RESUMO
We report a case of pyelocalyceal diverticulum. The patient was a 25-year-old woman and admitted to the Dept. of Urology B.N.U.H. because of mild left flank pain. The tentative diagnosis was made by IVP and confirmed by operation and histological examination. The roentgenogram of IVP revealed round cystic mass at lateral site of middle calyx, right.

Herein we report an experience of the vaginal ureterolithotomy. The patient was a 48-year-old woman with a juxtavesical left ureteral stone, which was vaginally palpable. Although this operation has limited indications, it is a safe and easy method of removing stones from the lowermost ureter especially in the women. This method seems to have considerable value to be attempted in the carefully select patients.

Penile fracture is rather rare. Authors recently experienced two cases of penile fracture and reviewed the literature. Case 1 was an 18-year-old single male who visited our emergency room on 7th Dec, 1980 with the chief complaint of painful swelling and deformity of the penis. The present illness started with a cracking sound. An emergency operation was done. The Buck's fascia and the tunica albuginea of the left corpus cavernosum were torn transversely with hematoma formation. The hematoma was removed and the tom tunica albuginea and Buck's fascia were sutured. On the 6th post-operative day, he discharged with improved symptoms. Case 2 was a 35-year-old married male who visited our emergency room on 5th Dec, 1981 with the chief complaint of painful swelling and deformity of the penis. He also experienced a cracking sound. The corpus cavernography showed the tearing of the corpus cavernosum on the proximal dorsal penis. An emergency operation was done. The Back's fascia and the tunica albuginea of the right cavernosum were torn about 2 cm transversely. The tom albuginea and the Buck`s fascia were sutured with 4-0 Dexon. On the 11th post-operative day, he discharged without penile deformities.

Priapism is a condition of prolonged erection, unassociated with sexual stimulation, which is usually painful We experienced two cases of idiopathic priapism, the one was cured by the corporal irrigation with normal saline containing heparin through the glans tip and the other by the corpora cavernosa-saphenous vein shunt. Postoperatively potency in these two cases was obtained.

We experienced two successful cases of left to right transureteroureterostomy recently. One had left ureterocutaneostomy resulted from the failure of left ureteroneocystostomy associated with partial cystectomy in bladder tumor, who was treated with left to right transureteroureterostomy without ureteral stents. The other had recurrent bladder tumor which had been treated with partial cystectomy, irradiation and TUR, who was underwent with cutaneous transureteroureterostomy (left to right) and subsequent total cystectomy by placing ureteral stents. In both cases, no complications were seen, such as urinary leakage, progressive bilateral and/or unilateral hydronephrosis

We herein report 2 cases of patient with urinary cystine stones. One was a 32-year-old woman and the other was a 52-year-old man. Each patient was found to have cystine crystals in urine and had a renal coral stone and multiple renal or ureteral stones. The female patient undertook a combined right pyelolithotomy and nephrolithotomy. But a few stones were remained in the kidney The removed stones consisted of a large staghorn stone and many small stones and were equivalent to cystine stones described in references macroscopically The male patient was treated conservatively. After discharge they have been treated with Thiola, large fluid intake and conversion of urine pH. And to date no recurrence has been noted in the female. There was decrease in size of left renal staghorn calculi in the male.

Congenital megacalicosis was first described by Puigvert in 1963. In pyelogram, it reveals dilated calyces with normal pelvis and ureter. And it should be differentiated from obstructive hydrocalicosis or hydronephrosis. In the last 6 years, we experienced two cases of megacalicosis at our urologic department. These cases consist of one boy and one woman.

Sarcomas of the prostate are the rare malignant tumors. In Korea. total 11 cases of prostatic sarcomas have been reported to date. Herein we report another 5 cases and discuss the multimodal treatment for prostatic sarcomas.

