RESUMO

OBJECTIVE@#To derive the Chinese medicine (CM) syndrome classification and subgroup syndrome characteristics of ischemic stroke patients.@*METHODS@#By extracting the CM clinical electronic medical records (EMRs) of 7,170 hospitalized patients with ischemic stroke from 2016 to 2018 at Weifang Hospital of Traditional Chinese Medicine, Shandong Province, China, a patient similarity network (PSN) was constructed based on the symptomatic phenotype of the patients. Thereafter the efficient community detection method BGLL was used to identify subgroups of patients. Finally, subgroups with a large number of cases were selected to analyze the specific manifestations of clinical symptoms and CM syndromes in each subgroup.@*RESULTS@#Seven main subgroups of patients with specific symptom characteristics were identified, including M3, M2, M1, M5, M0, M29 and M4. M3 and M0 subgroups had prominent posterior circulatory symptoms, while M3 was associated with autonomic disorders, and M4 manifested as anxiety; M2 and M4 had motor and motor coordination disorders; M1 had sensory disorders; M5 had more obvious lung infections; M29 had a disorder of consciousness. The specificity of CM syndromes of each subgroup was as follows. M3, M2, M1, M0, M29 and M4 all had the same syndrome as wind phlegm pattern; M3 and M0 both showed hyperactivity of Gan (Liver) yang pattern; M2 and M29 had similar syndromes, which corresponded to intertwined phlegm and blood stasis pattern and phlegm-stasis obstructing meridians pattern, respectively. The manifestations of CM syndromes often appeared in a combination of 2 or more syndrome elements. The most common combination of these 7 subgroups was wind-phlegm. The 7 subgroups of CM syndrome elements were specifically manifested as pathogenic wind, pathogenic phlegm, and deficiency pathogens.@*CONCLUSIONS@#There were 7 main symptom similarity-based subgroups in ischemic stroke patients, and their specific characteristics were obvious. The main syndromes were wind phlegm pattern and hyperactivity of Gan yang pattern.

RESUMO

Objective:To investigate the effect of SCN1A gene polymorphism (SCN1A-rs3812718) on the alterations of spontaneous brain activity using amplitude of low-frequency fluctuations (ALFF) of MR in patients with temporal lobe epilepsy (TLE).Methods:A total of 37 TLE patients (TLE group) admitted to the Epilepsy Center of the 900th Hospital of Joint Logistic Team from March 2018 to August 2019 were retrospectively analyzed, and another 28 healthy volunteers matched for gender, age, and years of education with the TLE group were selected as the healthy control group (HC group). Sixty-five subjects were divided into four groups by genotype and diagnosis: 34 cases in AA/AG-TLE subgroup, 3 cases in GG-TLE subgroup, 20 cases in AA/AG-HC subgroup and 8 cases in GG-HC subgroup. All subjects underwent sagittal 3D-T 1WI and resting-state functional MRI using a Siemens 3.0 T Trio Tim MR scanner. Then ALFF values of the four groups were calculated using DPABI by the MATLAB 2010 platform. The ALFF values between two groups were compared by independent samples t-test. The ALFF values of different genotypes at rs3812718 locus in TLE and HC group were analyzed by multivariate analysis of variance to find out the corresponding brain regions with interaction, and then post hoc simple effect analysis was performed. Results:The ALFF values in TLE group significantly increased in left marginal lobe, left parahippocampal gyrus, left fusiform gyrus, left hippocampus, right insular lobe and right inferior temporal gyrus (Alphasim corrected P<0.001) and decreased in the left superior frontal gyrus, left middle frontal gyrus, left inferior frontal gyrus, right middle frontal gyrus, right precuneus, left precuneus, bilateral cingulate gyrus and right angular gyrus (Alphasim correction P<0.05) compared with HC group. Subjects carrying the non-risk G allele had higher ALFF values in the right inferior temporal gyrus, right fusiform gyrus, and right cerebellum than subjects carrying the risk A allele ( t=3.30, Alphasim corrected P=0.002). There was a significant interaction effect on posterior cerebellar lobe, left anterior cerebellar lobe, left inferior temporal gyrus, left superior parietal lobule and right precuneus of TLE patients with SCN1A-rs3812718 genotype. Post-hoc simple effect analysis showed that ALFF significantly increased in the left posterior cerebellar lobe, left anterior cerebellar lobe, left inferior temporal gyrus and left fusiform gyrus in GG-TLE subgroup ( t=5.97, P<0.001), but significantly decreased in the right superior parietal lobule, right precuneus, right posterior cerebellar lobe in AA/AG-TLE subgroup compared to the HC group. Compared with GG-TLE subgroup, ALFF in left posterior cerebellar lobe, left fusiform gyrus and left inferior temporal gyrus decreased in AA/AG-TLE subgroup. Conclusion:SCN1A gene polymorphism in the rs3812718 locus affects spontaneous neural activity in resting state, which may be one of the pathophysiological mechanisms of TLE.

