RESUMO

Introduction: The National Family Health Survey-4 data from Odisha shows, adolescent growth spurt and nutritional level needs to be studied comprehensively for designing appropriate policies. Kandra community of Odisha is a Schedule Caste (SC) community with low socio-economic status. No study has been done on growth spurt of girls from this community. Hence, the paper examined growth and nutritional trend and test for significant difference on height, weight, and Body Mass Index (BMI). Methodology: Cross-sectional study with 78 sample were collected from one Gram Panchayat of Ganjam district, India. Somatometric measurements i.e., height, weight, chest, and calf circumference were collected from school-going adolescent, randomly. Mean difference between early and middle adolescent were seen through Levene's Test for equality of variances and independent sample-t-test. Result: Findings say, adolescent growth spurt was highest at age 13-14 years. The overall median height and weight were reported less than Indian and National Center for Health Statistics (NCHS) standards. The difference between 12-13 years and 14-16 years were significant at p<.001 for BMI. Conclusion: Findings were like the gaps reported in NFHS-4 nutritional indictors and is a concern as the girls are going to be future mother, and deficiency may pull their life into risk.

Background: To study the clinical profile, laboratory parameters, complications and outcome of scrub typhus in children. Methods: One hundred children with undiagnosed febrile illness admitted to our hospital from Aug 2014 to Nov 2014 were included in the study. All children were tested for scrub typhus using a commercial ELISA kit for specific IgM antibodies against Orientia tsutsugamushi. Results: Out of 100 children admitted to our hospital, only 50(50%) patients had positive IgM antibodies against O. tsutsugamushi. 30(60%) cases had fever of 5 to 10 days duration and 20(40%) cases had fever of more than 10 days duration. Vomiting with abdominal pain was reported in 20 (40%) cases, Lymphadenopathy in 18 (36%), hepatosplenomegaly in 35 (70%), generalized edema in 18 (36%) patients. Eschar was seen in 25(50%) patients. Most common abnormal laboratory parameters were raised SGOT, thrombocytopenia, raised bilirubin, raised CRP, leukocytosis and anemia. Most common complications were shock and electrolyte disturbances. Conclusion: In children Scrub typhus should be considered in the differential diagnosis of acute febrile illness associated with gastrointestinal symptoms, hepatosplenomegaly and lymphadenopathy .Prompt antibiotic treatment for scrub typhus should be given in cases with strong clinical suspicion to prevent morbidity and mortality.

Introduction: Femoral neck fractures are rare injuries in children, but the high incidence of long term complications make it an important clinical entity. The aim of this prospective study was to analyze the clinical outcomes of pediatric femur neck fractures. Methods: The study included 16 children (10 boys and 6 girls) who sustained femoral neck fractures and completed an average follow-up of 28 months. The children were treated with anatomical reduction and internal fixation with partially threaded cancellous screws. The outcomes were clinically and radio logically assessed for fracture healing, joint movements and implant failure. Results: The mean age of included patients was 10 years (range, 6 to 16 years) and the average followup was 28 months (range, 17 -48 months). Coxa- vara was the most common complication Other complications included nonunion associated with Partial a vascular necrosis and delayed union was seen in one case but later on it had been corrected. Two cases of slight coxa vara were seen radiologically but clinically patients had no problem in walking and daily activities. All children achieved union in a mean time of 10 weeks (range from 6 - 16 weeks) except two cases. Full weight bearing was possible in a mean time of 8.8 weeks. The results were excellent in 68.75% and good in 18.75% patients. Conclusions: We conclude that the early operation using anatomical reduction and internal fixation of pediatric femoral neck fracture offers the best results and provide early healing less complication with good results and is economical.

