RESUMO

ObjectiveTo observe the recovery of proprioception of the affected ankle over time after lateral ankle sprain accepting routine rehabilitation. MethodsFrom June, 2020 to June, 2022, 18 patients with lateral ankle sprain in Kunshan Rehabilitation Hospital underwent routine rehabilitation for twelve weeks. They were measured active and passive position sense of bilateral ankles using an isokinetic dynamometer before treatment, and four, eight and twelve weeks after treatment, respectively. ResultsThe active presentation difference of affected ankle reduced after treatment (F = 22.533, P < 0.001), but it was more than that of the healthy ankle at the same time (t > 4.419, P < 0.001). No significant improvement was found in passive presentation difference of affected ankle after treatment (F = 1.175, P > 0.05), and it was not significantly different from those of the healthy ankle at the same time (|t| < 0.646, P > 0.05). ConclusionProprioception of affected ankle has been impaired after lateral ankle sprain, and it can be recovered after rehabilitation, but cannot achieve the healthy level even after three months of training. Passive position sense as an index of proprioception needs more researches.

RESUMO

Bone defects caused by trauma, infection, tumor and other factors is a thorny problem in orthopedic clinic, and promoting bone repair and regeneration is the key and difficult point of treatment. In addition to autologous bone grafting, artificial bone materials are often used for large bone defects. β-tricalcium phosphate has good biocompatibility, bone conduction and bone induction properties, and has been studied deeply because of its excellent drug delivery performance and has shown broad application prospects. In this paper, the author will summarize the research progress of β-tricalcium phosphate composites loaded with different drugs in the treatment of bone defects caused by trauma, infection and tumor.

Objective To study the pharmacokinetic differences of pure salidroside and Kunfukang pills in rats and explore the pharmacokinetic changes of salidroside as a pure form or in the mixture. Methods The rats were divided into two groups. One group received pure salidroside and the other was administrated with kunfukang pills. Concentrations of salidroside in both groups were determined by LC/MS/MS method. The main pharmacokinetic parameters were calculated. Results AUC0−t of salidroside was (587.11±35.02) in pure salidroside group and (956.35±47.65)ng·h/ml in Kunfukang group. cmax was (85.81±15.66) and (143.86±46.91)ng/ml separately, with significant difference. Conclusion The results indicated that other components in kunfukang pills had influence on the absorption, distribution and elimination of salidroside.

Objective:To study the mechanism of protective effect of erythropoietin (EPO) on glucocorticoid-induced osteoporosis in rats through promoting vascular action mediated by VEGF, promoting osteogenesis, and inhibiting bone resorption.Methods:An animal experiment of 24 SD rats in total were divided into three groups: the osteoporosis group (20 mg/kg BW of methylprednisolone biwim for 6 weeks); the EPO group (MP 20 mg/kg biw+EPO 500 u/kg qdim); the nature salt group (0.9%NS). After 12 weeks, rats were harvested and received examination of histology (HE staining) for demonstration of protective effect, immunohistochemistry with CD31 stainingfor microvascular changes ,changes of VEGF and PCNA expressions using Western Boltfor microvascular and cell proliferation, and ELISA to detectOPN, PINP, CTX-1 in serumasbone turnover marker.Results:Hematoxylin and eosin staining in the model group showed that the bony trabeculae had become obviously narrow and sparse with discontinuity of the integrity. The integrity of the trabeculae was better in the EPO group. Immunohistochemical results: the EPO group CD31+ cell 16.60±4.88, the osteoporosis group 12.96±4.54, the NS group 25.84±7.97. CD31 expression was higher in the EPO group than the osteoporosis group. Western Bolt: in the NS groupthe ratio of VEGF/β-actin greyscalewas 0.570±0.022, with the osteoporosis group 0.446±0.083 and the EPO group 0.584±0.009; The ratio of PCNA/β-actingreyscale was 0.541±0.158 in the NS group, withthe osteoporosis group 0.187±0.099, the EPOgroup 0.733±0.257. VEGF and PCNA expression in the EPO group were higher than those in the osteoporosis group. ELISA: OPN results: the NS group 78.34±17.28 pg/ml, the osteoporosis group 368.48±97.23 pg/ml, the EPO group 217.62±39.11 pg/ml; P1NP results: the NS group 1 507.00±58.49 ng/ml, the osteoporosis group 1 196.00±91.32 ng/ml, the EPO group 1 621.00±65.57 ng/ml; CTX-1 results: the NS group 27.10±4.78 ng/ml, the osteoporosis group 39.46±9.23 ng/ml, the EPO group 31.17±4.11 ng/ml. The level of OPN and CTX-1 in the EPO group were lower than that in the osteoporosis group, and P1NP was higher than that in the osteoporosis group.Conclusion:EPO generates certain protective effect on bone of rats withglucocorticoid-induced osteoporosis. Its potential mechanism is to promote vascular action, promote osteogenesis, and inhibit bone resorption.

