RESUMO

Squamous cell carcinoma commonly originates from recalcitrant wound sites, including burn scars, pressure sores, stasis ulcers, osteomyelitis, and sites of frostbite. A 62-year-old male was referred to the dermatology department for skin necrosis of his right great toe and walking difficulty. He had a history of smoking, drinking alcohol, and frostbite of his right great toe 9 years prior, which deteriorated into osteomyelitis due to poor care. Although a skin biopsy was recommended before amputation, the two procedures were performed simultaneously due to a lack of toe function due to severe osteolysis. Biopsy of the amputated toe tip showed many lobules consisting of atypical keratinocytes with hyperchromatic nuclei, and severe dermal pleomorphism. After evaluation for distant metastasis, including a (99m) Tc-MDP bone scan, 18F-FDG positron emission tomography scan, computed tomography, and ultrasound, no metastasis was detected.

Atypical lymphocytic lobular panniculitis (ALLP) is a recently described entity characterized by self-limiting plaques that manifest a waxing and waning course. Although ALLP is known as a benign condition, it needs adequate follow-up to determine the risk of progression to subcutaneous panniculitis-like T-cell lymphoma (SPTCL) because ALLP has morphologic and biologic similarities with SPTCL. We report a case of ALLP in a 13-year-old girl who presented with an unusual tumor-like erythematous mass on the left thigh but without systemic symptoms or laboratory abnormalities. The histopathologic findings showed a dense and diffuse infiltration of the fat lobules by small-to-medium-sized atypical lymphocytes including focal atypical mitosis. Some cells showed characteristic peripheral rimming of individual adipocytes. However, prominent lymphoid atypia, fat necrosis, vasculitis, vascular thrombosis, erythrophagocytosis, mucin deposition, and the striking dominance of CD8+ lymphocytes seen in SPTCL were not detected. The lesion resolved spontaneously in 6 months and there was no recurrence over a 24-month follow-up period.

RESUMO

Pemphigoid gestationis, also known as herpes gestationis, is a rare blistering disease associated with pregnancy, caused by autoantibody to the basement membrane component. It occurs in the second or third trimester of pregnancy or directly after delivery, as erythematous, urticarial papules, and plaques on the whole body, which aggravate to vesicles and bullae with severe pruritus. Histopathologically, subepidermal blisters and eosinophilic spongiosis are shown, and linear deposition of C3 in the dermoepidermal junction is seen on direct immunofluorescence assay. We present the case of 35-year-old woman who was diagnosed with pemphigoid gestationis, with the involvement of the uncommon site of the face.

RESUMO

Lichen planus is a chronic inflammatory disease that mainly involves the skin and oral mucosa. We report the case of a 57-year-old female who presented with a 6-week history of localized violaceous papules on her back and both extremities. Prior to presentation, she had received acupuncture on her back and both extremities, and 2 weeks later skin lesions occurred along the treated area. From the histopathologic findings of the lesion, the patient was diagnosed with lichen planus and treated with intralesional and topical corticosteroids. Dermatologic diseases such as atypical mycobacterial infection, foreign body granuloma and contact urticaria associated with traditional Korean medicine treatments have been reported. To the best of our knowledge, there has been only one report of eruptive lichen planus triggered by acupuncture in English literature to date. Herein, we report this rare and interesting case of lichen planus related to acupuncture.

BACKGROUND: Famciclovir and valacyclovir are antiviral agents commonly used to treat herpes zoster. These medications not only reduce the time to complete cessation of zoster-associated pain, but also aid in the healing of the herpes zoster skin lesions. However, only few studies have compared these antiviral agents. OBJECTIVE: We conducted a randomized clinical trial to evaluate the extent of pain relief and wound healing, and the rate of postherpetic neuralgia associated with these drugs during 4 weeks of treatment. METHODS: The study included 69 immunocompetent adult inpatients diagnosed with herpes zoster randomly divided into 2 groups based on the antiviral agent administered. Patient age, date of visit from rash onset, and rash severity at baseline were recorded. Famciclovir or valacyclovir were administered orally for 7 days. Patients reported pain levels through a visual analog scale (VAS) score, and pain durations were assessed on days 1, 3, and 7, and at weeks 2, 3, and 4. Crust formation and reepithelialization times of skin lesions were also recorded. RESULTS: VAS scores, pain durations, ratios of patients undergoing postherpetic neuralgia, and skin lesion healing rates did not differ significantly between the 2 groups. However, rash severity independently correlated with the extent of pain experienced. CONCLUSION: Famciclovir and valacyclovir are comparable to each other in resolving zoster-associated pain, postherpetic neuralgia, and zoster wound healing. Early antiviral treatment before expansion of the skin lesion would be helpful for rapid relief of herpes zoster pain.