Objective:To perform unilateral patellar resurfacing and contralateral patellar retention in bilateral total knee arthroplasty (TKA) randomly,and to compare the clinical effects of patellar retention with patellar resurfacing in TKA.Methods:In the study,14 bilateral knee osteoarthritis (OA) patients were randomized in the bilateral TKA to receive unilateral patellar resurfacing and contralateral patellar retention,including 28 knees,all were females,53 to 78 years old,with average (66.9 ± 7.8) years,and the BMI was (26.3 ± 1.8) kg/m2.All subjects were followed up from 3 to 12 months.The clinical effects were evaluated based on measurements of American Knee Society score (KSS),range of motion (ROM),anterior knee pain,patellar clunk,and patellar tilt angle (PTA).Results:All the wounds healed primarily without significant complications,such as infection,aseptic loosening,patellar fracture and so on.The preoperative KSS scores of patellar resurfacing group were 38.9 ± 22.2,and the scores changed to be 92.4 ± 6.7 after operation,which were added by 53.5 ± 20.3.While in the patellar retention group,the KSS scores were 38.4 ± 20.5 preoperatively,and after operation,which were added to be 92.1 ±4.2,and improved by 53.7 ±21.4.The differences in the changed KSS scores between TKA with and without patellar resurfacing were not statistically significant (Independent t-test,P =0.98).The ROM was changed from 95.4° ± 13.5° preoperatively to 120.4° ± 8.9° postoperatively in the patellar resurfacing group and from 92.9° ± 19.1 ° preoperatively to 120.4 ± 8.4° postoperatively in the patellar retention group.The ROM of the two group were increased by 25.0° ± 14.5° and 27.5° ± 19.4° re spectively.However,no remarkable differences were observed between the 2 groups in the knee ROM (Independent t-test,P =0.70).At the end of the latest follow-up,3 knees in the patellar resurfacing group and 2 knees in the patellar retention group had knee anterior pain,the incidences of anterior knee pain were 21.4% and 14.3% respectively.There was no obvious difference for the incidence of post operative anterior knee pain (Chi-square test,P =0.62).The incidences of post-operative patellar clunk in the 2 groups were all with 3 knees (21.4%),which had no significant difference in the 2 groups (Chi-square test,P =1.00).The post-operative PTA were 2.6° ± 2.6° in the patellar resurfacing group and 3.6° ± 2.9° in the patellar retention group,respectively.There was also no statistical difference between the 2 groups (Chi-square test,P =0.36).Conclusion:For knee OA patients with mild or moderate patellar cartilage damage,performing patellar resurfacing or not didn't significantly affect anterior knee pain,patellar clunk,functional outcomes or patellar tracking after TKA.So we suggest retain patella in TKA for OA patients with mild or moderate patellar cartilage damage.

Objective To detect the SNR and geometric accuracy of MRI based on American College of Radiology (ACR)standards.Methods The SNR and geometric accuracy of Siemens Skyra 3.0T,Siemens Trio 3.0T and GE Excite HD 1.5T MRI were measured with ACR phantom,and the detection results were calculated according to the standards.Results The SNR values of Siemens Skyra 3.0T,Siemens Trio 3.0T and GE Excite HD 1.5T MRI were 589.98,438.50 and 277.12 respectively.Siemens Skyra 3.0T MRI had the values of geometric accuracy being-1.93% at X direction,-3.20% at Y direction and 0.68% at Z direction,Siemens Trio 3.0T MRI had the value of geometric accuracy being-0.87% at X direction,-2.33% at Y direction and 1.49% at Z direction,GE Excite HD 1.5T MRI had the values of geometric accuracy being 0.20% at X direction,-1.53% at Y direction and 1.69% at Z direction.Conclusion The detection of the SNR and geometric accuracy of MRI can effectively guarantee the image quality.

