RESUMO

Objective:To explore the application value of biomaterial breast model combined with traditional clinical teaching methods in the training of minimally invasive breast surgery. Methods:A total of 50 physicians who studied at the Thyroid and Breast Surgery Department of The First Affiliated Hospital of Air Force Medical University from March to December 2021 were selected as the research subjects and divided into the experimental group and the control group. The training time was one week, with three modules, namely, the study of Doppler ultrasound system and ultrasound theory, the study of the operating system for minimally invasive breast surgery, and the clinical observation and practice of minimally invasive breast surgery. Additionally, physicians in the experimental group practiced the surgical skills by using the biological material breast model. After the training, the assessment of breast minimally invasive surgery was performed, and the differences of minimally invasive operation and subjective evaluation of the two groups of physicians were analyzed. SPSS 22.0 was used to conduct independent-samples t-test and Chi-square test. Results:The time spent on breast mass localization, rotary cutter localization and residual rates of breast mass in the experimental group were (4.12±1.05) min, (12.12±2.61) min and 40%, respectively, and the corresponding results of the control group were (6.68±1.97) min, (15.32±2.89) min and 72%, which showed statistically significant differences between the two groups ( P<0.05). At the same time, the physicians in the experimental group were better than the control group in terms of self-evaluation of operational proficiency and clinical operation confidence. Conclusion:Compared with the traditional teaching method, the biomaterially simulated breast model combined with traditional teaching has significant advantages. It is an efficient teaching aid in the training and teaching of breast minimally invasive surgery, and plays an important role in the training of breast surgeons.

RESUMO

Objective:To investigate the expression of USP41 in triple-negative breast cancer (TNBC) and its correlation with malignant phenotype and adriamycin sensitivity. Methods:The expression of USP41 in TNBC resistant cell lines and clinical tissue samples was detected by Western blot and qPCR. Subsequently, the high expression of USP41 molecule was determined, and the role and possible mechanism of USP41 in the malignant phenotype and adriamycin resistance of TNBC were evaluated by cell biological methods such as CCK8, colony formation assay, transwell, Western blot, and CoIP-MS.Results:USP41 expression was significantly higher in triple-negative breast cancer samples than in adjacent non-cancerous tissues. USP41 expression was nearly 40-fold higher in the doxorubicin-resistant cell line MDA-MB-231/DXR, with an IC50 value of 6.86 μM. Interference with USP41 significantly increased the sensitivity of MDA-MB-231/DXR cells to doxorubicin. Interference with USP41 significantly inhibited cell proliferation, colony formation and migration of cells, with a decrease in the number of clones of 30%-80% and a decrease in the number of migrating cells of more than 70%, and the difference was statistically significant. In addition, USP41 knockdown improved the sensitivity of MDA-MB-231 cells to doxorubicin, with an IC50 decrease from 5.49 μM to 2.36 μM and 2.56 μM. CO-IP results showed that USP41 could directly interact with RACK1, and the expression of RACK1 was significantly higher in cancer tissues than in adjacent non-cancerous tissues. Interference with RACK1 inhibited MDA-MB-231 cell proliferation, with IC50 decreasing from 9.87 μM to 4.67 μM and 4.36 μM. Colony formation capacity decreased by more than 30% and the difference was statistically significant. USP41 knockdown decreased cell migration by more than 70% compared to control. Conclusion:High expression of USP41 is associated with malignant surface and adriamycin resistance in TNBC, and RACK1 may be a key molecule in the role of USP41.

