RESUMO

Background:Normal-pressure hydrocephalus (NPH) affects the older people and the presentations aregait disturbance, incontinenceand dementia. The reason is still unknown and there is increased CSF volume causing pressure to the periventricular brain causing the symptoms where there is no evidence of absorption failure or obstruction of the CSF pathway. Aims: The main objective of this study is to compare the surgical outcome of endoscopic third ventriculostomy (ETV) and VP shunt (VPS) in NPH.Methodology: Atotal of 24 patients were included in the study where 12 patients underwent ETV and 12 patients VPS. Patients having a definite cause of hydrocephalus and multipleco-morbid diseases were excluded from the study. Various Scoring systems were studied to see the efficiency of ETV. Results:About 60% of patients were successfully recovered. 40% needed to revision surgery either VP shunt or LP shunt. Some scales showed higher effieciency of ETV.Conclusion: ETV in NPH is a safe and effective procedure. Whether the CSF absorption is hampered at arachnoid granulation is doubtful for failure cases. ETV is a short procedure with a good success rate in selected patients

Objective: In this study our main aim is to evaluate impact ofSerum Total IgE and Disease Severity in Patients with AllergicAsthma.Method: This cross-sectional study was conducted at NIDCH,Mohakhali, Dhaka from one year (July 2012 - June2013).Among 150 asthma patients, 50 mild asthma patients, 50moderate and 50 severe asthma were selected in this study.Result: In our study among 150 patients, patients with midasthma belong to 15-19 years age group, where as patientswith moderate asthma and severe asthma belong to 25-29years and 35-39 years age group.21% patients with mildasthma unusually visit for regular checkup to doctor whereas48% patients with moderate asthma unusually visit regularly todoctor.47% patients with severe asthma level had >400 IU/mLserum level where as 19% patients with moderate asthma had>400 IU/mL serum level.Conclusion: From our study we can conclude that, serum totalIgE levels in adult patients with persistent allergic asthma fromBangladesh were high (two-thirds with levels >150 IU/mL) andvaried significantly. Further study is highly appreciated forbetter outcome.

Arachnoid cysts are present in 1% of the populace and generally found in the temporal, frontal, pineal and posterior fossa, frontotemporal fossa. Clinical and radiologic introductions can vary incredibly. In spite of serious research, it is as yet easily proven wrong which patients will profit by medical procedure. Objective: This study intends to research the pre-treatment parameters impacting the result after neuro-endoscopic treatment of arachnoid cysts. Methods: A review investigation of 14 patients who experienced an endoscopic fenestration of arachnoid cysts between 2012 to 2017. Results: In symptomatic patients, 85.71 percent of cases have improved clinically. The best results in treating symptoms related to intracranial hypertension, acute neurological defects and macrocrania and seizure were found. Conclusion: The clinical outcome and complication rate vary by technique and symptomatology, although the outcome after the endoscopic cysto cysternostomy is satisfactory.

Background: Surgical outcome in patients with cervicalspondylotic myelopathy may affects by variety of factors.Different study outcome suggest that the number of symptomsand involved levels, symptom duration and posterior approachsignificantly increased with increasing age, whereaspreoperative Japanese Orthopedic Association scoredecreased among the different age groups.Objective: The goal of this study is to analyze the surgicaloutcome by comparing younger and elderly patient groups onthe basis of preoperative radiological and clinical data.Methods: To find out the surgical outcome the clinical andradiological data of 32 patients who underwent expansivelaminoplasty were reviewed after their surgery had beenperformed. All of the Patients were divided into two groups,group (a): younger patient group (<65 yr of age; n = 13) andan elderly patient group (≥65 yr of age; n = 19). Patients wereassessed by use of the Japanese Orthopaedic Associationscale (JOA) to know the neurological status of them. Computedtomographic myelography and magnetic resonance imagingexamined for radiological feature. Finally, the effects of theclinical and radio-logical findings on neurological outcome wereinvestigated.Results: The scores of preoperative and postoperative meanof (JOA) in elderly patients were significantly lower thanyounger patients. In the elderly patients cases, the transversearea of the spinal cord at the level of maximum compressionand symptom duration were the factors that predicted anexcellent recovery on the other hand the transverse area wasthe only predictor of excellent recovery in younger patients.Intensity change on the spinal cord and age, preoperativeJapanese Orthopaedic Association score, canal diameter werenot predictive in either age range.Conclusion: Both younger and elderly patient groups thetransverse area of the spinal cord may be a reliable predictor ofexcellent recovery. In case of elderly patients shorter symptomduration was an important factor in the excellent recovery.