RESUMO

Eccrine syringofibroadenoma (ESFA) is a rare, benign adnexal neoplasm of eccrine differentiation. It is typically located on the limbs and presents as a plaque or a solitary hyperkeratotic nodule in an adult. However, there are several clinical subtypes, ranging from a solitary papule or nodule to multiple lesions with linear or diffuse distribution. Despite the diverse clinical presentation, ESFA is histologically similar. It commonly shows proliferation of anastomosing cords and strands of cuboidal epithelial cells with or without lumina embedded in a fibrovascular stroma. Herein, we report an unusual case of ESFA with verrucous surfaced plaque present on the scalp since birth in a 22-year-old man.

BACKGROUND: Acute pain and postherpetic neuralgia often interfere with life quality severely. Light emitting diode (LED) has been proposed as one of the effective therapeutic tools for pain reduction. However, little has been published in the Korean medical literature about the effect of LED on acute pain and the inhibitory effects of postherpetic neuralgia (PHN) in patients with herpes zoster. OBJECTIVE: The purpose of this study was to evaluate the efficacy of LED on acute pain and the inhibitory effects of PHN in patients with herpes zoster. METHODS: Sixty patients who were hospitalized for herpes zoster were evaluated according to their age, sex, associated disease, nature of pain, interval between diagnosis, therapy, and PHN. The patients were randomly placed into either a LED group treated with I.V acyclovir, analgesics and LED or a control group treated with I.V acyclovir and analgesics alone. The severity of pain was evaluated by patients themselves using a visual analogue scale (VAS) and a verbal rating scale (VRS) at one week intervals. The occurrence of PHN was assessed at 4 weeks after the initial development of the characteristic skin lesion of herpes zoster. RESULTS: The average days required for pain relief was significantly shorter in the LED group than the control group. The average weeks needed to reduce pain from 100 to 50 in the VAS, implicating relief of acute pain, were 1.4 weeks in the LED group and 1.8 weeks in the control group (p0.05). CONCLUSION: LED is an effective method which can be used to relieve acute herpes zoster pain quickly; however, LED cannot reduce the onset of PHN effectively.

BACKGROUND: Axillary osmidrosis causes significant emotional distress and embarrassment. Although numerous surgical procedures have been developed for the treatment of axillary osmidrosis, there is no decisively effective one. OBJECTIVE: This paper is to assess the efficacy and safety of suction-assisted cartilage shaver with the use of fibrin glue to treat axillary osmidrosis. METHODS: 18 axillary osmidrosis patients were treated with suction-assisted cartilage shaver followed by injection of fibrin glue. The effectiveness of the treatment used in this study was evaluated by measurement of axillary malodor both pre- and postoperatively. Patient satisfaction was surveyed as well. RESULTS: The postoperative test demonstrated that there was remarkable decrease in axillary malodor. Of the 18 patients, 17 patients (94.4%) expressed positive satisfaction. Complications including postinflammatory hyper-pigmentation (27.8%), hematoma and skin necrosis (5.6%), scar (5.6%), infection (5.6%) and hidradenitis suppurativa (2.8%) were reported. CONCLUSION: The use of suction-assisted cartilage shaver with fibrin glue proved itself an effective and safe treatment for axillary ormidrosis.

RESUMO

Multiple trichoepithelioma is a benign adnexal neoplasm characterized by the presence of many small papules that appear predominantly on the face. Clinical improvement have been demonstrated after several treatments such as carbon dioxide laser, cryotherapy, dermabrasion, and argon laser. However, these destructive methods have various disadvantages. We report a case of multiple trichoepithelioma treated with an intralesional bleomycin injection. Fewer lesions without scars and cosmetic improvements were observed. Bleomycin is a cytotoxic antitumor antibiotic that can be injected intralesionally by transcutaneous administration and displays excellent results for treating malignancies including skin tumors. We believe that intralesional bleomycin injection is an effective treatment option for this condition, and an easy method to conduct at the outpatient clinic.