RESUMO

The classical prescription Kaixinsan, which is recorded in an ancient medical book named Beiji Qianjin Yaofang,is one of the famous prescriptions used by ancient physicians to treat amnesia. Research on classical prescriptions has attracted more and more attention from scientific research institutions and related enterprises. Based on ancient books, textual research on origins and development of prescriptions, combing the evolution of prescriptions, preparations, oral ways, taboos and others are the important contents of the study on classical prescriptions. The research results show that the creation of Kaixinsan in Beiji Qianjin Yaofang can be traced back to Kaixinsan recorded in Jiyanfang and Dingzhiwan recorded in Gujinluyanfang. Later generations of physicians created many associated prescriptions in the process of applying Kaixinsan, and the efficacy of these prescriptions was constantly expanded with the development of the times. In the Tang and Song dynasties,Kaixinsan and its associated prescriptions were mainly used to treat amnesia, sorrow, fear, and other diseases. In the Jin and Yuan dynasties, these prescriptions were also used to treat convulsions and yawning. In the Ming dynasty, they were mainly for the treatment of hyperopia, myopia, sprematorrhea, and constipation. In the Qing dynasty, these herbs could be used to treat auricular deafness, aging and sweating. The dosage of Ginseng Radix et Rhizoma and Poria should be increased in the treatment of farsightedness, spermatorrhea and blurred urine, and in the treatment of nearsightedness, the dosage of Polygalae Radix and Haliotidis Concha should be increased. The main pathogenesis of the disease that Kaixinsan and its associated prescriptions treated could be summarized as the deficiency of heart and spleen, imbalance between heart-Yang and kidney-Yin,and the internal resistance of phlegm stagnation. By summarizing the contents of the preparation of tradition Chinese medicine products for Kaixinsan and its associated prescriptions,it is suggested that the dosage form of Kaixinsan can be pills, with the specification size confroming to the most record of ancient generations of physicians,as big as Firmiana platanifolia's fruit. The volume of a single pill is about 0.25 mL and the weight is about 0.3 g. The initial dosage is fifteen pills, which can be modified according to the severity of the illness, with no more than forty pills for each time, three times a day. Also, some excitant food like the sour food, sweet food and mutton should be avoided during the medication. The above research results can provide literature basis for the development of compound tradition Chinese Medicine preparation of Kaixinsan.

RESUMO

Kaixinsan recorded in an ancient medical book named Beiji Qianjin Yaofang is one of the famous classical formula, which is one of the common prescriptions used by ancient physicians to treat amnesia. In the process of using this prescription, later generations of physicians derived many kinds of associated prescriptions. The effect and indications of these prescription have been inherited and expanded from those in the Beiji Qianjin Yaofang because of the changes in dosages. Therefore,it is necessary to verify the effect and indications of the formulas and the rules of dosage changes. The research results showed that its basic effects included to nourish the mind, induce resuscitation, strengthen the spleen and calm the mind, and keep balance between heart-Yang and kidney-Yin. The main indications included amnesia, sorrow and sadness, fright and fear, and so on,which may differ slightly in different dynasties. In Song,Jin and Yuan dynasties,it also demonstrated the effect of warming the heart and Yang, clearing away heat and relieving wind besides the basic effects, with basically the same indications (slightly different from those in previous dynasties). In Ming dynasty,it demonstrated the effect of nourishing Yin,clearing away heat and nourishing blood besides the basic effects, and the indications expanded to farsightedness, nearsightedness, spermatorrhea and blurred urine. In the Qing dynasty, its effect also included to nourish the heart and kidney on the basis of the Ming dynasty, and the indications were basically the same with those in the previous dynasties. The compatibility ratio of ancient physicians in the application of this prescription and its associated prescriptions showed some remarkable features, for example,Ginseng Radix et Rhizoma and Poria should be increased and their ratio was≈1∶1 in the treatment of amnesia, sorrow, sadness, fright, fear, farsightedness, spermatorrhea and blurred urine, with ratio of Polygalae Radix to Acori Tatarinowii Rhizoma≈1∶1; the dosage of Polygalae Radix and Acori Tatarinowii Rhizoma should be increased in the treatment of nearsightedness, and their ratio was≈1∶1. The compatibility ratio of Polygalae Radix-Ginseng Radix et Rhizoma-Poria-Acori Tatarinowii Rhizoma=2∶3∶3∶2 was the most frequent, which basically included the indications of this prescription and its associated prescriptions. According to statistics, the average dosages that ancient physicians used were significantly higher than those in the modern times,Polygalae Radix 57 g,Ginseng Radix et Rhizoma 62 g,Poria 70 g,and Acori Tatarinowii Rhizoma 54 g,respectively in ancient times, while Polygalae Radix 11 g,Ginseng Radix et Rhizoma 15 g,Poria 17 g,and Acori Tatarinowii Rhizoma 9.5 g,respectively in modern times. The above textual research results can provide some reference for preparation of tradition Chinese medicine products of Kaixinsan.

