RESUMO

OBJECTIVE: Prevalence of coronary artery disease (CAD) has been reported to be high in patients with atherosclerotic peripheral artery disease (PAD) in lower extremities. Various inflammatory markers have been known to be associated with CAD. The aim of study was to explore the role of inflammatory makers for CAD in patients with PAD. METHODS: A total of 346 PAD patients (71.51+/-9.41 years, 337 males) who underwent percutaneous transluminal angioplasty from June 2006 to April 2012 were included in this study. Patients were divided into the Group I (PAD with CAD: n=151, 149 males) and the Group II (PAD without CAD: n=195, 188 males). RESULTS: Among 346 patients, 149 patients had CAD (43.6%). The prevalence of diabetes mellitus (DM) (p=0.023) and smoking (p=0.010) were significantly higher in the group I when compared withthe group II. The level of high sensitivity C-reactive protein (hs-CRP) increased (p or =3.0 mg/dL) (OR=2.595, 95% CI: 1.548-4.350, p<0.001), and age (OR=0.645; 95% CI, 0.454-0.915; p=0.014) were independent predictors for the development of CAD in PAD patients. CONCLUSION: 43.6% of patients with PAD also had CAD, and the predictors of CAD were smoking, DM, and high level of hs-CRP.

RESUMO

Ganglions are the most common mass occurred in the wrist and hand. Ganglion usually arises from the dorsal aspect of the radiocarpal joint that may lead to the wrist pain. We report a patient with a ganglion of the triangular fibrocartilage complex. Using arthroscopic technique, ganglion located in triangular fibrocartilage complex was successfully excised.

RESUMO

A 35-month-old girl visited our hospital with repetitive vomiting and abdominal distention; this was especially aggravated after the introduction of solid and semisolid foods. At 5 months of age, the patient, who had Down's syndrome, had undergone surgery for ventricular septal defect, atrial septal defect, and patent ductus arteriosus, and had subsequently been frequently hospitalized for respiratory infections and other viral infectious diseases. After her admission, the abdominal distension improved with fasting and intravenous fl uid therapy. Radiograph from a small-bowel series revealed a thin fi lling defect with a dilated duodenal bulb in the distal region of the second portion of the duodenum, suggesting a duodenal web, and endoscopy revealed duodenal stenosis. We therefore performed endoscopic resection with an insulated-tip knife because of the history of prior operations, fasting problems after operations, and respiratory infections. Seven days later, scar formation was noted on the second portion of the duodenum, the scope passed well at the excision site, and no retained food material was noted on the follow-up endoscopy. After the procedure, the patient's abdominal distention and repetitive vomiting subsided, and she was discharged with the ability to eat eat an age-appropriate normal diet. There were no specifi c symptoms or other complications for 1 year after the procedure.

RESUMO

Metatropic dysplasia is a rare spondylo epi metaphyseal dysplasia characterized by progressive kyphoscoliosis, short limbs with relatively large hands and feet and limited of motion and enlargement of the large joints. It is diagnosed based on the characteristic clinical and radiological features. Even though benign cases of this disease are frequently reported, metatropic dysplasia can often have a fatal outcome. We describe a sporadic case of the well-delineated lethal metatropic dysplasia.

RESUMO

PURPOSE: To compare the result of treatment between minimally invasive plate osteosynthesis (MIPO) using periarticular plate and intramedullary nailing in treatment of distal tibia fractures. MATERIALS AND METHODS: 28 cases of distal tibia fractures form Jan. 2006 to Mar. 2008 were divided into two group. Minimum follow-up was for 12 month. Group 1 consisted of 14 patients who were treated by MIPO technique and group 2 consisted of 14 patients who were treated by interlocking intramedullary nailing. The results were compared by assessing radiologic and clinical result. RESULTS: The mean bony union time was 14 weeks (8~17) in group 1 and 15 weeks (11~20) in group 2. Operation time was 58 minutes (55~65) in group 1 and 82.7 minutes (70~100) in group 2. The average angulation in AP view was 1.5 degrees (0~2) in group 1 and 2 degrees (0~5) in group 2, in lateral view was 1.8 degrees (0~4) in group 1 and 2.3 degrees (0~12) in group 2. The average range of motion for dorsi flextion was 17.5 degrees (15~20) in group 1 and 18 degrees (16~20) in group 2, for plantar flextion was 45 degrees (42~50) in group 1 and 44 degrees (42~50) in group 2, which means that there were no severe limitation of motion in all patients, resulting in satisfactory ambulation. There was no post operative complications such as skin irritation problem caused by internal device and no implant failure and superficial wound infection. CONCLUSION: There were no difference in bony union time, clinical result and anatomical reduction between Group 1 and Group 2 in distal tibia fractures, but operation time was shorter in MIPO than nailing.

