotefuran gel bait was 0.997 1(0.805 8-1.191 6) d, and increased by 3.85, 1.37 and 1.78 times, respectively in the first, second and third chain experiments. Conclusion In the laboratory killing efficacy test, 1% chlorpyrifos, 0.05% fipronil, and 0.5% dinotefuran gel baits are better than 2.15% imidacloprid gel bait.In the chain-killing efficacy test, 2.15% imidacloprid and 0.5% dinotefuran gel baits are better than 1% chlorpyrifos and 0.05% fipronil gel baits.Based on our results, we recommend the use of 0.5% dinotefuran gel bait for comprehensive and sustained killing effect.

Complex physical activity is a prerequisite for the completion of advanced sports skills, while trunk control is a prerequisite for complex physical activity. Cerebral palsy is often characterized by poor trunk control and poor balance response. An accurate measurement for trunk control function of patients is of great guiding significance for clinical treatment. Trunk Control Measurement Scale (TCMS) is applied to children with neuromotor dysfunction aged five years and above. TCMS has high reliability and validity, and high test-retest reliability and inter-rater reliability in evaluating spastic cerebral palsy, and it is a reliable, effective and appropriate measurement tool in evaluating the trunk control ability. It can combine with other functional scales to provide more valuable information for the function of patients with cerebral palsy because of its high correlation with other types of scales.

Background@#The development of auditory and speech perception ability of children with hearing loss is affected by many factors after they undergo cochlear implantation (CI). Age at CI (CI age) appears to play an important role among these factors. This study aimed to evaluate the development of auditory and speech perception ability and explore the impact of CI age on children with prelingual deafness present before 3 years of age.@*Methods@#Two hundred and seventy-eight children with pre-lingual deafness (176 boys and 102 girls) were included in this study, and the CI age ranged from 6 to 36 months (mean age, 19 months). Categorical auditory performance (CAP) was assessed to evaluate auditory ability, and the speech intelligibility rating was used to evaluate speech intelligibility. The evaluations were performed before CI and 1, 3, 6, 12, 18, 24, 36, 48, and 60 months after CI.@*Results@#The auditory ability of the pre-lingually hearing-impaired children showed the fastest development within 6 months after CI (k = 0.524, t = 30.992, P < 0.05); then, the progress started to decelerate (k = 0.14, t = 3.704, P < 0.05) and entered a plateau at the 24th month (k = 0.03, t = 1.908, P < 0.05). Speech intelligibility showed the fastest improvement between the 12th and 24th months after CI (k = 0.138, t = 5.365, P < 0.05); then, the progress started to decelerate (k = 0.026, t = 1.465, P < 0.05) and entered a plateau at the 48th month (k = 0.012, t = 1.542, P < 0.05). The CI age had no statistical significant effect on the auditory and speech abilities starting at 2 years after CI (P < 0.05). The optimal cutoff age for CI was 15 months.@*Conclusions@#Within 5 years after CI, the auditory and speech ability of young hearing-impaired children continuously improved, although speech development lagged behind that of hearing. An earlier CI age is recommended; the optimal cutoff age for CI is at 15 months.

OBJECTIVE: To investigate the time of clinical symptom disappearance, therapeutic efficacy and safety of Pudilan xiaoyan oral liquid and its effects on immune factors in the treatment of pediatric acute upper respiratory tract infection. METHODS: A total of 142 children aged 3-17 with acute upper respiratory tract infection selected from Chongqing Jiulongpo District Second People's Hospital during May 2015-May 2016 were divided into observation group (72 cases) and control group (70 cases) according to random number tablet. Both groups received symptomatic treatment as antivirus, antipyretic. Observation group was additionally given Pudilan xiaoyan oral liquid 10 mL each time, 3 times a day, treated for consecutive 7 d. The defervescence time, the disappearance time of rhinobyon, coughing, diarrhea and pharynx swelling were compared between 2 groups. The levels of TNF-α, IL-6 and immune factors (CD3+, CD4+ and CD4+/CD8+) were determined in 2 groups before and after treatment. Clinical efficacies and the occurrences of ADR were observed in 2 groups. RESULTS: Compared to control group, the defervescence time, the disappearance time of rhinobyon, coughing, diarrhea, pharynx red and swelling were significantly shortened in observation group (P<0. 05). Before treatment, there was no statistical significance in the serum levels of TNF-α, IL-6 or immune factors between 2 groups (P>0. 05). After treatment, the serum levels of TNF-α and IL-6 in 2 groups were significantly lower than before treatment (P<0. 05). The serum levels of CD3+, CD4+ and CD4+/CD8+ were significantly higher than before treatment of each group (P<0. 05), while the improvement of observation group was significantly better than that of control group (P<0. 05). Total response rate of observation group was significantly higher than that of control group (P<0. 05), but there was no statistical significance in the incidence of ADR between 2 groups (P>0. 05). CONCLUSIONS: Pudilan xiaoyan oral liquid can shorten the time of acute upper respiratory tract infection disapperance as defervescence, nasal congestion and cough. It combined with antiviral drug is better than antiviral drug alone, but antiviral drug combined with it or alone have same safety.