RESUMO

Drug use during pregnancy is unavoidable. Therefore, it is vitally important for medical workers to help pregnant women take drugs correctly to reduce the incidence of spontaneous abortion, premature birth, and low birth weight. In our study, drug screening model with induced pluripotent stem cells (iPSCs) was used to find some improper drugs which will result in woman's abortion. With 3D culture in vitro, iPSCs can form embryoid bodies (EBs) and cerebral organoids, which simulated in vitro development of early embryos, from inner cell mass to germ-layer differentiation. In the experiment, EBs were exposed to mifepristone (RU486), and three experimental groups were divided randomly. They were control group (without RU486), low-dose group (L-RU486, 10 μg·mL-1), and high-dose group (H-RU486, 20 μg·mL-1). After mifepristone exposure, EBs were observed at days 5, 8, and 11, including size of EB, cell apoptosis, and differentiation of germ layers, by using inverted optical microscope, TUNEL assay, and immunofluorescent staining. The results showed that through 3D culture, iPSCs could develop into embryoid bodies, neural rosettes, and finally cerebral organoids. After mifepristone exposure, EBs' sizes were decreased (P < 0.01); the levels of cell apoptosis in EBs were increased after mifepristone exposure (P < 0.01); the development of EBs' germ layer was affected. Mifepristone exposure could inhibit the proliferation of embryonic stem cells, reduce the differentiation of ectoderm (P < 0.01) and promote the development of mesoderm (P < 0.05). In conclusion, iPSCs can be used as a screening model for abortion drug, and EBs’ diameter, cell apoptosis, and differentiation changes of the germ layers can serve as criteria of abortion drug screening.

RESUMO

Spermatogonial stem cells (SSCs) have great applications in both reproductive and regenerative medicine. Primates including monkeys are very similar to humans with regard to physiology and pathology. Nevertheless, little is known about the isolation, the characteristics, and the culture of primate SSCs. This study was designed to identify, isolate, and culture monkey SSCs. Immunocytochemistry was used to identify markers for monkey SSCs. Glial cell line-derived neurotrophic factor family receptor alpha-1 (GFRA1)-enriched spermatogonia were isolated from monkeys, namely Macaca fascicularis (M. fascicularis), by two-step enzymatic digestion and magnetic-activated cell sorting, and they were cultured on precoated plates in the conditioned medium. Reverse transcription-polymerase chain reaction (RT-PCR), immunocytochemistry, and RNA sequencing were used to compare phenotype and transcriptomes in GFRA1-enriched spermatogonia between 0 day and 14 days of culture, and xenotransplantation was performed to evaluate the function of GFRA1-enriched spermatogonia. SSCs shared some phenotypes with rodent and human SSCs. GFRA1-enriched spermatogonia with high purity and viability were isolated from M. fascicularis testes. The freshly isolated cells expressed numerous markers for rodent SSCs, and they were cultured for 14 days. The expression of numerous SSC markers was maintained during the cultivation of GFRA1-enriched spermatogonia. RNA sequencing reflected a 97.3% similarity in global gene profiles between 0 day and 14 days of culture. The xenotransplantation assay indicated that the GFRA1-enriched spermatogonia formed colonies and proliferated in vivo in the recipient c-Kit

RESUMO

IκB kinase beta (IKKbeta) is a key kinase in nuclear factor kappa B (NF-kappaB) signaling pathway. After spinal cord injury, IKKbeta is activated, and the signal pathway of NF-kappa B is abnormally activated, which produces a large number of inflammatory factors, which has a negative impact on the recovery of spinal cord injury. This article mainly summarized the structure and function of IKKbeta and its application in inflammatory reaction after spinal cord injury, trying to find a new target for the treatment of spinal cord injury.