Objective: To compare the outcome of two regimes of mifepristone and misoprostol (oral vs vaginal and sublingual administration of misoprostol) for medical termination of early pregnancy (within 49 days). Study design: A randomized controlled study was conducted. On 75 women undergoing medical termination of pregnancy (MTP) within 49 days of amenorrhea (7 weeks). These women were divided into two groups – Group 1: received tablet mifepristone 200 mg orally followed by tablet misoprostol 600 mcg oral after 48 h. Group 2: received tablet Mifepristone 200 mg orally followed by misoprostol tablet 800 mcg, two tablets administered per vaginum and two tablets advised to be taken sublingually after 12 h. An ultrasonography pelvis was performed on all patients pre-MTP to confirm the location and age of gestation and post-MTP on day 15 to confirm no retained products of conception. Statistical analysis was done by two proportion tests for individual side effect with the null hypothesis. Results: The complete abortion rates in Group 1 were 91.4% as compared to 90% in Group 2. Only 11.4% of women in Group 1 and 12.5% in Group 2 had bleeding for more than 15 days. The duration and amount of bleeding were statistically similar in both groups. Conclusion: We concluded that after 48 h of administration of mifepristone, oral 600 mcg of misoprostol is as efficient as 800 mcg of misoprostol (administered vaginally, 400 mcg and sublingually, 400 mcg) in inducing medical abortion of pregnancy within 49 days of amenorrhea.

Introduction: To investigate the effect of minimally invasive percutaneous plate osteosynthesis for treatment of proximal humerus fractures in osteoporotic patients with philos plate. Methods: A prospective study was conducted at Department of Orthopedics, M.M.Medical College, Mullana Ambala, from July 2005 to November 2010. A Total of 108 cases of proximal humerus fractures were treated with minimally invasive percutaneous plate osteosynthesis with philos plate. According to Neer classification, 89 cases had two part fractures, 19 had tree part fractures. Results: All the 108 cases were followed up ranging from 12 to 18 months with an average of 14 months. Radio graphically, union was observed in 92 patients at the end of 10 weeks. In 10 patients, times to union were 16 weeks. Six patients underwent autogenous bone grafting because of nonunion after 16 weeks. The mean Constant-Murley score was 86 (range 74-92). The functional results of the shoulder, according to Muley scores, were classified as excellent in 85.15% cases, good in 9.25% cases and fair in 5.55 cases. Conclusion: Minimally invasive percutaneous plate osteosynthesis for treatment of proximal humerus fractures in osteoporotic patients with philos plate shows good results and less complication.

There have been no studies that have specifically looked at the management of subtrochanteric femoral fractures in skeletally immature adolescents. It was the purpose of this study to investigate the treatment of this injury in this unique patient population. Methods: This study was conducted in the Department of Orthopaedic surgery in M. M. Medical College from July 2006 to November 2008. Thirty-four patients were recruited from Emergency and out patient department having closed subtrochanteric femoral fracture. All patients were operated under general or spinal anesthesia. All patients were followed for twelve months. Results: All children achieved union in a mean time of 10 weeks (range from 6 - 16 weeks) depending on the type of long bone. Full weight bearing was possible in a mean time of 8.8 weeks. Mean duration of hospital stay was 9.8 days. The mean follow-up period was 28 months (17-48 months). Complications were recorded in 4 (11.77%) patients and included: two entry site skin irritations, one protrusion of the wires through the skin and one delayed union. The results were excellent in 97.06% and good in 2.97% patients. Conclusions: We conclude that Rigid and close interlocking nailing between the age of 9-16 years offered excellent fracture stability allowing early mobilization (early weight bearing) and joint motion in comparisons to the other groups and between the age of 6-8 years titanium elastic nail and bridging plate offered excellent result.

RESUMO

Indian spices that provide flavor, color, and aroma to food also possess many therapeutic properties. Ancient Indian texts of Ayurveda, an Indian system of medicine, detailed the medicinal properties of these plants and their therapeutic usage. Recent scientific research has established the presence of many active compounds in these spices that are known to possess specific pharmacological properties. The therapeutic efficacy of these individual spices for specific pharmacological actions has also been established by experimental and clinical studies. The medicinal effects traditionally ascribed to Indian spices are validated by modern pharmacological and experimental techniques, thus providing a scientific rationale to their traditional therapeutic usage. Many plant-derived molecules have shown a promising effect in therapeutics. Among the plants investigated to date, one showing enormous potential is the Piperaceae. Piperine is an alkaloid found naturally in plants belonging to the pyridine group of Piperaceae family, such as Piper nigrum and Piper cubeba. It is widely used in various herbal cough syrups and it is also used in anti inflammatory, anti malarial, anti leukemia treatment. So the present study was aimed to extract the phytochemical compounds in different solvent system in Piper nigrum and Piper cubeba. In preliminary screening and confirmatory test it was identified as alkaloid. High antioxidant activity was found in Piper cubeba ethanol extract i.e. 77.61±0.02% in comparison to Piper nigrum extracts with 74.61±0.02% with IC50 values10.54±0.12μg/mg and 14.15±0.02 μg/mg respectively.