BACKGROUND:In clinical practice,there are some patients with labial inclined anterior teeth which influence their appearance,but they refuse orthodontic treatment because of many factors such as long-term treatment.Alternatively,these patients can receive aesthetics restoration with re-directional post-core and crown.OBJECTIVE:To investigate the influence of three kinds of post-core materials on fracture resistant ability of labial inclined maxillary central incisor with different changed angles.METHODS:Seventy-five maxillary central incisors were randomly divided into three groups:group Ⅰ was restored with Co-Cr post-core and porcelain-fused-to-metal (PFM) crown,group Ⅱ was restored with 50% gold alloy post-core and PFM crown,group Ⅲ was restored with everStick fiber post and composite resin core and PFM crowns.Each group then was divided into five subgroups based on angles between the post-core and long axis of the tooth (0°,10°,20°,30°,40°).All specimens were embedded in acrylic resin and fixed in electronic universal testing machine.Force was loaded to each specimen until fracture occurred,and the maximum load values of fracture and the fracture mode were recorded.RESULTS AND CONCLUSION:The fracture resistance of group Ⅲ was superior to groups Ⅰ and Ⅱ (P < 0.05),but there was no significant difference between group Ⅰ and group Ⅱ (P > 0.05).The maximum load value of the three groups decreased as the changed angle increased.The fracture of group Ⅲ mostly appeared on the 1/3 of the root neck,while the fracture of groups Ⅰ and Ⅱ mostly appeared on the 1/3 of root central or the 1/3 of the root neck.So,the fracture resistance and fracture mode of central incisor under changed angles restored with everStick fiber post were superior to that with cast metal post-core.During re-directional restoration of labial inclined maxillary anterior teeth,everStick fiber post is recommended as a better choice to reduce the risk of catastrophic fractures.Also it suggests that excessive changed angle should be avoided to prevent root fracture.

Objective To compare the diagnostic value of DWI and ultrasound elastography(UE) in staging hepatic fibrosis in patients with chronic viral hepatitis B(CHB). Methods Fifteen healthy volunteers (control group) and 69 patients with CHB (disease duration more than 1 year) (patient group) were prospectively recruited. All of the subjects underwent DWI and UE experiments twice with the interval of less than 3 days. Liver ADC and shear wave velocity(SWV) values were obtained for subsequent analysis. Sixty?nine patients who had biopsy were grouped according to their pathological grading of fibrosis, from S0 to S4. One?way ANOVA was used to compare the ADC and SWV values between the five different fibrosis groups and control group. Spearman correlation analysis was used to analyze the correlations between the ADC and SWV values and those staging factors. Finally, receiver operating characteristic (ROC) curve analysis was performed to compare the diagnostic performance of ADC and SWV values in discriminating different stages of hepatic fibrosis. Results Based on the pathological results, the 69 patients were reclassified to five subgroups with 11, 13, 12, 15 and 18 patients in the S0, S1, S2, S3 and S4 groups respectively. ADC values were(1.39±0.09)×10-3,(1.39±0.08)×10-3,(1.38±0.10)×10-3,(1.20±0.06)×10-3,(1.12±0.07)×10-3 and(1.01±0.07)×10-3mm2/s for the control group and stages S0 to S4 respectively.SWV values were(1.17±0.07),(1.16±0.08),(1.23±0.10),(1.48±0.14),(1.85±0.14)and(1.97±0.12)m/s for stages S0 to S4 and the control group respectively. Statistically significant differences were observed among them (P<0.01). Both ADC (r=-0.894,P<0.01) and SWV (r=0.904,P<0.01) values were highly correlated with the stages of liver fibrosis. The area under ROC(AUC) for predicting fibrosis stages (≥S1, ≥S2, ≥S3 and S4) with ADC values was 0.893, 0.991, 0.966 and 0.952 respectively. Accordingly, the AUC for SWV values were 0.937, 0.993, 0.994 and 0.914. Conclusions The two imaging methods of DWI and UE showed good and similar diagnostic performance in discriminating the different stages of hepatic fibrosis.

