RESUMO

OBJECTIVE To clarify the core content of traditional Chinese medicine (TCM) policy in the provinces of China, so as to provide reference for optimizing the structure of the policy system of traditional Chinese medicine in China and assisting the inheritance and innovation of TCM industry in various regions. METHODS The websites of directly affiliated organs in 31 provinces, excluding Hong Kong, Macao and Taiwan, were retrieved to collect the TCM policies released from 2000 to 2021. The importance of keywords in the TCM policies of each province was measured based on term frequency-inverse documentation frequency (TF-IDF) keyword extraction method, and the similarities and differences were analyzed among TCM policies. RESULTS & CONCLUSIONS A total of 99 documents related to TCM policies of various provinces were obtained in this study, most of which were released after 2016. The theme of national TCM policy covered four aspects: building TCM talent team, perfecting TCM service system, strengthening TCM resource management and promoting TCM industry innovation. The TF-IDF values of “medical institutions”“traditional Chinese medicine”“medical treatment” were higher than other keywords in each province, indicating that the provinces paid more attention to the construction of TCM service system and the management of TCM resources than other aspects. Anhui and Jiangsu, Beijing and Henan, Hubei and Jilin, Hubei and Tianjin, and Hubei and Yunnan had the more degree of similarity in TCM policies, which all contained 16 of the same keywords. Therefore, the above regions should be encouraged to strengthen exchanges and cooperation and realize mutual promotion and joint development. Among all the keywords whose importance ratio was greater than 0.2,“ Tibetan medicine” was unique to Qinghai and Tibet,“ disease type” was unique to Guangdong, and the TF-IDF value of “supervision and management” in Beijing was higher, indicating that the emphasis of TCM policy formulation in different provinces was various. Meanwhile, the top 10 keywords of TF- IDF value in all provinces did not have words related to financial input, and the TF-IDF values of “informatization” in most provinces did not rank at the top. It is suggested to increase financial input or encourage social financing, and add “Internet+new business” in the field of TCM.

RESUMO

Objective:To discuss the development and management of full-time research assistants in China′s large hospitals. Methods:Through the methods of policy sorting, literature research, summarization, and classification, this study focused on the policy background, development opportunities, and the needs of the times for the position establishment of research assistants in China′s large hospitals, comprehensively classified the practical exploration and existing problems of employing and managing full-time research assistants in typical large hospitals in the country, and discussed the relevant management strategies learned from international experience. Results:In addition to hospitals directly affiliated with colleges and universities, dozens of large local hospitals in more than ten provinces have carried out beneficial practices of establishing the employment, management, and assessment systems for research assistants positions and achieved some progress. However, there are still common problems such as limited cognition of ″temporary positions″, insufficient coverage of the positions, low salary and welfare, and lack of training and development mechanisms as well as promotion and career development paths. Conclusions:The development and management of research assistants in large hospitals have gradually shifted their purpose from alleviating the employment issue of the graduates to reserving talented people in medical teaching, researching and managing, forming a high-quality, professional, and full-time medical scientific team, and continuously innovating. This article puts forward some suggestions about establishing the management system of research assistants, strengthening the standardized training for talented people, and constructing a diversified incentive mechanism combining performance appraisal and target management for research assistants.

RESUMO

Diabetic kidney disease (DKD) is one of the serious complications of diabetes mellitus, and it has become the leading cause of chronic renal failure in China. Podocytes are highly differentiated epithelial cells and are the important part of the glomerular filtration barrier. Apoptosis and shedding of podocytes, foot process fusion and decreased expression of slit membrane proteins can lead to proteinuria, which in turn affects the progression of DKD. Autophagy is an important process for eukaryotic cells to degrade cytoplasmic proteins and organelles, the increase of autophagy level helps to reduce podocytes damage. Endoplasmic reticulum stress (ERS) is the accumulation of misfolded proteins in cells. It allows the cells into stress state, and may be able to regulate cell damage in both directions. Autophagy and ERS are regulated by multiple signaling pathways and are considered to be closely related to the occurrence and development of DKD. This article explained some pathways and the role of podocyte autophagy and ERS in DKD, and the interaction between podocyte autophagy and ERS, which providing some potential targets for the treatment of DKD by interfering with podocyte autophagy and ERS.