Objective:To evaluate the activity of superoxide dismutase (SOD) in patients with hepatitis B and Hepatocellular carcinoma.Methods:63 cases of patients with hepatitis B,101 cases of patients with Hepatocellular carcinoma (HCC) and 58 cases of normal control were collected,serum enzyme activity of SOD was detected by enzyme method.Variance analysis were applied to analyze the difference of SOD activity in different groups.Receiver operating characteristic (ROC) were applied to analyze the diagnosis performance of SOD in hepatitis B and HCC patients.Results:SOD activity from high to low in turn was hepatitis B group191.500 U/mL,healthy contol group(179.766 ± 13.546 U/mL) and HCC group 150.000 U/mL,there were significant difference when compareing any two grougs (P<0.05).SOD=187.20 U/mL could be used as diagnosis threshold in hepatitis B patients,the sensitivity was 65.08% and the specificity was 75.86%.SOD=166.00 U/mL can be used as diagnosis threshold in HCC patients,the sensitivity was 82.18% and the specificity was 84.48%.SOD=170.40 U/mL could be used as diagnosis threshold in HCC patients with AFP<20 ng/mL,the sensitivity was 82.76% and the specificity was 79.31%.Conclusions SOD activity varies in healthy person,hepatitis B and HCC patients.The changes of SOD activity could help us to understand whether there was transformation from hepatitis B to HCC.

Objective To discuss the feasibility for quantitative assessment of nonalcoholic fatty liver disease by sequence of diffusion weighted imaging and hydrogen proton magnetic resonance spectroscopy.Methods Thirty patients with NAFLD and twenty-six normal volunteers were enrolled in an experiment group,and 25 healthy volunteers were selected into a control group.All the patients and volunteers underwent CT and MRI examinations.With the ratio of liver-to-spleen CT value as reference standard,the correlation and difference of ADC value,value of water peak,the area under water peak,values of lipid peak and area under lipid peak and fat fraction were measured and compared between the two groups.Results There was positive significant correlation between ratio of liver-to-spleen CT value and ADC value (r=0.92,P<0.05),and negative significant correlation between ratio of liver-to-spleen CT value and lipid peak and area of lipid peak (r=-0.91 and r=-0.96,respectively;all P<0.05),there was significant negative correlation between ADC value and lipid peak and area of lipid peak (r=-0.91,P<0.05),and ADC value,the values of water peak,lipid peak and area under peak and fat fraction were different between the two groups (P<0.05).Conclusion There is significant correlation and difference between DWI and 1H-MRS in evaluating NAFLD,which has the potential to quantitatively diagnose the severity and degree of fatty liver.

Objective To investigate the distribution characteristics of methicillin-resistant Staphylococcus aureus (MRSA)in a children's hospital,and provide basis for the prevention and control of MRSA infection in children. Methods Children who admitted to a children's hospital from 2011 to 2015 were analyzed retrospectively,clinical data of children,isolation of pathogens,types of specimens,and healthcare-associated infection(HAI)status were analyzed.Results From 2011 to 2015,a total of 911 children isolated Staphylococcus aureus (SA,1108 positive specimens),494 of whom isolated MRSA (599 positive specimens),54.23% of children isolated MRSA(isolation rate of specimens was 54.06%);there was no significant difference in the isolation rate of MRSA between children of different genders(P > 0.05);isolation rate of MRSA in different age groups was statistically significant(P <0.05).Isolation rates of MRSA from blood,puncture fluid,secretion,and pus were 68.97%,66.00%,55.81%, and 54.47% respectively.Isolation rate of SA and MRSA increased from 0.61% and 21.74% in 2011 to 1.40%and 75.59% in 2015 respectively,difference were both significant(both P <0.05).Incidence of SA and MRSA in-creased from 0.198% in 2011 to 2.697% and 2.119% in 2015 respectively,both showed an upward trend year by year(both P <0.05).Conclusion Isolation rate of MRSA and incidence of HAI in this children's hospital increased year by year,it is necessary to intensify management,use antimicrobial agents scientifically and rationally,timelyperform disinfection and isolation,so as to curb the emergence and spread of MRSA in hospital settings.