Dermatomyositis is a systemic inflammatory disease affecting the skin, skeletal muscle, and occasionally, other organs, such as the lung. Interstitial lung disease (ILD) is a serious lung complication, which can occur in patients with dermatomyositis. It affects the prognosis of the disease and requires a more aggressive therapeutic approach. Interstitial lung disease (ILD) may be associated with systemic inflammatory disorders and autoantibody production. In particular, anti Jo-1 antibody has long been recognized as an important predictive factor in systemic manifestations of ILD in dermatomyositis. We report here a case of a 42 year old woman with cutaneous lesions typical of dermatomyositis, showing clinical features of antisynthetase syndrome, which showed rapid progression to acute respiratory distress syndrome.

Blastomycosis-like pyoderma is a rare, vegetating skin lesion that is an unusual exaggerated tissue reaction possibly to prolonged primary or secondary bacterial infection. We report a case of blastomycosis-like pyoderma in a man with Down syndrome, diabetes mellitus and hypothyroidism who responsed poorly to conventional therapies, including antibiotics and correction of predisposing factors for a long time, but experienced dramatic improvement after 3 months of acitretin treatment.

Enterobiasis is a common disease among school-aged children, with worldwide distribution and high prevalence in countries with a temperate climate. Enterobius vermicularis usually inhabits the intestinal lumen, and the majority of clinical presentations are perianal pruritus. Extraintestinal infection is uncommon, mostly involving the female genital tract. It has been reported to occur in the peritoneal cavity, ovary, fallopian tube, endometrium, lung, liver, urinary tract and perianal tissue. In Korea, there has been only 1 case of enterobiasis presenting as perianal abscess in the Korean pathologic literature. So we present here an unusual case of perianal granuloma by Enterobius vermicularis in a 62-year-old woman.

Infantile hemangiomas are benign vascular tumors that commonly occur in infants. The management of hemangiomas ranges from simple observation and assurance to surgical resection. The commonly used treatment modalities include topical, intralesional and systemic steroids, pulsed dye laser and surgical excision. There are several reports of infantile hemangiomas treated with direct percutaneous injections of sclerosing agents such as ethanol, polidocanol or ethanolamine oleate. We report here on a case of infantile hemangioma that was treated with ethanolamine oleate by dermal injection. We think this is the first report of a dermal injection of ethanolamine oleate, and not intraluminal injection, for treating infantile hemangioma.

This study was performed to investigate the effect of chamomile, sage, and green tea intakes on anti oxidative capacity in 15-month-old rats. Dried powders of three plants were analyzed to determine the amount of total flavonoids, beta-carotene, vitamin C, vitamin E, and dietary fibers. In order to examine the change of antioxidative capacity in old rats, forty-eight Sprague-Dawley male rats weighing 621.2+/-9.5 g were divided into four groups according to body weight and fed for four weeks with each experimental diet of three dried powders and control diet. Plasma and liver thiobarbituric acid reactive substances (TBARS) levels and xanthine oxidase (XO) activities, erythrocyte and liver superoxide dismutase (SOD) activities and plasma vitamin A, C, E, and total carotenoids levels were measured. The total flavonoids and vitamin C contents were the highest in green tea powder. Beta-carotene and vitamin E contents were not significantly different among all three dried powders. Total dietary fiber contents also were not different among all three dried powders, but soluble dietary fiber contents of chamomile was higher than other two dried powders. Plasma TBARS level was found to be significantly lower in all the experimental groups as compared to control group. Chamomile powder group, especially, showed the lowest level among all experimental groups. Liver TBARS levels of experimental groups were also lower than that of control group and significant differences were observed in chamomile and green tea groups compared to control. Plasma XO activity of green tea group was significantly lower than control group. Liver XO activities of green tea and chamomile groups were significantly lower than control group. Erythrocyte SOD activity was not significantly different among all the groups. However, liver SOD activities of sage and green tea groups showed a significant increase as compared to control group. Plasma vitamin A level was not significantly different among all the groups. Plasma total carotenoids levels were found to be significantly higher in experimental groups as compared to control group. Plasma vitamin E level of chamomile group and vitamin C level of green tea group showed the highest level among all the groups. In conclusion, chamomile, sage, and green tea intakes had an effect on improving antioxidative capacity in 15-month-old rats. Effects of green tea and chamomile powders, especially, were higher than sage powder.