In the present study, impact of tannery and other industrial effluents on the physico-chemical characteristics of loamy drain water and their consequent impact on soil and plants irrigated with effluent have been studied. The study reveals most of the parameter pH, BOD5 and COD at sampling station I was higher than station II. Waste water quality at both Stations I and II exceeded prescribed limits (BIS) for safe disposal of effluents into the surface water Samples of soil and vegetables from the land irrigated with loamy drain water has been collected and analyzed for Cu, Zn, Ni, Cr Pb and Cd. The different metals showed different enrichment factor for loamy drain water irrigated soil and are as follows: Cd 30% (max), Pb 26%, Zn 18%, Cr 5%, Cu 5%, Ni 2% (min). For plant samples collected at polluted sites are Ni 46% spinach (whole plant) (max), Zn 42% spinach (whole plant), Cr 39% spinach (whole plant), Cu 33% spinach (whole plant), Pb 20% potato tuber, Cd 20% potato tuber (min). The levels of Zn 145, Cu 5.25, and Ni 39.25 microg/ g in spinach, Pb 29.25, Cr 38. 25 and Cd 3.2 microg/g in potato tuber grown on polluted soil irrigated with contaminated drain water were found more than the reference value, which may create chronic health hazard problem to human and cattle through food chain in long run. Accumulation of toxic heavy metals may be build up in the agriculturally productive land where it is treated with contaminated effluent enrich with metals in turn bio-concentrated in the edible fodder/plants.

RESUMO

The present study deals with the assessment of industrial water of an electronic component manufacturing unit with electroplating and its subsequent effects on soil and plants receiving the effluent. The physico-chemical parameters of the effluent samples showed higher value than that of ground water. The treated effluent was within the permissible limit. Microtox test was conducted and determined the degree of toxicity of untreated, treated effluents as well as the water sample collected at effluent discharge point of receiving river (confluence point). The physico- mechanical parameters of the soil samples were not changed due to irrigation of the treated effluent, but the concentration of metals were comparatively higher than the control soil. Higher accumulation of metals was found in the plant parts in naturally growing weeds and cultivated crop plant irrigated with treated effluent. Elevated accumulation of metals in Eichhornia crassipes and Marsilea sp. growing along the effluent channel has been identified as a potential source of biomonitoring of metals particularly for Cu and Ca and can be utilised for the removal of heavy metal from wastewater.

RESUMO

Fifteen (34.8%) of 43 patients of falciparum malaria screened for urinary abnormalities showed significant proteinuria (greater than 150 mg/24 h), haematuria (greater than 1/HPF) and casts, with or without azotaemia. Light microscopic examination of renal biopsy tissue from 12 patients revealed mesangial and endothelial proliferative change in 8, and acute tubular necrosis in one patient. Immunofluorescence showed IgM alone, or IgG and IgM along with C3, in 7 patients within the mesangium or along the capillary walls. Repeat kidney biopsy after 6 wk in 5 patients revealed no residual pathology indicating the reversible nature of the lesions.

Sputum and bleed samples were analysed from 330 cases of bronchial asthmatics and 50 normal persons without features of bronchial asthma. The prevalence of aspergilli and antibodies to different Aspergillus species were detected using cultural methods and serological technique like agar gel double diffusion (DD), counter-immuno-electrophoresis (CIEP) and enzyme-linked immunosorbent assay (ELISA). Out of total 6.06% yielded Aspergillus species in sputum specimens repeatedly. Precipitins against Aspergillus species were detected in 7.88% of cases by DD and CIEP. ELISA test showed 20% of cases to be having antibodies to A. fumigatus. Allergic broncho-pulmonary aspergillosis can be detected in the early course of the disease in bronchial asthmatics using highly sensitive technique like ELISA.
