Resumen La sarcopenia es la pérdida de masa y función muscular. Su presencia ensombrece el pronóstico de los pacientes cirróticos. El gold standard para la descripción de sarcopenia es el Skeletal Muscle Index (SMI). Se incluyeron 34 pacientes cirróticos. Se realizó medición a través de tomografía computarizada de ambos psoas por separado, que luego fueron sumados obteniendo lo que se denominó "suma de área de 2 psoas", además se midió área muscular total, SMI y área total de psoas. También se realizó ecografía y se midió el área transversal de recto anterior del cuádriceps por ecografía y Hand Grip (HG). Se analizó la corre lación entre las distintas mediciones comparando con el área muscular total, con área de cuádriceps (r = 0.39; P = 0.019), con área total de psoas (r = 0.71; P < 0.01), con suma de área de 2 psoas (r= 0.72; P < 0.001) y con HG (r = 0.45; P = 0.0069). Comparado con el SMI, en mujeres el HG tuvo 86% sensibilidad y 66% especificidad (AUC = 0.89). En hombres la suma de 2 psoas tuvo 91.7% de sensibilidad y especificidad (AUC = 0.82) y el área total de psoas tuvo 83.3% de sensibilidad y 76.9% de especificidad (AUC = 0.8). Hubo muy buena correlación entre los métodos de tomografía con el gold standard, y la correlación con la ecografía y la fuerza fue buena. Evaluando sensibilidad, especificidad y AUC, no se consideró un método útil para ambos sexos, comparado con el SMI


Abstract Sarcopenia is the loss of muscle mass and function. Its presence darkens the prognosis of cirrhotic patients. The gold standard for the description of sarcopenia is the Skeletal Muscle Index (SMI). Thirty-four cirrhotic patients were included. Measurement was carried out through CT of both psoas separately that were then added obtain ing the so called "sum of area of 2 psoas", in addition total muscular area, SMI and total area of psoas were measured. Ultrasonography was also performed and the transverse area of the quadriceps rectus anterior was measured by ultrasound and Hand Grip (HG). The correlation between the different measurements was analyzed comparing with the total muscle area, with quadriceps area (r = 0.39; p = 0.019), with total psoas area (r = 0.71; p <0.01), with sum of area of 2 psoas (r = 0.72; p <0.001) and with HG (r = 0.45; p = 0.0069). Compared with SMI, in women HG had 86% sensitivity and 66% specificity (AUC = 0.89). In men, the sum of 2 psoas had 91.7% sensitivity and specificity (AUC = 0.82) and the total area of psoas had 83.3% sensitivity and 76.9% specificity (AUC = 0.8). There was a very good correlation between the tomography methods with the gold standard. The correlation with ultrasound and strength was good. In the evaluation of sensitivity, specificity and AUC, it was not found a useful method when comparing with the SMI.

Background: Common cancers occurring in India are either preventable or curable, if detected early. Women’s organisation in Kerala working for poverty eradication was utilized to disseminate the message of cancer prevention in a southern district of Kerala state. Objectives: To create awareness among the community on cancer prevention and motivate them for early detection utilizing trained volunteers. Material and methods: Based on the three-tier system of functioning, master trainers at the top level of the organization were trained initially on primary prevention and early detection of common cancers viz. oral and breast. The trained resource persons further conducted awareness programmes in the second level and later at the grass root level also called the Neighborhood groups. Results: 174 master trainers were trained initially and further utilized to sensitize 920 members at the second level. At the NHG level, 1,00,257 volunteers were sensitized. 39169 house visits were conducted by trained volunteers to create cancer awareness in the community. Conclusion: Cancer literacy mission could effectively be implemented by empowered women in the community. Their acceptance in the community reinforces the fact that they can act both as a messenger for cancer prevention and a source of motivation for early detection.