Objective To investigate the diagnostic value of ADC histogram obtained from MR DW imaging for stage ⅠB cervical cancer. Methods Seventy three patients diagnosed by cervical smear screening as cervical cancer without priortreatment were included prospectively in the patient group, and staged according to the international federation of gynecology and obstetrics (FIGO) staging system. Forty three patients with uterine leiomyoma detected by gynecologic examination, ultrasonography or CT and with negative result of cervical smear screening who were scheduled for hysterectomy were included prospectively in the control group. The patients of both groups underwent routine pelvic MR sequences, dynamic contrast enhanced imaging and DWI before hysterectomy. ADC histograms of the entire tumor and cervix volume were generated by post-processing software. Features of ADC histogram for the 2 groups were observed. Histogram parameters such as mean ADC (ADCmean), median ADC (ADCmedian), the 25th percentile of ADC (ADC_25th), the 75th percentile of ADC (ADC_75th), skewness and kurtosis were recorded. Student's t test or Mann-Whitney U test depending on homogeneity of variance was employed for the comparison of those parameters. ROC analysis was employed for assessing the diagnostic performance of ADC histogram in distinguishing the 2 groups. Results Thirty five patients in the patient group were staged as FIGO IB. Five patients in the control group ended up with pathologic findings of cervical intraepithelial neoplasia grade 3. Therefore 38 patients in the control group were investigated. ADC histograms of the patient group were mostly skewed positively, while the curves were largely skewed negatively. ADCmean, ADCmedian, ADC_25th, ADC_75th, skewness and kurtosis for the IB stage patient group were (1.10±0.21)×10-3mm2/s, (1.05±0.21)× 10-3 mm2/s, (0.90 ± 0.19) × 10-3mm2/s, (1.26 ± 0.23) × 10-3mm2/s, 0.83 (median) and 1.25 (median) respectively. ADCmean, ADCmedian, ADC_25th, ADC_75th, skewness and kurtosis for the control group were (1.62 ± 0.25)×10-3mm2/s, (1.64±0.24)×10-3mm2/s, (1.42±0.24)×10-3mm2/s, (1.84±0.27)×10-3mm2/s,-0.11(median) and 0.29 (median) respectively. All parameters showed statistically different (t values were -9.693,- 11.117, -10.255, and -9.988 for ADCmean, ADCmedian, ADC_25th and ADC_75th respectively;Z values were -6.360 and -4.445 for skewness and kurtosis respectively; P< 0.01). ROC analysis indicated that ADCmedian had the highest diagnostic accuracy for differentiating the 2 groups, with the area under the curve being 0.97, a cutoff value of 1.21×10-3mm2/s, and a sensitivity of 95.6%and a specificity of 89.3%. Conclusion ADC histogram of DWI may be valuable for diagnosing stage IB cervical cancer by distinguishing stage IB cervical cancer from normal cervix or cervical benign lesions.

BACKGROUND:Someone regards that treatments such as root canal therapy and post-core restoration can increase the fracture probability of teeth. Can fiber reinforced composite post enhance the fracture resistance of the severe wedge-shaped defected premolar? Three-dimensional finite element can simplify complex dental systems into a model analyzed by computer, and conduct biomechanical research through the model analysis. OBJECTIVE:To analyze the stress distribution of sever wedge-shaped defected first premolar which restored by fiber reinforced composite post and composite resin. METHODS:We established the three-dimensional finite element models of severe wedge-shaped defected maxil ary first premolar before and after restoration with fiber reinforced composite post and composite resin. The maximum bite force was designed to 267 N that was applied at 45° angle. Then, we analyzed the distribution of maximum principal stress in finite element models. RESULTS AND CONCLUSION:The maximum principal stress was concentrated in the tip of defected region. After restoration, the distribution of maximum principal stress was similar to that of the normal premolar, and the tensile stress was distributed uniformly in the upper-central part of buccal root. It may be beneficial for improving fracture resistance ability of severe wedge-shaped-defect premolar by restoration with fiber reinforced composite post and composite resin.

Objective To improve the diagnostic accuracy of pulmonary mucormycosis(PM) by investigating imaging appearances of PM,including CT and PET-CT.Methods A retrospective analysis of unenhanced MSCT(n=14),dynamic enhanced MSCT(n=10) and 18F-FDG PET-CT(n=3) examinations was conducted in 14 patients with surgically and pathologically proven PM.Results CT findings:13 patients had multiple patchy consolidation.Nine had thick-walled cavitiess(n=26) and 4 had pulmonary embolism,1 had solitary irregular masses,3 had adjacent bronchus invasion,4 had pleural effusion,14 had no mediastinal adenopathy.All the patients after renal transplantation had varied ground-glass opacifications and interlobular septal thickenings.The solid portion of lesions had strong enhancement with CT attenduation of (25±8) HU.PET-CT findings:3 cases were found with homogeneously or inhomogeneously increased 18F-FDG uptake(SUVmax 6.2,4.6,3.6).Conclusions PM presents with widespread but lacked specific imaging features.A suggestive diagnosis could be made in some patients with imaging appearances of multiple thick-walled cavities,rapid changes,and complicated with pulmonary embolism or invaded bronchus and sleevelet sign.