RESUMO

The eukaryotic genome is folded into higher-order conformation accompanied with constrained dynamics for coordinated genome functions. However, the molecular machinery underlying these hierarchically organized three-dimensional (3D) chromatin architecture and dynamics remains poorly understood. Here by combining imaging and sequencing, we studied the role of lamin B1 in chromatin architecture and dynamics. We found that lamin B1 depletion leads to detachment of lamina-associated domains (LADs) from the nuclear periphery accompanied with global chromatin redistribution and decompaction. Consequently, the inter-chromosomal as well as inter-compartment interactions are increased, but the structure of topologically associating domains (TADs) is not affected. Using live-cell genomic loci tracking, we further proved that depletion of lamin B1 leads to increased chromatin dynamics, owing to chromatin decompaction and redistribution toward nucleoplasm. Taken together, our data suggest that lamin B1 and chromatin interactions at the nuclear periphery promote LAD maintenance, chromatin compaction, genomic compartmentalization into chromosome territories and A/B compartments and confine chromatin dynamics, supporting their crucial roles in chromatin higher-order structure and chromatin dynamics.

RESUMO

Objective:To investigate the pathogenic distribution of benign infantile convulsions with mild gastroenteritis(BICE)and explore the relevance of serum cytokines and BICE.Methods:Eighty BICE infants admitted in Inner Mongolia Maternal and Child Health Hospital from January 2017 to December 2019 were selected as BICE group, and 80 mild gastroenteritis infants without convulsion attack were selected as control group during the same period.Fluorometric real-time PCR was used to detect the pathogen of enterovirus.Serum cytokines including interleukin(IL)-6, tumor necrosis factor(TNF)-α, IL-8 and soluble interleukin-2 receptor(SIL-2R)were detected by using chemiluminescence method. The relevance of frequency and duration of convulsion in infants with BICE and the indicators above were analyzed. Results:Rotavirus infection was the main cause of BICE during the study period in this region.Among 80 cases in the BICE group, rotavirus positive infants accounted for 38.8%(31/80) and norovirus positive infants accounted for 10.0%(8/80). The levels of IL-6, IL-8 and SIL-2R in the BICE group were prominently higher than those in the control group( P<0.05); the difference of TNF-α level between two groups had no statistical significance( P>0.05). The levels of IL-6, TNF -α, IL-8, SIL-2R in the group with convulsion attack times ≥ 2 and convulsion duration ≥5 min were higher than those in the group with convulsion attack times<2 and convulsion duration<5 min( P<0.05). The frequency and duration of convulsions were positively correlated with the levels of IL-6, TNF -α, IL-8 and SIL-2R( P<0.05). Conclusion:Rotavirus is the main pathogen of BICE in this region. There is immunologic imbalance in children with BICE, especially the changes of IL-6 and SIL-2R levels, which may provide effective cytological and experimental data evidence for judging progression and prognosis of the disease.

RESUMO

Objective To analyze the diversity of salivary bacteria in patients with nonalcoholic fatty liv-er disease(NAFLD group)and healthy control group(HC group). Methods Saliva samples were collected from 24 patients with nonalcoholic fatty liver disease and 22 healthy control.Genomic DNA of the samples was extracted and extended to 16S rRNA V4-V5 hypervariable region after PCR with Illumina high-throughput sequencing tech-nology.All the data, including operational taxonomic units, diversity indexes and species annotation, etc.was ana-lyzed. Results 39118 optimized sequences in the saliva of each group were obtained, including nonalcoholic fatty liver group clustering OTUs about 308 ± 48 and healthy control group clustering OTUs about 305 ± 53.The diversi-ty analysis of NAFLD group and HC group showed that there was little difference in diversity of the whole salivary bacteria. The data of the species analysis of the saliva bacteria in two group indicate that the composition was simi-lar and the relative abundance was different in phylum and genus. In NAFLD group, the ratio of the abundance of firmicutes(20.42%)increased and the proportion of bacteroides(36.75%)decreased in phylum. Genus of the NAFLD group, the ratio of fusobacterium, porphyromonas and veillonella increased significantly, and the ratio of prevotella and[Prevotella]decreased. Conclusion In the NAFLD group, the abundance of firmicutes increased, and the abundance of the bacteria decreased. It was speculated that the imbalance of the salivary bacteria is associ-ated with nonalcoholic fatty liver disease.