RESUMO

While hallmarks of rodent spermatogonia stem cell biomarkers' heterogeneity have recently been identified, their stage and subset distributions remain unclear. Furthermore, it is currently difficult to accurately identify subset-specific SSC marker distributions due to the poor nuclear morphological characteristics associated with fixation in 4% paraformaldehyde. In the present study, testicular cross-sections and whole-mount samples were Bouin fixed to optimize nuclear resolution and visualized by immunohistochemistry (IHC) and immunofluorescence (IF). The results identified an expression pattern of PLZFhighc-KITpos in A1 spermatogonia, while A2-A4-differentiating spermatogonia were PLZFlowc-KITpos. Additionally, this procedure was used to examine asymmetrically expressing GFRA1 and PLZF clones, asymmetric Apr and false clones were distinguished based on the presence or absence of TEX14, a molecular maker of intercellular bridges, despite having identical nuclear morphology and intercellular distances that were <25 μm. In conclusion, this optimized Bouin fixation procedure facilitates the accurate identification of spermatogonium subsets based on their molecular profiles and is capable of distinguishing asymmetric and false clones. Therefore, the findings presented herein will facilitate further morphological and functional analysis studies and provide further insight into spermatogonium subtypes.

RESUMO

Objective To study the airway patency during sitting position and supine position in bronchoscopy during anesthesia. Method Forty-eight patients underwent bronchoscopy during Non-intubation anesthesia were randomly allocated to two groups. There were 24 cases in each group by observeing the airway patency and vital signs of the two groups during the procedure. Result Sitting position in bronchoscopy during anesthesia have the shorter average time to reach the carina, on securing a clear airway. The incidence of desaturation, hypertension and agitation were lower in sitting position during bronchoscopy. Conclusion Bronchoscopy anesthesia can reduce discomfort and fear, but traditional supine bronchoscopy anesthesia can cause displacement of the tongue, collapse of the upper airway in the oropharynx, soft palate and vocal cord related to gravity may seriously obstruct the upper airway, not only brings inconvenience to the inspection, but also increases the risk of airway correlation and hemodynamic instability, Sitting bronchoscopy ensure the airway patency and maintain a more stable hemodynamic state, so that sitting position in the sedation of bronchoscopy anesthesia is safer and more convenient.

RESUMO

Objective To detect the expression of Survivin and Caspase-9 gene in Uygur breast cancer pa-tients with different HER-2 phenotypes,to find out the difference and association of the two genes and to find out the potential roles of the two genes breast cancer pathogenesis. Methods We selected 72 Uygur patients diag-nosed as breast cancer initially and they were divided into group A with HER-2 positive(n = 39)and group B with HER-2 negative(n = 33). Another 40 Uygur patients with benign breast were involved as the controls. Immunohis-tochemistry and real-time RT-PCR were used to detect the two genes, and analyze the differences and association of each gene between the groups. Results (1)The expression of Survivin gene in group A and B were higher than that in the control group. Further analysis showed that the expression of Survivin gene was enhanced in group A when compared with that in group B(P < 0.05);while even the expression of Survivin gene in group B was higher than that in the control group but no statistical difference was found(P > 0.05).(2)The expression of Caspase-9 gene in group A and B were lower than that in the control group. Real-time RT-PCR showed that the expression of Caspase-9 gene of group A was decreased when compared with that in group B(P < 0.05);While the expression of Caspase-9 gene of group B was slightly lower that of the control group but it showed no statistical significance (P > 0.05). Immunohistochemical results showed there were no statistical differences of expression of Caspase- 9 gene in group A and B and control group(all P > 0.05). The expression of Survivin and Caspase-9 gene was nega-tively associated in group A and B(P < 0.01;P < 0.05). Conclusions In Uygur patients with HER-2 positive breast cancer, the expression of Survivin gene is enhanced but that of caspase-9 gene is decreased, and they are negatively associated. Through inhabiting caspase-9 gene,Survivin gene may potentially lead to the occurrence of HER-2 positive breast cancer.