<p><b>OBJECTIVE</b>To explore whether the dental restoration of nickel-chromium (Ni-Cr) based alloys will lead to extra excretions of urinary Ni and Cr.</p><p><b>METHODS</b>Urinary Ni and Cr were repeatedly measured in 33 patients before and 2 months after the dental restoration of Ni-Cr alloys. The associations between alloy restoration and urinary Ni or Cr were analyzed by paired t test and general linear model of repeated measures.</p><p><b>RESULTS</b>A slightly higher urinary Ni was found in patients after 2 month of the alloy restoration, but the difference was not statistically significant (before: 46.4 microg x mol(-1) crea; after: 67.6 microg x mol(-1) crea; P=0.063). This difference was only in female subjects (before: 44.8 microg x mol(-1) crea; after: 73.7 microg x mol(-1) crea; P=0.068). A significant higher urinary Cr was found in patients after 2 month of the alloy restoration (before: 57.0 microg x mol(-1) crea; after: 99.4 microg x mol(-1) crea; P=0.024). This significant difference was only in female subjects (before: 59.8 microg x mol(-1) crea; after: 124.4 microg x mol(-1) crea; P=0.023). General linear models of repeated measurements showed that urinary excretions of Ni and Cr were associated with the number of restoration and the area of metal basis uncovered with porcelain.</p><p><b>CONCLUSION</b>Dental restoration of Ni-Cr alloy might lead to the enhanced excretions of urinary Ni and Cr.</p>

<p><b>OBJECTIVE</b>To investigate the relationship of sperm morphology with reproductive hormones in infertile men and the pathogenesis of teratozoospermia.</p><p><b>METHODS</b>This study included 90 infertile men aged 25 - 40 years. We measured their testis volumes using the Prader orchidometer, conducted routine semen analyses according to the WHO laboratory standard, and determined the concentrations of reproductive hormones and sex hormone-binding globulin (SHBG) by chemiluminescence and the levels of free testosterone (FT) and bioavailable testosterone (BioT).</p><p><b>RESULTS</b>All the subjects showed normal sperm concentration. Based on the results of semen morphology analysis, the 90 infertile men were equally divided into groups 1 (morphologically normal sperm <4%), 2 (morphologically normal sperm > or = 4% and <10%), and 3 (morphologically normal sperm > or = 10%), with no significant differences in age among the three groups (P>0.05). The volumes of the left testis were (14.27 +/- 3.65) ml, (16.90 +/- 3.57) ml and (14.57 +/- 3.57) ml, respectively (P = 0.006 group 1 vs group 2, P = 0.741 group 1 vs group 3, P = 0.014 group 2 vs group 3), and those of the right testis were (14.60 +/- 3.70) ml, (16.60 +/- 3.35) ml and (14.67 +/- 3.54) ml, respectively (P = 0.050). There were no significant differences among the three groups in prolactin, follicle-stimulating hormone, luteinising hormone, estradiol, total testosterone and SHBG, (P>0.05). The levels of serum FT were (0.25 +/- 0.07) nmol/L, (0.29 +/- 0.07) nmol/L and (0.31 +/- 0.13) nmol/L (P = 0.086 group 1 vs group 2, P= 0.010 group 1 vs group 3, P= 0.364 group 2 vs group 3), and those of BioT were (5.81 +/- 1.58) nmol/L, (6.78 +/- 1.55) nmol/L and (7.29 +/- 3.02) nmol/L, respectively (P = 0.086 group 1 vs group 2, P = 0.010 group 1 vs group 3, P = 0.364 group 2 vs group 3). The percentage of morphologically normal sperm was positively correlated with the levels of serum FT and BioT (P<0.05).</p><p><b>CONCLUSION</b>The higher the levels of serum FT and BioT, the higher the percentage of morphologically normal sperm, which suggests that serum FT and BioT might be involved in the pathogenesis of teratozoospermia.</p>