Based on single factor tests,the optimum vacuum belt drying conditions of Qibai Pingfei granule were obtained through Box-Benhnken central combination design and RSM. In this study, drying time, drying temperature and extract density were chosen as independent variables, while transferring rate ginsenoside Rg₁, Re, Rb₁and astragaloside IV were taken as dependent variables. The optimum parameters are as follows: drying time of 112 min, drying temperature of 87 °C and extract density of 1.30 g · mL⁻¹. At the optimum condition, transferring rate ginsenoside Rg₁+ Re, Rb₁and astragaloside IV were 88.01%, 87.31%, 84. 34%. Above all, the optimum processing parameters of vacuum belt drying of Qibai Pingfei granule is reasonable and feasible, which can provide reliable basis for production.

To determine the optimum process conditions for dry granulating technique of Qibai Pingfei granule, granule excipient type, rolling wheel speed and pressure and feeding speed were studied. Taking shaping rate at a time, moisture absorption and dissolubility as index, the type and amount of granule excipient were determined. In addition, taking shaping rate at a time as index, parameters of rolling wheel speed and pressure and feeding speed were researched through single factor test and response surface methodology. The optimum parameters were as follows: lactose as excipient, dry extract powder to excipient at 1:2, rolling wheel speed and pressure at 10.9 Hz and 6.4 MPa and feeding speed at 7.2 Hz. After validation of three batches pilot-scale production, the optimum processing parameters for dry granulating technique of Qibai Pingfei granule is reasonable and feasible, which can provide reliable basis for production.

[ Objective ] To assess the resistance risk of Culex pipiens pallens to pyriproxyfen in Culexpipiens pallens, to predict the rate of resistance development and to provide the reference for scientific and rational application of insecticides. [ Methods ] The resistance strain was developed by group selection, and the realized heritability ( h2 ) of Culex pipiens pallens resistance to deltamethrin was estimated by Tabashnik's method. [ Results] The resistance of Culex pipiens pallens to pyriproxyfen had increased by 5.69-fold after 12 generations of indoor breeding, and the h2 of Culex pipiens pallens to pyriproxyfen was 0.02, indicating that a relatively risk of resistance development. [ Conclusion] The results indicate that Culex pipiens pallens has resistance risk to pyriproxyfen, so the application of the insecticide must be scientific and rational.