Introducción: Ante el difícil y cada vez más frecuente escenario de una revisión de cadera, existen diferentes alternativas para conseguir una fijación estable y duradera del componente femoral. El objetivo de este estudio fue evaluar la evolución clínica y radiográfica de los pacientes sometidos a una revisión del componente femoral con tallo cilíndrico de superficie rugosa extendida. Materiales y Métodos: Se llevó a cabo un estudio multicéntrico y retrospectivo de 148 pacientes, operados entre marzo de 1997 y marzo de 2010, a quienes se les realizó una cirugía de revisión femoral con un tallo cilíndrico con recubrimiento poroso. La edad promedio era de 63.1 años. Resultados: El seguimiento promedio fue de 7.7 años. En 134 (89,9%) pacientes, se observó una fijación ósea estable; en 12 (8%), una fijación fibrosa estable y, en 3 (2%), una fijación fibrosa inestable. El puntaje de Harris se modificó de 41 en el preoperatorio a 92 después de la cirugía. Las complicaciones fueron infección profunda (2 casos, 1,3%), fractura de fémur intraoperatoria (12,8%) y luxación protésica (3 casos, 2%). Conclusión: Los tallos cilíndricos con recubrimiento poroso han demostrado ser un eficaz recurso para solucionar la mayoría de las revisiones femorales por la posibilidad de obtener fijación estable a corto y largo plazo. Introduction: In the set of hip revision surgery there are different options to achieve a stable fixation of the femoral component. The objective of this study was to evaluate the clinical and radiographic outcomes of patients who underwent hip revision with cylindrical extensively porous stem. Methods: Between March 1997 and March 2010 a multicenter retrospective study was performed in 148 patients who underwent hip revision with cylindrical extensively porous stem. The mean age was 63.1 years. Results: The follow-up was 7.7 years. One hundred thirty-four (89.9%) patients achieved a stable bone ingrowth fixation; 12 (8%) a stable fibrous fixation and 3 (2%) a non-stable fixation. The Harris Hip Score improved from 41 points before surgery to 92 points in the postoperative period. Complications included: two deep infections (1.3%), 12 intra-operative femoral fractures (8%) and three hip dislocations (2%). Conclusion: The cylindrical extensively porous femoral stem seems to be a reliable technique for femoral hip revision surgery, as it is possible to obtain a short- and long-term stable fixation. La Enfermedad de Morquio (MPS IV-A) es producida por dos defectos enzimáticos diferentes. Desde que la terapia de sustitución enzimática (TSE) se encuentra disponible para algunas EDL y en desarrollo para otras, este grupo de patologías han despertado un gran interés en la comunidad científica debido a que la misma parecería una alternativa terapéutica prometedora para cambiar el curso evolutivo de los pacientes afectados. Se presenta un caso con presencia de albuminuria


Morquio's disease (MPS IV-A) is produced by two different enzymatic defects. Since enzymatic replacement therapy (ERT) is available for some LDD and others under development, this group of diseases have created great interest within the scientific community due to the fact that promising therapeutic alternative will be available to change disese course of affected patient. We present a case with albuminuria.

BACKGROUND AND AIM: Stapling devices are used for pharyngeal closure after laryngectomy for the past few decades although it has not gained wide acceptance. This study is aimed at evaluating the role of stapler in pharyngeal closure after laryngectomy. METHODS: Thirty consecutive patients who underwent stapled laryngectomy at our institution from October 2004 to February 2008 were evaluated retrospectively. Linear stapler (Proximate TX 60; Ethicon Inc.) was used for closure of neopharynx. RESULTS: There were 28 males and 2 females with mean age of 54.5 years (54.5 ± 11.2). Nineteen of these patients (63.3%) had salvage laryngectomy and two patients (6.7%) had laryngectomy for a second primary tumor. Twenty‑eight patients had total laryngectomy (TL), whereas two had extended TL. Eight patients had salivary leak (26.7%). Of these, 6 (75%) had prior radiation. All salivary leaks except one were managed conservatively. Follow‑up ranged from 7 to 54 months (median: 21 months). Seven patients (23.3%) developed recurrence, six at the stoma, of which 5 (83.3%) had initial extension of disease to the subglottis. Four‑year disease‑free survival was 54.4%. CONCLUSION: Pharyngeal closure by linear stapler is an efficient and safe method of fashioning the neopharynx after laryngectomy with no added risk of occurrence of pharyngocutaneous fistula in primary and salvage laryngectomies.