RESUMO

Biological performances of Ni-Cr porcelain alloy are highly correlated with released metallic ions. Released metal ions from Ni-Cr porcelain alloy, particularly Ni, Be can induce inflammation of the adjacent periodontal tissue and oral mucosa. In vitro evidence has indicated that the immune response can be altered by various metal ions. Allergic reactions due to metallic dental restorations have been documented. Ni has especially been identified as being highly allergenic. The cytotoxicity and corrosion level of Ni-Cr porcelain alloy is increased after recasting. The Ni-Cr porcelain alloy produced according to technology requirements has good biological safety. Ni-Cr porcelain alloy released a few of metal ions which might induce allergy and density of adjacent periodontal tissues, for the stimulation effect of these metal ions. There is no evidence to suggest that Ni-Cr porcelain alloy restorations has systemic toxicity or carcinogenic/genotoxic effect to human.

<p><b>OBJECTIVE</b>To study the rabies molecular biology features in animals between high incidence area of rabies and no rabies cases area in Hunan.</p><p><b>METHODS</b>detect saliva of dogs and brains of dogs and cats by direct immunofluorescence assay, review positive samples by RT-PCR, sequencing extract RNA virus for genetic analysis.</p><p><b>RESULTS</b>12 were detected rabies virus antigen and positive nucleoside acid in 82 dogs from Wugang city also 1 in 17 from Dongkou county; the positive rate: Wugang 14.63 percent, Dongkou 5.88 percent. No rabies virus was detected in 67 samples of dog brains from Fenghuang County. Also none in 28 samples of cat brains. Amplificating N gene of rabies virus from positive samples of dog brain's tissue (No Wg13, Dk13) by RT-PCR, it shows that homology of nucleoside acid between two strain of virus is 99.4 percent; also 99.1 percent of amino acid. The homology of nucleoside acid (amonio acid) among Wg13 stain and Chinese strain CTN and aG strain is 89.4 percent (98.2 percent) and 86.1 percent (95.1 percent); The homology of nucleoside acid (amonio acid) among Dk13 stain Chinese strain CTN and aG strain is 89.1 percent (98.0 percent), 86.1 percent (94.9 percent).Compare with isolated rabies virus from abroad, the homology between two strains and Indonesia is 92.8 percent and 93.2 percent, the most similar of them. The strains isolated from other countries including Japan, Sri Lanka and India are relatively lower; The sequence of gene Wg13 and Dk13 were taken replacement of amino acid.</p><p><b>CONCLUSION</b>Two strains are belong to type I rabies virus, comparing its N gene with current using vaccine strains, both are in same group, and homology are relatively higher.</p>

<p><b>OBJECTIVE</b>To investigate the distribution of hepatitis B virus (HBV) genotypes in patients with chronic HBV infection among 11 cities of China.</p><p><b>METHODS</b>A total of 1214 serum samples from patients with chronic HBV infection were collected in 11 cities of China, including Beijing, Qingyuan, Shenzhen, Shijiazhuang, Hanchuan, Nanjing, Changchun, Liaocheng, Jinan, Ningbo and Wenzhou. Genotypes of the 1214 HBV strains were identified by PCR method with type specific primers. Parts of the results were confirmed by direct sequencing analysis of PCR products.</p><p><b>RESULTS</b>Among the 1214 patients with chronic HBV infection, 0.7% (9/1214)were genotype A, 28.4% (345/1214)genotype B, 58.4% (709/1214) genotype C, and 12.4% (151/1214) genotype B and genotype C mixed infection. No other genotypes were found. Genotype C was predominant in the northern part of China, such as Changchun, Beijing, Shijiazhuang,while genotype B was more commonly seen in south of China. 71.4% (20/28) for patients from Qingyuan and 63.6% (70/110) from Shenzhen were infected with genotype B.</p><p><b>CONCLUSION</b>HBV genotypes had distinct geographic distribution. Genotype B and C the predominant strains in patients with chronic HBV infection in China. Genotype C was predominantly identified in the northern part of China versus genotype B the south.</p>

2 years),seizure frequency(≥1 time/month),persistence time(≥60 s),gene-ralized seizure were all associated with the incidence of the loss of neuron in ammonias.Multivariate Logistic regression analysis showed that the independent influencital factors for the loss of neuron in ammonias in children with temporal epilepsy included seizure frequency,persis-tence time and tape of seizure. Conclusions The loss of neuron in ammonias though 1H-MRS can be detected.The results of multivariate analysis verify that the development of the loss of neuron in ammonias may be associated with many factors including age of onset,course of di-sease,seizure frequency,persistence time and generalized seizure.In order to lower the incidence of the loss of neuron,early intervening treatment is very important.