Objective To explore the association of serum cytokine levels with disease activity in patients with dermatomyositis (DM) and clinically amyopathic dermatomyositis (CADM),especially their association with skin lesions and interstitial lung disease (ILD).Methods Enzyme-linked immunosorbent assay (ELISA) and cytometric beads array (CBA)were performed to detect the serum levels of interleukin (IL)-2,IL-4,IL-6,IL-10,IL-17A,IL-18,tumor necrosis factor (TNF) and interferon (IFN)-γin 40 patients with DM or CADM,as well as in 16 health checkup examinees (healthy control group).Then,the association of serum cytokine levels with skin lesions,inflammatory biomarkers and severity of ILD was analyzed.Results The patients with DM/CADM showed significantly higher serum levels of IL-6 (37.8 ±45.8 pg/ml),IL-10 (16.1 ± 7.2 pg/ml) and IL-18 (492.0 ± 193.1 pg/ml) compared with the healthy controls (12.0 ± 2.7 pg/ml,7.7 ± 1.4 pg/ml,191.1 ± 39.2 pg/ml,respectively,all P < 0.001),and there were no significant differences in the serum levels of the other 5 cytokines between the above 2 groups.The serum level of IL-6 was significantly higher in patients with elevated erythrocyte sedimentation rate (ESR) than in those with normal ESR (49.7 ± 46.8 pg/ml vs.29.1 ± 45.4 pg/ml,P =0.008).The patients with raised C-reactive protein (CRP) levels showed significantly higher serum levels of IL-6 (68.7 ± 59.7 pg/ml) and IL-18 (635.1 ± 232.8 pg/ml) compared with those with normal CRP levels (IL-6:30.6 ± 40.3 pg/ml,P =0.013;IL-18:440.2 ± 164.7 pg/ml,P =0.020).Moreover,the patients with elevated levels of lactate dehydrogenase (LDH) showed significantly higher serum levels of IL-10 (18.4 ± 6.9 pg/ml),IL-17A (19.6 ±6.7 pg/ml) and IL-18 (529.4 ± 197.2 pg/ml) compared with those with normal LDH levels (IL-10:10.7 ±4.8 pg/ml,P < 0.001;IL-17A:11.4 ± 6.6 pg/ml,P =0.001;IL-18:404.9 ± 158.0 pg/ml,P =0.037).No significant difference in the cytokine levels was observed between the patients with elevated creatine kinase (CK) levels and those with normal CK levels.The patients with Gottron's papules/sign showed significantly higher serum levels of IL-18 (513.7 ± 187.2 pg/ml) compared with those without Gottron's papules/sign (297.1 ± 140.4 pg/ml,P < 0.05).The serum levels of IL-10 and IL-18 were significantly higher in the patients with DM/CADM complicated by ILD (18.0 ± 6.7 pg/ml,552.3 ± 192.8 pg/ml,respectively) than in those without ILD (11.6 ± 6.5 pg/ml,351.4 ± 101.0 pg/ml,respectively,both P =0.001).Conclusion Serum levels of IL-6,IL-10 and IL-18 are highly associated with inflammatory biomarkers,skin lesions and ILD in patients with DM/CADM.

RESUMO

Objective To evaluate the outcome of the surgical-treated ankle fractures. Methods We reviewed the outcomes of 92 cases of ankle fracture which had complete follow-up data from January 2000 to April 2008. The outcomes were assessed by AOFAS (American Orthopaedic Foot & Ankle Society) scoring scale, VAS scale, satisfaction degree of patients and range of ankle motion. The differences in AOFAS scoring and ankle motive range between involved and contralateral extremity, AO fracture types, involved positions (solemaleolar, bimaleolar, trimaleolar), follow-up timing were statistically analyzed. Results Follow-ups averaged 55 months. All cases had fracture healed. The average AOFAS score was 93.6 points. The excellent to good rate was 100%. VAS score was 0.17 points on average. Satisfaction score was 9.62 points on average. Statistical analysis showed that the motive range of ankle joint between involved and con-tralateral extremity had a significant difference, but there were no significant differences in AOFAS scores and range of ankle dorsiflexion-plantarflexion between AO fracture types, involved positions and different follow-up durations, Conclusion Surgical treatment for ankle fractures can have a good clinical result.
