RESUMO

A five years retrospective study of total 1314 cases of intrauterine contraceptive device receptors in Gynae outdoor department of postgraduate Medical Institute, Lady Reading Hospital, Peshawar is presented here. Among these 1314 cases 2 had Copper T insertion and 482 women had lippes loop. A total of 890 cases reported with minor as well as major complications. In six cases coil had perforated the uterine wall and entered in the peritoneal cavity. These cases are discussed in detail along with their management

RESUMO

During last twelve years more than 100 cases of Manchester repair have been performed with a success rate of 80%. But here a review of only 50 cases of Manchester repair is presented, performed during the year 1985-89, because these cases were followed regularly and thoroughly. The results of these cases are very encouraging specially regarding relief of symptoms, subsequent conception and parturition and very low recurrence rate. Manchester repair is not liked by many young Gynaecologists because of certain disadvantages and difficult technique. Therefore, it is not done so frequently now a days, as it was done previously, but I have found it very advantageous in selected cases

RESUMO

A review of total 9676 laparoscopic procedures performed during last five years i.e. from January 1985 to December 1989, is presented. This study includes both contraceptive surgical as well as diagnostic laparoscopic procedures. Out of total 9676 cases, we performed 6754 laparoscopic sterilization and 2922 diagnostic laparoscopies in the Department of Obstetrics and Gynaecology, Postgraduate Medical Institute, Lady Reading Hospital, Peshawar. Contraceptive surgical procedures are not only performed in the hospital but also during mass extensive services programme. 3353 laparoscopic sterilization were performed in the hospital while 3401 cases were performed during extension services programme. A complete statistical record of individual patient is maintained and analysed at each successive follow up visit. Average age at the time of laparoscopic sterilization was 30 years, average parity was 6 and the age of last born baby at the time of surgical contraceptive procedure was one year, with a minimum failure rate, and a few complications with no mortality. Laparoscopy is the technique of visualisation of peritoneal cavity by means of an endoscope introduced through the abdominal wall. It has a limited effectiveness in surgery regarding certain intestinal disorders, but it provides a remarkable panoramic view of the female pelvic organs and lower abdominal structures. Therefore it plays an important role in many gynaecological diagnostic as well as in contraceptive surgical procedures. Laparoscopic sterilization is performed as an outpatient procedure under local anaesthesia using fallope rings. It is a simple economical, extremely rapid and quite safe surgical contraceptive method. Due to recent modifications and technical advance in the instrument, it is proved to be an excellent tool for research and study of factors controlling folliculation, ovulation and lutenisation. Treatment of ectopic pregnancy and endometriosis is now possible by laparoscopy. Second look laparoscopy has an important place for post-operative assessment following tubal surgery and in follow up and management of malignant ovarian and cervical diseases