<p><b>OBJECTIVE</b>To study hepatitis B virus (HBV) genotype and subtype distribution and its clinical significance in HBV-infected patients.</p><p><b>METHODS</b>We used type/subtype-specific primers and PCR to detect HBV genotype and subtype of 445 HBV-infected patients from Beijing, Changchun, Hanchuan Shenzhen, Qingyuan and Nanjing, including 7 acute hepatitis (AH), 36 asymptomatic HBV carriers (ASC), 352 chronic hepatitis (CH), 28 liver cirrhosis (LC), and 22 hepatocellular carcinoma (HCC) cases. Genotyping results were confirmed by PCR product sequencing.</p><p><b>RESULTS</b>Among 445 HBV-infected patients, the proportions of genotype B, C, and B/C were 32.6% (145/445), 53.7% (239/445), and 13.7% (61/445), respectively. In genotype C, 13 (5.4%) were subtype C1, 135 (56.5%) were subtype C2, and the remaining 91 (38.1%) were neither C1 nor C2. In genotype B, 100 (69.0%) were subtype Ba, 25 (17.2%) subtype Bj, and the other 20 (13.8%) were neither Ba nor Bj. In genotype B/C, 15 (24.6%) were Ba/C2, 8 (13.1%) Bj/C2, 6 (9.8%) Ba/C1, 3 (4.9%) Bj/C1, 11 (18.0%) Ba/neither C1 nor C2, 7 (11.5%) Bj/neither C1 nor C2, and 6 (9.8%) neither Ba nor Bj/neither C1 nor C2, 2 (3.3%) neither Ba nor Bj/C1, 3 (4.9%) neither Ba nor Bj/C2. The HBV genotype and subtype distribution we found exhibited significant differences in the various clinical types of HBV infection tested, and showed that genotype C was predominant among patients with liver cirrhosis (78.6%) and hepatocellular carcinoma (86.4%) while genotype B was predominant in asymptomatic carriers (72.2%). In addition, genotype and subtype distribution showed no significant differences between male and female patients, but genotype and subtype distribution showed significant differences in patients positive or negative with HBeAg.</p><p><b>CONCLUSION</b>Subtypes Ba and C2 are predominant in patients with hepatitis B from these 6 cities, and genotype C may be associated with the development of liver cirrhosis and hepatocellular carcinoma.</p>

Objective To evaluate the MRI features of osteosarcoma.Methods 38 cases of osteosarcoma proved pathologically were retrospectively analyzed.All of them were examined by plain MRI scan,25 cases were examined by enhanced scan.Results The diagnostic accuracy of osteosarcoma of MRI was 93.9%.There were 30 cases of the extension into the medullary cavity,18 cases of soft tissue mass,10 cases of the destruction in epiphysis and 4 cases of the invasion in articular cartilage,especially T2 weighted STIR image and enhanced MRI scans were advantageous in showing the images mentioned above and in judging of the change after treated.Conclusion MRI is very important in diagnosis and clinical therapy of osteosarcoma.

Objective To evaluate the capacity of consecutive three-dimensional time of flight(3D-TOF)MRA and velocity-encoding(Venc)three-dimensional phase-contrast MRA(3D-PC MRA)in determining cerebral arteries.Methods 15 persons being normal on MR images,consecutive 3D-TOF and 3D-PC MR angiograms were selected prospectively to evaluate the difference between two MRA techniques in determining cerebral arteries.The 38 normal sides of 43 cerebral arteriographic cases undergone MRA and DSA were selected retrospectively to evaluate the capacity of MRA technique in determining cerebral arteries with DSA criterion.Results The branch grades of ACA,MCA or PCA demonstrated by 3D-TOF technique were not respectively different from that by 3D-PC.But the former imaging quality was much better than that of the later.The branch grades of ACA,MCA or PCA shown by MRA were respectively different from those by DSA.These grades of ACA,MCA shown by MRA were one less than those by DSA,and that of PCA shown by MRA was a few different from that by DSA.Conclusion The capacity of consecutive 3D-TOF MRA in demonstrating cerebral arteries is less than that of DSA,but it is enough to most cerebral vascular diseases.