A 9 month-old male infant was admitted with the chief complaints of acute urinary retention and fever for 7 days. IVP showed the dilated upper urinary tracts. An expression cystogram demonstrated a bladder diverticulum without ureteral reflux. On cystoscopy, the left ureteral orifice was thought to be engulfed by the diverticulum and could not be seen The right ureteral orifice was normal findings. Diverticulectomy and left ureteroneocystostomy were performed. Postoperative course was uneventful and postoperative voiding cystography and IVP one month later showed an improvement of the upper urinary tracts. Diverticulum of the bladder it an infant was discussed.

A case of torsion of the right cryptorchid testis was herein reported. The patient was 6-year-old boy who was rolled on a staircase and traumatized his right inguinal area. There after painful and tender swelling developed on the area, and it spent 4 days prior to visit our department The involved cryptorchid testis showed dark reddish discoloration and external rotation of 360 degrees and was removed because of no re-establishment of blood supply in spite of detorsion for 30 minutes.

Case 1 was a 55-year-old housewife with the 1-year history of mild recurrent gross hematuria and recent history of massive hematuria following radiation treatment for uterine cancer 7 years ago. Various conservative treatments showed no effect but complete hemostasis was obtained immediately after instillation of 100 ml of 4% formalin solution for 30 minutes under sedation. And to date, there has been no recurrence of hematuria or significant complications for 5 months after the treatment. Case 2 was a 45-year-old housewife with the 3-month history of symptomless gross hematuria following irradiation for a gynecologic malignancy 8 years ago. Complete hemostasis was obtained one day after instillation of 100 ml of 4% formalin solution for 30 minutes under sedation. and to date, there has been no recurrence of hematuria or any complications for 2 months after the treatment. Postradiation vesical bleeding is a very difficult condition to control and may result in total cystectomy and urinary diversion. The treatment by intravesical instillation of formalin solution for the intractable vesical bleeding is simple to perform, shows excellent effect and no significant complications, and is a highly recommendable mode of treatment for massive, uncontrollable vesical bleeding before total cystectomy and urinary diversion.

In evaluation of primary hyperparathyroidism, one of the important causative diseases of urinary calculi, it is the prerequisite to determine the values of serum calcium and inorganic phosphorus along with the studies of clinical symptoms and signs, radiologic findings and other laboratory tests. The author obtained the following results through investigation of serum calcium and inorganic phosphorus in normal Koreans and urologic patients as the first step of research on the primary hyperparathyroidism. 1. Value of serum calcium in normal Koreans was 4.9% +/- 0.6 mEq/l, and no differences in sex and age distributions were noted. Serum inorganic phosphorus showed 3.5 +/- 0.8mg% in value, without any noticeable differences in sex distribution, and slightly higher value was noted in age group of under 20. 2. Values of serum calcium in out-patient group were within normal limit without regard to diseases and sex 3. In the in-patient group, lower value of serum calcium and higher value of inorganic phosphorus were noted in the renal failure group, and slightly higher value of inorganic phosphorus was seen in the obstruction group. 4. There was noted a mild inverse correlation between values of serum calcium and inorganic phosphorus.

This paper deals with clinical study of 23 cases of non-visualizing kidney on intravenous pyelography in children during 11 years from Jan. 1965 to Dec. 1975. The studying period was divided into the earlier term('68~'71) and the later term('72~'75) because since 1972 we have had special considerations in pediatric urology. The following results were obtained from the study. 1. More than half of the cases with non-visualizing kidney were seen in the age under 5 years. There were no remarkable difference between both sexes and terms of studying. 2. The chief complaints were flank mass, hematuria, incontinence and urinary frequency etc. in order of frequency without regarding to terms of studying. 3. In boys, the duration of symptoms was generally shorter than in girls. 4. As causative diseases, obstructive uropathy occupies more than half of the cases and more frequent in the earlier term of study than in later term. There was seen no difference between sexes. 5. In treatment of causative diseases, as possible conservative operations of the kidney were done.