Objective To explore the curative effect of oral cancer radical surgery plus free anterolateral thigh flap repair and analyze the influence factors of skin flap necrosis. Methods The data of 98 patients who underwent oral cancer radical surgery with concurrent free anterolateral thigh flap repair in our hospital from January 2010 to June 2015 were retrospectively analyzed. The complete survival rate of skin flaps and incidence of complications in all patients were statistically analyzed. The age, diabetes, infection in implanted skin flap area, smoking and mental status, etc.of patients with survived or necrotic skin flaps were compared between patients with survival flaps and patients with flap necrosis, and multivariate Logistic regression analysis was performed. Results Among the 98 cases,86 cases of flaps survived completely and the survival rate was 87.8%.Fifteen cases with the skin flap necrosis, the incidence of complications was 15.3%,including 7 cases of surgi-cal site infection and 5 cases of skin flap necrosis with massive hemorrhage. The donor site incisions of all patients were primary healing and the patients were satisfied with the postoperative appearance.Among the patients with flap necrosis, the incidence rates of ≥60 years old, with diabetes, infection in flap implanted area, smoking and poor mental state (41.7%,41.7%,58.3%,75.0%,83.3%)were higher than those corresponding proportions in the patients with with survival flaps(P <0.05).Multivariate Logistic regression analysis showed that age,diabe-tes,infection in implanted skin flap area, smoking and poor mental status were the influencing factors for flap necrosis. Conclusion Oral cancer radical surgery with concurrent free anterolateral thigh flap repair has good curative effects. The age, combined with diabetes,smoking,infec-tion in flap implanted area and poor mental state are the risk factors for flap necrosis in patients with oral cancer after flap repair, which should be given intervention to improve the survival rate of skin flap in clinic.

RESUMO

Objective:To analyze and discuss the influence of medical ethics on the quality of life and treatment compliance for breast cancer patients in clinical application, and to expound the positivehidden value of medical ethics to dealing with the doctor-patient relationship. Methods:176 cases of breast cancer patients in a hospital from December, 2012 to December, 2014 were selected to participate in this study. They were divided into experi-mental group (n=88) and control group (n=88) using the sequence number. The subjects in control group were treated according to the NCCN guidelines for breast cancer, while the experimental group was treated with medical ethics on the basis of the routine methods. The effect and safety were explored. Result:In evaluation of the quality of life, it was found that the excellent rate of patients in the experimental group was significantly higher than those in the control group (89. 8% vs. 68. 2%, P<0. 05). The incidence of complications in the study group was sig-nificantly lower than the control group ( P<0 . 05 ) . The complication incidence in the experimental group was sig-nificantly lower than that of the control group (73. 9% vs. 96. 6%,P<0. 05). Conclusion:Medical ethics has a great influence on the quality of life and treatment compliance for breast cancer patients, and its application has a positive indirect and clinical utility.

RESUMO

The aim of this study was to investigate the relationship between the acetylcholine concentration in the blood and gelsenicine-induced death in mice. Kunming mice were given intraperitoneal injections of normal saline, gelsenicine or different doses of acetylcholine chloride. Atropine was given to the mice which received gelsenicine or medium dose acetylcholine chloride injection. The blood was sampled immediately when the mice died or survived for 20 min after injection. The acetylcholine concentration and acetylcholinesterase activity in the blood were measured by the testing kits, and the mortality was calculated and analyzed. The results showed that half lethal dose of gelsenicine (0.15 mg/kg) reduced the acetylcholinesterase activity and increased the blood acetylcholine concentration. The blood acetylcholine concentration of the dead mice in the gelsenicine group was increased to 43.0 μg/mL (from 31.1 μg/mL in the control), which was lower than that (53.9 μg/mL) of the dead mice in the medium dose acetylcholine chloride group, but almost equal to that (42.7 μg/mL) of the survival mice in the medium dose acetylcholine chloride group. Atropine could successfully rescue the mice from acetylcholine poisoning, but its efficiency of rescuing the mice from gelsenicine intoxication was weak. These results suggest that gelsenicine can inhibit acetylcholinesterase activity and increase blood acetylcholine concentration, but the accumulation of acetylcholine may not be the only or main cause of the death induced by gelsenicine in mice.