RESUMO

Acute hematogenous osteomyelitis of the acetabulum is not a common disease with similar clinical symptoms and hematological findings to those of pyogenic arthritis of the hip joint. This symptom similarity might lead to a delayed diagnosis of the disease, and even to unnecessary surgery. We report two cases of acute hematogenous osteomyelitis of the acetabulum. One was a 13-year-old boy with non-specific radiological findings of the hip joint, who was initially diagnosed with fever of unknown origin. The other was an 11-year-old boy with hip joint pain and a recent history of minor trauma, which lead to a delayed diagnosis.

RESUMO

PURPOSE: We present our experience of operative results between in situ decompression of the ulnar nerve and medial epicondylectomy for cubital tunnel syndrome. MATERIALS AND METHODS: In 50 cases, we analyzed 32 cases followed more than 12 months. In 32 cases, 17 patients underwent in situ compression and 15 patients underwent medial epicondylectomy. After average follow-up period, we analyzed the operative results. The average follow-up period was 27 months(12~51 months). The results were divided into pain, sense, motor, and function using Gabel & Amadio grade (Table 1), and then estimated by 4 steps. Also, we divided into 3 grades using McGowan grade (Table 2), and then compared the score. RESULTS: The results of in situ decompression were 3 excellent cases, 5 good cases, 7 fair cases, and 2 poor cases by Gabel & Amadio grade. The results of medial epicondylectomy were 3 excellent cases, 4 good cases, 7 fair cases, and 1 poor case by Gabel & Amadio grade. However, there was no significant difference between the in situ decompression(5.45) and medial epicondylectomy(5.78). The results of McGowan grade I were 3 excellent cases, 1 good case. According to Gabel & Amadio grade, the average score was 8.5(range, 7~9) and the results of McGowan grade II were 3 excellent cases, 5 good cases, 5 fair cases, and 1 poor case. According to Gabel & Amadio grade, the average score was 6.7(range, 3~9). Finally, the results of McGowan grade III were 3 good cases, 9 fair cases, 2 bad cases. According to Gabel & Amadio grade, the average score was 4.85(range, 2~7), which was statistically significant difference. CONCLUSION: Statistically there was no significant difference between in situ decompression of the ulnar nerve and medial epicondylectomy for cubital tunnel syndrome. Both operative methods have short operation time, which makes it possible to exercise the joints earlier than other operations. In conclusion, we consider both methods are available for the treatment of cubital tunnel syndrome.

RESUMO

PURPOSE: To evaluate usefulness of Navigation system by comparing the radiographic results of femoral and tibial component position between conventional total knee replacement (TKA) and Navigation-assisted TKA. MATERIALS AND METHODS: From December 2003 to January 2006, TKA with Scorpio(R)(Osteonics, NJ,USA) were preformed for 53 knees of 41 patients. Among them, 26 knees (Group A) were operated by conventional TKA and 27 knees (Group B) were by Navigation-assisted TKA. We measured the femorotibial angle (omega), femoral component coronal (alpha) and sagittal angle(gamma), tibial component coronal (beta) and sagittal angle (delta) according to the radiographic evaluation of American Knee Society. We also measured the coronal femoral component angle to the femoral mechanical axis (theta) and femoro-tibial mechanical axis. RESULTS: alpha, omega and thetaangle showed no significant difference between two groups (p>0.05). beta, gammaand delta angle were 89.2+/-1.9, 4.3+/-1.7, 81.4+/-2.9 degree in group A and 90.5+/-1.2, 2.8+/-1.6, 86.6+/-2.2 degree in group B, respectively. There was statistically significant difference in beta, gammaand deltaangle (p<0.05). The range of deviation from desired angle showed no significant difference in femoro-tibial mechanical axis and betaangle. However, there was statistically significant difference in thetaangle with more accuracy in group B (p=0.0001) CONCLUSION: There were statistically significant differences in betaand gammaangle between conventional TKA and Navigation assisted TKA. We could get more accurate femoral component coronal angle to the mechanical axis with the Navigation system.

RESUMO

The purpose of this study id to investigate the change of the EGFR mRNA expression in the rat gingival epithelium by the experimental tooth movement. We applied reciprocal force between the upper anterior teeth using NiTi open coil spring and stainless steel wire for 1, 2 3, 7 days. For the detection of EGFR mRNA, in situ hybridization was done in the tissue samples which were taken from the pressure and tension sides of teeth. The results were as follows; 1. The expression of EGFR mRNA was increased application-time dependently. a Day 1: mild expression on the basal and spinous cell layers. b. Day 2: moderate expression on the whole layers. c. Day 3: severe expression on the basal and spinous cell layers. d. Day 7: severe expression on the whole layers 2. The expression level of EGFR mRNA in the pressure and tension sides were similar during the whole period of experiment except seven day application at which the cornified layer of the tension side showed moderate expression. 3. Removal of the appliance after 7-day force application lowered the level of EGRF mRNA expression. It was returned to the mild and control (rare) level at three and seven days after the removal, respectively. In conclusion, EFGR mRNA was increased by, the experimental tooth movement on the rat ginigval epithelium. Up-regulation of EGFR mRNA in the gingival epithelium can be regarded as responses to the possible changes caused byy the physical stersses to the oral environment to maintain the homeostatic conditions of the periodontium.