<p><b>BACKGROUND</b>Microalbuminuria (MAU) is a key component of metabolic syndrome (MetS) and is an early sign of diabetic nephropathy as well. Although routine Western medicine treatments are given to MetS patients to control high blood pressure, hyperglycemia and dyslipidemia, some patients still experience progressive renal lesions and it is necessary to modify and improve the treatment strategy for MetS patients.</p><p><b>OBJECTIVE</b>To investigate the efficacy of Yiqi Huaju Qingli Herb Formula, a compound traditional Chinese herbal medicine, in MetS patients with MAU when it is combined with routine Western medicine treatment.</p><p><b>DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS</b>Sixty patients with MetS were randomized into the Chinese herbal formula group (CHF, Yiqi Huaju Qingli formula treatment in combination with Western medicine) and control group (placebo in combination with Western medicine). All treatments were administered for 12 weeks.</p><p><b>MAIN OUTCOME MEASURES</b>Urinary microalbumin (MA), urinary albumin-to-creatinine ratio (UACR), 24-hour total urine protein (24-hTP), body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR), fasting plasma glucose (FPG), 2-hour postprandial plasma glucose (2-hPPG), glycosylated hemoglobin (HbA1c), homeostasis model assessment for insulin resistance (HOMA-IR), blood lipid profile and blood pressure were observed.</p><p><b>RESULTS</b>Compared with the control group, CHF treatment significantly decreased BMI (P<0.05), WC (P<0.01) and WHR (P<0.01). Both groups had significant decreases in FPG, 2-hPPG, HbA1c, HOMA-IR, MA, and UACR, with CHF treatment showing better effects on these parameters compared with the control treatment (P<0.05). Both treatments significantly reduced the levels of total cholesterol, low-density lipoprotein cholesterol and triacylglycerol (TAG), and a greater reduction in TAG was observed with CHF treatment (P<0.05). The level of high-density lipoprotein cholesterol did not change in the control group after treatment (P>0.05), whereas it significantly increased with CHF treatment (P<0.01). Compared with before the treatment, significant decreases in systolic blood pressure, diastolic blood pressure and mean arterial blood pressure were observed in both groups (P<0.01). However, there was no significant difference between the two groups (P>0.05).</p><p><b>CONCLUSION</b>Combined treatment of Yiqi Huaju Qingli Formula and Western medicine significantly alleviated MAU, which may correlate with the improvement of insulin sensitivity and glucose and lipid metabolism. <p><b>BACKGROUND</b>Patients with hypertension coupled with metabolic syndrome (MetS) are among the high risk population in cardiovascular and cerebrovascular diseases. To reduce the prevalence of cardiovascular and cerebrovascular diseases, it is essential to appropriately control blood pressure together with other cardiovascular risk factors.</p><p><b>OBJECTIVE</b>The current study was designed to investigate the therapeutic effects on blood pressure, blood pressure variability and other cardiovascular risk factors by giving Yiqi Huaju Formula, a compound traditional Chinese herbal medicine, in addition to routine treatment to hypertensive patients coupled with MetS.</p><p><b>DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS</b>A total of 43 patients with hypertension coupled with MetS were recruited into this study. The enrolled patients were randomly divided into the Chinese herbal formula group (anti-hypertensive drugs plus Yiqi Huaju Formula, CHF) and the control group (anti-hypertensive drugs plus placebo). The CHF group enrolled 22 patients while the control group received 21 cases. Treatments were given for 12 weeks in both groups.</p><p><b>MAIN OUTCOME MEASURES</b>Parameters examined include 24-hour ambulatory blood pressure monitoring, body mass index, waist circumference, waist-to-hip ratio, homeostatic model assessment for insulin resistance (HOMA-IR), fasting glycosylated hemoglobin (HbA1c), fasting plasma glucose, 2-hour postprandial plasma glucose (PPG), fasting plasma insulin, serum lipid, etc.</p><p><b>RESULTS</b>Compared with the control group, the CHF group had significant improvement (P<0.01) in anthropometric parameters, FPG, HOMA-IR, blood pressure amplitude, blood pressure variability and blood pressure load.</p><p><b>CONCLUSION</b>This study showed that integrated traditional Chinese and Western medicine treatment can achieve better results in controlling blood pressure as well as other cardiovascular risk factors. The mechanism of controlling of blood pressure may be associated with the improvement of insulin sensitivity due to the Yiqi Huaju intervention. <p><b>OBJECTIVE</b>The aim of this study is to investigate the development of auditory skills in Mandarin-speaking infants with normal hearing using IT-MAIS, set up normal comparison data for evaluating the auditory performance of children with hearing loss and provide a basis for establishing an appropriate hearing and speech rehabilitation program for them.</p><p><b>METHODS</b>A total of 183 infants with Mandarin-speaking patents participated in this investigation which was conducted in Beijing, China. 160 infants aged from 1 to 36 months were finally included, whose hearing were considered normal according to the history collection, high-risk registers for hearing loss and hearing screening using DPOAE. All infants were divided into 8 groups with 20 infants in each group by their ages. They were 1 month, 2-3 months, 4-6 months, 7-9 months, 10-12 months, 13-18 months, 19-24 months and 25-36 months group. The IT-MAIS/MAIS were administered to evaluate their development of auditory skills. All statistical analyses were executed using the MATLAB R2010a.</p><p><b>RESULTS</b>The detection scores improved with age and reached ceiling at 19 months in infants with normal hearing, the regression function for prediction of scores from age was score = 0.26×ln(age) + 0.23 and prediction of age from score was age = e([score-0.23])/0.26, r(2) = 0.93. The recognition scores also increased with age and reached ceiling at 24 months in infants with normal hearing, the regression function for prediction of scores from age was score = 0.26×ln(age)-0.07 and prediction of age from score was age = e([score+0.07])/0.34, r(2) = 0.93. The overall scores which combine the above two aspects augmented with age and reached ceiling at 22 months. The regression function for prediction of scores from age was score = 0.3×ln(age)+0.09 and prediction of age from score was age = e([score-0.09])/0.3, r(2) = 0.95.</p><p><b>CONCLUSIONS</b>Auditory skills showed a growth trend with age in infants with normal hearing. Scores of different auditory skills can be predicted according to their age. Age can also be predicted according to their scores of different auditory skills.</p>

Objective To compare efficacy of two intensive therapies:continuous subcutaneous insulin infusion(CSⅡ) and multiple subcutaneous insulin infusion (MSⅡ) for treatment of type 2 diabetes mellitus.Methods 60 patients with type 2 diabetes mellitus were randomly divided into 2 groups. Patients in CSⅡ group were treated by insulin (Novolin R) through a infusion pump. Patients in MSⅡ group were treated by Novolin R before meals (3 times a day) and Novolin N at bedtime. Blood glucose was monitored the whole day before and after treatment. Time required for blood glucose to reach the standard level, insulin dosage and hypoglycemia incidence were compared between the 2 groups before and after treatment. Results Both of the 2 therapeutic methods effectively controlled blood glucose (P<0.05). However, the 2 groups had significant difference in terms of the time required for blood glucose to reach the standard level ( 3.6 ± 1.2 d vs. 9.4 ± 3.2 d, P < 0. 01 ), daily insulin consumption (35.2 ± 8.5 u vs. 43.2 ± 10. 1 u, P <0. 01 ) and hypoglycemia incidence (2. 1% vs.9.7%, P <0.01 ). Conclusions CSⅡ and MSⅡ are effective for treatment of type 2 diabetes mellitus. CSⅡ is superior to MSⅡ due to its advantages of quick response, safety, and less insulin consumption.