OBJECTIVE: To determine the morbidity and survival of surgically treated locally advanced carcinoma larynx and hypopharynx in a tertiary referral center in South India, a prospective cohort study was carried out. MATERIALS AND METHODS: Patients who had undergone laryngectomy or laryngopharyngectomy from January, 2006 to January, 2011 at our institute were prospectively studied for factors affecting morbidity tumor recurrence and disease free survival (DFS). DFS was calculated for the whole group and for the larynx and hypopharynx cancer subgroups separately, using Kaplan Meir Method and the survival differences of the larynx and hypopharynx groups and between salvage and primary surgical cases were evaluated using the Cox’s regression scale. RESULTS: A total of 154 patients with ages ranging from 23 to 78 (mean 56.3 + standard deviation 9.2) were studied, which included 145 males and 9 females. Pre‑operative tracheostomy and previous radiotherapy were the most significant factors contributing to post‑operative morbidity. Survival difference between the larynx and hypopharynx cancers was statistically significant and the DFS was significantly affected by primary site wound infection, primary site margin and node positivity. CONCLUSION: The results of laryngectomy can be optimized by “proper case selection and morbidity risk assessment”.

RESUMO

Chemoradiotherapy is increasingly used in advanced laryngeal cancers. Failures are generally managed by surgery. They include histologically confirmed recurrent or residual disease or a symptomatic life threatening treatment sequelae. Tumour recurrence or residivism can be managed by chemotherapy when radical surgery is either refused by the patient or if the general condition of the patient do not permit it. However surgery becomes inevitable when life threatening treatment sequelae like absolute pharyngo-oesophageal stricture and aspiration sets in.

BACKGROUND: Cancer of the oral cavity is one of the commonest cancers among males. AIMS: To assess the aetiological factors, patient characteristics, treatment and the outcome in young patients with oral cancer. SETTINGS AND DESIGN: A retrospective descriptive study of patients under the age of 35 years with cancer of the oral cavity treated between 1982-1996, with the last follow-up till 2001, using the tumour registry data of Regional Cancer Centre (RCC), Trivandrum, Kerala, India. SUBJECT AND METHOD: The detailed clinical, treatment and follow-up data were obtained from the computerised records of RCC and recorded on a preset proforma. This was analysed with emphasis on age, sex, risk factors, site, histology, clinical extent and treatment methods and survival in the study group. STATISTICAL ANALYSIS: The survival analysis was carried by Kaplan-Meier method and the difference in survival was analysed using log-rank test. RESULTS: Out of 264 patients analysed, tongue was the commonest site identified in 136 (52%) patients followed by buccal mucosa in 69 (26%) patients. A male female ratio of 2.3:1 was observed with a significantly higher male preponderance in buccal mucosa (4.3:1). Prior exposure to tobacco or alcohol was noted in 59.4% patients, with more habitués in buccal mucosa cancer. Histological confirmation was present only in 83.7% patients and among them most were squamous cell carcinoma (85.9%). Radiotherapy, surgery or combined modalities of treatment were employed for majority of patients. The 5-year survival was 57.3%. T stage of the tumour was found to be significant in predicting disease free survival (P=0.03). CONCLUSIONS: The importance of early detection for clinical down staging is stressed. There is a need to investigate the aetiology of intra oral cancers in younger patients since a significant proportion (almost 40%) of these patients do not have associated risk factors for cancer.

RESUMO

OBJECTIVE: To assess the incidence of acute abdomen in patients receiving chemotherapy and to evaluate the factors affecting the clinical decision-making. MATERIAL AND METHODS: Retrospective analysis of surgical oncology references for the patients developing acute abdomen while receiving chemotherapy was carried out. A surgical oncologist reviewed each referral and a full work-up was carried out. Surgical interventions were carried out as and when deemed necessary. Response to treatment, whether surgical or conservative, was used as main outcome variable. RESULTS: Mean age of the patients was 37 years (7-58 years). Half of the patients were receiving chemotherapy for lymphoproliferative disorders, 13 for haematological malignancies and 3 for ovarian cancer. Clinical diagnosis was of acute appendicitis in 13, paralytic ileus in 9, typhilitis in 3 and intestinal perforation in 2, acute intestinal obstruction in 3, and obstructed hernia and intussception in one each. Six patients underwent surgery. Two patients expired postoperatively of progressive septicaemia and multi-system organ failure. CONCLUSION: Evaluation of cancer patients on chemotherapy is difficult. Tenderness, presence of peritoneal signs and absence of exaggerated bowel sounds are most important clinical signs, however they may be blunted by progressive neutropenia or corticosteroid administration. Decision to operate, should be made with extreme caution as mortality and morbidity after surgery is high.