RESUMO

Objective: To investigate the inlfuence of angiotensin converting enzyme 2 (ACE2) on lectin-like oxidized low density lipoprotein receptor-1 (LOX-1) protein expression and to explore the protective effect of ACE2 on vascular endothelial cells. Methods: Our work includedin vitro andin vivo studies. For in vitro experiment, the human umbilical vein endothelial cell (HUVEC) were cultured and transfected with replication deficient recombinant adenovirus of ACE2 (Ad-ACE2), and LOX-1 protein expression stimulated by angiotensin 2 was examined by Western blot analysis. Forin vivo study, atherosclerosis plaques were induced in 20 apolipoprotein E-deifcient (ApoE-/-) mice, and then randomly divided them into 2 groups: ACE2 group, the mice received Ad-ACE2 (2.5×109 pfu/ml) injection through caudal vein, EGFP (enhanced green lfuorescent protein) group, the mice received equal replication deifcient recombinant adenovirus of EGFP (Ad-EGFP) injection through caudal vein.n=10 in each group. The animals were executed after 1 month treatment to collect abdominal aorta. Lipid content in atherosclerosis plaque was evaluated by Oil red O staining and LOX-1 protein expression was examined by immunohistochemistry and Western blot analysis. Results: Bothin vitro andin vivo experiments conifrmed that endothelial cell LOX-1 protein expression was signiifcantly inhibited by ACE2 transfection. The lipid content in ACE2 group was obviously lower than that in EGFP group byin vivo study. Conclusion: ACE2 may inhibit LOX-1 protein expression and therefore reduce the progress of atherosclerosis.

RESUMO

BACKGROUND: Many study results proved that electroacupuncture can cause change of endogenous opioid peptide system.OBJECTIVE: To observe the change of blood sugar and blood lipoprotein (a) in rats with diabetes mellitus after electroacupuncture therapy and probe into the effect of opioid peptide system in the treatment for diabetes with electroacupuncture.DESIGN: Randomized controlled trial with experimental animals as the subjects.SETTING: The experiment was conducted in the Department of Pathophysiology, Medical College of Shandong University.MATERIALS: The experiment was completed in the Department of Pathophysiology, Medical College of Shandong University from July 2004 to December 2004. Totally 140 male Wistar rats were involved.METHODS: Ten Wistar rats were collected randomly as the blank control group. The other 130 rats were given 20 g/L streptozotocin 50 mg/kg intraperitoneally and the rats in blank control group were given the same volume of saline intraperitoneally. Three weeks later, the tails were cut off and the blood sugar was measured 2 hours after food intake. The rats with obvious rapid sugar tolerance were given foods with high calories for 28 days as the model rats. There were 64 rats were successful models and they were classified into 3 groups randomly, namely model control group( n = 20), electroacupuncture therapy(n=22) and electroacupuncture with naloxone groups( n = 22) . The blood sugar and blood lipoprotein(a) were detected after treatment for the diabetic rats with streptozotocin.MAIN OUTCOME MEASURES: Blood sugar level and blood lipoprotein(a) of each rat after the final therapy.RESULTS: Totally 74 rats entered the result analysis. Electroacupuncture therapy could reduce the blood sugar and blood lipoprotein(a) of diabetes mellitus rats, while rats with naloxone injection before therapy were insensitive to electroacupuncture. [The blood sugar value were(13.91 ± 3.79),( 10.33 ± 2.36), (12.96 ± 2.23)mmol/L in the model control group, electroacupuncture therapy group, electroacupuncture with naloxone group respectively; while the level of blood lipoprotein(a) were(13.75 ±2.69),( 10.81 ± 2.96), (13.01 ± 3.23)mg/L respectively].CONCLUSION: Electroacupuncture therapy can reduce the level of blood sugar and the risk of atherosclerosis in rats with diabetes mellitus. Although the mechanism can not be explained through the meridian theory, according to the experimental result that blood sugar and blood lipoprotein(a) were no longer intervened by electroacupuncture therapy after injecting naloxone, it is presumed that electroacupuncture therapy can exert its intervention effect on blood sugar and blood lipoprotein(a)through opiate system.

RESUMO

Objective: To investigate the effects of different concentrations of vitamin E (VE) on angiogenesis and its possible mechanism. Methods: The rings of rat aorta were embedded in gels of collagen and cultured in a serum free medium for 28 days. Curves of microvessels growth were generated by counting the number of newly formed microvessels every day with an inverted microscope. Photos were taken at the same time. Expression of factor FVIII related antigen (FVIII RAg) in newly formed microvessels was detected by immunohistochemistry. Concentration change of FVIII R in medium was evaluated by ELISA. Results: Both 0.2 g/L and 0.1 g/L VE showed significant inhibitory effect on angiogenesis (P0.05). The inhibitory effect of VE was expressed in the phases of microvessel growth and decline (10-25 d). Conclusion: 1.Three dimensional collagen gels culture of microvessel in serum free medium can be used as a sensitive assay for study of soluble and solid phase angiogenic agonists and antagonists. 2. Both 0.1 g/L and 0.2 g/L VE inhibit angiogenesis, and VE has no toxicity and no drug resistance.
